In addition, she had progressive speech problems and liquid swallowing difficulty. Physical examination revealed bilateral upper motor neuron (UMN) signs, bilateral hyperreflexia, lower cranial nerves impairment with preserved gag reflex, and no sensory deficits, lingual amyotrophy or fasciculations. Several diagnostics test were performed including brain and cervical MRI and electromyography (EMG) were unremarkable, motor evoked potential showed a mild alteration in corticospinal tract at cervical levels, with no transmission impairment for lower spinal levels. A lumbar puncture was also performed and showed a mild hyperproteinorraquia.</p><p id="par0010" class="elsevierStylePara elsevierViewall">In the case of a patient with gait disorders, multiple differential diagnoses should be taken into account. Among them an important group are parkinsonian syndromes: Parkinson disease, progressive supranuclear palsy, corticobasal syndrome, Lewy body dementia, multiple system atrophy (parkinsonian or cerebellar), normal pressure hydrocephalus, vascular parkinsonism and Alzheimer disease; although they can associate a variable degree of pyramidal signs and swallowing disorders, these are accompanied by bradykinesia, rigidity, disturbance of postural control, accompanied or not by myoclonus, gaze alteration, dysautonomia and/or cognitive impairment.</p><p id="par0015" class="elsevierStylePara elsevierViewall">It is also described the association between frontotemporal dementia and amyotrophic lateral sclerosis (ALS), but the most frequent finding is to observe cognitive deficits in more advanced clinical stages.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Due to physical examination findings and clinical suspicion of motor neuron disease a [<span class="elsevierStyleSup">18</span>F]FDG PET/CT was performed, showing bilateral, predominantly left, primary motor neuron hypometabolism, (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A–C); so the patient was finally diagnosed of primary lateral sclerosis (PLS). Within the appropiate clinical context, hypometabolism over the motor cortex is known as the “stripe sign”.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> This hypometabolic pattern also has been described in ALS, so [<span class="elsevierStyleSup">18</span>F]FDG PET/CT is not able to differentiate between these two diseases.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> The severity and extent of hypometabolism is associated with the severity of symptoms and is a cognitive decline predictor.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Debate as to whether PLS belongs to an ALS spectrum, or is a distinct disorder is ongoing.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">PLS diagnosis is mostly clinical and requires of a minimum age of 25, progressive symptoms and signs of UMN (spasticity, weakness and hyperreflexia) in at least two of the following locations: upper extremities, lower extremities or medulla oblongata.
