Patient was referred for an <span class="elsevierStyleSup">18</span>F-FDG PET study to assess metabolic activity in retroperitoneal lymph nodes in order to discern its residual, inflammatory or infiltrative origin. Given our availability and pancreas history, we decided to perform a PET/MRI exam.</p> <p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Whole-body MIP image:</p> <p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">• No active infradiaphragmatic lymph nodes are seen.</p> <p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">• Intense and irregular <span class="elsevierStyleSup">18</span>F-FDG uptake in the right breast along with moderate uptake in the left breast.</p> <p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">• Slightly symmetrical increased uptake in supra-aortic and subclavian vessels (score 2), considered as physiological as it is a digital PET, with no significant activity in the remaining vascular territories.</p> <p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">Selective breast imaging: T2 MR sequence and axial, coronal and sagittal fusion images:</p> <p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">• Pseudotumoral mass in the superointernal quadrant of the right breast, which reaches 9 cm in diameter, with low signal on T1 and T2 sequences, with restriction of diffusion in the MRI component, and intense and irregular <span class="elsevierStyleSup">18</span>F-FDG uptake (SUVmax 10.3).</p> <p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">• Irregular infiltration in the inferointernal quadrant of the left breast, showing low signal on T1 and T2 sequences and diffusion restriction on the MRI, along with with moderate irregular <span class="elsevierStyleSup">18</span>F-FDG uptake (SUVmax 5.1). Clinical assessment confirmed an irregular lesion in the right breast, at which time the patient reported an increase in glandular size in recent months. Lesion density in the left pubic bone increased and SUVmax decreased to 2.3.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">A 24-year-old woman complained of lower abdominal pain for two weeks with no history of hematuria, weight loss, or other relevant medical history. CT at an external institution revealed a cystic-solid mass in the ovary, approximately 95 × 85 mm. At our institution, blood chemistry showed an elevated CA125 level (62.4 U/mL; normal limit, <35 U/mL). Subsequently, [<span class="elsevierStyleSup">18</span>F]-FDG PET/CT was performed. Axial images show multiple nodules with increased metabolic activity in the abdomen and the SUVmax for all lesions ranged from 11.0 to 17.8 (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Suspecting an ovarian tumor, an exploratory laparotomy was performed and a 0.5 cm diameter nodular tumor was found in the left ovary. Pathology detected the characteristic chromosomal translocation (EWSR1–WT1). In combination with cytology, morphology, and immunohistochemistry, the final diagnosis of the desmoplastic small round cell tumour (DSRCT) invading the ovary was made. The patient underwent surgical resection and received postoperative chemotherapy and bone repair including vincristine, iso-cyclophosphamide, etoposide, and osteopontin gel tablets. [<span class="elsevierStyleSup">18</span>F]-FDG PET/CT was repeated 10 months after surgery (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). DSRCTs are rare, aggressive, and malignant tumors that occur mainly in adolescent males.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The symptoms of abdominopelvic DSRCT patients are frequent abdominal pain and a palpable mass with no specific manifestation. DSRCT is associated with a t(11;22)p13;q12 translocation. This results in the fusion of the EWS and WTI genes.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The [<span class="elsevierStyleSup">18</span>F]-FDG PET/CT findings of DSRCT include multiple masses with heterogenous density, peritoneal seeding, peritoneal effusion, retroperitoneal lymphadenopathy, and without a definite organ origin.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> In female patients, DSRCT may be mistaken for ovarian cancer and may be overlooked by clinicians.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> We report the case of a young woman with an [<span class="elsevierStyleSup">18</span>F]-FDG PET/CT presentation resembling a primary ovarian tumour, with pathology confirming DSRCT. Therefore, DSRCT should be considered in the differential diagnosis when working up a young woman with an ovarian mass. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members.
The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process.
In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
