    "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Colorectal carcinoma &#40;CCR&#41; is the second cause of cancer and the fourth cause of mortality by cancer in Western countries<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>&#46; Around 20&#37; of patients with CCR will have metastasis at diagnosis&#44; with the liver being the most frequent site of metastasis&#46; Hepatic metastatic involvement is considered responsible for most deaths of patients with metastatic CCR<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2&#44;3</span></a>&#44; and is associated with a 5-year survival of only 11&#46;7&#37;<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">The only curative treatment of liver metastases in CCR is surgical resection&#46; The techniques associated with the greatest survival include liver metastatectomy and thermic ablation<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>&#44; although only 20&#8211;30&#37; of the cases are considered candidates for this type of therapy <a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6&#44;7</span></a>&#46; The remaining cases are considered irresectable or non treatable by the previous techniques due to multiple factors such as the localization of the metastasis&#44; the proximity to critical structures&#44; tumor size&#44; the number of lesions&#44; insufficient residual hepatic volume or the presence of extrahepatic disease<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>&#46; In this group of patients the implementation of multidisciplinary committees has been one of the most relevant methods for indicating the most adequate therapy<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">In the last two decades&#44; local therapies have been developed for the treatment of metastatic liver disease&#44; and they are emerging as a treatment option for both salvage therapy in the process of reducing the disease for allowing curative resection as well as palliative treatment of non resectable hepatic metastases&#46; One of these options is transarterial radioembolization&#44; better known as TARE&#44; which has demonstrated promising results as a tool for the management of non resectable liver metastasis in CCR<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>&#46; This therapy is indicated in chemorefractory patients with dominant liver disease&#44; alone or in combination with chemotherapy&#44; in patients who wish to avoid systemic chemotherapy or as radical lobectomy for hypertrophy of the hepatic remnant in potentially resectable patients<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">TARE is an increasingly used local therapy for the treatment of liver metastases of chemorefractory CCR and has been included in the clinical practice guidelines of the European Society of Medical Oncology<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>&#46; Radioembolization is based on the fact that the supply of blood to hepatic tumors is different from that to the normal hepatic parenchyma&#46; Primary and metastatic liver tumors receive most of the blood supply from the hepatic arteries&#44; in contrast to the normal hepatic parenchyme which is predominantly portal&#46; Consequently&#44; the injection of radioactive microspheress into the hepatic artery irrigating metastastic lesions determines that a major proportion of microspheres remain located in the tumoral microvasculature&#44; and thus&#44; it is the tumoral tissue that receives the greatest dose of radiation compared to non tumoral tissue<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13&#44;14</span></a>&#46; This is also due&#44; in part&#44; to the size of the microspheres which are sufficiently small to enter the tumoral vessels but too large to pass to the capillary bed and reach the venous circulation&#46; The tissue damage generated by the beta radiation of <span class="elsevierStyleSup">90</span>Yttrium &#40;<span class="elsevierStyleSup">90</span>Y&#41; is the fundamental reason for the efficacy of the treatment and its toxicity<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">The results obtained by different studies evaluating TARE have reported an overall survival &#40;OS&#41; ranging from 8 to 13 months<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14&#44;15</span></a>&#46; TARE has a series of advantages derived by the physical properties of the radionuclide itself as well as its short half life and low mean depth of penetration of only a few mm&#44; allowing the radiation to be