Although prosthesis can improve the quality of life, post-operative complications have been reported.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">1</span></a> While infection is rare, occurring in 1–2% of postoperative prosthetic surgeries,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">2</span></a> it is the most serious complication. Moreover, prosthetic joint infection is not easily distinguished from aseptic loosening of the prosthesis,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">3</span></a> but a correct diagnosis is crucial for proper treatment planning and a good treatment response.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">4</span></a> Laboratory tests, such as blood count, C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) are nonspecific.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">5</span></a> The specificity of invasive diagnostic methods, including blood and tissue cultures, is consistently high (>90%), whereas sensitivity is variable (28–92%).<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">6,7</span></a> X-ray imaging does not distinguish infection from aseptic loosening, similar to computed tomography (CT) and magnetic resonance imaging; this is because the metallic prosthesis creates intense artifacts.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">8</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">By contrast, radionuclide imaging methods play an important role in the diagnosis of prosthetic joint infection.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">8</span></a> Bone scintigraphy presents high sensitivity, albeit low specificity.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">9</span></a><span class="elsevierStyleSup">67</span>Ga-citrate, <span class="elsevierStyleSup">18</span>F-fluorodeoxyglucose (FDG), labeled monoclonal antibodies, and radiolabeled antibiotics, vitamins and antimicrobial peptides are the other agents used in the diagnosis of infections.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">9</span></a> Of these, radioactive labeled leukocyte scintigraphy (<span class="elsevierStyleSup">99m</span>Tc/<span class="elsevierStyleSup">111</span>In) is the most effective imaging technique for the diagnosis of prosthetic infections. Its accuracy, sensitivity and specificity for prosthetic infection and acute osteomyelitis are 90%,<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">10,11</span></a> 88% and 91%,<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">12–15</span></a> respectively. The most important factor in the efficacy of labeled leukocyte scintigraphy is the in vivo/in vitro labeling of leukocytes migrating to the site of infection.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">16</span></a> However, the presence of physiologic uptake of labeled leukocytes in the bone marrow and, on the other hand, insufficient anatomic localization are the disadvantages of radionuclide imaging. These drawbacks can be overcome by combining bone marrow scintigraphy and radioactive-labeled leukocyte scintigraphy. This approach allows the physiologic uptake of labeled leukocytes to be differentiated from the uptake in infection.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">17,18</span></a> Planar images may not be sufficient to detect infection, but the sensitivity of planar imaging can be increased using single-photon emission tomography (SPECT).<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">19</span></a> However, because SPECT imaging alone does not allow anatomic characterization, it should be combined with CT imaging. The latter also increases the accuracy of labeled leukocyte scintigraphy.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">20</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to evaluate the contribution of SPECT/CT imaging to <span class="elsevierStyleSup">99m</span>Tc-hexamethylpropyleneaminoxime (HMPAO)-labeled leukocyte scintigraphy for the diagnosis of infection in patients with suspected knee and hip infections related to prosthesis implantation.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patient population</span><p id="par0020" class="elsevierStylePara elsevierViewall">The 37 patients included in this retrospective study underwent <span class="elsevierStyleSup">99m</span>Tc-HMPAO-labeled leukocyte scintigraphy (<span class="elsevierStyleSup">99m</span>Tc-WBC) due to suspicion of knee/hip prosthesis infection. The 11 male and 26 female patients presented an age range from 38 to 84 years, with a mean (±SD) age of 65.7 (±5.6) years. Knee prosthesis had been implanted in 21 patients (56.7%), and a hip prosthesis in 16 (43.3%).</p><p id="par0025" class="elsevierStylePara elsevierViewall">All patients were assessed for clinical findings (swelling, temperature increase, redness, pain, etc.), and underwent clinical laboratory testing (leukocyte count, ESR and CRP values) and three-phase bone scintigraphy with <span class="elsevierStyleSup">99m</span>Tc-methyl diphosphate (MDP). In all patients, 3-phase bone scintigraphy showed activity involvement in the periprosthetic region and <span class="elsevierStyleSup">99m</span>Tc-WBC was performed for the differential diagnosis of infection/aseptic relaxation. In all patients antibiotic therapy was withdrawn at least 30 days before scintigraphy. The median time between arthroplasty and scintigraphy was 1 year (range 5 months to 5 years). Patients with suspected infection of the prosthesis site were referred for HMPAO-labeled leukocyte scintigraphy. The study was approved by the Faculty Ethics Committee of our institution (60116787-020/13945).