            "en" => "<p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Patient who had undergone total thyroidectomy for follicular carcinoma with a trabecular oncocytic variant with foci of clear cells&#46; Ablative therapy with 100<span class="elsevierStyleHsp" style=""></span>mCi <span class="elsevierStyleSup">131</span>I&#46; The patient later underwent surgery for single costal metastasis &#40;6th left costal arch&#41; and was treated with a new therapeutic dose of 180<span class="elsevierStyleHsp" style=""></span>mCi of <span class="elsevierStyleSup">131</span>I&#46; Progressive increase in Tg values &#40;28&#46;04<span class="elsevierStyleHsp" style=""></span>ng&#47;ml&#41; with negative <span class="elsevierStyleSup">131</span>I whole body scan &#40;A&#41;&#46; A whole body <span class="elsevierStyleSup">18</span>F-FDG PET&#47;CT scan was performed in April 2014 &#40;B&#41; showing hypermetabolic foci at the level of the sternum&#44; 2nd and 10th left costal arches and left ischiopubian branch suggestive of new bone metastasis&#44; and treatment with tyrosin kinase inhibitors &#40;sorafenib&#41; was initiated&#46; Sequential studies at 6 months &#40;October 2014&#41; and at 14 months &#40;June 2015&#41; after the initiation of therapy demonstrate the persistence of the hypermetabolic foci in the axial skeleton &#40;sternal body&#44; 2nd and 10th left costal arches&#44; ischiopubian branch&#41; with a similar size and intensity of uptake&#46; In the remainder of the study no hypermetabolic foci were observed suggesting functional stabilization of the disease&#46; A new control <span class="elsevierStyleSup">18</span>F-FDG PET&#47;CT study performed at 19 months after the beginning of therapy &#40;December 2015&#41; showed 3 foci of uptake in the nodular images in both pulmonary fields suggesting disease progression&#46;</p>"

Therefore&#44; several studies have evaluated the radiation dose to which the public is exposed due to contact with patients undergoing diagnostic studies involving conventional nuclear medicine procedures<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">1&#44;2</span></a> as well as positron emission tomography &#40;PET&#41;&#46;<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">3&#44;4</span></a> In addition&#44; the possible radiological impact of high levels of radiation received during therapy have led to studies on the radiation dose received by third parties in contact with patients treated with radiopharmaceuticals&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a> National practical guidelines have been established with the aim of unifying the application of basic international recommendations for radiological discharge of patients treated with radiopharmaceuticals&#46;<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> On the other hand&#44; some studies have assessed the dose received by technical personnel during diagnostic procedures including PET&#46;<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">7&#44;8</span></a> Technical staff are not considered members of the public since these professionals are exposed to ionizing radiation&#46; However&#44; little information is available on the radiation dose produced by patients during the period of radiopharmaceutical incorporation&#46;<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">9</span></a> Estimation of the dose and the recommendations derived thereof should be made based on worst-case scenarios&#44; that is&#44; those in which the dose rates are highest&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">Nuclear medicine departments have rooms in which the patients remain from the time the radiopharmaceutical is administered until the imaging study is performed&#46;<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">10&#44;11</span></a> This so-called waiting room for patients who have been administered the radiopharmaceutical is designed according to the ALARA principle which establishes that the dose should be as low as reasonably possible&#46; While the patient is in this room other patients will be exposed to radiation but not members of the public not related to nuclear medicine procedures&#46; When the diagnostic procedure is completed and the patient leaves the nuclear medicine department there continues to be a risk of radiation exposure&#44; and yet there are no norms for radiological protection&#46; Taking this into account it would be of interest to know the radiation dose received by patients on being in the waiting room and compare the dose received by members of the public if the patients do not remain in the waiting room during incorporation of the radiopharmaceutical and the dose the public receives when the contact is made upon the patient having completed the diagnostic test&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to evaluate the radiation dose members of the public can receive from contact with nuclear medicine patients during the radiopharmaceutical incorporation and compare this dose with that produced by patients after having completed the diagnostic study in order to establish recommendations taking into account that current legislation requires that the public should not receive doses greater than 1<span class="elsevierStyleHsp" style=""></span>mSv over one year&#46;<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">12</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients and diagnostic procedures</span><p id="par0020" class="elsevierStylePara elsevierViewall">We prospectively studied 194 patients attending the Department of Nuclear Medicine for any of the following diagnostic procedures&#58; bone scintigraphy &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>145&#41;&#44; parathyroid scintigraphy &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>4&#41;&#44; ventilation study&#47;pulmonary perfusion &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>6&#41;&#44; glomerular filtration &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>3&#41;&#44; scintigraphy of the salivary glands &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>2&#41;&#44; myocardial perfusion study &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>2&#41;&#44; sentinel lymph node detection &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>12&#41;&#44; scan with <span class="elsevierStyleSup">67</span>Ga &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>2&#41;&#44; somatostatin receptor scintigraphy with <span class="elsevierStyleSup">111</span>In &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>2&#41;&#44; isotopic lymphography &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>5&#41; and leukocytes labeled with <span class="elsevierStyleSup">99m</span>Tc &#40;<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>3&#41;&#46; The following data of each patient were collected&#58; gender&#44; age&#44; body mass and height as well as the radioactivity of the radiopharmaceutical and the time of administration&#46;</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Measurement of the dose rate</span><p id="par0025" class="elsevierStylePara elsevierViewall">A MiniTRACE ¿ Geiger-M&#252;ller detector &#40;Genitron Instruments&#44; Frankfurt&#44; Germany&#41; was used to determine the dose rate measuring the ambient dose equivalent <span class="elsevierStyleItalic">H</span>&#42;&#40;10&#41;&#46; Since the equipment is calibrated with a source of <span class="elsevierStyleSup">137</span>Cs&#44; the calibration factor for <span class="elsevierStyleSup">99m</span>Tc was determined using the gamma constant to measure the ambient dose equivalent&#46;<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">13</span></a> Thirty measurements of a point source of 750<span class="elsevierStyleHsp" style=""></span>MBq of <span class="elsevierStyleSup">99m</span>Tc in air were made&#46; The measurements showed a statistical variability of 3&#37;&#44; and the calibration factor was 1&#46;28&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">On the other hand&#44; to characterize the angle response of the measuring equipment a series of 30 measurements were made varying the angle position between the detector and the source &#40;0&#176;&#44; &#177;30&#176;&#44; &#177;45&#176;&#44; &#177;60&#176; and &#177;90&#176;&#41;&#46; The results obtained showed that the maximum difference of the dose rate measured with respect to the central position &#40;0&#176;&#41; was less than 10&#37;&#46; This factor was not taken into account because the distribution of radioactivity of the patient and thus&#44; the incidence of the photon angle in the detector was unknown&#46;</p><p id="par0035" class="elsevierStylePara elsevierViewall">In general the dose rate of each patient was measured after administration of the radiopharmaceutical prior to the imaging study &#40;pre-miction&#41; and when the patient left the Nuclear Medicine Department&#46; The dose rate was measured at the level of the sternon and in an orthostatic position at 0&#46;1&#44; 0&#46;5 and 1<span class="elsevierStyleHsp" style=""></span>m of distance&#46;<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">2&#44;5&#44;8&#44;14</span></a> Likewise&#44; background radiation was also measured and the time of each measurement was registered&#46; Lastly&#44; the dose rate was calculated by unit of radioactivity administered&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">Based on the acquisition protocol of the diagnostic test and the time of incorporation of the radiopharmaceutical&#44; different sets of measurement were performed&#46; <a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a> shows the time sequence of the different measures for each study together with the time of incorporation of the radiopharmaceutical and the time of image processing&#46; The values presented are those used in our department which have been adapted from the Serena Puig and Campos Villarino protocols&#46;<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">15</span></a></p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">On the other hand&#44; we estimated the time of incorporation of the radiopharmaceutical &#40;time from the time of administration of the radiopharmaceutical to the beginning of the imaging study&#41; and the total time the patient remained in the Nuclear Medicine Department&#46; This total time included the incorporation of the radiopharmaceutical&#44; the time the patient was in the washroom and the dressing room&#44; correct positioning in the equipment&#44; image acquisition and the waiting time until the physician decided that no further studies were needed and the patient could leave&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Estimation of the effective dose</span><p id="par0050" class="elsevierStylePara elsevierViewall">We calculated the radiation dose to which determined subjects such as a relative&#44; or social contact or co-worker might be exposed on contact with a patient undergoing a nuclear medicine diagnostic test&#46; We determined the dose produced from the measured dose rates and the time of permanence&#44; considering only radioactive decay without taking into account the biological elimination of the radiopharmaceutical&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">To do this we first obtained the ambient dose equivalent using the law of radioactive decay&#58;<elsevierMultimedia ident="eq0005"></elsevierMultimedia>where <span class="elsevierStyleItalic">t</span><span class="elsevierStyleInf">1</span> and <span class="elsevierStyleItalic">t</span><span class="elsevierStyleInf">2</span> are&#44; respectively&#44; the times at which the contact begins and finishes&#44; <span class="elsevierStyleItalic">T</span> is the period of semidecay of the radiopharmaceutical&#44; and D&#729;0 is the dose rate measured at the corresponding time point&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">Finally&#44; the magnitude of the ambient dose equivalent was converted into the effective dose from the coefficients shown in table A2 of the ICRU 47<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">16</span></a> &#40;conversion of ambient dose equivalent