(d) Bone lesion in the right L2 vertebral hemibody only detected with <span class="elsevierStyleSup">68</span>Ga-DOTATATE e) (yellow arrows).</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Prostate cancer is the second most frequent malignant tumor in men and one of the most frequent malignant diseases worldwide.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The detection of lesions associated with recurrence of prostate cancer within the context of biochemical relapse (BCR) is an important challenge for all imaging modalities.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">2</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Positron emission tomography/computerized tomography (PET/CT) with radiopharmaceuticals derived from choline are widely used for diagnosing recurrence of prostate cancer in patients with BCR. However, numerous studies have described relatively low values of sensitivity and specificity, especially at low prostate specific antigen (PSA) levels.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">3,4</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Despite the therapeutic success of the initial strategies, most patients with advanced prostate cancer become independent of androgens and progress with elevated PSA levels when on hormone deprivation therapies. Hormone-resistant prostate cancer is defined by disease progression despite androgen deprivation treatment. This situation is a clinical challenge since treatment for patients with hormone-resistant prostate cancer with distant metastasis is very limited. New therapeutic strategies able to control the evolution of patients with this stage of the disease are urgently needed.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">A series of studies evaluated the possible neuroendocrine differentiation (NED) of prostate cancer cells in the phase of resistance to castration.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">6–8</span></a> Indeed, NED has gained increasing interest in the last years due to its prognostic and therapeutic implications taking into account disease progression despite conventional treatment.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">NED is characterized by disperse groups of differentiated neuroendocrine cells within a predominant population of adenocarcinoma cells and has prognostic and therapeutic potential in prostate cancer.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">9</span></a> NED has been described in up to 85% of patients with hormone-resistant prostate cancer.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a> Neuroendocrine cellular hyperactivation may play a key role in the pathogenesis of castration-resistant prostate cancer by the production and secretion of bioactive neuropeptides.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">11</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Some neuroendocrine cells in the normal prostate form a communication network linked to the regulation of cellular proliferation<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">9</span></a> and are independent of androgen stimulation.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">These cells express somatostatin receptors (SSTR) and can be identified with somatostatin analogs labeled with <span class="elsevierStyleSup">68</span>Ga (<span class="elsevierStyleSup">68</span>Ga-DOTA-peptides) which are alternative PET radiotracers that are of interest for the evaluation of patients with hormone-resistant prostate cancer.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">6</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Different studies have analyzed the overexpression of SSTR using <span class="elsevierStyleSup">68</span>Ga-DOTA-peptides in different tumors, especially neuroendocrine tumors<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">11</span></a> as well as in patients with prostate cáncer.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">12,13</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">In this regard it has been demonstrated that some prostate cancer lesions can be visualized with these radiotracers using PET/CT, thereby confirming the expresson of these receptors.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">14,15</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">These results led our group to carry out preliminary studies using <span class="elsevierStyleSup">68</span>Ga-DOTATATE PET/CT in a small series of patients with advanced prostate cancer in BCR. Using this technique we were able to visualize all the lesions previously detected by bone scintigraphy and, moreover, prostatic lesions and pelvic lymph nodes.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">Combined therapies with somatostatin analogs have recently emerged as possible therapeutic options in patients with advanced androgen-independent prostate cancer using β-emitting radiopharmaceuticals such as <span class="elsevierStyleSup">90</span>Y or <span class="elsevierStyleSup">177</span>Lu for radiotherapy.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Accurate determination of the presence of neuroendocrine cells in prostate cancer and the identification of subgroups of potential responders to these therapies are of clinical relevance.