All of them are inactivated since no attenuated vaccines are administered post-HT.</p> <p id="sp0030" class="elsevierStyleSimplePara elsevierViewall">DTP: diphtheria–tetanus–pertussis; HBV: hepatitis B; Hib: <span class="elsevierStyleItalic">Haemophilus influenzae</span> type b; MenACWY: meningococcal ACWY; MenB: meningococcal B; MenC: meningococcal C; Polio: polio; T Td: tetanus–diphtheria; TV: triple virus; VCN13: pneumococcal conjugate 13-valent; VPN23: pneumococcal polysaccharide 23; VZV: varicella zoster.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="s0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0065">Introduction</span><p id="p0005" class="elsevierStylePara elsevierViewall">The number of children on immunosuppressive therapy in developed countries is increasing, because it is possible to treat and intervene in complex, previously incurable, diseases.<a class="elsevierStyleCrossRef" href="#bb0005"><span class="elsevierStyleSup">1</span></a> Recipients of solid organ transplantation (SOT), such as heart transplantation (HT) or haematopoietic stem cell transplantation, and patients on immunosuppressive therapy require specialist multidisciplinary care.</p><p id="p0010" class="elsevierStylePara elsevierViewall">The immunosuppressants they receive impair their humoral and cellular response.<a class="elsevierStyleCrossRef" href="#bb0010"><span class="elsevierStyleSup">2</span></a> This condition, coupled with organ failure, puts them at high risk of contracting infectious diseases, with a potentially serious outcome.<a class="elsevierStyleCrossRef" href="#bb0015"><span class="elsevierStyleSup">3</span></a> Vaccination is the most effective preventive measure to reduce the risk of infection; however, the response to vaccination is diminished in these patients.<a class="elsevierStyleCrossRef" href="#bb0020"><span class="elsevierStyleSup">4</span></a> There are accelerated schedules that can be administered before commencement of immunosuppression.<a class="elsevierStyleCrossRef" href="#bb0025"><span class="elsevierStyleSup">5</span></a></p><p id="p0015" class="elsevierStylePara elsevierViewall">The main indications for HT are cardiomyopathies and congenital heart disease,<a class="elsevierStyleCrossRef" href="#bb0030"><span class="elsevierStyleSup">6</span></a> whose follow-up is usually complex from birth, with multiple hospital admissions and surgical interventions, and therefore these patients are not always correctly vaccinated by the time of HT. Furthermore, the administration of attenuated vaccines is contraindicated from the time they are included on the HT waiting list, as they must be administered at least 1 month before transplantation, and this cannot be guaranteed. Patients remain immunosuppressed for life after HT, and attenuated vaccines are contraindicated because there is a risk of infection by the micro-organism included in the vaccine after administration.<a class="elsevierStyleCrossRef" href="#bb0035"><span class="elsevierStyleSup">7</span></a></p><p id="p0020" class="elsevierStylePara elsevierViewall">The main objective of this study is to describe the pre- and post-transplant vaccination status of children undergoing HT. Possible demographic and clinical factors that could influence this will be studied as secondary objectives.</p></span><span id="s0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0070">Methods</span><span id="s0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0075">Design</span><p id="p0025" class="elsevierStylePara elsevierViewall">Retrospective descriptive study conducted in patients under 18 years of age undergoing HT in a tertiary hospital between January 2000 and September 2020. A total of 158 patients were assessed and 75 were finally included after excluding 83 patients. Exclusion criteria were death (55; 66.2 %), incomplete data (19; 22.8 %), having undergone a retransplant (6; 7.2 %), concomitant illness precluding vaccination, such as decompensated organ disease (2; 2.4 %), or refusal of vaccination (1; 1.2 %).</p><p id="p0030" class="elsevierStylePara elsevierViewall">Access to information was through the electronic medical record and the HORUS computer programme in patients from the Community of Madrid. Otherwise, they were contacted by telephone to request the record of their vaccination schedule. Informed consent was obtained from all patients, their parents, or legal representatives, as appropriate. Data were collected in an encrypted Excel database, to which only the study investigators had access. The study was approved by the hospital ethics committee (code 154/19) and complies with the Declaration of Helsinki.</p></span><span id="s0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0080">Definitions</span><p id="p0035" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Complete vaccination schedule:</span> For each of the vaccines, this was considered to be the case when the patient had received the number of doses for their age according to the vaccination schedule in force at that time in each autonomous community.