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Imaging protocol</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Leukocyte labeling</span><p id="par0030" class="elsevierStylePara elsevierViewall">A 50<span class="elsevierStyleHsp" style=""></span>mL injection syringe containing 7<span class="elsevierStyleHsp" style=""></span>mL of acid citrate dextrose was used to withdraw 43<span class="elsevierStyleHsp" style=""></span>mL of blood, which was then transferred to a tube containing 10<span class="elsevierStyleHsp" style=""></span>mL of hydroxyethylstarch (Hespan). After 45<span class="elsevierStyleHsp" style=""></span>min of incubation at 37<span class="elsevierStyleHsp" style=""></span>°C, a cell-rich pellet and plasma could be seen in the top layer. The plasma was separated by centrifugation for 10<span class="elsevierStyleHsp" style=""></span>min at 1380<span class="elsevierStyleHsp" style=""></span>rpm. After the plasma was removed, the leukocytes in the pellet were mixed with 5<span class="elsevierStyleHsp" style=""></span>mL of saline and centrifuged for 5<span class="elsevierStyleHsp" style=""></span>min at 1480<span class="elsevierStyleHsp" style=""></span>rpm. Freshly obtained 30<span class="elsevierStyleHsp" style=""></span>mCi <span class="elsevierStyleSup">99m</span>Tc was labeled with HMPAO using a kit (Ceretec, Amersham Inc., London, UK). The label was added to the pellet generated by the latter centrifugation and the mixture was centrifuged for 5<span class="elsevierStyleHsp" style=""></span>min at 1650<span class="elsevierStyleHsp" style=""></span>rpm. The pelleted <span class="elsevierStyleSup">99m</span>Tc-HMPAO-labeled leukocytes were injected intravenously into the patient (mean, 8–12<span class="elsevierStyleHsp" style=""></span>mCi).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Imaging</span><p id="par0035" class="elsevierStylePara elsevierViewall">Planar and SPECT/CT imaging was conducted using a SPECT/CT device equipped with a dual-head hybrid gamma camera, and combined within the same gantry with a flat-panel CT with a low dose X-ray tube (2.5<span class="elsevierStyleHsp" style=""></span>mA, 120<span class="elsevierStyleHsp" style=""></span>kVp; Philips Brightview XBT; Cleveland, OH, USA).</p><p id="par0040" class="elsevierStylePara elsevierViewall">After the injection, planar images of the prosthetic area were obtained at 2, 4 and 24<span class="elsevierStyleHsp" style=""></span>h. The images were captured using a low-energy high-resolution collimator (140<span class="elsevierStyleHsp" style=""></span>keV photoelectric pic, 20% energy window), with a 128<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>128 matrix and 10-min counting. Additionally, a 10-min planar image of the thorax was obtained from each patient during the first hour of injection, to visually evaluate the binding activity of the labeled leukocytes based on their activity in the lungs. SPECT/CT images of the prosthetic area were obtained at 4<span class="elsevierStyleHsp" style=""></span>h after injection. CT images were generated using a 512<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>512 matrix with a slice thickness of 1<span class="elsevierStyleHsp" style=""></span>mm. The SPECT images were acquired using a 64<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>64 matrix (64 frames; 30<span class="elsevierStyleHsp" style=""></span>s per frame). Images in each plane were captured by turning the patient 180°. The images were reconstructed using a Butterworth filter and the filtered back projection method (cutoff, 0.5; order, 6) (AutoSPECT Pro; Philips Intellispace Portal). Attenuation correction was performed using transverse, sagittal and coronal CT imaging. The SPECT and CT images were fused to generate SPECT/CT images.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Analysis and statistics</span><p id="par0045" class="elsevierStylePara elsevierViewall">Three experienced nuclear medicine specialists evaluated the planar images and then the SPECT/CT fusion images. The information obtained from each imaging series was compared and evaluated. Infection at the peri-prosthesis site was defined as one or more foci of increased activity within the bone structure and soft tissue of the peri-prosthetic field.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Definitive diagnosis of prosthetic infection</span><p id="par0050" class="elsevierStylePara elsevierViewall">The diagnosis of prosthetic infection was confirmed surgically in 12 (32.5%) patients, and by cell culture following intra-articular fluid aspiration (IAFAC) in 11 (29.7%) patients. In the 14 (37.8%) patients who did not undergo surgery, and for whom no culture was obtained, the diagnosis was based on a long clinical follow-up by an experienced orthopedic specialist.</p><p id="par0055" class="elsevierStylePara elsevierViewall">True positive and true negative results on SPECT/CT, in cases diagnosed with infection, were determined by comparing the findings of planar and SPECT/CT images against the definitive diagnosis parameters. The utility of SPECT/CT for accurate anatomic localization of the involved areas, and differentiation of bone and soft tissue, was evaluated and the sensitivity, specificity, and positive/negative predictive values were calculated.