trapped within the tumoral bed&#44; respecting the surrounding healthy parenchyma&#46; Data regarding its long-term safety are limited&#44; although previous studies have described bile duct damage as a possible complication&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">The selection of optimal patients for the treatment of TARE is key since this determines obtaining the best results possible with a minimum cost of medical care by avoiding non beneficial therapies&#46; That is why previous studies have focused on the identification of these parameters or biomarkers that can guide the selection of patients who are candidates to receive TARE&#44; albeit with controversial results<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>&#46;</p><p id="par0035" class="elsevierStylePara elsevierViewall">The objective of the present study was to analyze the therapeutic efficacy and safety of TARE with <span class="elsevierStyleSup">90</span>Y microspheres in patients with liver metastasis by CCR and study the prognostic value of different biomarkers&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0040" class="elsevierStylePara elsevierViewall">We performed a prospective&#44; longitudinal&#44; observational study including 23 patients with CCR liver metastasis who underwent a total of 30 consecutive TARE treatments from November 2015 to June 2020&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">All the patients were evaluated and proposed as candidates for TARE by a multidisciplinary team made up of oncologists&#44; radiologists&#44; nuclear medicine physcians and surgeons specialized in hepatobiliary surgery&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall">The inclusion criteria were&#58; age&#8239;&#62;&#8239;18 years&#44; metastatic liver disease diagnosed by imaging techniques &#40;computerized tomography &#91;CT&#93; or magnetic resonance &#91;MR&#93;&#41; Eastern Cooperative Oncology Group &#40;ECOG&#41; score between 0 and 1&#44; dominant hepatic metastatic disease&#44; refusal to undergo surgery and capacity to provide informed consent&#46; All the patients had undergone treatments for CCR and metastatic disease including&#58; surgery of the primary tumor&#44; systemic chemotherapy&#44; transarterial chemoembolization and&#47;or local ablation techniques&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">The exclusion criteria were&#58; previous treatment with external liver radiation&#44; presence of ascites or hepatic insufficiency &#40;serum bilirubin greater than 2&#8239;mg&#47;dl&#41;&#44; hepatopulmonary shunt &#62;20&#37; or dose absorbed in the lung greater than 30&#8239;Gy&#44; vascular anomal&#237;es not subsidiary of embolization and may condition the migration of particles labeled with <span class="elsevierStyleSup">90</span>Y to other localizations &#40;gallbladder&#44; stomach&#44; among others&#41; and tumoral volume calculated by CT or MR&#8239;&#62;&#8239;70&#37; of the total hepatic volume&#46; Prior to planning the procedure&#44; conserved liver function &#40;maximum aspartate aminotransferase &#40;AST&#41;&#47;alanine aminotransferase &#40;ALT&#41; limited up to 5-fold the maximum laboratory value and bilirubin&#8239;&#8804;&#8239;2&#8239;mg&#47;dl&#41; was determined in all the subjects by blood analysis 30 days prior to TARE&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">The study was carried out according to the prinicples of the Declaration of Helsinki&#44; and all patients provided signed informed consent&#46;</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Procedure</span><p id="par0065" class="elsevierStylePara elsevierViewall">After the patients had been evaluated by the multidisciplinary team and proposed as candidates for TARE&#44; accurate planning of the procedure was performed together with the inteventionist Department of Radiology with the aim of performing safe and effective administration of treatment&#46; In all cases&#44; the procedures was performed according to the following steps&#58; <ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0070" class="elsevierStylePara elsevierViewall">First&#44; 2&#8211;3 weeeks prior to TARE and before evaluation of the arterial tree in the CT study&#44; hepatic arteriography was performed to evaluate the arterial anatomy and define hepatic and tumoral vascularization&#46; Arterial vessels that could condition the <span class="elsevierStyleSup">90</span>Y spheres reaching extrahepatic organs &#40;fundamentally the gallbladder and the antropiloric