</p><p id="p0040" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Specialist vaccination consultation:</span> Paediatric infectious disease or preventive medicine consultation specialising in childhood vaccination.</p></span><span id="s0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0085">Variables</span><p id="p0045" class="elsevierStylePara elsevierViewall">Demographic variables were collected: Sex, age, nationality, autonomous community of residence, and diagnosis (congenital heart disease, cardiomyopathies, and tumours) and clinical variables: vaccines administered before and after the HT until January 2021, time from diagnosis to HT, time from the HT to the start of revaccination, attendance at the specialist vaccination consultation, time from HT to the consultation, and request for HBV and HAV serology testing.</p><p id="p0050" class="elsevierStylePara elsevierViewall">The correct administration of the vaccines recommended by the scientific societies before and after the HT was assessed:<a class="elsevierStyleCrossRef" href="#bb0025"><span class="elsevierStyleSup">5</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bb0040"><span class="elsevierStyleSup">8</span></a> Hepatitis B (HBV), inactivated poliomyelitis or attenuated poliomyelitis, diphtheria, tetanus, pertussis with a high load, diphtheria, tetanus, pertussis with a low load, tetanus, diphtheria, <span class="elsevierStyleItalic">Haemophilus influenzae</span> type b (Hib), heptavalent pneumococcal conjugate, decavalent pneumococcal conjugate, 13-valent pneumococcal conjugate, 23-valent pneumococcal polysaccharide, triple virus (TV), varicella zoster (VZV), tetraviral, meningococcal B, meningococcal C (MenC), tetravalent meningococcal ACWY,<a class="elsevierStyleCrossRef" href="#bb0045"><span class="elsevierStyleSup">9</span></a> annual influenza vaccine, and rotavirus.<a class="elsevierStyleCrossRef" href="#bb0050"><span class="elsevierStyleSup">10</span></a></p><p id="p0055" class="elsevierStylePara elsevierViewall">HAV and HBV serostatus was determined before and after the HT.</p><p id="p0060" class="elsevierStylePara elsevierViewall">Pre-HT, the vaccines were studied individually and grouped into inactivated and attenuated vaccines. This determined whether the schedule was complete or not at that time.</p><p id="p0065" class="elsevierStylePara elsevierViewall">Post-HT, we recorded whether each vaccine had been administered correctly following the Ministry of Health recommendations for patients undergoing SOT,<a class="elsevierStyleCrossRef" href="#bb0055"><span class="elsevierStyleSup">11</span></a> there being no specific recommendations for these patients at the regional level. In addition to analysing each vaccine individually, we considered 2 groups: funded and non-funded vaccines. We determined whether the schedules were up to date for each vaccine and for each group of vaccines.</p></span><span id="s0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0090">Statistical analysis of the data</span><p id="p0070" class="elsevierStylePara elsevierViewall">Quantitative variables are presented as median and interquartile range (IQR). Qualitative variables are expressed as frequencies and percentages. The association between qualitative variables was made using the χ<span class="elsevierStyleSup">2</span> test or Fisher's exact test as appropriate, and the Mann–Whitney <span class="elsevierStyleItalic">U</span> test used for association between quantitative variables. A <span class="elsevierStyleItalic">P</span>-value <<span class="elsevierStyleHsp" style=""></span>.05 was considered statistically significant, in which case the odds ratio (OR) was calculated with a 95.0 % confidence interval (CI). IBM SPSS version 25 (SPSS 25.0; SPSS, Inc., Chicago, IL) was used for the statistical analysis of all data as well as the graphs).</p></span></span><span id="s0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0095">Results</span><p id="p0075" class="elsevierStylePara elsevierViewall">Seventy-five patients were included, with a median age at HT of 36 months (IQR 7.3–108.5), 68.0 % were male.</p><p id="p0080" class="elsevierStylePara elsevierViewall">Patient demographic data are shown in <a class="elsevierStyleCrossRef" href="#t0005">Table 1</a>. Indications for CT were cardiomyopathies (37; 49.3 %), congenital heart disease (36; 48.0 %), and tumours (2; 2.7 %). The median time from diagnosis to HT was 15.0 months (IQR 4.5–89.5). The median time from HT to starting to update vaccination status was 1.0 month (IQR 0.0–2.0), with the exception of influenza vaccination.