In general&#44; probably because of its tendency to present at an early stage&#44; endometrial carcinoma has a good prognosis&#46;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The initial treatment of endometrial carcinoma patients involves surgery and the surgical staging&#46; The post-treatment surveillance of patients with endometrial carcinoma includes physical examination&#44; conventional imaging &#40;CI&#41; procedures and post-treatment serum levels of CA 125&#46; Post-treatment serum level of CA 125 has been shown to be a useful indicator of recurrence&#46; However&#44; elevated tumour markers are known to be associated with both benign and malignant conditions&#46;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Moreover&#44; the site of recurrence cannot be demonstrated with tumour markers&#46; Positron emission tomography&#47;computed tomography &#40;PET&#47;CT&#41; as a new imaging modality has the advantage of imaging the entire body in a single study&#46; PET and PET&#47;CT with fluorodeoxyglucose &#40;FDG&#41;&#44; have been widely used for follow-up of various cancer patients&#46; However&#44; there have been few reports to date establishing their value in the management of post-treatment endometrial carcinoma patients&#46;<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3&#8211;11</span></a> To best of our knowledge&#44; there have been no reports on comparison of PET&#47;CT with CA 125 as a serum tumour marker for different cut-off values&#46; This retrospective study was designed to assess the value of FDG-PET&#47;CT imaging in the post-treatment evaluation of patients with endometrial carcinoma and to compare PET&#47;CT scan with CI including computed tomography &#40;CT&#41;&#44; ultrasonography &#40;US&#41; and magnetic resonance imaging &#40;MRI&#41; and CA 125 with both 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml and 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml cut-off values&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Patients</span><p id="par0010" class="elsevierStylePara elsevierViewall">Between April 2008 and August 2010&#44; a total of 31 women &#40;age range&#44; 48&#8211;69 years&#59; mean age&#44; 61 years&#41; who had primary cytoreductive surgery for histopathologically proven endometrial adenocarcinoma&#44; underwent FDG-PET&#47;CT examination for restaging and suspected recurrence&#46; The clinical characteristics of patients are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#46; Two women had two PET&#47;CT studies&#44; whereas one patient had three studies&#46; Thirty five PET&#47;CT studies were performed in 31 women with previously treated endometrial carcinoma&#46; Chemotherapy &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>3&#41;&#44; radiation therapy &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>8&#41; or concurrent chemoradiation &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>6&#41; were the adjuvant treatments after surgery&#46; Recurrence was suspected on the basis of elevated tumour marker level &#40;CA 125<span class="elsevierStyleHsp" style=""></span>&#62;<span class="elsevierStyleHsp" style=""></span>35<span class="elsevierStyleHsp" style=""></span>U&#47;ml&#41; in one study&#44; because of abnormal findings on CI in six studies and because of both elevated tumour markers and findings on CI in two studies&#46; Another 26 studies were performed because of clinical suspicion of recurrence in symptomatic patients&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Imaging protocol</span><p id="par0015" class="elsevierStylePara elsevierViewall">All patients were fasted for 4<span class="elsevierStyleHsp" style=""></span>h before 370 MBq &#40;10<span class="elsevierStyleHsp" style=""></span>mCi&#41; FDG injection&#46; Patients were kept at rest in a silent room and voided just before starting the acquisition to avoid urinary tract artifacts&#46; Urinary bladder catheterization was not used&#46; PET&#47;CT scans were obtained 60<span class="elsevierStyleHsp" style=""></span>min after injection using an integrated scanner &#40;Biograph&#44; Siemens&#41;&#46; Whole-body CT was performed without intravenous contrast administiration with 130<span class="elsevierStyleHsp" style=""></span>kV&#44; 50<span class="elsevierStyleHsp" style=""></span>mAs&#44; a pitch of 1&#46;5&#44; a section thickness of 5<span class="elsevierStyleHsp" style=""></span>mm and a field of view 70<span class="elsevierStyleHsp" style=""></span>cm&#46; PET scan was performed immediately