Vaccination protects us from common preventable diseases like tuberculosis, poliomyelitis, diphtheria, tetanus, pertussis, and measles. There is no doubt that since its introduction in the 18th century, vaccination has made an enormous contribution to global health. Major infections like smallpox had been eradicated and poliomyelitis which has previously infected the world has been dramatically reduced.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Global coverage of vaccination against many important infectious diseases of childhood has been enhanced dramatically since the creation of WHO's Expanded Program of Immunization in 1974 and the Global Alliance for Vaccination and Immunization in 2000. However, from its introduction, questions and concerns about the safety of vaccines have always been voiced out. However, the protection against feared, deadly diseases that vaccines offer encourages the majority of health care professionals and laypeople to continue supporting immunization.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">2</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Recently, the Philippines has been experiencing a vaccine scare due to the outrage of the risks and complications brought by the dengue vaccine, <span class="elsevierStyleItalic">Dengvaxia</span>. Based on the Department of Health Dengue Adverse Events Surveillance, 3281 <span class="elsevierStyleItalic">Dengvaxia</span> vaccine recipients were hospitalized due to dengue vaccine associated illnesses from March 2016 to March 2018.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">3</span></a> The Vaccine Confidence Project™ measured the impact of the crisis and showed a dramatic drop in vaccine confidence from 93% “strongly agreeing” that vaccines are important in 2015 to only 32% in 2018.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">4</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">As more parents became hesitant even on routine vaccination, an outbreak in measles was reported. The alarming case even prompted the WHO to publish update on the current situation of measles in the Philippines. In their report, they cited that the country's Department of Health has officially reported over 12,700 cases with 203 deaths from January 1 to February 23, 2019 with over half of these cases involving children under the age of 5. WHO further identified the decreased in vaccine confidence to be due to <span class="elsevierStyleItalic">Dengvaxia</span>.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">5</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Although there are multiple factors that may contribute to the low immunization coverage, parental decisions are crucial for increasing the immunization rate, compliance, and for decreasing any possible vaccination error. Barriers against immunization include misinformation, adverse effects, and disease development after the administration of vaccines. Depending on the parental knowledge, this can lead to either many errors or immunization completeness.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a> Given the huge social benefits of immunization, any inequities in the knowledge, attitude, and practices that leave out large sections of the most deprived populations are a cause of serious policy concern.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">One study conducted in the Philippines show that parents living in urbans areas, with tertiary level of education, and being married are more likely to have their children fully vaccinated.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">8</span></a> However, no published study in the Philippines is available as to the immunization knowledge, attitude, and practices of mothers living in rural areas. Moreover, the hesitation that precipitated from the <span class="elsevierStyleItalic">Dengvaxia</span> vaccine, decrease in confidence toward immunization, and the outbreak of measles in different parts of the country<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">3–5</span></a> prompted the researchers to assess the mothers’ knowledge, attitude, and practices toward childhood immunization and correlate their demographic profile to these variables.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methodology</span><p id="par0035" class="elsevierStylePara elsevierViewall">The study was a cross sectional survey conducted from February to May 2019 involving 240 mothers from one rural area in Pampanga, Philippines. All respondents must have at least one child who is 2 years old or below. Mothers visiting the rural health units (RHU) during the survey period were recruited to participate in the study. The researchers handed the self-administered questionnaire in a face-to-face manner to willing participants after obtaining their informed consent.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The questionnaire used was adopted from two different international studies<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">7,9</span></a>; both were translated in Filipino and pilot tested to fit the local setting. The final questionnaire consisted of 4 sections. The first section gathered the socio-demographic profile of the respondents while the remaining sections elicited the current knowledge, attitude, and practice of the mothers regarding childhood immunization.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In scoring the knowledge, respondents who rated scores above 5 out of 9 were classified as having high knowledge whereas respondents with a score of 4 and below were considered as having low level of knowledge.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Ethical considerations</span><p id="par0050" class="elsevierStylePara elsevierViewall">Ethical approval was obtained and granted by a reputable ethics review committee. Participation was voluntary and all respondents were asked to sign an informed consent before data collection. All information collected was treated with utmost confidentiality.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Data analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">Frequency, percentage, and mean were used for all categorical data and Pearson's chi-square was used to determine the association of the mother's socio-demographic profile with their corresponding level of knowledge, attitude, and practices. Statistical significance was set at alpha error of 0.05.