</p><elsevierMultimedia ident="t0005"></elsevierMultimedia><p id="p0085" class="elsevierStylePara elsevierViewall">At the time prior to HT, 49 patients (65.3 %) had a complete vaccination schedule, with better compliance in those older than 12 months than in those younger (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.15). The analysis for each vaccine is shown in <a class="elsevierStyleCrossRef" href="#t0010">Table 2</a>. This trend was not maintained in vaccination post-HT. The results of vaccination rates for each vaccine and vaccines grouped into attenuated or inactivated vaccines are shown in <a class="elsevierStyleCrossRef" href="#f0005">Fig. 1</a>.</p><elsevierMultimedia ident="t0010"></elsevierMultimedia><elsevierMultimedia ident="f0005"></elsevierMultimedia><p id="p0090" class="elsevierStylePara elsevierViewall">With respect to influenza, before HT, 30 patients older than 6 months (49.2 %) had received some dose of vaccine. After HT, this percentage rose to 82.7 % (62 patients). No significant differences in influenza vaccination were found between any of the age ranges.</p><p id="p0095" class="elsevierStylePara elsevierViewall">After HT, 10 (13.3 %) of the patients' vaccination status had been correctly updated, taking only funded vaccines into account. Of these, 4 patients (5.3 %) were up to date with all non-funded vaccines. <a class="elsevierStyleCrossRef" href="#f0010">Fig. 2</a> shows the analysis of the post-HT updated vaccine schedule grouped into funded and non-funded and of each vaccine individually.</p><elsevierMultimedia ident="f0010"></elsevierMultimedia><p id="p0100" class="elsevierStylePara elsevierViewall">Of the total number of patients whose pre-HT schedule was not complete, 4 (8.2 %) patients' global vaccination schedule had been correctly updated post-HT. No significant differences were found between having a correct schedule pre-HT and the degree of global updating post-HT (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.29), or for each of the vaccines.</p><p id="p0105" class="elsevierStylePara elsevierViewall">There were no differences in terms of sex with respect to the degree of global vaccination updating post-HT. The individual analysis for each vaccine according to sex is shown in <a class="elsevierStyleCrossRef" href="#t0015">Table 3</a>.</p><elsevierMultimedia ident="t0015"></elsevierMultimedia><p id="p0110" class="elsevierStylePara elsevierViewall">Of the 75 patients, 10 (13.3 %) attended consultation for vaccination counselling; all of them were assessed between 2015 and 2020, the final years of the study, and only 4 of them (40.0 %) started follow-up pre-HT. The median time from HT to consultation was 8.8 months (IQR 2.2–28.0). Of the 65 who had not attended consultation, only 1 patient (1.5 %) had an updated schedule. Of the 10 who had attended consultation, 3 patients (30.0 %) had a completely updated schedule (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.01; OR 27.5 [2.5–300.5]). <a class="elsevierStyleCrossRef" href="#t0020">Table 4</a> shows the analysis of the relationship between vaccination updating and specialist consultation attendance.</p><elsevierMultimedia ident="t0020"></elsevierMultimedia><p id="p0115" class="elsevierStylePara elsevierViewall">Regarding request for HBV serology testing, 41 patients (54.7 %) had undergone at least 1 HBV serology test at some point in their progression, 21 of them after HT. Regarding HAV, 38 patients (50.7 %) had undergone at least 1 serology test, 17 of them post-HT.</p></span><span id="s0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0100">Discussion</span><p id="p0120" class="elsevierStylePara elsevierViewall">This is the first study to analyse the optimisation of the childhood vaccination schedule in a cohort of HT patients, as well as the recommended vaccination protection status in paediatric HT patients in Spain. The tertiary hospital where the study was conducted has among the highest volume of paediatric HTs in the country, and therefore our sample of patients is nationally representative. Patients with complex diseases are treated in specialist hospitals, and this is reflected in our sample, since more than two-third of our patients live in autonomous communities other than the Community of Madrid, thus reflecting how vaccination updating for paediatric HT patients takes place throughout Spain.</p><p id="p0125" class="elsevierStylePara elsevierViewall">In our series, children under 12 months are the most poorly vaccinated, especially with regard to polio, Hib, and MenC. This age group is the most vulnerable to infections, given the immaturity of their immune system and the lower number of vaccine doses received before transplantation. A study in SOT recipients found that children undergoing HT, especially before the age of 2 years, were at increased risk of hospitalisation for a vaccine-preventable infection.