after unenhanced CT&#44; and acquired from the skull base to the upper thigh&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Image analysis</span><p id="par0020" class="elsevierStylePara elsevierViewall">All lesions with increased FDG uptake detected by PET&#47;CT imaging were recorded&#46; PET&#47;CT scans were reported as negative when no areas of abnormal FDG uptake were seen&#46; A site of increased FDG uptake was defined as benign&#44; and unrelated to cancer&#44; when it was located in an area of known physiological uptake of the tracer or in a known non-malignant process such as active pneumonia&#47;pneumonitis&#44; post-surgical changes&#44; traumatic injury&#44; inflammatory&#47;reactive lymph nodes&#46; When a focal FDG uptake&#44; with higher intensity than that of the surrounding tissues&#44; in areas unrelated to physiological or benign processes&#44; it was defined as malignant and grouped as belove or above the diaphragm &#40;<a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a>&#41;&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">All studies were interpreted retrospectively and were reviewed with knowledge of the patient&#39;s clinical history&#46; Conventional imaging &#40;CI&#41; including computed tomography &#40;CT&#41;&#44; ultrasonography &#40;US&#41; and magnetic resonance imaging &#40;MRI&#41; were used for comparison with PET&#47;CT scan&#46; Lesion status was determined on the basis of clinical follow-up including radiological imaging &#40;follow-up CT scans&#41; at least 6 months and response to therapy&#46; The results of previous imaging studies from the time of initial diagnosis were also included&#46; In this study&#44; a lesion was considered benign if no change was seen on previous or CT scans after a minimum 6 months of follow-up&#46; A lesion was considered malignant if follow-up CT scans showed a 30&#37; decrease in longest diameter &#40;partially response&#41; or disappearance &#40;complete response&#41; after chemoradiotherapy or a 20&#37; increase in longest diameter &#40;progressive disease&#41; on follow-up CT imaging&#46; These criteria were based on response evaluation criteria in solid tumours &#40;RECIST&#41; criteria&#46;<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12&#44;13</span></a> When a patient had follow-up PET&#47;CT imaging&#44; definition of FDG tumour response was based on EORTC 1999 criteria&#46;<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> Appearance of new FDG uptake in metastatic lesions or visiable increase in the extent of FDG tumour uptake &#40;&#62;20&#37; in the longest dimension&#41; or an increase in FDG tumour SUV of greater than 25&#37; within the tumour region defined on the baseline scan was classified as progressive metabolic disease&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">A CA 125 cut-off value of 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml and 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml as the threshold for recurrence were used according to previous reports&#46;<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15&#44;16</span></a></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">The sensitivity&#44; specificity&#44; accuracy&#44; and positive and negative predictive values were calculated both on the basis of study and lesion analysis according to the classical definitions&#46;</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall">A total of 31 patients were included in the study&#46; All 31 patients had histopathologically proven malignancies&#46; The mean patient age was 61 &#40;age range&#59; 48&#8211;69&#41; years&#46; Thirty five PET&#47;CT studies were performed in 31 women with endometrial adenocarcinoma&#46; Recurrent disease was observed in 13 &#40;37&#37;&#41; studies&#44; whereas 22 &#40;63&#37;&#41; of the 35 FDG-PET&#47;CT studies were negative&#46; On study-based analysis the overall sensitivity&#44; specificity&#44; accuracy of FDG-PET&#47;CT imaging were 100&#37;&#44; 96&#37; and 97&#37; respectively&#44; whereas the corresponding information for CI were 46&#37;&#44; 87&#37; and 74&#37;&#46; A CA 125 threshold of 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml corresponded to a sensitivity of 45&#37;&#44; specificity of 88&#37;&#44; accuracy of 74&#37;&#46; A CA 125 cut-off value of 35 U&#47;ml corresponded to a sensitivity of 27&#37;&#44; specificity of 100&#37;&#44; accuracy of 78&#37;&#44; respectively &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#41;&#46; Measurements