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">Therefore, the aim of this study was to prospectively compare the rate of detection of <span class="elsevierStyleSup">68</span>Ga-DOTATATE PET/CT versus <span class="elsevierStyleSup">11</span>C-choline PET/CT in patients with prostate cancer and BCR and evaluate the in vivo expression of SSTR with the objective of developing therapies aimed at these molecular targets.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0075" class="elsevierStylePara elsevierViewall">According to internationally accepted criteria, we prospectively analyzed 64 patients with BCR referred to our department from March 2012 to January 2014. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the characteristics of the patients. Forty-two (65%) had undergone radical prostectomy and 22 patients were treated with radiotherapy (associated or not with hormone deprivation). The mean age of the patients was 64.98<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.03 years. The median PSA level was 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL, ranging from 0.22 to 291<span class="elsevierStyleHsp" style=""></span>ng/mL.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Ethical considerations</span><p id="par0080" class="elsevierStylePara elsevierViewall">The study was approved by the Ethics Committe of our center and was carried out according to the Declaration of Helsinki, with the respective amendments and the requisites of the International Committee of Medical Journal Editors (ICMJE). In addition, all the study participants provided informed consent and authorization to publish the results, maintaining patient anonymity.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Radiopharmaceuticals</span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090"><span class="elsevierStyleSup">11</span>C-choline</span><p id="par0085" class="elsevierStylePara elsevierViewall">[N-methyl-<span class="elsevierStyleSup">11</span>C] choline was synthesized from <span class="elsevierStyleSup">11</span>CO2 and dimetylaminoethanol in an automatic module.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The reaction and purification was done in a Sep-Pak Accell Plus CM (Millipore).</p><p id="par0095" class="elsevierStylePara elsevierViewall">The radiochemical purity of <span class="elsevierStyleSup">11</span>C-choline was greater than 95%.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095"><span class="elsevierStyleSup">68</span>Ga-DOTATATE</span><p id="par0100" class="elsevierStylePara elsevierViewall">The elution was obtained in a <span class="elsevierStyleSup">68</span>Ge/<span class="elsevierStyleSup">68</span>Ga generator (ITG, Germany).</p><p id="par0105" class="elsevierStylePara elsevierViewall">A quantity of peptide of 20<span class="elsevierStyleHsp" style=""></span>nmol or less was heated for 10<span class="elsevierStyleHsp" style=""></span>min at 100<span class="elsevierStyleHsp" style=""></span>°C with a pH 3.5–4.2. This was then purified with SPEC (SepPak C 18 light) eluted with ETOH 30% and formulated with NaCl 0.9%, thereafter achieving the radionuclide and radiochemical purity of <span class="elsevierStyleSup">68</span>Ga-DOTATATE.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Acquisition of images</span><p id="par0110" class="elsevierStylePara elsevierViewall">Patients were not required to have fasted or discontinued their usual medication.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Patient preparation included the placement of a intravenous line and hydratation with 500<span class="elsevierStyleHsp" style=""></span>mL of saline solution at 0.9%.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Within an interval no greater than 2 weeks two PET/CT studies were performed: one with <span class="elsevierStyleSup">11</span>C-choline and the other with <span class="elsevierStyleSup">68</span>Ga-DOTATATE in no specific order, and the studies were then analyzed.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The studies were acquired in 64-slice PET/CT equipment (General Electric Discovery 690 VCT, 64 slices, Waukesha, WI, USA) immediately after the injection of the 6.0<span class="elsevierStyleHsp" style=""></span>MBq/kg of <span class="elsevierStyleSup">11</span>C-choline or 30<span class="elsevierStyleHsp" style=""></span>min after the intravenous administration of 2.0<span class="elsevierStyleHsp" style=""></span>MBq/kg of <span class="elsevierStyleSup">68</span>Ga-DOTATATE.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Images were acquired from the cranium to the thigh. The acquisition and processing parameters of the PET images were the same for <span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE.</p><p id="par0135" class="elsevierStylePara elsevierViewall">CT was carried out with 80–180<span class="elsevierStyleHsp" style=""></span>mA, 120<span class="elsevierStyleHsp" style=""></span>kV (index noise 30) and 3.75<span class="elsevierStyleHsp" style=""></span>mm of slice thickness, pitch of 1.375, rotation time of 0.8<span class="elsevierStyleHsp" style=""></span>s and a table movement of 55<span class="elsevierStyleHsp" style=""></span>mm/rotation.