She received two failed renal transplants. In 1996, she was diagnosed with secondary hyperparathyroidism and underwent total parathyroidectomy with auto transplantation into the left sternocleidomastoid muscle. In March 2017, she showed recurrence of hyperparathyroidism with the following biochemical parameters: PTH 1571<span class="elsevierStyleHsp" style=""></span>U/L, serum calcium 2.43<span class="elsevierStyleHsp" style=""></span>mmol/L, phosphate 1.51<span class="elsevierStyleHsp" style=""></span>mmol/L. Parathyroid scintigraphy with 740<span class="elsevierStyleHsp" style=""></span>MBq of <span class="elsevierStyleSup">99m</span>Tc-MIBI was performed, including SPECT/CT of the neck and mediastinum. Early and delayed planar images (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) showed a large focal area of tracer uptake in the upper lateral left neck that was confirmed on SPECT/CT (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). With the suspicion of adenoma/hyperplasia within the auto transplanted parathyroid tissue, a left cervicotomy and exeresis of the cervical mass was performed. The major axis of the lesion measured 4.8<span class="elsevierStyleHsp" style=""></span>cm. Pathological examination confirmed the presence of hyperplasia of the auto transplanted tissue.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Secondary hyperparathyroidism is a major complication associated with CRF that can be controlled with medical treatment in many cases but from 5 to 10% of patients will need surgical treatment.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> Unfortunately, early surgical failure with persistent disease may occur in 5–10% of patients and recurrence reaches 20–30% at 5 years.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> The use of parathyroid scintigraphy with MIBI and the introduction of hybrid techniques (SPECT/CT) that combine anatomical and functional information allows to obtain higher diagnostic accuracy and improves the management of patients with hyperparathyroidism resistant to medical therapy.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">3</span></a></p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "NotaPie" => array:1 [ 0 => array:2 [ "etiqueta" => "☆" "nota" => "<p class="elsevierStyleNotepara" id="npar0005">Please cite this article as: Zeidan Ramón N, García Lucero ME. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