<a class="elsevierStyleCrossRef" href="#bb0060"><span class="elsevierStyleSup">12</span></a></p><p id="p0130" class="elsevierStylePara elsevierViewall">In the pre-transplantation period, efforts should be directed towards ensuring that the patient has the highest possible vaccination coverage, with adopting strategies of accelerated vaccination schedules. Pre-HT immunization has been shown to generate higher antibody titres compared to vaccines administered post-HT. Moreover, attenuated vaccines cannot be administered after HT.<a class="elsevierStyleCrossRef" href="#bb0065"><span class="elsevierStyleSup">13</span></a></p><p id="p0135" class="elsevierStylePara elsevierViewall">In our study, we observed pre-HT vaccination rates of more than 60 %, whereas in other studies, such as that of Cortina et al. with 30 patients, 3 of whom were HT recipients, rates do not reach 50 %,<a class="elsevierStyleCrossRef" href="#bb0070"><span class="elsevierStyleSup">14</span></a> this is perhaps influenced by their smaller sample size. Cortina et al. also found that the main differences between SOT recipients were in regard to live virus vaccines, achieving 100 % pre-transplant rates in patients on the HT list. In our study, higher pre-transplant vaccination coverage was also achieved with live virus vaccines (98.7 % vs. 68.0 % with inactivated vaccines, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.01). A possible explanation could be the lower number of doses required for attenuated vaccines to be considered adequately administered.</p><p id="p0140" class="elsevierStylePara elsevierViewall">Post-HT, the percentage of patients in our study complying with updating of all vaccines studied is low. These results are consistent with other studies such as that by Cenc et al. who published coverage for vaccines such as measles or HBV of less than 40 % in paediatric kidney transplant recipients,<a class="elsevierStyleCrossRef" href="#bb0075"><span class="elsevierStyleSup">15</span></a> or the study by Dehghani et al. who reported a low immunization rate for HBV, DTP, and TV in liver transplant recipients.<a class="elsevierStyleCrossRef" href="#bb0080"><span class="elsevierStyleSup">16</span></a> Another study by Chaves et al. in renal transplant patients showed that only 4.3 % of their population was correctly vaccinated. However, they found that more than half of them had received conventional vaccines after transplantation, without taking into account the specific recommendations for these patients.<a class="elsevierStyleCrossRef" href="#bb0085"><span class="elsevierStyleSup">17</span></a> According to Cortina et al., the reasons for this insufficient vaccination status were both medical, such as early age of transplantation, and non-medical, including parental refusal of vaccination.<a class="elsevierStyleCrossRef" href="#bb0070"><span class="elsevierStyleSup">14</span></a> In our study, we found 1 isolated case of vaccination refusal. This reflects the need to provide accessible and understandable information to ensure all patients and their families understand the importance of vaccination.</p><p id="p0145" class="elsevierStylePara elsevierViewall">Post-HT administration of attenuated vaccines is still contraindicated. However, in other SOTs, such as kidney and liver, attenuated vaccines are already considered in certain circumstances.<a class="elsevierStyleCrossRef" href="#bb0090"><span class="elsevierStyleSup">18</span></a> Although this is not the case in HT, 2 patients in our cohort received attenuated vaccines by mistake without adverse effects. In fact, with the aim of increasing vaccine protection in immunocompromised patients, prevention strategies such as clinical trials with the herpes zoster vaccine in patients with SOT are being undertaken.<a class="elsevierStyleCrossRef" href="#bb0095"><span class="elsevierStyleSup">19</span></a></p><p id="p0150" class="elsevierStylePara elsevierViewall">In the post-transplant setting, inactivated vaccines can be started as early as 3 months post-transplant, with the exception of influenza which can be administered from the first month post-transplant.<a class="elsevierStyleCrossRef" href="#bb0100"><span class="elsevierStyleSup">20</span></a> In our study, the median time to start updating was 1 month, due to the high influenza vaccination coverage in these patients, as we observed that more than 80 % had received the post-transplant influenza vaccine, as previously described in the scientific literature.<a class="elsevierStyleCrossRef" href="#bb0105"><span class="elsevierStyleSup">21</span></a></p><p id="p0155" class="elsevierStylePara elsevierViewall">Our study also reflects that serology testing for HAV and HBV is not routinely requested in these patients, as a large percentage had not undergone serology testing at any time during their progression. A survey of 18 lung transplant centres showed that less than half of the patients had undergone post-transplant serology testing.<a class="elsevierStyleCrossRef" href="#bb0110"><span class="elsevierStyleSup">22</span></a> It is recommended that TV, VZV, HAV, and HBV serology tests be requested every 1–2 years.