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">A total of 240 mothers from the rural community participated in the study. Majority of the mother-respondents were married, within the age range of 30–39 years old, having 2–3 children, secondary level of education, and were mostly unemployed (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Mothers’ knowledge on childhood immunization</span><p id="par0065" class="elsevierStylePara elsevierViewall">High level of knowledge (score<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>5) was observed in 67.50% of the respondents (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2a</a>) whereas a high proportion (93.30%) answered that routine vaccination prevents children from infectious diseases and complications (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 2b</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">It was also noted that 81.70% of the mothers answered the correct timing of the first dose in the schedule of vaccination. A considerable number of 73.80% noted that the incidence of most diseases against which children are vaccinated occur during the first years of life and 75.00% of respondents answered that administration of multiple doses of the same vaccine given at intervals are significant for child immunity. More than half of the respondents (62.90%) answered that synchronous administration of multiple vaccines has no adverse effects on child immunity. They also agree on the importance of vaccinating children during immunization campaigns (83.80%) and that vaccination of children against seasonal illness is important (82.90%).</p><p id="par0075" class="elsevierStylePara elsevierViewall">On the other hand, 94.60% do not believe that immunization is associated with autism whereas only 45.80% agree that common colds, ear infection, and diarrhea are not contraindications for vaccination.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Mothers’ attitude toward childhood immunization</span><p id="par0080" class="elsevierStylePara elsevierViewall">Mothers’ attitude toward childhood immunization is shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 3</a>. The majority of the mothers (96.30%) strongly agree and agree that childhood immunization is important, 92.10% of them considered immunization to be more beneficial than harmful with 95.80% strongly agreeing and agreeing that vaccines are safe. Out of the 240 mothers, 211 (88.00%) strongly disagree and disagree that childhood immunization is prohibited in religion and only 14.60% feel that immunization is associated with side effects. Eighty-five percent (85.00%) and 98.80% respectively strongly agree and agree that compliance to immunization schedule is important and immunization keep their children healthy.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Mothers’ practice toward childhood immunization</span><p id="par0085" class="elsevierStylePara elsevierViewall">From the data presented in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 4</a>, it was observed that 90.40% of the respondents answered that their children were completely vaccinated and that 91.70% of the mothers complied with the vaccination schedule of their children. Two hundred nineteen (219) out of 240 respondents visit public health care facility mostly because of its free services (50.83%) and accessibility (39.20%). The study further revealed that health care workers were the main source of information for mothers.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">Results show that there was no statistically significant difference between mothers’ demographic profile (civil status, number of children, educational background, and employment) and their level of knowledge as well as their attitude and practice toward childhood immunization.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">The study shows that despite living in a rural community, majority of the mothers show high level of knowledge about basic information on vaccination. The results are similar with previous studies showing that mothers were in favor of vaccinating children, thought that immunization can prevent diseases, and that vaccination is “very important”.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">9,10</span></a> As mentioned in the results, the respondents obtained the information from health care workers. The current data coincide with previous studies showing that health staffs are the major source of information of mothers.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">11–13</span></a> This emphasizes the significant role played by community health workers in improving the overall knowledge of mothers as they are the key personnel who can reach out to the masses and motivate them. In the case of measles outbreak in the Philippines due to vaccine scare, the health care workers are in the best position to encourage mothers to have their children vaccinated and correct any misconception associated with immunization.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Apart from the importance of community health workers, it is also noteworthy to say that other studies reported that access to the nearest health facilities is one of the well-known factors associated with vaccination status. This is further supported by Mugada et al. (2017) where researchers concluded that a strong primary health care approach leads to improved mother's knowledge and subsequently, good coverage on immunization.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">14</span></a> Both the present and previous studies are highly suggestive that the government should strengthen the rural health units and community centers to positively influence mothers regarding immunization.</p><p id="par0105" class="elsevierStylePara elsevierViewall">In the Philippines, a house-to-house campaign is a common scenario to inform and convince the parents to vaccinate their children. The strategy has higher coverage level (100.00%) compared with fixed site immunization (86.00%). Although considered to be more expensive, house-to-house vaccination is still considered as the most cost-effective strategy to universal access to immunization.