No papillae can be observed (H&E, original magnification 100×).</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">A 69 year old man, former smoker, had a CT diagnosis of two lung nodules, one of 1 cm in the right lower lobe and one of 3 cm in the middle lobe, both suspected for lung cancer. During the staging exams, a 18-FDG PET-TC was performed, revealing an increased uptake at the level of the sub-pleural nodule with central fibrosis and irregular margins in the lower right lobe, while no uptake was present at the level of the middle lobe nodule.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The case was multidisciplinary discussed, also because the position of the middle lobe nodule was not favourable for bronchoscopy or trans-parietal biopsy. However, the radiological characteristics of the middle lobe nodule (peribronchovascular solid pulmonary nodule with lobulated margins) were also compatible with lung carcinoid, so a 68Ga-DOTATOC was indicated.</p><p id="par0015" class="elsevierStylePara elsevierViewall">68Ga-DOTATOC showed and increased uptake at the level of the middle lobe nodule, while no uptake was detected at the level of the lower lobe nodule, suggesting the carcinoid histology.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Finally, a middle lobectomy and a wedge resection of the right lower lobe were performed, associated with hilo-mediastinal lymphadenectomy. The final pathological exam showed the presence of a predominantly papillary adenocarcinoma at the 3 cm nodule of the middle lobe, while the lower lobe nodule resulted an acinar adenocarcinoma. Lymph nodes were negative from metastases (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">The 68Ga-DOTATOC is specific for tumors expressing somatostatin receptors such as neuroendocrine tumours,<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> while its uptake in lung adenocarcinomas is rare, also if it is possible that a percentage of these tumours may present an increased expression of somatostatin receptors.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">The possible use of 68Ga-DOTATOC for indeterminate pulmonary nodules is an intriguing argument, for the possibility to obtain more information in lung nodules located in unfavourable positions for typing. Indeed, preliminary studies showed that 68Ga-DOTATOC was more specific and less sensitive than 18-FDG PET-TC for malignant nodule detection, suggesting that the association of the two radiotracers may be more efficient for indeterminate nodules characterization (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">This is the first case reported, in our knowledge, regarding the 68Ga-DOTATOC uptake in papillary adenocarcinomas, suggesting the possible use of this radio-tracer for this histological sub-type.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Considering the absence of 18-FDG PET-TC uptake, 68Ga-DOTATOC may be considered also for mediastinal staging to avoid the risk of unexpected nodal metastases that may change the prognosis in these patients. Moreover, this metabolic behaviour in papillary adenocarcinoma may be taken in account when limited resection are planned for suspected peripheral carcinoids, suggesting the intraoperative histologic exam execution for a certain diagnosis that permits to perform the appropriate lung resection and mediastinal lymphadenectomy.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In conclusion, our experience showed an interesting and unique metabolic behaviour in papillary adenocarcinoma, suggesting that 68Ga-DOTATOC may be considered for nodule characterization and staging in this histology when 18-FDG PET-TC is negative.
