F-18 FDG PET/CT revealed intense FDG uptake with a SUVmax of 15.2 in the heterogenous lesion with a diameter of 6 × 5 cm located in the right lobe of the thyroid. Biopsy was directed at the lesion and histopathologic examination of the tissue revealed mucosa-associated lymphoid tissue (MALT) lymphoma. Histopathologic examination of the tumor margin showed diffuse lymphocytic infiltration in the normal thyroid parenchyma, compatible with Hashimoto's thyroiditis. After four cycles of chemotherapy followed by external radiation therapy, a repeat F-18 FDG PET/CT depicted significant metabolic response (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Primary thyroid lymphomas (PTL) accounts for 1%–5% of all thyroid malignancies. Almost all of thyroid lymphomas origin from B cells and MALT lymphoma is a rare form of thyroid lymphomas.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Most of the patients often have a history of chronic thyroiditis and present as an enlarging neck mass that may be associated with dysphagia, hoarseness or dyspnea. Chemotherapy with or without radiation therapy is the choice of treatment while surgery has a limited role. Due to the coexistence of chronic thyroiditis and thyroid MALT lymphomas, FDG PET/CT plays a crucial role in initial staging and follow-up to detect residual/recurrent tumor.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Chronic thyroiditis can also demonstrate diffuse FDG uptake but they are shown to have lower SUVmax values (cutoff value of 14.1) and decreased CT density (cutoff value of 51 HU) compared with PTL. Therefore, these parameters in FDG PET/CT may be useful for differential diagnosis between PTL and chronic thyroiditis.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> F-18 FDG PET/CT is proved to be useful in assessing disease activity in PTL.