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">There was increased <span class="elsevierStyleSup">99m</span>Tc-WBC activity in 29 of 37 patients. Scintigraphic findings in 17 of 29 patients determined prosthetic infection. While 16 of 17 patients had a definitive diagnosis of prosthesis infection, 1 patient had a diagnosis of non-infectious pathology (false positive). Scintigraphic findings did not determined infection in 12 of 29 patients. In all patients, definitive diagnosis was determined as non-infectious pathologies (7 aseptic loosening, 5 degenerative changes). There was no periprosthetic increased <span class="elsevierStyleSup">99m</span>Tc-WBC activity in 8 of 37 patients with increased activity in three-phase bone scintigraphy. Two of these 8 patients were definitely diagnosed with IAFAC and 6 of them were definitely diagnosed after the clinical follow-up as true negative for prosthetic infection (degenerative change) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><p id="par0065" class="elsevierStylePara elsevierViewall">Among all 37 patients, <span class="elsevierStyleSup">99m</span>Tc-WBC+SPECT/CT was true-positive for infection in 16 patients, and true-negative in 20 patients (<a class="elsevierStyleCrossRefs" href="#tbl0005">Tables 1 and 2</a>). Accurate anatomic localization of the foci positive for infection was consistently achieved with SPECT/CT. In the remaining patient, <span class="elsevierStyleSup">99m</span>Tc-WBC+SPECT/CT yielded a false-positive result for clinical infection based on focal uptake in the right femoral lesser trochanter area, as seen on planar images, and increased sclerosis and focal uptake in the same area on the SPECT/CT images (no. 17 of <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">The sensitivity, specificity, negative predictive value (NPV) and positive predictive value (PPV) were 100%, 59.1%, 100%, and 62.5% in the planar images obtained with <span class="elsevierStyleSup">99m</span>Tc-WBC scintigraphy, versus 100%, 90.1%, 100%, and 88.2% in those of planar<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>SPECT/CT imaging, respectively. SPECT/CT had no significant diagnostic benefit in 15 of the 37 patients (40.6%), but was of significant benefit in the remaining 22 patients (59.4%). No <span class="elsevierStyleSup">99m</span>Tc-WBC uptake was seen on the planar<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>SPECT/CT images of 10 of the 15 patients for whom SPECT/CT had no significant diagnostic benefit, and no infection was detected clinically. In the other 5 patients, the findings were similar between planar and SPECT/CT images. While SPECT/CT did not change the diagnosis, it accurately revealed the anatomic localization of all foci (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">In 12 of the 22 patients for whom SPECT/CT had significant diagnostic benefit, SPECT/CT supported the diagnosis of infection via accurate anatomic localization, improved visualization of foci not clearly identified on the planar images, and differentiation of infection in bone versus soft tissue. Infection was clinically proven in all of these patients.</p><p id="par0080" class="elsevierStylePara elsevierViewall">In 5 of the 22 patients, SPECT/CT provided a clearer picture of the peri-prosthetic localization and the patterns of activity in the involved sites on planar images. The findings initially suggested non-infectious causes, which correlated with the clinical diagnosis.</p><p id="par0085" class="elsevierStylePara elsevierViewall">In 5 of the 22 patients, the mild activity around the prosthesis seen on the planar images was not clearly observed on the SPECT/CT images. No infection was diagnosed in these patients, a result in accordance with the clinical diagnosis.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0090" class="elsevierStylePara elsevierViewall">Accurate diagnosis of a peri-prosthetic infection, including the exact location of the infected site, is necessary for appropriate treatment, but the most effective noninvasive diagnostic test is currently a matter of debate. <span class="elsevierStyleSup">99m</span>Tc and <span class="elsevierStyleSup">111</span>In-labeled leukocyte scintigraphy are considered to be the most effective methods, but variation in the sensitivity and specificity of labeled leukocyte scintigraphy has been reported.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">3,12–15,21–23</span></a> In our study, the sensitivity, specificity, NPV and PPV based on <span class="elsevierStyleSup">99m</span>Tc-WBC scintigraphic static images were 100%, 59.1%, 100% and 62.5%, respectively. In 16 of the 17 infection-positive patients, the infection was confirmed (true-positive), and only one patient had a non-confirmed infection (false-positive). Devillers et al. evaluated 116 patients with suspected infection: 74 with infection of the prosthesis site, 24 with suspected osteomyelitis, and 18 with suspected septic arthritis. The overall sensitivity and specificity were 95% and 90%, while in the prosthetic infection group the sensitivity and specificity were 97% and 89%.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">3</span></a> The latter, high sensitivity values were similar to those of our study. Glithero et al. reported a sensitivity for <span class="elsevierStyleSup">99m</span>Tc-WBC of 50% in their study of 52 patients with suspected infection of the prosthesis. The low sensitivity was attributed to possible chronic infection due to the long latency (0.83–7.02 years) between prosthesis surgery and imaging.