to kerma in air&#41; and the article by Zankl et al&#46;<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">17</span></a> &#40;conversion of kerma in air to an effective dose&#41;&#46; For energies of 80 and 300<span class="elsevierStyleHsp" style=""></span>keV the conversion factor of the ambient equivalent dose to the effective dose varies from 0&#46;83 to 0&#46;85&#46; Since this factor is lower for lower energies&#44; from a conservative point of view a factor of 0&#46;84 of effective dose&#47;ambient equivalent dose was used&#46; Several suppositions were made in relation to the different circumstances of contact which may be produced&#44; selecting the adequate distances and time for each situation&#46; In addition&#44; it was taken into account that this contact may take place immediately or a certain time after the test&#46; Therefore&#44; the doses were calculated immediately and applying delay times&#58; 1&#44; 2 and 3<span class="elsevierStyleHsp" style=""></span>h in the case of bone scintigraphies and 1<span class="elsevierStyleHsp" style=""></span>h for the remaining diagnostic tests&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">The following conditions were considered during the incorporation of the radiopharmaceutical&#58;<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">&#8226;</span><p id="par0070" class="elsevierStylePara elsevierViewall">Nuclear Medicine Department waiting room&#58; injected patient in the waiting room who is receiving radiation from other patients waiting in the same room who have also been administered ionizing radiation&#46; The contact times corresponded to the incorporation of the radiation dose&#46; In addition&#44; times of 1&#44; 2&#44; 3&#44; and 4<span class="elsevierStyleHsp" style=""></span>h were considered for bone scintigraphies&#46;<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> All considered distances of 0&#46;1&#44; 0&#46;5 and 1<span class="elsevierStyleHsp" style=""></span>m&#46;</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">&#8226;</span><p id="par0075" class="elsevierStylePara elsevierViewall">Accompanying person&#58; adult accompanying the patient during the study&#46; It is assumed that this person remained with the patient at a distance of 0&#46;5<span class="elsevierStyleHsp" style=""></span>m during a time equal to the incorporation period of the radiopharmaceutical&#46; The waiting time used corresponded to the incorporation time determined for each patient&#46;</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">&#8226;</span><p id="par0080" class="elsevierStylePara elsevierViewall">Coffee shop and restaurant&#58; users of coffee shops &#40;during 30<span class="elsevierStyleHsp" style=""></span>min&#44; at 0&#46;5<span class="elsevierStyleHsp" style=""></span>m&#41; and restaurants &#40;during 1<span class="elsevierStyleHsp" style=""></span>h at 0&#46;5<span class="elsevierStyleHsp" style=""></span>m&#41;&#46;</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">&#8226;</span><p id="par0085" class="elsevierStylePara elsevierViewall">Medical visit&#58; physician attends a patient who has undergone a nuclear medicine study&#46; The patient is considered to stay a total of 20<span class="elsevierStyleHsp" style=""></span>min in the consulting office with the following distances and contact times&#58; distance of 1<span class="elsevierStyleHsp" style=""></span>m during the first 5<span class="elsevierStyleHsp" style=""></span>min&#44; 0&#46;5<span class="elsevierStyleHsp" style=""></span>m during the following 10<span class="elsevierStyleHsp" style=""></span>min and 0&#46;1<span class="elsevierStyleHsp" style=""></span>m the last 5<span class="elsevierStyleHsp" style=""></span>min&#46;</p></li></ul></p><p id="par0090" class="elsevierStylePara elsevierViewall">Apart from the medical office&#44; coffee shop and restaurant the following scenarios were also evaluated on completion of the diagnostic test in the Nuclear Medicine Department&#58;<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">&#8226;</span><p id="par0095" class="elsevierStylePara elsevierViewall">Transportation&#58; users of public transport &#40;during 1<span class="elsevierStyleHsp" style=""></span>h at 0&#46;1<span class="elsevierStyleHsp" style=""></span>m&#41; and private vehicle &#40;during 1<span class="elsevierStyleHsp" style=""></span>h at 1<span class="elsevierStyleHsp" style=""></span>m&#41;&#46;<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">3&#44;5</span></a></p></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">&#8226;</span><p id="par0100" class="elsevierStylePara elsevierViewall">Work place&#58; co-workers&#46; A maximum period of 8<span class="elsevierStyleHsp" style=""></span>h of contact at a distance of 1<span class="elsevierStyleHsp" style=""></span>m was assumed&#46;<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">3&#44;5</span></a></p></li><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">&#8226;</span><p id="par0105" class="elsevierStylePara elsevierViewall">Family&#58; adult living with the patient&#46; It was assumed that the patient is with this person during the first 6<span class="elsevierStyleHsp" style=""></span>h at a distance of 1<span class="elsevierStyleHsp" style=""></span>m and during the following 8<span class="elsevierStyleHsp" style=""></span>h &#40;sleeping&#41; at 0&#46;1<span class="elsevierStyleHsp" style=""></span>m&#46;<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">3&#44;5</span></a></p></li></ul></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analyses</span><p id="par0110" class="elsevierStylePara elsevierViewall">Taking into account that in general the variables studied do not have a normal distribution &#40;Shapiro&#8211;Wilk test&#41; the median&#44; maximum&#44; and percentiles were used to express the variables&#46; Specifically&#44; the results of the characteristics of the patients were expressed as median and range while in the case of bone scintigraphies<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a> the dosimetric results obtained were evaluated