It is a global health problem and resulted in 884,400 deaths from complications of chronic hepatitis in 2015 alone. Around 257 million people are known to be living with hepatitis B virus (HBV) infection.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The worldwide prevalence of HBV infection in the general population was 3.5% in 2015. The proportion of chronic HBV infection remains high among people born before the HBV vaccine became available, and even higher in the African (6.1%) and Western (6.2%) regions,<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> with high prevalence in regions such as Western Europe, China, and Taiwan.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–4</span></a> In Latin America, the prevalence of HBV infection is low to intermediate.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Peru has an average prevalence of HBsAg carriers of 1%–2%. However, in some areas of the Peruvian Amazon there is high endemicity and it persists as a public health problem.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6–9</span></a> Endemicity is medium to high in some parts of the Peruvian Andes, such as Abancay and Huanta.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Many countries have gradually adopted HBV vaccination in national childhood immunisation programmes on the recommendation of the World Health Organisation (WHO). Due to the importance of vaccination and the vulnerability of Peru’s indigenous peoples to HBV, the HBV vaccine has been incorporated into the expanded programme on immunisation (EPI) for children under one year of age living in areas of intermediate and high endemicity since 1996 and the vaccine has been made universal for children under one year of age throughout the national territory since 2003.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> On this basis adolescents over 14 years of age and adults would remain unprotected in most of the country.</p><p id="par0025" class="elsevierStylePara elsevierViewall">HBV vaccination triggers an immune response, generating antibodies against hepatitis B surface antigen (anti-HBs). Values equal to or greater than 10 mIU/mL confer seroprotection against HBV infection. The administration of HBV vaccine in 3 doses (at 0, 1 and 6 months) is the standard considered to achieve seroprotection.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> The efficacy of vaccination in children has been extensively studied; nationally the prevalence of protective antibodies in children aged 1–4 years against HBV was between 95%–97%.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> It is known that the younger the age of vaccination, the higher the levels of protective antibodies. In a study conducted in a population with an average age of 23.5 years, 100% seroprotection was achieved with the third dose of the vaccine, with average antibody levels ranging from 532.7 mIU/mL to 1237.2 mIU/mL.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Another national study evaluating HBV vaccine seroprotection in susceptible health care workers under 40 years of age, found 97% seroprotection at the end of an unconventional 0-1-2-month schedule.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">A study conducted in the USA found a marked variation in the prevalence of hepatitis B among different age groups, the prevalence among adults aged 50 years and older being 1.7 and 12.8 times higher than in individuals aged 20–49 years and 6–19 years, respectively.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Older adults (≥50 years) can be severely affected by HBV, with consequences such as fulminant hepatitis, liver cirrhosis and hepatocellular carcinoma.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> Immunosenescence results in both increased susceptibility to infection and reduced response to vaccination; levels of anti-HBs tend to decline significantly with age, from 33% to 60% in those aged over 60 compared to younger adults.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18</span></a> Despite the extraordinary efficacy of second-generation HBV vaccines, immunisation can fail due to multiple factors.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">19,20</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">One meta-analysis observed that many factors influence HBV vaccine response, especially at older ages. However, over the last decade, numerous emerging reports focusing on the HBV vaccine seroprotection rate in adults are still inconclusive on the age population that require increased seroprotection.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a></p><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Study objective</span><p id="par0040" class="elsevierStylePara elsevierViewall">The primary aim of the study was to analyse risk factors associated with the non-development of seroprotection against hepatitis B virus in a population of adults titrated to anti-HBs.