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">21</span></a> The high sensitivity determined in our study can be explained by the fact that in all of our patients acute osteomyelitis was suspected based on clinical and laboratory findings. In the 324 patients with suspected osteomyelitis who were included in the study of Wolf et al., the sensitivity and specificity were 97.7% and 96.8%. The most sensitive method for the detection of acute osteomyelitis is labeled leukocyte scintigraphy, but the group in that study was heterogeneous (prosthetic infection, osteomyelitis, septic arthritis).<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">23</span></a> According to Acker et al., the sensitivity, specificity and PPV of <span class="elsevierStyleSup">99m</span>Tc-WBC scintigraphy, determined in 21 patients with knee prostheses, were 100%, 56%, and 42%, respectively.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">24</span></a> These results are also similar to those of our study. The low specify was due to the evaluation of all focal activity considered suspicious for infection. Better knowledge of the anatomic localization of the infection site will improve the specificity of <span class="elsevierStyleSup">99m</span>Tc-WBC scintigraphy.</p><p id="par0095" class="elsevierStylePara elsevierViewall">The insufficient anatomic localization obtained with planar images is compensated by hybrid imaging. Hybrid SPECT/CT systems, created by combining a low-dose X-ray tube with a variable angle dual head gamma camera,<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">25</span></a> generate anatomic and functional images during the same imaging session. Those images correctly reveal the anatomic location of the accumulated radioactivity. Hybrid SPECT/CT imaging is used in many clinical applications.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">25</span></a> The significant contribution of SPECT/CT has been shown by Mandegaran et al. In their study, the specificity of the examination was increased from 30% to 75% by SPECT/CT, which was added to the three-phase bone scintigraphy performed primarily to patients with suspected prosthetic infection.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">26</span></a> In this study, some SPECT/CT images showed that it made a significant contribution to the identification of infection in patients undergoing labeled leukocyte scintigraphy.<a class="elsevierStyleCrossRefs" href="#bib0295"><span class="elsevierStyleSup">27–30</span></a> In our study, SPECT/CT imaging was performed in all patients and provided additional anatomical information in 45.9%, with a significant diagnostic contribution seen in 22 patients (59.4%). Especially in cases of infection, SPECT/CT anatomically identified the infection site and differentiated between bone and soft tissues. In our study, the sensitivity, specificity, NPV and PPV of planar imaging was 100%, 59.1%, 100%, and 62.5%, with corresponding values for planar<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>SPECT/CT images of 100%, 90.1%, 100% and 88.2%. Thus, SPECT/CT significantly increased the specificity and PPV. In the study of Kim et al., 71 patients with hip prostheses and 93 with knee prostheses, the sensitivity, specificity, NPV and PPV of planar imaging was 82%, 88%, 80.5%, and 89%, versus 93.3%, 93.3%, 94.3%, 93.3%, respectively, for planar<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>SPECT/CT.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">27</span></a> While these findings are similar to those of our study, the sensitivity values differed, presumably because of the lower number of patients in our study. Bar Shalom et al. were able to localize all the areas of increased uptake defined on planar and SPECT images, and to differentiate bone from soft tissue on SPECT/CT images.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">29</span></a> In the study of Graute et al., the localization of foci of infection could be classified by SPECT/CT as peri-prosthetic, intra-prosthetic, or corticomedullary, which contributed to an exact diagnosis.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">30</span></a> In the study of Luca Filippi et al., SPECT/CT also located all positive foci in 28 patients with suspected osteomyelitis correctly (13 prosthetic and 15 non-prosthetic infections). SPECT/CT played a significant role in achieving a definitive diagnosis in 35.7% of the patients.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">28</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">The ability to differentiate increased uptake by bone versus soft tissue is important because the treatment of infections in these areas differs. Treatment also varies according to the location and extent of the infection, and may involve antimicrobial treatment alone, debridement with prosthesis protection, single-stage exchange arthroplasty, two-stage exchange arthroplasty and prosthesis resection, etc.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">31</span></a> Accurate determination of the surgical area shortens the duration of surgery and reduces the probability of postoperative complications.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">30</span></a> Kim et al. described a series of patients in whom appropriate treatment was made possible by correct anatomic localization of the infection site and bone-soft tissue differentiation, both achieved with SPECT/CT imaging. In addition, unnecessary surgery was eliminated in patients with infections of soft-tissue origin.