gastric regions&#41; were embolized &#40;<a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a>&#41;&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0075" class="elsevierStylePara elsevierViewall">Thereafter&#44; in the same procedure&#44; 148&#8239;MBq &#40;4&#8239;mCi&#41; of <span class="elsevierStyleSup">99m</span>Tc- macroaggregated albumin &#40;<span class="elsevierStyleSup">99m</span>Tc-MAA&#41; were injected through selected arteries for the administration of the treatment&#44; obtaining whole body scintigraphic images and single photon emission computed tomography &#40;SPECT&#47;CT&#41; of the abdomen before 2&#8239;h after the injection to avoid the appearance of free technetium&#46; Planar scintigraphic images and SPECT&#47;CT were used to calculate the percentage of hepatopulmonary shunt and asssess the intra- and extrahepatic distribution of the particles to detect unidentified extrahepatic arterial communications in the hepatic arteriography&#44; evaluate the perfusion of the hepatic volume to treat &#40;objective&#41;&#44; calculate the tumoral&#47;non tumoral ratio &#40;TNR&#41; and estimate the dose absorbed by the lung &#40;<a class="elsevierStyleCrossRef" href="#fig0010">Fig&#46; 2</a>&#41;&#46; The activity of <span class="elsevierStyleSup">90</span>Y labeled spheres was estimated to achieve a dose absorbed by the liver to treatment &#8805;120&#8239;Gy&#46; Personalized dosimetry was calculated using the Simplicity&#174; software &#40;Mirada Medical&#41; &#40;<a class="elsevierStyleCrossRef" href="#fig0015">Fig&#46; 3</a>&#41;&#46; A hepatopulonary shunt &#62;20&#37; or a dose absorbed in the lung &#62;30&#8239;Gy &#40;50&#8239;Gy if the dose was accumualted in consecutive treatments&#41; was considered as a relative contraindication for performing the procedure due to the risk of radiation pneumonitis&#46; It may be necessary to repeat the procedure for planning posterior treatments&#44; estimate the accumulated dose at a pulmonary level or to reevaluate gastrointestinal diversion&#46; The patients were evaluated and discharged after 24&#8722;48&#8239;h with the indication of analgesia based on an alternating regimen of paracetamol and non steroid antiinflammatories according to the presence of pain&#46; In cases with bilobar involvement&#44; treatment planning was sequential&#44; first treating the lobe with the greatest tumoral load&#46;</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">-</span><p id="par0080" class="elsevierStylePara elsevierViewall">Later&#44; at 14&#8211;21 days&#44; TARE with glass beads labeled with <span class="elsevierStyleSup">90</span>Y &#40;Therasphere&#174;&#59; Boston Scienti&#64257;c&#46; Massachusetts&#44; USA&#41; was performed&#46; The dosimetry was based on the characteristics of the tumors according to institutional protocols of radiological safety&#46; The focus of treatment was lobar or selective&#46; Patients with unilobar disease underwent a single TARE&#44; and those with bilobar disease underwent two sessions of TARE separated by 2 weeks&#46; In general&#44; the patients were discharged after 24&#8722;48&#8239;h&#46;</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">-</span><p id="par0085" class="elsevierStylePara elsevierViewall">Finally&#44; a <span class="elsevierStyleSup">90</span>Y PET&#47;CT study was performed in all the cases in order to visualize the distribution of the <span class="elsevierStyleSup">90</span>Y labeled spheres in the hepatic volume to treat and rule out the presence of extrahepatic foci &#40;<a class="elsevierStyleCrossRef" href="#fig0020">Fig&#46; 4</a>&#41;&#46;</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia></li></ul></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Follow-up and evaluation of response</span><p id="par0090" class="elsevierStylePara elsevierViewall">All the patients were evaluated from the beginning of treatment with TARE until the last clinical examination or death&#46; The clinical history and laboratory data prior to TARE were collected from the electronic registry of the health history of the patients&#46; The follow-up visit was carried out 30 days after the completion of TARE and included a CT study and full blood analyses including the hepatic profile&#46; Afterwards&#44; the patients were followed at 3&#44; 6 and 12 months&#46; Serum AST&#44; ALT alkaline phosphatase&#44; total bilirubin and albumin were studied as well as a complete hemogram that allowed calculating the neutrophil to lymphocyte ratio &#91;NLR&#41; and