</p></span></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Material and methods</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Type of study</span><p id="par0045" class="elsevierStylePara elsevierViewall">We conducted an analytical observational case-control study in a population of patients treated at a Level III hospital centre between 2015 and 2016, who were titrated to anti-HBs.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Selection criteria</span><p id="par0050" class="elsevierStylePara elsevierViewall">The inclusion criteria were patients over 18 years of age, of both sexes, vaccinated against HBV whose last dose of the vaccine had been administered more than one year previously, aged 18 years or older at the time of vaccination and registered in the computerised vaccination system (SISVAC), who had an anti-HBs titre measured in the period from January 2015 to December 2016, and without chronic hepatitis B or with cured or resolved hepatitis B, as defined by the presence of HBsAg or the presence of anti-HBs prior to vaccination. The exclusion criteria were any patients with incomplete information in their medical records.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Definition of cases and controls</span><p id="par0055" class="elsevierStylePara elsevierViewall">The cases were defined as any patient with an anti-HBs titre <10 mIU/mL (without seroprotection) and the controls were patients with an anti-HBs titre ≥10 mIU/mL (with seroprotection).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Definition of variables</span><p id="par0060" class="elsevierStylePara elsevierViewall">The following variables were included: age at vaccination (as a continuous variable), as well as categorised age (cut-off points at 40 and 60 years), and sex. Human immunodeficiency virus (HIV) infection, chronic kidney disease on haemodialysis (CKD-HD), and type 2 diabetes mellitus (DM2) were considered comorbidities present at the time of vaccination. The diagnosis of HIV infection was made by third or fourth generation ELISA and a positive test confirming the infection; CKD-HD was categorised according to whether the patient was on the haemodialysis programme for patients with chronic kidney disease. DM2 was diagnosed by fasting blood sugar, glucose tolerance test, or glycosylated haemoglobin.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The type of activity variable was included because most of the vaccinated population were military personnel, assessing whether they belonged or had belonged to the Peruvian Air Force as military personnel (not civilian attachés). The variable “time since first vaccination” was the time in years from the first dose of HBV vaccine to anti-HBs titration. The number of vaccine doses was stratified into 1, 2 or 3 doses.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Sample</span><p id="par0070" class="elsevierStylePara elsevierViewall">We chose to include all patients vaccinated against HBV at the Central Hospital of the Peruvian Air Force (Hospital Central FAP), a level III hospital in the city of Lima, over the period 2015–2016, and achieved 39 cases and 166 controls. A post hoc analysis of this sample showed that this number and proportion of patients gave the study a power of 91% at a significance level of .05 to be able to run two-tailed statistical tests, in relation to the main variables of analysis (age at vaccination and number of doses). This confirms that the sample included is adequate for the type of design and analysis proposed. The post hoc analysis was performed with G*Power statistical software (Heinrich Heine University, 2020, Düsseldorf, Germany).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Procedures</span><p id="par0075" class="elsevierStylePara elsevierViewall">The data of the participants were identified through the computerised vaccination system of the epidemiology department of Hospital Central FAP, including anti-HBs and HBV antibody titres (HBsAg, anti-HBc), dates of all HBV vaccination doses, as well as the comorbidities DM2 and CKD-HD. The diagnosis of HIV infection had to be corroborated by a member of the Local STI/HIV-AIDS Prevention and Control Strategy of Hospital Central FAP, as well as by the study's research infectiologist by verifying the result of the third or fourth generation ELISA test, as well as a positive test confirming infection. The patient's confidential data were safeguarded at all times. The procedure for anti-HBs titration used chemiluminescence microparticle assay (CIMA). The vaccine administered to participants is a recombinant DNA (rDNA) vaccine containing the purified surface antigen of the virus obtained by genetically engineered cell culture from the yeast Hansenula polymorpha, which meets WHO requirements (TRS 786) and is produced by the Serum Institute of India Ltd., Hadapsar, Pune 411028, India. The adult vaccine was used, in which each 1 mL intramuscular dose contains 20 mcg of purified hepatitis B surface antigen.