with the 95th percentile&#46; The maximum value<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">2</span></a> was used in the remaining imaging studies due to the small size of the sample&#46; The statistical analyses were performed using the SPSS Statistics 20 &#40;IBM&#41;&#46;</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0115" class="elsevierStylePara elsevierViewall">A total of 194 patients were studied&#44; 190 of whom were finally included &#40;124 women and 66 men&#41;&#46; Patients with measurements demonstrating any type of error were excluded due to a lack of concordance &#40;3 bone scintigraphies and one isotopic lymphography&#41;&#46;</p><p id="par0120" class="elsevierStylePara elsevierViewall">The characteristics of age&#44; body mass and height of the patients studied &#91;median &#40;range&#41;&#93; were&#58; 58 &#40;11&#8211;93&#41; years&#44; 69&#46;5 &#40;34&#8211;125&#41;<span class="elsevierStyleHsp" style=""></span>kg and 164 &#40;147&#8211;188&#41;<span class="elsevierStyleHsp" style=""></span>cm&#44; respectively&#46;</p><p id="par0125" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0010">Fig&#46; 2</a> shows the distribution of the radioactivity administered in each diagnostic test&#46; The radioactivity administered varied greatly according to the diagnostic procedure&#44; with the maximum activity corresponding to cardiac studies with a median of 1460<span class="elsevierStyleHsp" style=""></span>MBq and 744<span class="elsevierStyleHsp" style=""></span>MBq for bone scintigraphies&#46; <a class="elsevierStyleCrossRefs" href="#fig0015">Figs&#46; 3 and 4</a> show the time the patients were in the waiting room during incorporation of the radiopharmaceutical and the total time they remained in the Nuclear Medicine Department for bone scintigraphies and the remaining studies&#44; respectively&#46; The medians of incorporation time and the total study time for the bone studies were 160 and 220<span class="elsevierStyleHsp" style=""></span>min&#46;</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the dose rates measured at 0&#46;1&#44; 0&#46;5 and 1<span class="elsevierStyleHsp" style=""></span>m after the administration of the radiopharmaceutical&#44; before the imaging test and when the patient left the Nuclear Medicine Department&#46; The 95th percentile of the bone scintigraphies and the rate of maximum dose for the remaining diagnostic studies are shown&#46; In addition&#44; the dose rate per unit of radioactivity administered is shown in parenthesis&#46; Upon leaving the Nuclear Medicine Department the maximum dose rates at 0&#46;1&#44; 0&#46;5 and 1<span class="elsevierStyleHsp" style=""></span>m were 258&#46;4&#59; 44&#46;5 and 18&#46;6<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#47;h&#44; for the myocardial perfusion studies&#46; However&#44; the highest dose rate values normalized to the radioactivity administered were found for the sentinel lymph node tests&#46; <a class="elsevierStyleCrossRef" href="#fig0025">Fig&#46; 5</a> shows the distribution of the dose rates determined in patients undergoing bone scintigraphy for each of the distances and measurement time points&#46; The reduction in the dose rate with distance was more marked after the administration of the radiopharmaceutical than at the end of the test&#46; In some diagnostic studies measurements of the dose rates were not performed after radiopharmaceutical administration or before image acquisition because they were either dynamic studies which were initiated simultaneously after the administration of the radiopharmaceutical &#40;renal studies&#41; or the time until the image was very short&#46; Neither were measurements made if the patient remained in the administration room and went to the imaging room &#40;thyroid <span class="elsevierStyleSup">99m</span>Tc&#41; or if the time of incorporation was over several days and the patient left the Nuclear Medicine Department &#40;studies with <span class="elsevierStyleSup">67</span>Ga&#41;&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0025"></elsevierMultimedia><p id="par0135" class="elsevierStylePara elsevierViewall">The dose received by a person remaining in the patient waiting room during incorporation of the radiopharmaceutical can be determined by the values shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#46; This table shows the maximum doses produced by the patients at different distances considering the time of contact to be equal to that of the incorporation to each patient&#46; In addition&#44; other irradiation times were assumed for the bone studies and are expressed as the 95th percentile&#46; Thus&#44; a person exposed for 2<span class="elsevierStyleHsp" style=""></span>h at 0&#46;5<span class="elsevierStyleHsp" style=""></span>m of distance from a patient who had undergone bone scintigraphy receives a dose of 80&#46;3<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#46; However&#44; if this person were to sit for 4<span class="elsevierStyleHsp" style=""></span>h next to 2 patients&#44; one at 10<span class="elsevierStyleHsp" style=""></span>cm and the other at 1<span class="elsevierStyleHsp" style=""></span>m&#44; the dose would be of 641<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#46; The highest doses were produced by bone scintigraphies followed by somatostatin receptor studies &#40;<span class="elsevierStyleSup">111</span>In-Octreotide&#41;&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">Based on the conditions established&#44; the doses were determined when the contact took place with the patient during incorporation of the radiopharmaceutical &#40;<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>&#41; and when this contact was produced after the patient had left the Department of Nuclear Medicine &#40;<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>&#41;&#46; If the contact occurred during the period of radiopharmaceutical incorporation&#44; the worst-case scenario was that of the person accompanying the patient undergoing the diagnostic study since the dose received could be