Glandular external auditory canal tumors represent 0.00025% of all cases at one large referral institution and represented 2.4% of all ear neoplasms.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Ceruminous glands are modified apocrine glands located primarily in the skin lining cartilaginous/membranous portion of the EAC.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> CT and MRI are used to determine the extent of the tumor and involvement of the middle ear, while discharge culture can be added to identify possible infections; and in selected cases, auditory tests are required to determine the type of hearing loss and extent of middle and inner ear involvement. Recurrence, nodal or systemic metastases occur frequently despite adequate surgical resection with negative margins. Recurrence and distant metastasis rates reach 40% and 21%, respectively.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Distant metastasis most common occurs to the lungs and then to the brain and bones, respectively. CT and MRI have been used in the detection of metastatic disease. <span class="elsevierStyleSup">18</span>F-FDG PET/CT imaging is of great importance in observing the various clinical behaviors of different tumors, evaluating the diagnosis and treatment efficiency, determining the long-term prognosis and choosing treatment alternatives. In the literature review, no cases were found in which aggressive ceruminal carcinoma was shown by <span class="elsevierStyleSup">18</span>F-FDG PET/CT. There was also intermittent yellow discharge and progressive hearing loss. Ceruminal carcinoma was reported in the excisional biopsy, performed due to ongoing local complaints after topical antibiotherapy, and <span class="elsevierStyleSup">18</span>F-FDG PET/CT imaging was planned for staging due to pleural nodules in thoracic CT and low back pain newly developed. In addition to moderate <span class="elsevierStyleSup">18</span>F-FDG uptake (SUVmax: 4.8) seen in the primary tumor in the right external auditory canal (1A, 1B, 1C black and white arrows), metastatic involvement was observed with mild <span class="elsevierStyleSup">18</span>F-FDG uptake (SUV max: 3.4) on the local nodulations in the left hemithorax pleura (2A, 2B, 2C black and white arrows). The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
