He also referred weight loss and occasional diarrhea. PET/CT showed a large, solid, well-delimitated lobulated mass in the left hypochondrium/lateral region (largest diameter was 18.1<span class="elsevierStyleHsp" style=""></span>cm), located in the anterior pararenal space. It displaced the fourth part of the duodenum and the pancreas, it was in close relation to the descending colon and presented an intense, heterogeneous <span class="elsevierStyleSup">18</span>F-FDG uptake with a SUVmax of 9.74, imaging findings that suggested malignancy (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">The patient underwent partial colectomy, being inflammatory myofibroblastic tumor (IMT) the final pathologic diagnosis (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">IMT is a rare neoplasm of mesenchymal origin<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> and unknown etiology. Inflammation following minor trauma or surgery, immune-autoimmune conditions and previous infections have been proposed as potential causes.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,3</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Clinical symptoms are unspecific, being often incidentally detected on imaging studies or during diagnostic evaluation for unexplained fever, weight loss or anemia.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Many of its features can be related to the production of inflammatory mediators such as cytokines and particularly interleukin-1.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,3</span></a><span class="elsevierStyleSup">18</span>F-FDG uptake will depend on the tumor cellularity, the biological behavior of its cells (nuclear atypia and proliferative index), the composition and proportion of inflammatory cells, and the extent of activation of those cells at the time of examination.</p><p id="par0035" class="elsevierStylePara elsevierViewall">IMTs have traditionally been classified as benign lesions,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> even with reports of spontaneous regressions,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> but they can be locally aggressive and present local recurrences, which are more common during the first year (though described up to 9 years after surgery). It has also been reported rare cases of distant metastases<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> and even progression to a true malignant tumor,<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,3</span></a> so it is recommended to consider them as neoplasms with intermediate or low-grade malignant potential,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> being complete resection the treatment of choice.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Anti-inflammatory therapy may be useful before surgery to down-size the tumor or in patients with recurrent or non-resectable tumors.<span class="elsevierStyleSup">18</span>F-FDG PET/CT can be performed to evaluate the tumor and to search for other potential malignant lesions, as well as for treatment evaluation and follow-up,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> which is crucial for early detection of tumor recurrences.</p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "NotaPie" => array:1 [ 0 => array:2 [ "etiqueta" => "☆" "nota" => "<p class="elsevierStyleNotepara" id="npar0005">Please cite this article as: Díaz Silván A, Allende Riera A, Cabello García D, Vilahomat Hernández O, Martínez Gimeno E. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members.
The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process.
In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