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Ethical aspects</span><p id="par0080" class="elsevierStylePara elsevierViewall">The data obtained from the records did not include the names of the research subjects. The names of the participants were known only to one of the researchers, and the confidentiality of the individual study subjects was maintained at all times. Due to the study methodology, this did not pose any additional risk to the subjects included. The study was approved by the Institutional Ethics Committee of Hospital Central FAP.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Statistical analysis</span><p id="par0085" class="elsevierStylePara elsevierViewall">The data were processed in STATA version 14 (StataCorp., 2015, TX, USA). A descriptive analysis of the variables was performed using frequencies and percentages for categorical variables and by determining the median and interquartile range for numerical variables, as the latter were not normally distributed. A bivariate analysis was also performed, using the χ2 test for categorical variables and the Mann-Whitney <span class="elsevierStyleItalic">U</span> test for numerical variables. Statistical analyses were performed at a significance level of .05.</p><p id="par0090" class="elsevierStylePara elsevierViewall">To quantify the strength of association, the crude odds ratio (OR) was calculated for each variable in relation to seroprotection status, presenting the respective confidence interval (95% CI). The Mantel-Haenszel test was also used to assess confounding variables.</p><p id="par0095" class="elsevierStylePara elsevierViewall">The binary logistic regression method was used to determine whether or not the different variables under study were a risk factor for the non-development of HBV seroprotection, including the variables that were significant in the bivariate analysis using stepwise regression (forward/backward), using the forward stepwise method. To determine the model, variables with a <span class="elsevierStyleItalic">P</span> < .05 value were considered significant. The magnitude of risk among the variables considered risk factors for the non-development of seroprotection was calculated using the adjusted OR for each variable. Two logistic regression models were considered, the first including all the covariates that were significant in the bivariate analysis, and a second model only with the variables that showed a predictive value when adjusted in model 1. A graph of the estimated probabilities according to the final model was generated, as well as the ROC curve for this model.</p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Results</span><p id="par0100" class="elsevierStylePara elsevierViewall">The study sample comprised 205 participants. The median anti-HBs titres in the seroprotected patients were 520 mIU/mL (IQR 128.81−1000 mIU/mL) compared to the non-seroprotected patients, where the median was 1.34 mIU/mL (IQR .41–3.8 mIU/mL). Of the 205 participants, 39 had no HBV seroprotection (cases), and 166 had HBV seroprotection (controls).</p><p id="par0105" class="elsevierStylePara elsevierViewall">Regarding sex, 77.7% were male in the case group and 66.7% in the control group, no difference was found in terms of sex between the groups (<span class="elsevierStyleItalic">P</span> = .148).</p><p id="par0110" class="elsevierStylePara elsevierViewall">The median age for the seroprotected group of participants was 41 years (IQR 30−51 years), in contrast to the non-seroprotected group, whose median age was 54 years (IQR 47−66 years). There was a difference between the two groups in terms of age (<span class="elsevierStyleItalic">P</span> ≤ .001), <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">When categorising the participants’ ages into 18−39 years and ≥40 years, we found that 54.8% of HBV seroprotected patients were ≥40 years old, compared to the non-seroprotected group, where 79.5% of the patients were in this age range, there being a difference between both groups (<span class="elsevierStyleItalic">P</span> = .005). Moreover, we found an OR of 3.19 (1.32−8.49) for not developing seroprotection for those aged ≥40 years compared to those aged 18−39 years. Likewise, when categorising age into 18−59 and ≥60, we found that 7.2% of the seroprotected patients were ≥60 years old, compared to the non-seroprotected group, where 41.03% of the patients were in this age range, there being a difference between the two groups (<span class="elsevierStyleItalic">P</span> < .001) with an OR of 8.93 (3.42−23.3) for not developing seroprotection for those ≥60 compared to those aged 18−59 years.