In addition, some hypermetabolic adenopathies are observed in the right inguinofemoral territory (asterisk).</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">We present the case of a 50-year-old patient receiving chemotherapy for stage IV lung adenocarcinoma who was referred to our center to undergo a control <span class="elsevierStyleSup">18</span>F-FDG positron emission tomography/computerized tomography (PET/CT) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The patient reported intense pain of recent onset in the right gluteus. The cranial vertex image at half of the thighs showed hypermetabolic pulmonary, mediastinal and left cerebral cortex involvement. In addition, soft tissue edema was identified with metabolically active subcutaneous emphysema affecting the perineal and right gluteal regions (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B, C, and D), suggesting the presence of an acute inflammatory/infectious process secondary to active treatment (antibiotics and/or surgery) as the first option. In view of these findings, in-hospital antibiotherapy was initiated and urgent surgical debridement was successfully performed, confirming the diagnosis of perineal necrotizing fasciitis (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>), also known as Fournier’s gangrene.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Fournier’s gangrene is an aggressive anaerobic necrotizing infection of genital and perineal soft tissues, which is associated with chronic comorbidities, peripheral vascular disease and abuse of drugs such as those used in chemotherapy.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> This infection usually presents as disproportionate pain with respect to the external visual cutaneous involvement. Diagnosis by imaging is key to rule out superficial from deep soft tissue involvement, which requires urgent surgical intervention.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a> In the metabolic images, infectious/inflammatory processes, such as that mentioned previously, represent the most frequent cause of false positives for malignancy. Thus, special attention must be given to the characteristics of the deposit of the radiotracer and its morphological correspondence. In addition, on visualization of anomalies in the PET/CT, it is necessary to carry out complementary tests such as selective morphological study (ultrasound, magnetic resonance, among others) or biopsy. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