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">15</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">Even in the midst of <span class="elsevierStyleItalic">Dengvaxia</span> scare, Filipino mothers from the rural community continue to have a positive outlook on vaccination. It may be because the measles scare is a bigger harm to handle if their children fail to avail of the immunization. Highlighting factual information about the dangers of communicable diseases is an effective method to positively impact people's attitudes to vaccination.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">16</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">The positive attitude observed in the present study is similar to the study of Awosan et al. (2018) where majority of the participants showed encouraging attitude with regards to child vaccination.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">17</span></a> While immunization may be related with common body manifestations and reactions, the benefits still outweigh the side effects, and the serious adverse events are sporadic.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">18</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">At this present time, there is no scientific evidence which supports the mothers’ fears regarding combined vaccines that can cause immune overload. According to CDC, administration of vaccines at the same time doesn’t cause any health issues. Studies have been done to assess the effects of giving lots of combinations of vaccines, and when every new vaccine is licensed, it has been tested along with the vaccines that were recommended for a specific age group.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">19</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">A surprising result is the self-reported immunization coverage for the respondents. The data is inconsistent with the acquired report from the FHIS which shows only 65% vaccination rate compared with the 90.40% self-reported complete immunization. According to Ricco et al. (2017), “self-referred vaccination rates are notoriously inaccurate, in particular for pediatric-age immunizations”.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">20</span></a> Moreover, there are also studies showing that electronic record on immunization and self-report do not always agree. Previous study showed that over-reporting was frequent among respondents who have lower income level which may have been the same case for the inconsistencies in the present study.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">21</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">While the results of the present study show that majority of the mothers have high level of knowledge, positive attitude, and good practice on vaccination, this should not lead to complacency on the part of health authorities. There are still mothers who continue to have misconceptions about vaccination and thus, hinder them from having their children vaccinated. In previous studies, researchers noted that one of the main reasons on why parents are hesitant toward vaccinating their children is the side effects of the vaccine.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">20,22</span></a> Other reported their husbands’ condemnation (52.60%) and lack of information on vaccination schedule (26.30%) as barriers. Other notable findings show that incomplete immunization status was associated with low economic income, fewer antenatal care visits, inadequate supplies of vaccine service; lacking information on immunization program; and lower educational level of caregivers.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">12,14</span></a> Thus, these barriers should be taken into consideration when developing policies related to vaccination.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Even though only 25.00% of the respondents have a tertiary level of education, it is impressive that 67.50% of them showed high level of knowledge on childhood immunization. However, the study failed to establish a significant relationship between the mothers’ demographic variables and their knowledge, attitude, and practice. The results are similar with the findings of Anokye et al. (2018), in which maternal age, religion, and educational attainment has no significant association on the vaccination of their children.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">23</span></a> Data also coincide with the study of Sankar, Rameh, and Sunny (2018), in which educational attainment of mothers has no correlation with the immunization condition of their children. However, they have found out that marital and employment status of the respondents have significant association toward childhood immunization.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">24</span></a> The lower number of respondents of the present study and differences in race, habits, tradition, and beliefs of the mothers across certain geographical location may explain differences in the results.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusion</span><p id="par0140" class="elsevierStylePara elsevierViewall">The mothers from the rural community showed a high level of knowledge, positive attitude, and good practice regarding childhood immunizations. Majority of the mothers also have good practices showing that 90.40% of them have children who are completely immunized, and 91.70% of them complied with the vaccination schedule. It was also reported that 63.75% obtain information about immunization from health care workers. Majority of them (91.30%) go to government facilities to avail of immunization because of the free services and easy accessibility. La mayoría de las madres mostró también buenas prácticas, y reflejó que el 90,4% tenía hijos completamente inmunizados, y que el 91,7% había cumplido el programa de vacunación. También se reportó que el 63,75% obtenía información sobre la inmunización de los profesionales sanitarios. La mayoría de ellas (91,3%) acudía a las instituciones gubernamentales para disponer de inmunización, debido a la gratuidad de los servicios y a su fácil accesibilidad.