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Nine percent of the HBV seroprotected patients had a diagnosis of HIV compared to the non-seroprotected patients, where only 5.1% had this diagnosis, which did not exist between groups (<span class="elsevierStyleItalic">P</span> = .425). On the other hand, of those seroprotected against HBV only 2.4% had a history of CKD-HD, in contrast to the non-seroprotected patients, where 17.9% had this condition (<span class="elsevierStyleItalic">P</span> = .001), with an OR of 8.85 (2.07–43.1) for not developing seroprotection in those with a history of CKD-HD compared to those without the condition.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In the HBV seroprotected group, 81.9% of the patients received three doses of HBV vaccine, 9.6% received two doses, and 8.4% received only one dose; whereas in the non-seroprotected group, 61.5% of the patients received three doses, 7.7% received two doses and 30.8% received only one dose. When categorising the number of doses into 1 dose and ≥1 dose, we found that only 8.4% achieved seroprotection in the first group, in contrast to the second group where 30.8% achieved seroprotection, there being a difference between both groups (<span class="elsevierStyleItalic">P</span> ≤ .001). Moreover, we found an OR of 4.82 (1.8–12.5) for not developing seroprotection in those who had received only one dose, compared to those who had more than one dose.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Of those seroprotected against HBV, 10.2% had a diagnosis of DM2 compared to those who were not seroprotected, where the proportion was 33.3%, with a difference between the two groups (<span class="elsevierStyleItalic">P</span> ≤ .001, with an OR of 4.38 (1.72−10.8) for not developing seroprotection in those with DM2 compared to those without the condition.</p><p id="par0135" class="elsevierStylePara elsevierViewall">We should mention that the vaccination schedule used was 0, 1 and 6 months, although in the study population these times between doses were not strictly complied with, with a gap between the first and second dose of around 1.6 months (IQR 1.0–2.4), and between the second and third dose a median of 5.2 months (IQR 4.2−9.0). This finding occurred in both the seroprotected and non-seroprotected groups, with no difference between the two.</p><p id="par0140" class="elsevierStylePara elsevierViewall">On the other hand, categorising the time from the start of vaccination to the date of HBV antibody titration as greater than or equal to 7 years and less than 7 years showed that in 55.4% of the HBV seroprotected patients the time from the start of vaccination to titration of their antibodies was 7 years or more, unlike the non-seroprotected patients where the proportion of those at 7 years or more was 30.8%, with a difference between groups (<span class="elsevierStyleItalic">P</span> = .006), with an OR of .36 (.15−.79) for not developing seroprotection.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Similarly, the median time from first dose to anti-HBs titration in the controls was 7.33 years (IQR 2.9–8.64) compared to the case group where it was 3.55 years (IQR 1.77–7.88), with a difference between the two groups (<span class="elsevierStyleItalic">P</span> = .036). The median time for the second and third dose to anti-HBs titration showed no difference between cases and controls (<span class="elsevierStyleItalic">P</span> = .273 and <span class="elsevierStyleItalic">P</span> = .607, respectively).</p><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Multivariate model</span><p id="par0150" class="elsevierStylePara elsevierViewall">Having analysed the variables that were significant in the bivariate analysis, a first model (model 1) was generated with all the significant variables according to the results of the bivariate analysis (CKD on haemodialysis, number of doses, age at vaccination, both in categorical and continuous variables, DM2 and time from the first dose to anti-HBs titration). The reason for including this first model was to analyse by adjusting for all initially significant variables to exclude non-significant variables in the final model. However, variables such as CKD-HD, DM2, time from first dose to anti-HBs titration did not show a consistent adjusted OR that would predict non-development of seroprotection, and therefore model 2 was generated only with the number of doses (1dose vs. >1 dose) and age at vaccination variables, these being the only variables capable of predicting the development of seroprotection in vaccinated adult subjects. The adjusted OR for number of doses was 2.74 for those who had more than 1 dose (CI 1.03−7.27) and the adjusted OR for age at vaccination was 1.05 (CI 1.02−1.08) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). This logistic model to assess the probability of not developing HBV seroprotection in vaccinated adult subjects is 74% explained by the number of vaccination doses (1 dose vs. >1 dose) and age at vaccination, as shown by the ROC curve for the proposed model (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), demonstrating that they are independent predictors for not developing seroprotection. Since age was shown to be an important independent predictor, an estimated variance of the probability of non-seroprotection for HBV and its confidence intervals was generated according to this variance (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Discussion</span><p id="par0155" class="elsevierStylePara elsevierViewall">Age and number of HBV vaccine doses (>1 dose) were predictors for the development of HBV seroprotection in the adult vaccinated population; both showed high predictive capacity according to the study findings.