</p><p id="par0140" class="elsevierStylePara elsevierViewall">PET was acquired in 3D, with a matrix size of 128<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>128 pixels with an acquisition time of 3 or 4<span class="elsevierStyleHsp" style=""></span>min per each bed position (<span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE, respectively), with slice overlapping of 11<span class="elsevierStyleHsp" style=""></span>mm.</p><p id="par0145" class="elsevierStylePara elsevierViewall">The images were reconstructed using an Ordered Subset Expectation Maximization (OSEM) iterative reconstruction algorithm with 28 subsets/2 iterations with correction by flight time and reformed in transversal, coronal and sagittal slices.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The images were evaluated together by two specialists in nuclear medicine and by two radiology physicians. Both sets of images were assessed together. Disagreement was resolved by consensus.</p><p id="par0155" class="elsevierStylePara elsevierViewall">All the lesions visually considered as malignant were taken into account and analyzed in relation to their localization (local recurrence, bone metastasis, lymph nodes or soft tissue) and their standardized uptake values (SUVmax).</p><p id="par0160" class="elsevierStylePara elsevierViewall">Suspicious lesions were defined by any focal uptake, in one or more localization, of greater intensity than the background uptake or surrounding background or in comparison with normal tissue, excluding zones of articulations and areas of physiological uptake.</p><p id="par0165" class="elsevierStylePara elsevierViewall">The pattern of comparison considered the following:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">(1)</span><p id="par0170" class="elsevierStylePara elsevierViewall">Histopathology of the lesions by histological study.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">(2)</span><p id="par0175" class="elsevierStylePara elsevierViewall">Comparison of the PET/CT findings with other imaging techniques: CT with intravenous contrast, magnetic resonance, bone scintigraphy, transrectal ultrasonography, or new PET/CT studies.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">(3)</span><p id="par0180" class="elsevierStylePara elsevierViewall">Biochemical and clinical follow-up of the patient over a period greater than 5 months after the PET/CT studies. To do this, clinical histories were reviewed and telephone interviews were made with the patients and/or treating physicians.</p></li></ul></p><p id="par0185" class="elsevierStylePara elsevierViewall">The PET/CT were clinically considered as true positive results with the presence of one or more of the following circumstances.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Histopathological confirmation</span><p id="par0190" class="elsevierStylePara elsevierViewall">Confirmation of the lesion by concordant results with other imaging techniques whether in reevaluation of previous studies or posterior studies.<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">(1)</span><p id="par0195" class="elsevierStylePara elsevierViewall">Confirmation of the lesion in posterior PET/CT studies with an increase in size/uptake of <span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE and/or the appearance of new lesions.</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">(2)</span><p id="par0200" class="elsevierStylePara elsevierViewall">A reduction of greater than 50% in PSA values in patients treated after the PET/CT studies.</p></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">(3)</span><p id="par0205" class="elsevierStylePara elsevierViewall">Persistence and worsening of the symptoms and/or a rise in PSA levels.</p></li><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">(4)</span><p id="par0210" class="elsevierStylePara elsevierViewall">Results were considered as clinically true negative results in patients showing persistent stabilization over time (>6 months) or spontaneous normalization of PSA values during follow-up, without the appearance of lesions in other imaging studies or the control PET/CT study.</p></li></ul></p><p id="par0215" class="elsevierStylePara elsevierViewall">Images were analyzed using a General Electric AW 4.6 platform (HP Workstation and LINUX OS) and OsiriX v6.5.2 (OS X El Capitan).</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Statistical analysis</span><p id="par0220" class="elsevierStylePara elsevierViewall">The SUVmax values of the same lesions detected with both <span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE were statistically analyzed using Wilcoxon analysis for paired data (Wilcoxon-Signed test).</p><p id="par0225" class="elsevierStylePara elsevierViewall">The normality of both radiotracers was evaluated separately, graphically and using the Shapiro–Wilk test of normality.</p><p id="par0230" class="elsevierStylePara elsevierViewall">Correlation was made between the SUVmax values of <span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE for concordant lesions using the Spearman rho non parametric correlation coefficient.</p><p id="par0235" class="elsevierStylePara elsevierViewall">The rate of <span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE detection was analyzed at different PSA cut-off points.