greater than 0&#46;1<span class="elsevierStyleHsp" style=""></span>mSv&#46; Nonetheless&#44; when the contact is produced after the diagnostic study people living with the patient are at higher exposure and may receive a dose of up to 0&#46;63<span class="elsevierStyleHsp" style=""></span>mSv &#40;for a cardiac perfusion test&#41;&#46; In the settings studied&#44; bone scintigraphy produced the greatest dosimetric impact during incorporation of the radiopharmaceutical&#46; On the other hand&#44; bone scintigraphy produced the lowest dose when the contact between the patient and the public was produced after the diagnostic procedure&#46;</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0145" class="elsevierStylePara elsevierViewall">The present study evaluated the radiation dose members of the public may be exposed to due to contact with nuclear medicine patients during incorporation of radiopharmaceuticals and compared this with the dose produced by patients after having undergone the diagnostic study&#46; Indeed&#44; one of the objectives of the study was to develop recommendations to reduce radiation doses from the evaluation of the different contact scenarios studied&#46; These recommendations are dependent on the dose administered and thus&#44; a reduction in dose that does not affect the quality of the diagnostic study would allow a reduction in the dose to which individuals are exposed&#46; A reduction in the radiation dose would not only reduce the individual but also the collective dose produced by the irradiation of members of the public by patients attending the Department of Nuclear Medicine&#46;</p><p id="par0150" class="elsevierStylePara elsevierViewall">For the measurements of the ambient dose equivalent the energetic response of the equipment for the energy of 140<span class="elsevierStyleHsp" style=""></span>keV of <span class="elsevierStyleSup">99m</span>Tc was taken into account &#40;98&#37; of the cases studied&#41;&#46; For the detector used in this study the angle response for a source point varied less than 10&#37; with respect to the central position&#46; However&#44; neither the angle factor nor the geometric response of the equipment was taken into account because the distribution of the radiation dose in the patient was not known&#46; Indeed&#44; similar to our study other authors did not include these measurement corrections despite recognizing that the response of the detector varies from a phantom to a person&#46;<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">7</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">One method to know the response of the measurement equipment to a complex source of radiation is simulation using Monte Carlo methods which allow different geometric and angle variables to be evaluated&#46; Nonetheless&#44; the results obtained are limited to the geometric measurements simulated&#44; which differ from the patient measurements&#44; and thereby make it difficult to transfer the associated uncertainties&#46;</p><p id="par0160" class="elsevierStylePara elsevierViewall">For this evaluation the dose rates produced at 0&#46;1&#44; 0&#46;5 and 1<span class="elsevierStyleHsp" style=""></span>m of distance from the patient in orthostatic position were measured at the sternon similar to what has been described in other studies&#46;<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">2&#44;5</span></a> In the present study we made measurements not only at the end of the diagnostic study but also after administration of the radiopharmaceutical and before image acquisition&#46;</p><p id="par0165" class="elsevierStylePara elsevierViewall">The study scenarios in which members of the public were exposed to irradiation represent situations of usual contact &#40;users of public and private transport&#44; relatives or co-workers&#41;&#44; and the times and distances of contact were adjusted to real circumstances similar to the scenarios studied by other authors&#46;<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">3&#44;5&#44;8</span></a> A new scenario&#44; however&#44; was that of the physicians attending patients in their offices during incorporation of the radiopharmaceutical and after having undergone the nuclear medicine diagnostic test&#46; On the other hand&#44; we also studied the people accompanying the patients when they did not remain in the patient waiting room during radiopharmaceutical incorporation&#46; In contrast to other studies&#44; however&#44; the dose received by the technical personnel performing the diagnostic tests was not evaluated&#44;<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> In the case of bone scintigraphy 142 patients were studied&#44; and the dose in the different conditions was estimated with the 95th percentile while the maximum values were used for the remaining diagnostic tests&#46; Consequently&#44; the method to evaluate the doses in the different scenarios was conservative&#44; that is&#44; the worst-case scenario was assessed&#46;</p><p id="par0170" class="elsevierStylePara elsevierViewall">An important variation was observed in the dose rates measured according to the type of diagnostic test performed mainly due to differences in the physical characteristics of the patients&#44; the biodistribution and radioactivity of the radiopharmaceutical administered and the different gamma emission of the radiopharmaceutical&#46; When the patient had finished the diagnostic test&#44; the dose rates measured &#40;95th percentile&#41; were lower in the present study than values previously reported for <span class="elsevierStyleSup">18</span>F-FDG &#40;174&#44; 69 and 29<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#47;h at 0&#46;1&#44; 0&#46;5 and 1<span class="elsevierStyleHsp" style=""></span>m&#44; respectively&#41;&#44; except in the case of cardiac studies&#46;<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a> In our series the maximum and minimum dose rates determined after having completed bone scintigraphy were similar to previous descriptions&#44;<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">1&#44;2&#44;18</span></a> taking