Abstract
Background and objectives

To assess the feasibility of performing selective parathyroidectomy without intraoperative parathyroid hormone (PTHio) determination when first-line preoperative localization tests (ultrasonography and [99mTc]Tc-MIBI) are negative and/or discordant, and second-line [18F]F-Colina PET-CT, is positive.

Materials and methods

Retrospective cohort study, including patients with negative or discordant ultrasound and MIBI scans and positive [18F]F-Colina PET-CT, who underwent selective parathyroidectomy between 2019 and 2022. Groups were compared based on PTHio determination. Study variables were: gender, mean age, biochemical cure assessed by PTH value (pg/mL) and corrected calcium by albumin (mg/dL) at 6 months post-surgery follow-up, and histopathological analysis.

Results

The final sample included 42 patients. At 6 months post-surgery, in the PTHio group (20 patients), PTH values were 64.50 pg/mL and calcium 9.30 pg/mL, with 19 adenomas and 1 hyperplasia found. In the non-PTHio group (22 patients), PTH values were 61 pg/mL and calcium 9.37 pg/mL, with 22 adenomas found. No statistically significant differences were found between both groups.

Conclusions

Based on the results obtained in our patient cohort, selective parathyroidectomy could be considered with negative or discordant first-line tests and positive [18F]F-Colina PET-CT, without intraoperative PTH determination.

Antecedentes y objetivos

Evaluar si es factible la realización de paratiroidectomía selectiva sin determinación intraoperatoria de paratohormona (PTHio) cuando las pruebas de localización preoperatorias de primera línea (ecografía y [99mTc]Tc-MIBI) son negativas y/o discordantes y se realiza como prueba de segunda línea el PET-TC [18F]F-Colina, siendo éste positivo.

Material y métodos

Estudio de cohorte retrospectivo, donde se incluyen pacientes con ecografía y MIBI negativos o discordantes y PET-TC [18F]F-Colina positivo, intervenidos de paratiroidectomía selectiva entre 2019 y 2022. Se comparan los grupos en función de la determinación de PTHio. Las variables de estudio fueron: género, la edad media y la curación bioquímica mediante el valor de PTH (pg/mL) y la Calcemia corregida por albúmina (mg/dL) en el seguimiento a los 6 meses de la cirugía, y finalmente el análisis anatomopatológico.

Resultados

la muestra final fue de 42 pacientes incluidos. A los 6 meses tras la cirugía, en el grupo con PTHio (20 pacientes) los valores de PTH fueron de 64,50 pg/mL y Calcemia de 9,30 pg/mL, encontrándose 19 adenomas y 1 hiperplasia. En el grupo sin PTHio (22 pacientes) los valores de PTH fueron de 61 pg/mL y de Calcio 9,37 pg/mL, encontrándose 22 adenomas. No se encontraron diferencias estadísticamente significativas entre ambos grupos.

Conclusiones

con los resultados obtenidos en nuestra cohorte de pacientes se podría plantear la paratiroidectomía selectiva con pruebas de primera línea negativas o discordantes y PET-TC [18F]F-Colina positivo sin la determinación de PTH intraoperatoria.