</p><p id="par0240" class="elsevierStylePara elsevierViewall">The relationship between PSA levels and the SUVmax of both studies was evaluated.</p><p id="par0245" class="elsevierStylePara elsevierViewall">Statistical significance was considered with <span class="elsevierStyleItalic">α</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.05.</p><p id="par0250" class="elsevierStylePara elsevierViewall">The statistical analyses were performed using: GraphPad Prism (version 7.00 fora Mac OS X, La Jolla, CA, USA), R software version 3.2.2 (R Core Team (2015). RStudio: Integrated Development for R. RStudio, Inc., Boston, MA URL: <a id="intr0015" class="elsevierStyleInterRef" href="http://www.rstudio.com/">http://www.rstudio.com/</a>).</p></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Results</span><p id="par0255" class="elsevierStylePara elsevierViewall">The rate of global detection per patient was 48.43% (31/64) for <span class="elsevierStyleSup">68</span>Ga-DOTATATE and 46.87% (30/64) for <span class="elsevierStyleSup">11</span>C-choline.</p><p id="par0260" class="elsevierStylePara elsevierViewall">Both techniques were positive in 25 patients (39%) and negative in 28 patients (43.70%). Six patients were only positive with <span class="elsevierStyleSup">68</span>Ga-DOTATATE (9.4%) and 5 only with <span class="elsevierStyleSup">11</span>C-choline (7.8%). The results were concordant in 53 cases (82.81%) (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0265" class="elsevierStylePara elsevierViewall">A total of 215 lesions were detected with at least one radiotracer: 149 for <span class="elsevierStyleSup">68</span>Ga-DOTATATE and 197 for <span class="elsevierStyleSup">11</span>C-choline. The localization of the lesions by each radiotracer is shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><p id="par0270" class="elsevierStylePara elsevierViewall">The normality of the two radiotracers was evaluated separately, graphically and with a test of normality. Neither of the two measures had a normal distribution (Shapiro–Wilk test, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001).</p><p id="par0275" class="elsevierStylePara elsevierViewall">The capacity of disease detection by PET/CT (positive test) was also assessed based on different PSA values: 1<span class="elsevierStyleHsp" style=""></span>ng/mL, 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL and 5<span class="elsevierStyleHsp" style=""></span>ng/mL (4.25<span class="elsevierStyleHsp" style=""></span>ng/mL corresponds to the median PSA distribution observed in the patient samples and the other two values correspond to values of clinical interest obtained from the literature).</p><p id="par0280" class="elsevierStylePara elsevierViewall">Thus, with a PSA less than 1<span class="elsevierStyleHsp" style=""></span>ng/mL, pathological uptake was detected with <span class="elsevierStyleSup">11</span>C-choline in 6% of the patients and in 12% of the patients with <span class="elsevierStyleSup">68</span>Ga-DOTATATE. With values between 1 and 5<span class="elsevierStyleHsp" style=""></span>ng/mL, the percentages of detection rose to 54% and 36%, respectively. Finally, percentages of 72% and 80% (<span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE respectively) were obtained with PSA values greater than or equal to 5<span class="elsevierStyleHsp" style=""></span>ng/mL.</p><p id="par0285" class="elsevierStylePara elsevierViewall">At least one lesion characteristic of prostate cancer was identified in 25% of the patients with <span class="elsevierStyleSup">11</span>C-choline PET/CT, with PSA values less than or equal to 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL (sample median), while 65.75% of the patients presented pathological lesions with PSA values greater than 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL.</p><p id="par0290" class="elsevierStylePara elsevierViewall">Using the same threshold for <span class="elsevierStyleSup">68</span>Ga-DOTATATE PET/CT, 21.8% of the patients presented at least one lesion with a PSA level of 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL and less. In addition, 75% of the patients presented pathological lesions with PSA values greater than 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL.</p><p id="par0295" class="elsevierStylePara elsevierViewall">The SUVmax of <span class="elsevierStyleSup">11</span>C-choline was signficantly greater than that corresponding to <span class="elsevierStyleSup">68</span>Ga-DOTATATE for all the concordant lesions (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>130): 6.17 (1.70–15.50) versus 4.38 (1.37–26.70), median (range), for each radiotracer, respectively (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001).</p><p id="par0300" class="elsevierStylePara elsevierViewall">There was a significant correlation between the SUVmax of the two radiotracers (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.41, <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>130, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001).</p><p id="par0305" class="elsevierStylePara elsevierViewall">The sensitivity and specificity values per patient with their 95% confidence intervals (95% CI) were the same for both techniques: 0.82 (0.65–0.93) and 0.90 (0.73–0.98), respectively.