A PET/CT (A-J) performed for primary diagnosis revealed a hypermetabolic nodular lesion in the left lobe of the thyroid gland (SUVmax: 4.6) measuring approximately 3.5 x 3 cm (arrow heads), and increased FDG uptake in the right humerus diaphysis (SUVmax: 3.0) (dotted arrow), in addition to hypermetabolic sacrum lesion (SUVmax:8.2) (thick arrows). Furthermore, a lesion with a ground glass density in the apicoposterior segment of the left lung upper lobe without FDG uptake (thin arrows) was observed. Thyroid cancer was asserted as the primary diagnosis according to PET findings, but a histopathological evaluation revealed a benign thyroid nodule. Because the lung lesion was not regressed and it progressed despite antibiotic treatment, a wedge resection was performed to the patient and she was diagnosed with lepidic type lung adenocarcinoma in the follow-up period. The patient was dead at the time of the analysis, and progression-free and overall survival were 2 months.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Small-cell lung cancer (SCLC) accounts for 15–20% of all lung cancers, and is a fast-growing tumor with a poor prognosis.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> SCLC is staged as a limited disease (LD) and extensive disease (ED) according to the Veterans’ Administration Lung Study Group (VALSG) staging system, based on the extensiveness of disease. LD is defined as a disease that is confined to the ipsilateral hemithorax that can be covered by a single tolerable radiation port, whereas ED is defined as a disease that extends beyond the ipsilateral hemithorax, or accompanied by a malignant pleural effusion.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> The therapy administered to patients with SCLC depends primarily on the tumor stage; although additional prognostic factors, such as performance status and weight loss also guide the therapy to be administered. Fluorine-18 2-fluoro-2-Deoxy-d-glucose (<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>F FDG) positron emission tomography/computerized tomography (PET/CT) is an effective imaging method used in the diagnosis and staging of SCLC, and in evaluating the response to therapy. There is, however, only limited data on the prognostic value of PET/CT in SCLC.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,4–7</span></a> The present study evaluates the relationship between the maximum standardized uptake value (SUVmax), SUVmean, metabolic tumor volume (MTV) and total lesion glycolysis (TLG) values of the primary tumor and progression-free survival (PFS) and overall survival (OS).</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0010" class="elsevierStylePara elsevierViewall">This retrospective study included 63 patients with histopathologically confirmed diagnosis of SCLC who underwent baseline <a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>F FDG PET/CT prior to the initial therapy in the Department of Nuclear Medicine of the Ege University Faculty of Medicine between April 2012 and January 2016. All patients met the following inclusion criteria: 1) having sufficient records on the staging and follow-up, 2) having received at least one course of chemotherapy after a PET/CT scan. The patients were group as those with LD and those with ED, according to the VALSG staging system. The age, gender, pretreatment albumin level and baseline PET parameters related to the primary tumor (SUVmax, SUVmean, MTV, TLG) were recorded in all patients.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Treatment and response evaluation</span><p id="par0015" class="elsevierStylePara elsevierViewall">Patients with LD received combined simultaneous chemotherapy and radiotherapy, whereas those with ED underwent systemic chemotherapy. Cisplatin and etoposide-based chemotherapy regimens were used as the first line therapy in the patients in both groups, and patients with complete or partial response to initial therapy underwent prophylactic cranial irradiation (PCI). Response to therapy was evaluated according to the response evaluation criteria for solid tumors (RECIST 1.1) and positron emission tomography response criteria in solid tumors (PERCIST 1.0) criteria based on the imaging methods.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085"><a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>F FDG PET-CT imaging</span><p id="par0020" class="elsevierStylePara elsevierViewall">The images were captured with Biograph high-definition 16-slice CT Siemens Healthcare® PET/CT device. OSEM iterative reconstruction (3 iteration, 21 subset) method was used (True X, FWHM: 4.0<span class="elsevierStyleHsp" style=""></span>mm, zoom: 1). Intravenously 0.15<span class="elsevierStyleHsp" style=""></span>mCi/kg <a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>F FDG was administered to patients with a blood glucose level of <200<span class="elsevierStyleHsp" style=""></span>mg/dl following a 6<span class="elsevierStyleHsp" style=""></span>h fasting period. Patients were rested in a calm and warm room for 60<span class="elsevierStyleHsp" style=""></span>min after the injection. Muscle relaxants and oral or intravenous contrast were not given to the patients. When 60<span class="elsevierStyleHsp" style=""></span>min have expired, patients emptied the bladder and they were laid in the PET/CT scanner bed. The CT (5-mm slice thickness, 130<span class="elsevierStyleHsp" style=""></span>keV, 120<span class="elsevierStyleHsp" style=""></span>mA) and PET images (1.5<span class="elsevierStyleHsp" style=""></span>min per bed position) between the vertex of the skull and femur were captured and loaded onto a workstation. The CT data was used for attenuation correction. PET/CT images with CT attenuation correction were examined as standard. If necessary, images without attenuation correction were also evaluated.