the platelet to lymphocyte ratio &#91;PLR&#41; before and after TARE&#46; Serological toxicities were registered and evaluated using the Common Terminology Criteria for Adverse Events &#40;CTCAE version 5&#46;0&#41;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> of the five grade Cancer Therapy Evaluation Program&#44; defining grade 3 as severe hepatotoxicity &#40;AST&#44; ALT or alkaline phosphatase &#8805;5-fold the upper limit of the normal range or evaluation of total bilirubin &#8805;3-fold the upper limit&#41;&#46;</p><p id="par0095" class="elsevierStylePara elsevierViewall">For evaluating response to treatment with TARE the traditional crieria of radiological response &#40;WHO and RECIST 1&#46;1&#41; based on the reduction of the number and tumoral size are of limited utility<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>&#46; Thus&#44; to evaluate response to TARE we used both the RECIST 1&#46;1 &#40;reduction of the number&#41; and the mRECIST criteria &#40;reduction of contrast uptake&#41;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>&#46; Response to TARE can be extended to up to 6 months after its administration&#44; and therefore&#44; hepatic or MR studies were performed in the follow-ups at 3 and 6 months after TARE and were compared with the studies prior to TARE &#40;pre-TARE&#41; applying the RECIST&#41; 1&#46;1 and mRECIST criteria of evaluation of response in solid tumors <a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>&#46; The radiological imaging tests were reviewed by 2 experienced radiologists &#40;PPM&#44; JJCD&#41;&#44; resolving any disagreements by consensus when necessary&#46;</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Variables of efficacy</span><p id="par0100" class="elsevierStylePara elsevierViewall">The variables of response included&#58; &#40;a&#41; OS defined as survival in months since TARE until the last clinical evaluation or patient death&#59; &#40;b&#41; progression-free survival &#40;PFS&#41; defined as the time in months from TARE until evidence of hepatic disease progression in imaging techniques&#59; &#40;c&#41; objective response rate &#40;ORR&#41; defined as the proportion of patients achieving complete &#40;CR&#41; or partial response &#40;PR&#41; based on the RECIST 1&#46;1 and mRECIST criteria of response&#44; and &#59; &#40;d&#41; the disease control rate &#40;DCR &#41;&#44; defined as the proportion of patients who achieved CR or PR or stablization of the disease as the best tumoral response during the study based on the RECIST 1&#46;1 and mRECIST criteria of response<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>&#46;</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0105" class="elsevierStylePara elsevierViewall">For the descriptive analysis central tendency &#40;mean&#41; and dispersion &#40;standard deviation&#44; range&#41; were used for continuous variables&#44; and the distribution of absolute and relative frequencies was used for categorical variables&#46; For comparison of quantitative data between the two groups of ORR and DCR&#44; the Student&#8217;s t test was used for independent parametric data while the Mann&#8211;Whitney U test was used for non parametric data&#44; and the association with categorical variables was evalauted using contingency tables applying the Chi-square or Fisher&#8217;s test&#46; The probability of OS and PFS was obtained with the Kaplan-Meier method&#46; Cox regression was used to study the association of the different independent variables with OS and PFS&#46; The data were processed using IBM SPSS Versi&#243;n 24&#46;0 software &#40;IBM Corp&#44; Armonk&#44; New York&#41;&#46; A p value &#8804;0&#46;05 was considered statistically significant&#46;</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Study population</span><p id="par0110" class="elsevierStylePara elsevierViewall">The study included 23 patients &#40;13 men and 10 women&#41; who underwent a total of 30 radioembolization procedures with <span class="elsevierStyleSup">90</span>Y labeled spheres or TARE&#46; The mean age was 61&#46;61&#8239;&#177;&#8239;9&#46;13 years &#40;range&#58; 45&#8211;74 years&#41;&#46; The mean follow-up was 10&#46;07&#8239;&#177;&#8239;7&#46;54 months &#40;range&#58; 1&#46;0&#8211;29&#46;0 months&#41;&#46; Sixteen patients &#40;69&#46;6&#37;&#41; were in functional ECOG stage 0&#46; Of the 30 procedures carried out&#44; 23 &#40;76&#46;7&#37;&#41; treated synchronous liver metastasis&#44; with an hepatic tumoral load &#60;50&#37; in 