of CA 125 were 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml or less in 86&#37; &#40;19&#47;22&#41; of PET negative cases&#44; whereas the same value were &#62;20<span class="elsevierStyleHsp" style=""></span>U&#47;ml&#44; in 14&#37; &#40;3&#47;22&#41; of PET negative cases&#46; There was no patient with a measurement of CA 125 of &#62;35<span class="elsevierStyleHsp" style=""></span>U&#47;ml in PET negative cases&#46; The total number of both benign and metastatic lesions was 74 in 35 PET&#47;CT studies&#46; Of the 74 sites&#44; 42 sites were metastatic&#46; PET&#47;CT accurately localized the site of metastasis below the diaphragma in 24&#37; &#40;10&#47;42&#41; and above the diaphragma 76&#37; &#40;32&#47;42&#41;&#46; <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> reveals a comparison of PET&#47;CT and CI on lesion-based analysis&#46; On lesion-based analysis the overall sensitivity&#44; specificity&#44; and accuracy of FDG-PET&#47;CT imaging were 100&#37;&#44; 97&#37; and 99&#37; respectively&#44; whereas the corresponding information for CI were 60&#37;&#44; 75&#37; and 66&#37;&#46; On lesion based analysis&#44; CI had 17 false negative and 8 false positive lesions&#46; Of the 17 false negative lesions 13 were lymph nodes with &#60;1<span class="elsevierStyleHsp" style=""></span>cm in diameter &#40;2 supraclavicular&#44; 3 paraaortic and pelvic and 8 mediastinal and hilar&#41;&#46; Two peritoneal metastasis&#44; 1 liver and 1 spleen metastasis were not reported and seen on CI&#46; Of the 8 false positive results&#44; 4 were lymph nodes with &#62;1<span class="elsevierStyleHsp" style=""></span>cm diameter &#40;2 mediastinal and hilar&#44; 2 inguinal&#41;&#46; One liver&#44; 1 spleen and 1 lung lesion due to benign conditions were reported as distant metastases and 1 pelvic lesion due to post-radiotherapy chances was falsely reported as pelvic recurrence&#46; On lesion-based analysis&#44; FDG-PET&#47;CT revealed only one false positive case which was a patient with physiological asimetric FDG uptake in the epiglottis that was confirmed not to be a second malignancy by MRI imaging&#46; Because of the asymmetric FDG uptake in the epiglottis&#44; this was suspected as a second malignancy on PET&#47;CT imaging&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">The overall lesion-based site-by-site results were shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>&#46; In none of 21 patients with negative 22 PET&#47;CT studies&#44; no subsequent clinical or radiological recurrences were observed with a follow-up of at least 6 months&#46;</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Discussion</span><p id="par0050" class="elsevierStylePara elsevierViewall">The clinical role of PET&#47;CT in the management of post-treatment endometrial carcinoma patients is relatively less described&#46; Several authors have reported the clinical diagnostic accuracy of integrated PET&#47;CT for post-therapy surveillance of patients with endometrial carcinoma&#46;<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6&#8211;11</span></a> Chung et al&#46;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> evaluated the accuracy of PET&#47;CT in 31 post-treatment endometrial carcinoma women for suspected recurrence and found high sensitivity &#40;100&#37;&#41;&#44; specificity &#40;94&#46;7&#37;&#41; and accuracy &#40;92&#46;3&#37;&#41;&#46; In a study evaluating the accuracy of integrated PET&#47;CT by Kitajima et al&#46;&#44;<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> FDG-PET&#47;contrast-enhanced CT is found more accurate modality for assessing recurrence of uterine cancer&#44; and led to more appropriate clinical management than that of PET alone or enhanced CT&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">To best of our knowledge&#44; this is the first study on a comparison of FDG-PET&#47;CT imaging with measurements of CA 125 as a tumour marker both 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml and 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml cut-off values in post-treatment evaluation of patients with endometrial adenocarcinoma&#46; The value 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml for serum CA 125 level has been employed as a clinical cut-off level for most gynecological diseases including endometrial carcinoma&#46; In a retrospective cohort study by Sebastianelli et al&#46; including 254 patients who underwent hysterectomy and full staging for endometrial cancer&#44; a statistically higher