</p><p id="par0025" class="elsevierStylePara elsevierViewall">SUV is the most commonly used parameter for semi-quantitative evaluation of PET images and shows the normalized tumor uptake that based on dispersion volume. It is calculated by dividing the average activity (mCi/ml) within a selected area of interest (ROI) to the dose injected per patient weight (mCi/kg). In the present study the highest standardized uptake value at the primary lung tumor site was determined as the SUVmax. The average SUV within ROI was identified as the SUVmean. Metabolic tumor volume (MTV) was defined as the sum of voxels that showed an SUV of 2.5 or more. SUVmean and MTV were calculated considering 41% of SUVmax as the threshold value. SUVmax, SUVmean and MTV values were obtained with automatic semi quantitative software program. The total lesion glycolysis (TLG) value was calculated manually using the following formula: TLG<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>SUVmeanXMTV.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Patient survival and statistical analysis</span><p id="par0030" class="elsevierStylePara elsevierViewall">OS was defined as the time from the date of diagnosis to the date of death from any cause. PFS was defined as the date of first progression or death from any cause without previous progression. The OS and PFS curves of the patients were created using the Kaplan–Meier method. A log-rank test was used to determine the survival differences between the groups. Age and PET parameters were considered as continuous variables and analyzed using a Cox proportional hazards model. The results of the analysis were evaluated within a 95% confidence interval and a p value of below 0.05 was considered statistically significant.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">The study included a total of 63 patients, 10 of which were female and 53 were male, with a mean age 64.8 years and an age range 43–82 years. Of these patients, 22 had LD and 41 had ED, and 22 had hypoalbuminemia prior to therapy. The mean values of primary tumor SUVmax, SUVmean, MTV and TLG were 17.0<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.11, 9.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.19, 77.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.07<span class="elsevierStyleHsp" style=""></span>mL and 731.1<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.01<span class="elsevierStyleHsp" style=""></span>g, respectively.</p><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Overall survival</span><p id="par0040" class="elsevierStylePara elsevierViewall">Of the total, two patients were alive at the time of analysis and 61 were dead. The median OS was 11<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.6 months. Statistical analysis indicated that disease stage was a prognostic factor affecting OS. The OS was significantly higher in patients with LD than in patients with ED (15<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.9 vs. 10<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.9 months; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). That said, there was no statistically significant relationship between gender, albumin level and age, and the SUVmax, SUVmean, MTV and TLG values related to the primary tumor and OS (all p values >0.05) (<a class="elsevierStyleCrossRefs" href="#tbl0005">Table 1 and 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Progression-free survival</span><p id="par0045" class="elsevierStylePara elsevierViewall">A total of 2 patients were progression-free during the analysis. The median PFS for all patients was 8.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.9 months. The PFS was significantly higher in patients with LD than in patients with ED (10<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.7 vs. 6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.6 months; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.014) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B). Gender, albumin level, age and SUVmax, SUVmean, MTV and TLG values related to the primary tumor did not appear to be a predictive factor for PFS (all p values >0.05) (<a class="elsevierStyleCrossRefs" href="#tbl0005">Table 1 and 2</a>).</p><p id="par0050" class="elsevierStylePara elsevierViewall">Two case samples with similar PET parameters but different survival durations are presented in <a class="elsevierStyleCrossRefs" href="#fig0010">Figs. 2 and 3</a>.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0055" class="elsevierStylePara elsevierViewall">SCLC, which accounts for 15–20% of all patients with lung cancer, is a fast-growing tumor with a poor prognosis.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> The disease initially shows a good response to therapy, but the relapse rate is high.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Disease staging is regarded as the strongest prognostic factor<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> and SCLC is staged as LD or ED according to the VALSG staging system, which is based on the extent of the disease. Patients with an ED account for 60–70% of all patients with SCLC,<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> and 65% of the patients in the present study had ED.