into account the difference in dose administered&#46; The dose rate measured prior to the imaging test was an average of 20&#37; less than expected for the dose rate after administration considering only radioactive decay&#44; thereby demonstrating biological elimination&#46; Nonetheless&#44; in studies such as cardiac perfusion&#44; the dose rate is greater prior to image acquisition than following administration of the radiopharmaceutical due to redistribution of the activity in the patient&#44; and consequently&#44; different attenuation&#44; despite a reduction related to radioactive decay&#46; In the case of studies using <span class="elsevierStyleSup">111</span>In and <span class="elsevierStyleSup">67</span>Ga&#44; despite having a lengthy semidecay period&#44; the dose rates measured prior to image acquisition were reduced by approximately half compared to those measured after administration of the radiopharmaceutical&#46; This is mainly due to different biodistribution&#46; On the other hand&#44; in sentinel lymph node and myocardial perfusion studies&#44; the dose rates measured at 0&#46;1<span class="elsevierStyleHsp" style=""></span>m were much higher than at 0&#46;5 and 1<span class="elsevierStyleHsp" style=""></span>m since their distribution takes place in tissues or organs close to the thoracic zone&#44; and thus&#44; closer to the point of measurement&#46;</p><p id="par0175" class="elsevierStylePara elsevierViewall">The design of all nuclear medicine departments usually includes a waiting room for irradiated patients&#44; and several patients may be inside during incorporation of the radiopharmaceutical for the same or different diagnostic tests&#46; The use of this waiting room follows the ALARA principle in which the radiation dose received should be as low as reasonably possible&#46; With this objective patients remain isolated during the incorporation of radiopharmaceutical so that neither accompanying persons nor the general public are exposed&#46; In the waiting room the patient will also be irradiated due to confluence with other patients in the same room&#44; several of whom may be undergoing diagnostic bone scintigraphy&#46; In this setting the results of the present study demonstrate that if a patient remains with 2 other patients for 4<span class="elsevierStyleHsp" style=""></span>h the radiation dose they receive may be greater than 1<span class="elsevierStyleHsp" style=""></span>mSv&#46; This dose is 25&#37; of the effective dose a patient receives in a bone scintigraphy&#44; that is&#44; 3&#46;8<span class="elsevierStyleHsp" style=""></span>mSv &#40;5&#46;1<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#47;MBq&#41; for 740 MBq of <span class="elsevierStyleSup">99m</span>Tc-HDP&#46;<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">19</span></a> However&#44; if the patient is waiting for 2<span class="elsevierStyleHsp" style=""></span>h to undergo a sentinel lymph node study the effective dose is of 0&#46;8<span class="elsevierStyleHsp" style=""></span>mSv &#40;5&#46;7<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#47;MBq&#41;&#44;<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">19</span></a> similar to the dose received when with 2 patients undergoing bone scintigraphy &#40;0&#46;7<span class="elsevierStyleHsp" style=""></span>mSv&#41;&#46; This comparison indicates the need to apply the ALARA principle in the waiting room and suggests the need to maximize the distance between patients and distribute them based on the diagostic test to be performed&#46; There is also the possibility of telling the patient to remain with their accompanying person in a public place with few people such as a park&#44; and thus&#44; the only person irradiated would be the accompanying person who would receive an estimated dose of 0&#46;01&#8211;0&#46;1<span class="elsevierStyleHsp" style=""></span>mSv if accompanying a patient undergoing isotopic lymphography or bone scintigraphy&#44; respectively&#46;</p><p id="par0180" class="elsevierStylePara elsevierViewall">Using dosimetry&#44; Harding et al&#46;<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">9</span></a> measured the dose received by people accompanying patients for a bone study received&#46; The median dose ranged from 11 to 13<span class="elsevierStyleHsp" style=""></span>&#956;Sv depending on whether the Nuclear Medicine Department had one or 2 waiting rooms or if the patient could leave the department&#46; However&#44; the maximum values &#40;84<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#41; were compatible with our results &#40;116<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#41; at a distance of 0&#46;5<span class="elsevierStyleHsp" style=""></span>m&#44; considering the difference of dose administered and that not registered as the mean distance between the patient and the accompanying person&#46; With these mean values Harding et al&#46; showed that from the point of view of radiologic protection there was no need to have 2 waiting rooms&#44; one being exclusively for injected patients&#46; The results of the present study were more conservative on considering the maximum dose and the 95th percentile&#46;</p><p id="par0185" class="elsevierStylePara elsevierViewall">If a patient is to be accompanied by someone during incorporation of the radiopharmaceutical&#44; the dose received for remaining at 0&#46;5<span class="elsevierStyleHsp" style=""></span>m from the patient during the incorporation of <span class="elsevierStyleSup">99m</span>Tc-HDP during 4<span class="elsevierStyleHsp" style=""></span>h &#40;0&#46;14<span class="elsevierStyleHsp" style=""></span>mSv&#44; 95th percentile&#41; is greater than that produced by a patient undergoing a diagnostic test by PET during an incorporation time of 1<span class="elsevierStyleHsp" style=""></span>h &#40;0&#46;05<span class="elsevierStyleHsp" style=""></span>mSv&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> However&#44; the doses received by a person at 0&#46;1&#44; 0&#46;5 and 1<span class="elsevierStyleHsp" style=""></span>m