</p><p id="par0310" class="elsevierStylePara elsevierViewall">In the group of patients with PSA <4.25 the sensitivity calculated was 0.63, while patients with PSA >4.25 presented a sensitivity of 0.89 for both techniques.</p><p id="par0315" class="elsevierStylePara elsevierViewall">There was no relationship between the SUVmax of the two radiotracers with regard to the type of lesion detected and the PSA level.</p><p id="par0320" class="elsevierStylePara elsevierViewall">Of the whole population, only 4 patients with positive results underwent surgery, and histopathological study of the surgical pieces showed inflammatory alterations.</p><p id="par0325" class="elsevierStylePara elsevierViewall">Comparison of the uptake of <span class="elsevierStyleSup">11</span>C-choline with that of <span class="elsevierStyleSup">68</span>Ga-DOTATATE identified malignant lesions with SSRT expression which could be a promising specific target for therapy with <span class="elsevierStyleSup">177</span>Lu-DOTATATE. Although there was a high concordance between the two techniques, the level of uptake and number of lesions detected seemed to be greater with <span class="elsevierStyleSup">11</span>C-choline.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Discussion</span><p id="par0330" class="elsevierStylePara elsevierViewall">After applying a treatment with intention to cure in clinically localized prostate cancer with surgery or radiotherapy, 15–30% of the patients present biochemical progression which precedes recurrence clinically detectable at a pelvic level or with metastatic disease by a period of months or years.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">17</span></a></p><p id="par0335" class="elsevierStylePara elsevierViewall">The utility of PET/CT with radiolabeled choline for the evaluation of these patients has been widely demonstrated in several clinical studies<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a> which confirmed a preferential uptake by prostate cancer, the lymph nodes and metastasis.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a> However, choline is not specific for either intra- or extra-prostatic cancer, and in addition, benign prostatic hyperplasia has shown a high affinity to this radiotracer. Consequently, discrimination betweeen benign and malignant intraprostatic tissue is hindered by a low specificity.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a> Moreover, the sensitivity of choline with PET/CT for detecting locally recurrent prostate cancer is low, especially in cases with low PSA values.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">21,22</span></a></p><p id="par0340" class="elsevierStylePara elsevierViewall">On the other hand, PET/CT with SSRT uses <span class="elsevierStyleSup">68</span>Ga-DOTA-peptide radiotracers (DOTA-TOC, DOTA-TATE, DOTA-NOC) which specific bind to these receptors.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">12–14</span></a> The most relevant difference between the different compounds is the binding affinity variable for the different subtypes of receptors.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> There are 5 subtypes (1–5); 2A and 2B are the most commonly expressed in neuroendocrine cells (95%).<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> Overexpression of SSTR2 has been described in patients with recurrent prostate cáncer.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a></p><p id="par0345" class="elsevierStylePara elsevierViewall">Therefore, confirmation of a sufficient number of SSTR is of clinical value in order to plan therapy directed against these molecular targets by labeling with β-emittor isotopes such as <span class="elsevierStyleSup">90</span>Y or <span class="elsevierStyleSup">177</span>Lu. This combined procedure has the potential to perform a therapeutic approach based on molecular images (theranostics or diagnotherapy).<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">8,16,25</span></a></p><p id="par0350" class="elsevierStylePara elsevierViewall">We prospectively analyzed 64 patients in BCR after conventional treatment for prostate cancer. Within an interval of less than 2 weeks two PET/CT studies were performed: one with <span class="elsevierStyleSup">11</span>C-choline and the other with <span class="elsevierStyleSup">68</span>Ga-DOTATATE.</p><p id="par0355" class="elsevierStylePara elsevierViewall">Pathological findings were detected in the <span class="elsevierStyleSup">68</span>Ga-DOTATATE PET/CT study in 48.3% of the patients. This is similar to data from the literature which describe the uptake of somatostatin analog radiotracers in different types of tumors expressing these receptors.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,16,26</span></a></p><p id="par0360" class="elsevierStylePara elsevierViewall">On the other hand, in 46.87% of the patients undergoing the <span class="elsevierStyleSup">11</span>C-choline PET/CT study, at least one lesion characteristic of prostate cancer was found, similar to previous reports.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0365" class="elsevierStylePara elsevierViewall">Both techniques were positive in 25 patients (39%) and negative in 28 patients (43.70%). Six patients were positive only with <span class="elsevierStyleSup">68</span>Ga-DOTATATE (9.4%), and 5 were positive only with <span class="elsevierStyleSup">11</span>C-choline (7.8%). The results were concordant in 53 cases (82.81%).