The tumor consists of nests and cords of cells with pleomorphic nuclei and high nuclear-to-cytoplasmic ratio (b, 20×).</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">A 72-year-old man was under surveillance for a 2-year history of low-grade prostate cancer with 2 serial prostate biopsies consistently showing Gleason 6 (3<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>3) glandular lesions. He was evaluated for a prostate-specific antigen (PSA) serum level increase from 9<span class="elsevierStyleHsp" style=""></span>ng/ml to 11.4<span class="elsevierStyleHsp" style=""></span>ng/ml. Magnetic resonance (MR) imaging showed an 18<span class="elsevierStyleHsp" style=""></span>mm prostatic lesion abutting the left capsule and an additional lesion of the right uretero-vesical junction (UVJ) with right hydronephrosis and hydroureter (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). A subsequent <span class="elsevierStyleSup">18</span>F-Fluciclovine PET/CT confirmed the MR bladder finding as a hypermetabolic right UVJ nodule, with no obvious focal tracer-avid prostatic lesion (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Cystoscopy with right UVJ tissue sampling established the diagnosis of invasive high-grade urothelial carcinoma, with tumor penetrating the muscularis propria (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). The patient had 4 cycles of chemotherapy for the urothelial carcinoma with positive response on most recent scans. For the prostate cancer, the patient received a 3-month injection of Lupron, initiated 2 months prior to the PET/CT, with PSA serum level decreasing to 0.27<span class="elsevierStyleHsp" style=""></span>ng/ml from 114<span class="elsevierStyleHsp" style=""></span>ng/ml. Cystoprostatectomy has been scheduled for the patient.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Fluciclovine is a synthetic amino acid which functions like a branched chain amino acid such as leucine, isoleucine and valine.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> The branched chain amino acids are transported into cells via the sodium-independent large amino acid transporter, particularly L-type amino acid transporter 1 (LAT-1). The large amino acid transporters are the key route for uptake of essential amino acids into the cell and are normally upregulated in proliferating tissues such as the placenta, brain, and ovary. LAT-1 has been reported with an overexpression in several types of cancer with LAT-1 linked to the angiogenesis, proliferation and metastatic potential.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a><span class="elsevierStyleSup">18</span>F-Fluciclovine has been approved for PET/CT imaging of patients exhibiting biochemical evidence of prostatic cancer recurrence. <span class="elsevierStyleSup">18</span>F-Fluciclovine is also involved in the PET/CT research imaging for breast cancer and gliomas, and has been reported with incidental PET/CT finding of recurrent pancreatic adenocarcinoma and nodal metastasis from penile squamous cell carcinoma.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,3</span></a> The underlying mechanism of <span class="elsevierStyleSup">18</span>F-Fluciclovine avidity for pancreatic cancer, squamous cell carcinoma and, in our presented case, urothelial carcinoma is not well understood, possibly in part related to the LAT-1 overexpression of these malignancies. Since <span class="elsevierStyleSup">18</span>F-Fluciclovine has a very limited renal excretion, the tracer-avid UVJ urothelial carcinoma was well visualized on PET/CT with a high target-to-background activity ratio. To the best of our knowledge, our case is the first reported instance of <span class="elsevierStyleSup">18</span>F-Fluciclovine PET/CT demonstration of urothelial carcinoma. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members.
The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process.
In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
