Patient presented with increased PSA level (3.18 ng/mL), with a doubling time of 6 months, and therefore he underwent a <span class="elsevierStyleSup">18</span>F-Choline PET/MRI study, given our availability of a hybrid equipment Signa PET/RM 3T (GE Healthcare).</p><p id="par0010" class="elsevierStylePara elsevierViewall">Prostate PET/MRI imaging showed local recurrence on multiparametric MRI, corresponding to an <span class="elsevierStyleSup">18</span>F-Choline uptake at the vesicourethral junction level (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A and B). In addition, the <span class="elsevierStyleSup">18</span>F-Choline PET image showed a focus of choline kinase activity in the right testis, corresponding to a nodular lesion on MRI (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A and C).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Given this incidental finding, the subsequent clinical examination revealed a non-painful induration of the right testis, for which reason an orchiectomy was decided to perform. Histopathology study revealed a tumour pattern with interstitial infiltration and distortion of the glandular architecture, with the presence of tumour cells with little cytoplasm, irregular nuclei and prominent nucleoli. The immunohistochemical study was negative for Placental Alkaline Phosphatase-PLaP (germinal origin), Leukocyte Common Antigen-LCA (lymphoid origin) and Neuronal Specific Enolase-NSA (neuroendocrine origin), and positive for Prostate Specific Antigen-PSA (prostate origin). Therefore, this incidental finding on the <span class="elsevierStyleSup">18</span>F-Choline PET study was consistent with a testicular metastasis of prostatic origin.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Testicular metastases are very rare, except for malignant infiltration by leukaemia and lymphoma, due to the adverse testis environment, related to the relatively low temperature found in the scrotum. Adenocarcinoma of the prostate is the primary tumour that most frequently spreads to testicular tissue, with other origins being lungs, gastrointestinal tract, melanoma and kidneys. The most common metastases are unilateral and with little symptomatology, so that in most cases they are diagnosed by autopsy (incidence 0.02–2.5%) or after therapeutic orchiectomy (2–4%), especially before hormone analogues were introduced.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a><span class="elsevierStyleSup">–,</span><a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Several mechanisms of dissemination of prostate adenocarcinoma to the testicle have been described: retrograde venous spread from the prostatic urethra (its involvement increases this probability), arterial embolism, lymphatic extension (due to anatomical connections), and endocanalicular spread.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">In our case, the diagnosis of right testicular metastasis therefore affected our patient's prognosis and prompted a change in the therapeutic strategy, with radiotherapy on the testicular bed being performed in addition to radiotherapy on the local recurrence, and normalisation of PSA levels was achieved 6 months after its completion.</p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "NotaPie" => array:1 [ 0 => array:2 [ "etiqueta" => "☆" "nota" => "<p class="elsevierStyleNotepara" id="npar0005">Please cite this article as: Garcia JR, Compte A, Bassa P, Soler M, Buxeda M, Riera E.