</p><p id="par0370" class="elsevierStylePara elsevierViewall">There is currently controversy regarding the efficacy of <span class="elsevierStyleSup">11</span>C-choline at different PSA cut-offs, and different authors have established values between 1 and 5<span class="elsevierStyleHsp" style=""></span>ng/mL as being discriminative in positive or negative PET studies.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">21,22,27,28</span></a></p><p id="par0375" class="elsevierStylePara elsevierViewall">In our study, we evaluated the capacity of detection of disease by PET/CT (positive study results) based on PSA values and found a relationship between the two parameters. Thus, with a PSA less than 1<span class="elsevierStyleHsp" style=""></span>ng/mL pathological uptakes were detected with <span class="elsevierStyleSup">11</span>C-choline in 6% of the patients and in 12% of the patients with <span class="elsevierStyleSup">68</span>Ga-DOTATATE. The percentages of detection rose up to 54% and 36%, respectively with PSA values between 1 and 5<span class="elsevierStyleHsp" style=""></span>ng/mL. Finally, percentages reached 72% and 80% (<span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE, respectively) with PSA value greater than or equal to 5<span class="elsevierStyleHsp" style=""></span>ng/mL.</p><p id="par0380" class="elsevierStylePara elsevierViewall">For <span class="elsevierStyleSup">11</span>C-choline PET/CT with PSA values less than or equal to 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL (sample median) at least one lesion characteristic of prostate cancer was identified in 25% of the patients, while 65.75% of the patients presented pathological lesions with PSA values greater than 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL. These results are similar to those reported in the literature.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">4,5,18,21,22,27,28</span></a></p><p id="par0385" class="elsevierStylePara elsevierViewall">Using the same threshold for <span class="elsevierStyleSup">68</span>Ga-DOTATATE, 21.8% of the patients presented at least one lesion with a PSA level of 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL or less. In addition, 75% of the patients presented pathological lesions with PSA values greater than 4.25<span class="elsevierStyleHsp" style=""></span>ng/mL.</p><p id="par0390" class="elsevierStylePara elsevierViewall">Although some studies have analyzed the SUVmax values of the <span class="elsevierStyleSup">68</span>Ga-DOTA-peptides in patients with neuroendocrine tumors,<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> no study has related SUVmax values to PSA levels in patients with prostate cancer.</p><p id="par0395" class="elsevierStylePara elsevierViewall">Our results demonstrate that the rate of detection of recurrent prostate cancer and metastasis improves with an elevation of the tumor marker. This closely coincides with previously reported data.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">21,27,28</span></a></p><p id="par0400" class="elsevierStylePara elsevierViewall">In addition, there was a correlation between the SUVmax values of <span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATE using a non parametric correlation coefficient (rho de Spearman) showing a statistically significant, albeit weak, correlation between the two radiotracers (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.41, <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>130, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001).</p><p id="par0405" class="elsevierStylePara elsevierViewall">However, <span class="elsevierStyleSup">11</span>C-choline detected a larger number of lesions compared to <span class="elsevierStyleSup">68</span>Ga-DOTATATE.</p><p id="par0410" class="elsevierStylePara elsevierViewall">The SUVmax of <span class="elsevierStyleSup">11</span>C-choline was significantly greater than that corresponding to <span class="elsevierStyleSup">68</span>Ga-DOTATATE for all the concordant lesions (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>130): 6.17 (1.70–15.50) and 4.38 (1.37–26.70), <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001; median (range), respectively.</p><p id="par0415" class="elsevierStylePara elsevierViewall">Our results are similar to previous reports in the literature.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">11,13–15,23,24</span></a></p><p id="par0420" class="elsevierStylePara elsevierViewall">The sensitivity and specificity per patient with the 95% CI were the same for both techniques: 0.82 (0.65–0.93) and 0.90 (0.73–0.98), respectively.</p><p id="par0425" class="elsevierStylePara elsevierViewall">Although there was a trend toward a lower sensitivity in patients with lower PSA levels, the difference was not statistically significant: 0.63 vs. 0.89; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.13; with the same values for both radiotracers for patients with PSA <4.25 and ≥4.25<span class="elsevierStyleHsp" style=""></span>ng/mL, respectively. This may have been influenced by the sample size, and the fact that most of the patients presented elevated PSA levels, and this may be a limitation of the study. Unfortunately, our study did not include enough patients with lower PSA levels.