<a class="elsevierStyleCrossRef" href="#bb0040"><span class="elsevierStyleSup">8</span></a></p><p id="p0160" class="elsevierStylePara elsevierViewall">Most of the vaccinations administered to these patients throughout their progression are given in primary care centres.<a class="elsevierStyleCrossRef" href="#bb0115"><span class="elsevierStyleSup">23</span></a> In our study, we found statistically significant differences in the adequacy of vaccination between patients who did and did not attend specific vaccination counselling consultations, vaccination updating being better in those who did attend. Following the results of this study, it is proposed as an objective to refer all patients on the HT waiting list for specialist vaccination counselling consultations. At these consultations, the patient's immunization schedule is reviewed, the appropriate recommendations are made, and the vaccination schedule of household members is also reviewed.<a class="elsevierStyleCrossRef" href="#bb0120"><span class="elsevierStyleSup">24</span></a> Nevertheless, only 3 of the 10 patients who attended the consultation had a fully updated schedule. This may be due to the difficulty in accessing vaccines, especially in patients from other autonomous communities. Therefore, intrahospital circuits would be advisable to ensure the administration of vaccines especially in referral centres where complex immunosuppressed patients or those receiving immunosuppressive treatments are treated.</p><p id="p0165" class="elsevierStylePara elsevierViewall">Finally, we believe there is a need to analyse the obstacles that lead to suboptimal immunization of HT recipients in order to develop action plans to improve their vaccination status.<a class="elsevierStyleCrossRef" href="#bb0125"><span class="elsevierStyleSup">25</span></a> One of the patients had not been vaccinated due to parental refusal. This raises an ethical question as they have to take care of an organ as valuable as a heart and, if they refuse vaccination, they may also refuse immunosuppressants. This reflects the importance of good information to families to ensure that they are aware that vaccination should be part of the routine medical care of transplant patients.</p><p id="p0170" class="elsevierStylePara elsevierViewall">This study has several limitations. It is retrospective, and thus has the inherent limitations that this entails. In addition, we considered the vaccines recorded in the patients' vaccination schedules, and there may be vaccines administered in other settings (hospital or private) that are not always noted in the usual vaccination schedules. Coronavirus type 2 (SARS-CoV-2) vaccination could not be considered, as this study describes the situation between 2000 and 2020, prior to the approval of SARS-CoV-2 vaccines in children.<a class="elsevierStyleCrossRef" href="#bb0130"><span class="elsevierStyleSup">26</span></a> The main strength of the study is that it was conducted in an HT referral hospital with a very large number of patients, and a representative sample of this population in our country. It also made it possible to optimise the vaccination schedule in HT patients included in the study and, in turn, to create specialist follow-up protocols in prevention and vaccination in paediatric HT.</p></span><span id="s0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0105">Conclusions</span><p id="p0175" class="elsevierStylePara elsevierViewall">Reviewing and optimizing vaccination schedules in BT patients remains a challenge. Patients should come for HT with the highest possible vaccination coverage. After HT, vaccination updating should be started following current recommendations. The objectives of this study are to describe and evaluate the vaccination status of a group of pediatric HT recipients.</p></span> <span id="as0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0015">Material and methods</span><p id="sp0070" class="elsevierStyleSimplePara elsevierViewall">Retrospective descriptive study of 75 pediatric patients who underwent HT at a tertiary hospital between January 2000 and September 2020. Demographic, clinical (immunization before and after HT, and attendance to vaccination counselling consultation), and analytical (hepatitis A and B serology before and after HT) variables were collected.</p></span> <span id="as0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0020">Results</span><p id="sp0075" class="elsevierStyleSimplePara elsevierViewall">Patients ><span class="elsevierStyleHsp" style=""></span>12 months reached the HT with better vaccination coverage than those <<span class="elsevierStyleHsp" style=""></span>12 months: 78.9 % versus 60.7 % had a complete vaccination schedule (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.15). Post-HT vaccination update rate is low; only 13.3 % of patients had a complete schedule according to their age.