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">27</span></a> In our study, the increased uptake seen in the lateral adjacent areas of the prosthesis on planar SPECT/CT images denoted soft tissue. These infections were treated with antibiotics and surgery was not required for any patient.</p><p id="par0105" class="elsevierStylePara elsevierViewall">In our study, SPECT/CT contributed significantly to the diagnosis of infection in 59.4% (22 of 37) of the patients, which was a higher proportion than reported in the literature. The planar images revealed infection in 12 of 22 patients, including the anatomic location of the infection with respect to non-prosthetic or prosthetic site, bone or soft tissue, and cortical or medullary bone separation. Furthermore, bone marrow activity could be excluded. In patients with a suspected infection despite negative results on planar images, but also in those with positive foci indicating infection, SPECT/CT has an important role in achieving a definitive diagnosis. However, Filippi and Horger did not find any evidence of a significant contribution of SPECT/CT in patients with infection-negative planar images, unlike in our study, in which SPECT/CT contributed significantly to a definitive diagnosis in patients with mild uptake on planar images. In 5 of the 22 patients, suspicious uptake on planar images was classified as infection-negative, because the SPECT/CT images suggested aseptic loosening and degenerative changes. There was no other evidence of prosthetic-site infection. Similarly, Kim et al. showed that identification of bone marrow activity in patients diagnosed as negative for infection, and the indication of aseptic loosening on CT, made a significant contribution to the clinical diagnosis of SPECT/CT.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">27</span></a> In the review of Van der Bruggen et al., it has been emphasized that there are not sufficient large prospective studies in the diagnosis of prosthetic infection. In a limited number of studies conducted in this area, it is stated that SPECT/CT makes a significant contribution especially to negative predictive value.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">32</span></a></p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusion</span><p id="par0110" class="elsevierStylePara elsevierViewall">SPECT/CT, as a hybrid imaging modality, contributes significantly to the diagnosis of prosthetic-site infection by providing anatomic and metabolic information. It increases the specificity and PPV of labeled leukocyte scintigraphy, by providing meaningful additional information. In addition, the ability of SPECT/CT to distinguish bone from soft tissue involvement, and to recognize physiological bone marrow activity, provides significant clinical benefit with respect to achieving a definitive diagnosis. Based on the findings of our study, to diagnose the patient with suspected prosthesis infection, we recommend to begin with a three-phase bone scintigraphy with SPECT/CT and then, to evaluate with leukocyte scintigraphy in cases that cannot be excluded. The aim of this retrospective study was to evaluate the usefulness of SPECT/CT performed simultaneously using a hybrid imaging device of prosthesis infections.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Materials and methods</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">99mTc-HMPAO scintigraphy was performed on 37 patients (11 men and 26 women; age range 38-84 years; mean age±SD, 65.7±5.6 years). Planar scans were acquired 2.,4. and 24. hour after injection. SPECT/CT was obtained 4 h after injection, using a dual-head hybrid gama camera coupled with a low-power x-ray tube. In all patients, scintigraphic results were matched with the results of surgery, cultures and clinical follow-up.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">Seventeen (45,9%) out of 37 patients had prosthesis infection and 20 (54,1%) out of 37 patients had non-infectious prosthesis pathologies with 99mTc-HMPAO scintigraphy and SPECT/CT. The 99mTc-HMPAO scintigraphy was true-positive for infection in 16 of 37 patients and true-negative in 20 of 37 patients. SPECT/CT provided an accurate anatomic localization of all positive foci. With regard to the final diagnosis, SPECT/CT added a significant clinical contribution in 22 of 37 patients(59,4%). Se obtuvieron imágenes planares 2, 4 y 24 horas después de la inyección. A las 4 horas se realizó SPECT/TC usando una gammacámara híbrida de doble detector con TC de baja energía. En todos los pacientes se compararon los resultados de la gammagrafía con los resultados de la intervención quirúrgica, el cultivo y el seguimiento clínico.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">De 37 pacientes estudiados con leucocitos marcados con 99mTc-HMPAO, con gammagrafía y SPECT/TC, diecisiete (45,9%) tenían infección protésica, y 20 (54,1%) tenían patologías protésicas no infecciosas. La gammagrafía con leucocitos marcados con 99mTc-HMPAO fue verdadero positivo para infección en 16 de 37 pacientes y verdadero negativo en 20 de 37 pacientes. La SPECT/TC proporciona una localización anatómica precisa de todos los posibles focos. Considerando el diagnostico final, la SPECT/TC aportó una contribución clínica significativa en 22 de 37 pacientes (59,4%).