</p><p id="par0430" class="elsevierStylePara elsevierViewall">There were some discordant results (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). The different uptake between the radiotracers may be due to different factors such as, for example, previous therapies, the influence of androgen deprivation treatment, different histological variables of prostate cancer, different biological characteristics such as the expression of SSRT or choline transporter or tumoral factors, among others.</p><p id="par0435" class="elsevierStylePara elsevierViewall">Four patients with positive PET/CT results underwent surgery (patients number 7, 32 and 46 presented positive lymph nodes and patient number 19 showed uptake in the prostatic bed), and the histological results showed inflammatory alterations (<a class="elsevierStyleCrossRefs" href="#fig0015">Figs. 3 and 4</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0440" class="elsevierStylePara elsevierViewall">It has been shown that SSRT are also expressed in macrophages which play a role in inflammatory processes, and an increase in choline kinase activity of these cells can also be found when they are activated.</p><p id="par0445" class="elsevierStylePara elsevierViewall">The uptake of <span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE in inflammatory or infectious processes can limit the specificity of these techniques for the detection of occult metastatic disease and should be taken into account when deciding invasive manuevers.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">29</span></a></p><p id="par0450" class="elsevierStylePara elsevierViewall">Indeed, in a previous study we described the presence of possible false positive results with these techniques.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a></p><p id="par0455" class="elsevierStylePara elsevierViewall">This series of patients showed that both <span class="elsevierStyleSup">11</span>C-choline and <span class="elsevierStyleSup">68</span>Ga-DOTATATE PET/CT studies have the capacity to detect pathological lesions by different mechanisms in the study of occult BCR in prostate cancer, and this seeems to be dependent on the PSA level.</p><p id="par0460" class="elsevierStylePara elsevierViewall">For diagnosis, <span class="elsevierStyleSup">68</span>Ga-DOTATATE seeks to identify SSRT in prostate cancer. As a therapeutic application the same somatostatin analogs can be labeled with beta neative radioisotopes (<span class="elsevierStyleSup">177</span>Lutetium) with the aim of specifically irradiating these tumor cells.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a> Therapy with <span class="elsevierStyleSup">177</span>Lu-DOTATATE can be effective in patients with hormone-resistant prostate cancer who have no other therapeutic options and present sufficient uptake of <span class="elsevierStyleSup">68</span>Ga-DOTATATE in the tumoral lesions observed in the PET/CT.</p><p id="par0465" class="elsevierStylePara elsevierViewall">Taking into account the high coincidence between the two techqniues (albeit with <span class="elsevierStyleSup">11</span>C-choline being better in the level of uptake and the number of lesions detected), we believe that radioguided therapy would be feasible in these patients.</p><p id="par0470" class="elsevierStylePara elsevierViewall">In the present study, image acquisition was done 30<span class="elsevierStyleHsp" style=""></span>min post <span class="elsevierStyleSup">68</span>Ga-DOTATATE injection. Several studies have acquired images at 45–60<span class="elsevierStyleHsp" style=""></span>min post-injection. It would be of interest to evaluate whether the time of acquisition influences the results.</p><p id="par0475" class="elsevierStylePara elsevierViewall">Our study has limitations. It would have been useful to correlate the Gleason score with the results since it has been reported that neuroendocrine differentiation seems to be related to aggressiveness.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">9</span></a> Neither were data of specific tumor markers of neuroendocrine tumors such as chromogranine A available in our patients.</p><p id="par0480" class="elsevierStylePara elsevierViewall">Finally, it would have been interesting to carry out histological studies of the lesions in order to determine the true significance of <span class="elsevierStyleSup">68</span>Ga-DOTATATE uptake.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Conclusions</span><p id="par0485" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleSup">68</span>Ga-DOTATATE PET/CT and <span class="elsevierStyleSup">11</span>C-choline PET/CT studies seem to have a high capacity to detect pathological lesions in the evaluation of patients with prostate cancer in BCR.</p><p id="par0490" class="elsevierStylePara elsevierViewall">Further studies are needed with the aim of establishing the possible complementary clinical value of these PET/CT techniques and <span class="elsevierStyleSup">68</span>Ga-DOTATATE for the potential planning of therapies measured by SSRT (<span class="elsevierStyleSup">177</span>Lu-DOTATATE).</p></span><span id="sec0070"