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Although decreasing seroprotection responses have been reported as the age at vaccination increases, it is not known at what age immunogenicity begins to decrease.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> It is important to note that age is highly relevant as a predictor for seroprotection in this population, as it is a non-modifiable factor. In this regard, our results show that, when assessing variation in age, we would have a .7% increase in the probability of not developing seroprotection for each additional year.</p><p id="par0165" class="elsevierStylePara elsevierViewall">Although it is true that the multivariate analysis did not show predictability in seroprotection for ages categorised above 40, these patients had a higher risk of not developing seroprotection compared to the young adults. This finding is supported by evidence gathered from several studies that looked at age as a risk factor, evidence synthesised in a meta-analysis which reported that adults over 40 are 85% more likely not to develop seroprotection than younger adults.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> Other studies that only included adults under 40 years of age did not assess the relationship of age with level of seroprotection;<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a> however, more recent studies, that included older adults, have shown that the vaccine is more effective when given at a younger age.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> One study also suggests that the lower response to HBV vaccination in adults over 60 years of age may be due to decreasing immunity with age, with a seroprotection rate of only 22.5% in this age group.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a></p><p id="par0170" class="elsevierStylePara elsevierViewall">Although male sex and comorbidities have been demonstrated to have some influence,<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> especially in the population with CKD-HD,<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">26–28</span></a> on lower levels of seroprotection after vaccination, the results of the multivariate analysis showed no association for sex, DM2, HIV and CKD-HD when adjusted for age and number of doses. Although in the present study there is no evidence on the association between comorbidities and seroprotection, they could play a significant role in decreasing the efficacy of HBV vaccination, mainly due to their impact on immunity.</p><p id="par0175" class="elsevierStylePara elsevierViewall">It is important to note that our study population differs from other reported populations,<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> with less access to local health services and therefore less possibility of receiving the HBV vaccine or receiving the required doses, which directly impacts the level of seroprotection in this subgroup of patients.</p><p id="par0180" class="elsevierStylePara elsevierViewall">Post-marketing studies have shown that a schedule of three doses induces protective levels of anti-HBs titres in more than 95% of healthy infants, children, and adolescents, and in more than 90% of healthy adults under 40 years of age. However, it is common to find patients who have had only one dose of the vaccine at the local level, and we need to determine their probability of not developing HBV seroprotection according to the level of risk corresponding to their age, to prioritise preventive measures in the adult population with incomplete doses. The results of this study showed that, for example, for a 60-year-old patient with a single vaccine dose, the probability of not developing seroprotection is 57.9%, whereas, if this same patient receives 2 or 3 doses, the probability is reduced to 31.6%. This supports the need for much more rigorous and comprehensive vaccination schedules in the adult population to reduce their vulnerability to HBV infection, as well as adequate serological follow-up and successive booster doses to achieve seroprotection that persists over time.</p><p id="par0185" class="elsevierStylePara elsevierViewall">These findings suggest that new alternatives should be implemented for at-risk populations, especially older adults, and patients with comorbidities. In this regard, it is important to note that a new generation of recombinant HBV vaccines have been developed in several countries that are useful in risk groups that do not respond to conventional HBV vaccines, including older adults, the obese, healthcare workers, patients with kidney failure and on dialysis, transplant patients, and patients with HIV.