The patient received anti-infective therapy, which was ineffective; he was admitted for further evaluation. During the evaluation, he underwent a radionuclide salivagram to evaluate the possibility of aspiration pneumonia. The radiopharmaceutical (2 mCi of <span class="elsevierStyleSup">99</span>Tc<span class="elsevierStyleSup">m</span>-Sulfur colloid) was diluted into 1 mL of normal saline and administered on the tongue (dynamic imaging). Subsequently, planar scan imaging was performed at 1 and 2 h. The 1-h planar scan image (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) most clearly shows that the local radioactivity of the upper esophagus is slightly increased, which extended to the periphery (dashed arrow). The middle thoracic portion of the esophagus had focally increased radioactivity and protruded to the right thoracic cavity (small arrows).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">The patient underwent a fused SPECT/CT of the chest to determine the site of any abnormality (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Notably, the axial SPECT (A), axial CT (B), and axial fused image (C) revealed a fistula (curved arrow) of about 0.5 cm, connecting the upper esophagus and mediastinum (dashed arrow) with radioactive saliva passing through. The mediastinal gas accumulation (dashed arrow) and radioactive saliva were seen locally in the mediastinum on the axial images (D–F). Additionally, the upper esophagus was connected to the adjacent trachea through a fistula (curved arrow) measuring approximately 0.6 cm in diameter, and only the mildly increased radioactivity of the esophageal wall was shown on the axial images (G–I). No fistula was shown to connect the middle esophagus to the right lung tissue on CT. However, a considerable amount of the radioactive saliva had leaked into the dorsal segment of the right lower lobe and had been wrapped by granulation tissue to form a lung abscess (small arrows), stimulating inflammatory changes in the surrounding lung tissue as seen on the axial images (J–L).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Axial SPECT, axial CT and axial fused image (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>) demonstrates slightly elevated radioactivity in the intrabronchial portion of the right middle lobe (arrow). CT shows exudation with patches in the right lung. Gastroscopy performed 2 days later showed that the esophagus was erosive. Multiple fistula-like openings were seen in the lumen. Esophageal wall biopsy confirmed the presence of esophageal squamous cell carcinoma.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">In adults, most acquired esophageal fistulas are associated with neoplastic diseases, the most common being primary esophageal or lung cancer.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> A radionuclide salivagram was commonly used to check for aspiration pneumonia in pediatric patients.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Our case shows that this approach is not only useful for the diagnosis of aspiration pneumonia,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> but also important for the identification of multiple esophageal fistulas. Combined with low dose SPECT/CT, radionuclide salivagram helps to accurately determine the location, morphology, and surrounding tissue of a fistula and can aid in differential diagnosis of aspiration pneumonia.