number of patients from the stage III or IV group had a serum CA 125 level &#8805;35<span class="elsevierStyleHsp" style=""></span>kU&#47;l &#40;58&#37;&#41; compared with the stage I or II group &#40;16&#37;&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> In a review by PG Rose et al&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> serial CA 125 levels were elevated &#40;greater than 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml&#41; in 19 of 33 patients &#40;58&#37;&#41; with recurrent disease&#46; The authors concluded that the use of serial CA 125 assays is most beneficial in diagnosing recurrence in a high-risk population&#44; including patients with papillary serous carcinomas&#46; In a study including 112 women with endometrial carcinoma by Cherchi et al&#46;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> during the follow-up the most reliable marker was found to be CA 125&#58; values more than 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml of this marker resulted positive in 50&#37; of relapsed cases and only in 5&#46;1&#37; of disease-free cases&#44; thus demonstrating a high specificity&#46; This view is mostly consistent with the present study&#46; Because in the present study&#44; a CA 125 cut-off value of 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml has been shown to have 100&#37; specificity&#46; Although&#44; according to the findings in the current study&#44; we found lesser sensitivity &#40;27&#37;&#41; with CA 125 cut-off value of 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml&#46; In a study by Alagoz et al&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> CA 125 values were found 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml or less in 95&#37; of disease free patient who had undergone surgical treatment of endometrial carcinoma&#46; The authors suggested that 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml could detect early relaps of endometrial carcinoma&#46; Similarly&#44; in another report by Kurihara et al&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> a CA 125 level of 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml reported to be potential indicator of the recurrence of endometrial carcinoma&#46; This study has shown that 97&#37; of measurements of CA 125 were 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml and less in disease-free patients who had undergone surgical treatment of endometrial carcinoma&#46; Starting from this point&#44; we decided to compare FDG-PET&#47;CT findings with CA 125 values both for 20 and 35<span class="elsevierStyleHsp" style=""></span>U&#47;ml cut-off values&#46; Measurements of CA 125 were 20<span class="elsevierStyleHsp" style=""></span>U&#47;ml or less in 86&#37; &#40;19&#47;22&#41; of PET negative cases&#44; whereas the same value were &#62;20<span class="elsevierStyleHsp" style=""></span>U&#47;ml in 14&#37; &#40;3&#47;22&#41; of PET negative cases&#46; There was no patient with a measurement of CA 125 of &#62;35<span class="elsevierStyleHsp" style=""></span>U&#47;ml between PET negative cases&#46; All patients with CA 125 &#62;35<span class="elsevierStyleHsp" style=""></span>U&#47;ml were PET&#47;CT positive&#46; An elevated serum level of CA 125 after initial treatment has been shown to be highly specific for both cut-off values in the determination of recurrences in endometrial carcinoma patients in accordance with the literature&#46; However&#44; an additional imaging method for identifiying recurrent area and&#47;or areas is needed after tumour marker elevation&#46; Moreover&#44; sensitivities were found to be less for both cut-off values&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">Although sensitivity of PET&#47;CT was found 100&#37; in this study&#44; a 100&#37; reported PET&#47;CT sensitivity in itself has to be questioned&#46; Certainly&#44; it is not likely for this study in the absence of surgical proof to identify lesions&#44; which are not seen on PET imaging&#46; Thus&#44; PET is the de facto gold standard&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">Our study had some limitations&#46; First&#44; it was a retrospective study&#46; Second we used clinical follow-up and response to therapy in all cases&#46; Histological confirmation of all findings of course is the most reliable method for analysis&#46; However&#44; investigating all PET&#47;CT positive lesions with biopsy would be no doubt very invasive&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">In conclusion&#44;