from the patient during the first hour of the incorporation period of <span class="elsevierStyleSup">99m</span>Tc-HDP in bone scintigraphy &#40;0&#46;17&#44; 0&#46;042 and 0&#46;016<span class="elsevierStyleHsp" style=""></span>mSv respectively&#41; are comparable to those described by Benatar et al&#46;&#44;<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> who evaluated the dose received by people accompanying patients who had been administered 100<span class="elsevierStyleHsp" style=""></span>MBq of <span class="elsevierStyleSup">18</span>F-FDG &#40;0&#46;16&#44; 0&#46;05 and 0&#46;02<span class="elsevierStyleHsp" style=""></span>mSv at the same distances and in 1<span class="elsevierStyleHsp" style=""></span>h&#41;&#46;</p><p id="par0190" class="elsevierStylePara elsevierViewall">The radiological impact of going to a coffee shop or restaurant during incorporation of a radiopharmaceutical is 3-fold less &#40;in bone scintigraphy&#41; and 2-fold less &#40;lymphography and leukocyte tests&#41; if done after having finished the diagnostic study&#46; In some cases&#44; such as in cardiac studies or sentinel lymph node detection&#44; the reduction of the dose is very slight&#46; The worst scenario is the hospital cafeteria and particularly a worker of this establishment where a greater number of nuclear medicine patients likely go&#46; The estimated dose is less than 1<span class="elsevierStyleHsp" style=""></span>&#956;Sv per patient since dose reduction factors must be applied in the scenario of a person accompanying a patient in a cafeteria at a distance of 0&#46;5<span class="elsevierStyleHsp" style=""></span>m and 30<span class="elsevierStyleHsp" style=""></span>min of contact &#40;with a dose less than 18&#46;2<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#41; because of the increase in distance and reduction in the contact time&#46;</p><p id="par0195" class="elsevierStylePara elsevierViewall">The proportion of radiation received by a physician in the consultation office attending a patient who has undergone a nuclear medicine diagnostic study is decreased similar to the scenario in the coffee shop and restaurant if the visit is made after completing the test instead of during the incorporation of radiopharmaceutical&#46; Nonetheless&#44; the annual dose may surpass the limit of 1<span class="elsevierStyleHsp" style=""></span>mSv if the physician attends a significant number of patients in the office&#46; Thus&#44; during incorporation of the radiopharmaceutical&#44; the worst-case scenario is that produced by patients undergoing bone scintigraphy since 44 visits a year to the physician would reach the annual dose limit&#46; However&#44; exposure is greatest after having completed cardiac studies since the physician would reach the annual dose limit with 40 patients while this would be achieved on seeing 128 patients who had undergone bone scintigraphy&#46; Restrictions would be necessary if a physician were to perform more than this number of consultations per year&#46;</p><p id="par0200" class="elsevierStylePara elsevierViewall">Restrictions with regard to the use of transportation are necessary for patients undergoing radiometabolic treatments with <span class="elsevierStyleSup">131</span>I&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a> However&#44; this is not necessary in diagnostic studies&#46; Nonetheless&#44; it is recommended that patients use private rather than public transport to reduce the dosimetric impact by an average factor of 6 for bone scintigraphy and up to a factor of 50 for sentinel lymph node studies&#46; To calculate the dose received during the trip&#44; the difference considered between public and private transport is the distance factor rather than the reduction in time with the use of a private vehicle&#46; In this case the reduction for the use of private transportation would be even greater&#46; In addition&#44; the patient normally irradiates more members of the public on public transport thereby increasing the collective dose&#46; However&#44; at an individual level it is very unlikely that one person would be affected by the irradiation of several patients who have undergone a nuclear medicine procedure&#46; In most of the diagnostic tests studied the dose received by members of the public was lower than that in studies with <span class="elsevierStyleSup">18</span>F-FDG &#40;145 and 24<span class="elsevierStyleHsp" style=""></span>&#956;Sv for public and private transportation&#44;<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a> respectively&#41;&#46;</p><p id="par0205" class="elsevierStylePara elsevierViewall">The highest dose received by a member of the public occurs in the case of an accompanying person who may receive up to 0&#46;63<span class="elsevierStyleHsp" style=""></span>mSv from a patient undergoing a cardiac study or 0&#46;18<span class="elsevierStyleHsp" style=""></span>mSv in the case of bone scintigraphy&#46; This dose is similar to that estimated for studies with <span class="elsevierStyleSup">18</span>F-FDG &#40;0&#46;11<span class="elsevierStyleHsp" style=""></span>mSv&#41; considering the same scenario&#46;<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a></p><p id="par0210" class="elsevierStylePara elsevierViewall">The dose received by co-workers in the worst-case scenario of a patient immediately returning to work after having undergone the tests is 82<span class="elsevierStyleHsp" style=""></span>&#956;Sv &#40;cardiac study&#41; similar to the dose received by co-workers of a patient diagnosed with <span class="elsevierStyleSup">18</span>F-FDG&#44;<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a> and less than the other scenarios considered in the present study&#46; In addition&#44; it is very unlikely for a person to be irradiated by several co-workers who have undergone a nuclear medicine diagnostic test in the same year&#46; In any case&#44; this dose significantly decreases if the patient does not return to work until the day after the intervention&#46;</p><p id="par0215" class="elsevierStylePara