86&#46;9&#37; of the cases&#46; The mean interval between the diagnosis of CCR and TARE was 25 months &#40;range&#58; 10&#8211;66 months&#41;&#46; Sixteen patients &#40;69&#46;6&#37;&#41; were treated with a single TARE session&#44; while the remaining 7 &#40;30&#46;4&#37;&#41; required 2 sessions due to lesions in both hepatic lobes&#46; Only one case underwent re-treatment&#46; <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the baseline demographic and clinical characteristics and those related to treatment in the study population&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Local tumoral response</span><p id="par0115" class="elsevierStylePara elsevierViewall">Tumoral response was evaluated at 10&#8211;12 weeks after TARE&#46; Based on the RECIST v&#46;1&#46;1 and mRECIST criteria&#44; stablization was observed in 36&#46;7&#37; of the lesions treated&#44; PR was found in 13&#46;3&#37;&#44; with CR in 3&#46;3&#37; and disease progression in the remaining 46&#46;7&#37;&#46; Thus&#44; the ORR &#40;CR or PR&#41; at 12 weeks was 16&#46;7&#37; and the DCR &#40;PR&#47;CR or stable disease&#41; was 53&#46;3&#37;&#46; Six months after TARE&#44; stabilization was observed in 20&#37; of the lesions treated&#44; while the remaining 80&#37; showed progression&#44; with neither PR nor CR being observed&#44; Therefore&#44; at 6 months the DCR was 20&#37; &#40;see tumoral response in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">The ORR at 3 months&#44; Two patients presented late complications &#40;8&#46;7&#37;&#41;&#58; one case developed grade 3 afebrile neutropenia which was successfully resolved and the other presented hepatic failure induced by TARE which resolved within 72&#8239;h&#46;</p></span></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Discussion</span><p id="par0145" class="elsevierStylePara elsevierViewall">This study analyzed the different clinical&#44; analytical and genetic biomarkers of imaging related to metastatic liver disease in CCR and therapy with TARE associated with therapeutic response&#44; OS and PFS&#46;</p><p id="par0150" class="elsevierStylePara elsevierViewall">The role of TARE in the management of liver metastases in CCR is being fully developed<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a>&#46; The literature has publised several clniial trials studying the combination of TARE with different lines of chemotherapy&#44; showing slight improvement in OS depending on the inclusion phase&#44; achieving increases of up to 2 months&#46; At present&#44; the ideal time for including TARE combined with systemic chemotherapy remains unclear <a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20&#44;21</span></a>&#46; Additionally&#44; some studies have shown that the inclusion of TARE together with first line mFOLFOX6 chemotherapy in patients with right hepatic tumors achieves a significant increase in survival of greater than 4 months versus chemotherapy alone<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a>&#46; It remains to be seen which prognostic biomarkers should be taken into account for better selection of patients who would benefit from the combination of the two therapies in terms of an increase in survival&#46;</p><p id="par0155" class="elsevierStylePara elsevierViewall">There are few studies on the biomarkers of response to TARE&#44; and most are described for the treatment of hepatocellular carcinoma&#46; The results obtained in different studies and with different populations are quite heterogeneous with respect to the ORR at 3 months&#44; ranging between 0&#8211;71&#37;<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20&#44;21&#44;23&#8211;25</span></a>&#44; likely due to the heterogeneity of the population included and the time at which TARE was indicated&#46; In our case&#44; the ORR at 3 months was 16&#46;7&#37;&#44; being at the lower limit described in other studies&#46; The biomarkers showing a statistically significant association with the ORR at 3 months were age at diagnosis&#44; previous treatment with bevacizumab&#44; plasma hemoglobin levels prior to the procedure and previous NLR&#44; while those for DCR at 3 and 6 months included previous treatment with bevacizumab&#44; albumin&#44; NLR prior to TARE and dose absorbed by the tumor &#62;115&#8239;Gy&#46; Most of these biomarkers have been associated in previous studies on biomarkers of therapeutic response&#46; Of note is the review by Dendy <span class="elsevierStyleItalic">et al</span>&#46;<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>&#44; which