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p><p id="par0190" class="elsevierStylePara elsevierViewall">It is important to highlight that this study has several limitations, such as the population not being homogeneous in terms of sex (only 24.4% of the participants were female), a variable that seems to be related to not developing HBV seroprotection in the adult population, according to reports. We should also clarify that, as this was a retrospective observational study, not all variables that might influence anti-HBs titres, such as the gradual variation of seroprotection over time (early anti-HBs levels, effect of revaccination), could be measured. While the regression model allowed adjustment of variables by number of doses, seroprotection should have been assessed in a larger sample of individuals with three vaccine doses, which is the current standard of intervention.</p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Conclusions</span><p id="par0195" class="elsevierStylePara elsevierViewall">Age at vaccination and number of doses (1 dose vs. >1 dose) were the only risk factors able to predict non-development of seroprotection against HBV in adult subjects. These findings flag up the need to improve hepatitis B vaccination programmes aimed at this population, as well as the processes related to the immunisation of susceptible adults, given our population’s poor access to health services.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Contribution of the authors</span><p id="par0200" class="elsevierStylePara elsevierViewall">All the authors participated in the study design. Adults (>18 years) vaccinated against HBV, titrated to anti-HBs < 10 mIU/mL, and who did not present chronic hepatitis B or previous history of HBV infection were defined as cases. The controls were defined with the same characteristics of the cases, the only difference being a titre of anti-HBs ≥ 10 mIU/mL. The factors analyzed were age, sex, number of doses, and some comorbidities such as HIV infection, type 2 diabetes mellitus, and chronic kidney disease on hemodialysis. A bivariate analysis was performed using the Mann-Whitney Chi2 and <span class="elsevierStyleItalic">U</span> tests, as well as a multivariate analysis using binary logistic regression to generate a predictive model.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">The cohort of patients with anti-HBs titers was composed of 205 participants, which included 39 cases and 166 controls (power: 91%, significance of 0.05). The bivariate analysis showed that the variables age (OR 3.19, <span class="elsevierStyleItalic">P</span> = .005), number of vaccine doses (OR 4.82, <span class="elsevierStyleItalic">P</span> = .001), hemodialysis CKD (OR 8.85, <span class="elsevierStyleItalic">P</span> = .001), Diabetes mellitus 2(OR 4.38, <span class="elsevierStyleItalic">P</span> = .001), and the time from onset of vaccination (OR 0.36, <span class="elsevierStyleItalic">P</span> = .006), were associated with non-development of seroprotection against HBV. Likewise, age groups >40 years and >60 years were highly significant in this analysis, OR 3.19 and OR 8.93, respectively. In the multivariate analysis, only age as a numerical variable (OR: 1.05) and number of doses (OR: 2.74), after being adjusted by the other variables, were the only ones that explained the non-development of seroprotection in adults. Se realizó un análisis bivariante mediante las pruebas de Chi2 y U de Mann-Whitney, así como un análisis multivariado mediante regresión logística binaria para generar un modelo predictivo.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">La cohorte de pacientes con titulación de anti-HBs estuvo formada por 205 participantes, la cual incluyó 39 casos y 166 controles (potencia: 91%, significancia α de 0.05). El análisis bivariante mostró que las variables edad (OR 3.19; <span class="elsevierStyleItalic">P</span> = .005), número de dosis vacunal (OR 4.82; <span class="elsevierStyleItalic">P</span> = .001), ERC en hemodiálisis (OR 8.85; <span class="elsevierStyleItalic">P</span> = .001), Diabetes mellitus (OR4.38; <span class="elsevierStyleItalic">P</span> = .001), y el tiempo desde el inicio de la vacunación (OR 0.36; <span class="elsevierStyleItalic">P</span> = .006), se asociaron al no desarrollo de seroprotección contra el VHB. Asimismo, los grupos de edad ≥40 años y ≥60 años, resultaron altamente significativos en este análisis, OR 3.19 y OR 8.93 respectivamente. En el análisis multivariado, solo la edad como variable numérica (OR: 1.05) y el número de dosis (OR: 2.74), tras ser ajustadas por las demás variables, fueron las únicas que explicaron el no desarrollo de seroprotección en adultos.