elsevierViewall">In studies with <span class="elsevierStyleSup">99m</span>Tc&#44; the dose reduces approximately 10&#37; by delaying the contact of the patient with members of the public after leaving the nuclear medicine department by 1<span class="elsevierStyleHsp" style=""></span>h&#46; However&#44; the same cannot be said for studies with <span class="elsevierStyleSup">67</span>Ga and <span class="elsevierStyleSup">111</span>In since they have a semidecay period of several days&#46;</p><p id="par0220" class="elsevierStylePara elsevierViewall">One limitation of this study is the small number of patients undergoing diagnostic procedures other than bone scintigraphy included due to the healthcare profile of the center&#46; Nonetheless&#44; we considered it of interest to evaluate the dose produced by patients undergoing less frequently performed studies to thereby detect which tests induce greater exposure from a radiological protection point of view&#46; On the other hand&#44; adults made up the study population&#46; The dose for children in the different scenarios would be lower than that produced by adults due to the lower radioactivity administered to a child&#46; In contrast&#44; the dose rate per unit of radioactivity administered would be greater because of the reduction in body mass&#46;<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">14</span></a></p><p id="par0225" class="elsevierStylePara elsevierViewall">The results of the present study are conservative for 2 reasons&#46; Firstly&#44; they are based on measurements of dose rates made from the anterior side of the patient&#46; Iin one study determining the doses to personnel in procedures involving <span class="elsevierStyleSup">18</span>F-FDG at several distances it was found that anterior and posterior measures were similar and greater than those made from the sides of the patient&#46;<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">7</span></a> Thus&#44; anterior measurements overestimate the dose since the subjects exposed may move around the patient&#46; Secondly&#44; the results of dose estimation for bone scintigraphy were presented as the 95th percentile thereby excluding extreme values and maintaining an acceptable probability that an individual in the worst-case scenario remained within the dose limits&#46;<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">18</span></a></p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conclusions</span><p id="par0230" class="elsevierStylePara elsevierViewall">The irradiation received by a patient while remaining in a waiting room with other patients during the incorporation of the radiopharmaceutical may be similar to the dose received by the diagnostic process itself&#46; It is therefore recommended to maximize the distance between patients and distribute patients based on the type of diagnostic study to be performed&#46; In the case of a patient attending a medical office&#44; coffee shop or restaurant&#44; the irradiation to which third parties are exposed may be 3-fold greater during radiopharmaceutical administration than after completion of the diagnostic test&#44; with the accompanying person receiving the highest dose&#46; Once the test has finished it is recommended that the patient use private rather than public transport&#46; Although it is the accompanying person who receives the greatest radiation dose&#44; restrictions are not needed&#46; However&#44; it may be necessary to limit the number of patients undergoing nuclear medicine diagnostic tests attended by physicians in their consulting offices&#46;</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conflict of interests</span><p id="par0235" class="elsevierStylePara elsevierViewall">The authors declare no conflict of interest&#46;</p></span></span>"

        "resumen" => "<span id="abst0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Objectives</span><p id="spar0005" class="elsevierStyleSimplePara elsevierViewall">The aim of this study was to assess the dose received by members of the public due to close contact with patients undergoing nuclear medicine procedures during radiopharmaceutical incorporation&#44; and comparing it with the emitted radiation dose when the test was complete&#44; in order to establish recommendations&#46;</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">A prospective study was conducted on 194 patients&#46; <span class="elsevierStyleItalic">H</span>&#42;&#40;10&#41; dose rates were measured at 0&#46;1&#44; 0&#46;5&#44; and 1&#46;0<span class="elsevierStyleHsp" style=""></span>m after the radiopharmaceutical administration&#44; before the image acquisition&#44; and at the end of the nuclear medicine procedure&#46; Effective dose for different close contact scenarios were calculated&#44; according to 95th percentile value &#40;bone scans&#41; and the maximum value &#40;remaining tests&#41;&#46;</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">During the radiopharmaceutical incorporation&#44; a person who stays with another injected patient in the same waiting room may receive up to 0&#46;59<span class="elsevierStyleHsp" style=""></span>mSv&#46; If the patient had a medical appointment&#44; or went to a restaurant or a coffee shop&#44; members of the public could receive 23&#44; 43&#44; and 22<span class="elsevierStyleHsp" style=""></span>&#956;Sv&#44; respectively&#46; After finishing the procedure&#44; these doses are reduced by a factor 3&#46; In most of the studies&#44; the use of private instead of public transport may reduce the dose by more than a factor 6&#46;</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusion</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">It is recommended to increase the distance between the patients during the radiopharmaceutical incorporation and to distribute them according to the diagnostic procedure&#46; Patients should be encouraged to use private instead of public transport&#46; Depending on the number of nuclear medicine outpatients per year attended by a physician&#44; it could be necessary to apply restrictions&#46;</p></span>"