describes that the tumoral dose absorbed as well as lymphocyte depletion &#40;leading to an increase in NLR&#41; and elevated serum amylase levels are associated with worse response&#46; These results contribute to sustaining the scientific evidence on clinical prognostic factors that should be taken into account for improving the selection of patients who are candidates to <span class="elsevierStyleSup">90</span>Y TARE&#46;</p><p id="par0160" class="elsevierStylePara elsevierViewall">With regard to survival&#44; the results of the different studies and systematic reviews in the literature are more homogeneous&#44; with a median OS of between 8 and 13 months and a PFS of between 3&#8211;16 months<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a>&#46; In this sense&#44; our results are similar to those previously described&#44; with a median OS of 12 months and PFS of 3 months&#46; Our results suggest that resection of the primary tumor&#44; absence of KRAS oncogene mutations&#44; hematological status prior to TARE and early DCR at 3 months are associated with a greater OS&#44; while biomarkers associated with PFS include the presence of synchronous liver metastasis at the diagnosis of CCR&#44; extrahepatic disease at the time of TARE&#44; the development of complications following TARE&#44; total bilirubin after TARE and the DCR at 3 and 6 months&#46; These findings are in accordance with those reported in previous studies and even in other types of tumors such as the case of cholangiocarcinoma&#44; since they translate into the presence of a more advanced and likely more aggressive disease<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13&#44;26&#44;27</span></a>&#46;</p><p id="par0165" class="elsevierStylePara elsevierViewall">The choice of adequate treatment protocols remains to be determined&#44; requiring the establishment of both the time of inclusion of TARE therapy and the type of treatment&#46; This has been demonstrated in some multicenter studies evaluating the combined effect of treatment with TARE with second lines of chemotherapy<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> or studies evaluating selective treatment &#40;segmentectomy&#41;<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a>&#46; Nonetheless&#44; the ideal time to treat hepatic metastases of CCR by TARE in the therapeutic algorithm for CCR continues to be uncertain&#46;</p><p id="par0170" class="elsevierStylePara elsevierViewall">No acute complications were registered during the TARE procedure&#46; After the procedure 2&#47;23 patients &#40;8&#46;7&#37;&#41; presented a late complication related to the procedure&#46; One case developed grade 3 afebrile neutropenia which was uneventfully solved&#44; while the other patient developed a self-limiting post-radioembolizationsyndrome &#40;radioembolization induced liver disease&#41; at 72&#8239;h&#44; reflecting the safety of the technique and being similar to the results ofother studies in the literature<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a>&#46;</p><p id="par0175" class="elsevierStylePara elsevierViewall">The main limitation of this study lies in the limited number of cases included and the lack of a control group&#46; Therefore&#44; our study focused on results less prone to being misinterpreted and biases&#44; in order to determine survival&#44; therapeutic response and treatment safety&#46; The results of the study may be confounded by the variability in the number of lines of treatment prior to TARE&#46; However&#44; despite the differences in oncological management of the underlying disease&#44; the results of survival and rate of therapeutic response are concordant with those reported in previous studies&#46; Another limitation of the study is the short follow-up period which limits evaluation of long-term survival&#46; Nonetheless&#44; the results of this study may be useful to establish the bases of future studies with a longer study period which will clarify the impact of biomarkers in relation to the selection of a subgroup of patients who would most benefit from TARE&#46;</p><p id="par0180" class="elsevierStylePara elsevierViewall">In conclusion&#44; in heptatic metastatic disease in CCR TARE is a safe and effective technique&#46; In our study&#44; the biomarkers able to predict the prognosis and therapeutic response to TARE include biochemical parameters as well as factors related to the primary tumor and with estimation of tumoral dosimetry&#46;