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The patients with LD in the present study had a longer OS and PFS. Similar to our findings, various studies have found significantly longer survival in patients with LD than in patients with ED.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11</span></a> The present study found no statistically significant relationship between gender, albumin level and age and OS or PFS. The factors affecting prognosis have been investigated in patients with SCLC<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12–14</span></a> and various studies have shown that young age<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16</span></a> and female gender<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> favorably affect survival. Cohen et al. suggested that pretreatment laboratory parameters could be used as a prognostic factor in patients with SCLC, and found that patients with elevated albumin levels had a higher rate of survival than those with low albumin levels.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Our results that are inconsistent with the literature may be attributable to patients with LD being evaluated together with patients with ED and the lack of a subgroup analysis. Furthermore, the lower number of female patients than male patients in the study may have contributed to the finding of gender being of no prognostic value (10 vs. 53).</p><p id="par0065" class="elsevierStylePara elsevierViewall">Apart from the mentioned prognostic factors, additional prognostic factors that better correlate with the tumor burden are sought in order to determine high-risk patients requiring more aggressive therapy, considering the low survival and high relapse rates in patients with SCLC. FDG uptake by the tumor in PET/CT reflects metabolic glucose activity, and may provide information about the tumor burden. The present study attempted to investigate whether the SUVmax, SUVmean, MTV and TLG values reflecting FDG uptake by the tumor can be used to predict prognosis. However, no statistically significant relationship was identified between the values of SUVmax, SUVmean, MTV and TLG related to the primary tumor and PFS or OS in the present study. There are a limited number of studies in literature investigating the prognostic value of PET parameters in patients with SCLC, with conflicting results. In a study of 82 patients newly diagnosed with SCLC, Kim et al. grouped patients according to SUVmax (8.2) values in a pretreatment PET/CT, and found no significant difference between the groups in terms of OS and PFS.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> In another study involving 106 patients with SCLC, Oh et al. showed that SUVmax had no effect on prognosis, although they also stated that patients with high whole body MTV had poorer prognosis than patients with low whole body MTV.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> In a retrospective study of 55 patients with SCLC, Zer et al.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> divided patients into two groups according to the median value of PET parameters. The authors investigated the prognostic value of PET parameters related to the primary tumor and the hilar-mediastinal lymph nodes together with the primary tumor, if any (expressed in total). The SUVmax and MTV values that are related to the primary tumor and total SUVmax were found to have no effect on OS and PFS. However, it was stated that total TLG could be a prognostic factor for OS and total MTV could be a prognostic factor for PFS. In another study, among patients with SCLC at both LD and ED stage, those with high MTV were considered at higher risk of disease progression/death.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> As mentioned previously, there are studies in literature that support the findings of the present study, while some advocate the opposite. The study by Zer et al. found that MTV related to the primary tumor did not affect prognosis (a similar finding to that of the present study), whereas MTV related to the hilar-mediastinal lymph nodes together with the primary tumor was a prognostic factor for PFS. In the study by Oh et al, the whole body MTV was evaluated. The fact that the present study evaluated only the PET parameters related to the primary tumor may have affected the current findings. Furthermore, the suggested threshold value of 41%<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> was used in the present study while acquiring SUV-derived parameters. However, there is a lack of consensus on the optimal threshold value, and the use of different threshold values in other studies may have also contributed to the different results obtained in the present study. In addition, PET parameters are affected by various patient-related factors, such as blood glucose level, image acquisition and reconstruction parameters, and time between tracer injection and imaging.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> Furthermore, SCLC falls within the group of pulmonary neuroendocrine tumors (NET), which are a heterogeneous group of tumors with variable degrees of differentiation.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> The benefits of FDG PET in the diagnosis of NETs depend on the degree of differentiation of the tumor and its biological aggressiveness.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,22</span></a> In the present study, however, we had no access to data indicating the degree of differentiation of the primary tumor.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Among the limitations of the present study are the relatively small sample size and the retrospective design.
