Twelve serogroups have been discovered, of which six have been identified as dangerous to health: A, B, C, W, X, and Y.<a class="elsevierStyleCrossRefs" href="#bb0005"><span class="elsevierStyleSup">1–3</span></a> This bacterium can spread through the central nervous system, causing invasive meningococcal disease (IMD), leading to meningitis and/or sepsis.<a class="elsevierStyleCrossRef" href="#bb0005"><span class="elsevierStyleSup">1</span></a></p><p id="p0010" class="elsevierStylePara elsevierViewall">It is the leading cause of bacterial meningitis in children and the second in adults, with an overall lethality of around 10% (7.7% in Spain in the 2017/2018 season),<a class="elsevierStyleCrossRef" href="#bb0015"><span class="elsevierStyleSup">3</span></a> and a risk of sequelae of 20%–30%. Serogroup B predominates, especially in countries with routine immunisation against serogroup C.<a class="elsevierStyleCrossRef" href="#bb0010"><span class="elsevierStyleSup">2</span></a></p><p id="p0015" class="elsevierStylePara elsevierViewall">The incidence of meningitis in Europe is 0.2–14/100 000 inhabitants and in Spain 0.3/100 000 inhabitants. The highest incidence rates are in children under 5 years of age, especially in children under 1 year of age, with a rate in Spain of 5.85 in the 2017/20183 season. In adolescents, the carrier rate can be as high as 30%, and the bacteria can be transmitted to young children and adolescents themselves, which are the groups with the highest incidence of IMD.<a class="elsevierStyleCrossRef" href="#bb0020"><span class="elsevierStyleSup">4</span></a></p><p id="p0020" class="elsevierStylePara elsevierViewall">Currently, one of the preparations on the market against serogroup B is the multicomponent vaccine 4CMenB (Bexsero). This vaccine is not included in the routine vaccination schedule in Spain, although the Vaccine Advisory Committee of the Spanish Paediatric Association recommends its inclusion.<a class="elsevierStyleCrossRef" href="#bb0025"><span class="elsevierStyleSup">5</span></a> In 2015, when it began to be marketed in Spain, the technical data sheet indicated guidelines according to the age of onset:</p><p id="p0025" class="elsevierStylePara elsevierViewall">- Infants aged 2–5 months: 3 separate doses, 1 month between them, plus a booster dose between 12 and 15 months (6 months after the last one).</p><p id="p0030" class="elsevierStylePara elsevierViewall">- Infants 6–11 months: 2 separate doses, 2 months apart, plus a booster dose in the second year of life (6 months after the last dose).</p><p id="p0035" class="elsevierStylePara elsevierViewall">- Infants 12–23 months: 2 separate doses, 2 months apart, plus a booster dose 12–23 months after the last dose.</p><p id="p0040" class="elsevierStylePara elsevierViewall">- Children 2–10 years: 2 separate doses, 1 month apart, no booster.</p><p id="p0045" class="elsevierStylePara elsevierViewall">- Adolescents 11 years and older and adults: 2 separate doses, minimum 1 month apart, no booster.</p><p id="p0050" class="elsevierStylePara elsevierViewall">In June 2018, a change in the guideline was introduced, whereby infants aged 3–5 months were considered to require only 2 doses of primary immunisation with 2 months between them, plus a booster dose between 12 and 15 months (6 months after the last dose).</p><p id="p0055" class="elsevierStylePara elsevierViewall">Finally, in 2020, a study was published in the UK demonstrating sufficient protection in infants aged 2 months with only 2 doses of primary immunisation plus a booster dose.<a class="elsevierStyleCrossRef" href="#bb0030"><span class="elsevierStyleSup">6</span></a> Following this study, 2-month-old infants were included in the 2<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>1 schedule (2 doses 2 months apart, plus a booster dose between 12 and 15 months, at least 6 months after the last dose). <a class="elsevierStyleCrossRef" href="#f0005">Fig. 1</a> shows the vaccination schedule in force in 2021.<a class="elsevierStyleCrossRef" href="#bb0035"><span class="elsevierStyleSup">7</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bb0040"><span class="elsevierStyleSup">8</span></a></p><elsevierMultimedia ident="f0005"></elsevierMultimedia><elsevierMultimedia ident="f0010"></elsevierMultimedia><p id="p0060" class="elsevierStylePara elsevierViewall">The objectives of this study were to describe the vaccination schedule of the patients included in the study who received the 4CmenB vaccine (Bexsero), and to study the relationship that may exist between the variables analysed, all over a period of 5 years.</p></span><span id="s0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0070">Subjects and method</span><p id="p0001" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Study design: Retrospective, observational, descriptive study:</span></p><p id="p0065" class="elsevierStylePara elsevierViewall">Participants: 11 776 patients of both sexes were included, belonging to 13 health centres in Sector I of Zaragoza, aged between 0 months and 16 years, inclusive, with a history of primary meningitis B vaccination between January 2015 and December 2019.</p><p id="p0070" class="elsevierStylePara elsevierViewall">Variables: were collected on sex (male/female), date of birth and date of first vaccination, age at first vaccination (years and months), health centre and vaccination schedule (correct, incorrect due to missing doses, incorrect due to interval error, and incomplete in time for correction).</p><p id="p0075" class="elsevierStylePara elsevierViewall">From 2015 to 2018, the administration of 3 doses plus a booster dose in infants up to 5 months of age was considered correct. From 2018 onwards, both the administration of 2 or 3 doses of primary vaccination plus booster in both cases was considered correct.</p><p id="p0080" class="elsevierStylePara elsevierViewall">Incomplete vaccination was considered to be correct for those for whom no further data were available as of December 2019 but who were still in time to receive the remaining doses.</p><p id="p0085" class="elsevierStylePara elsevierViewall">Vaccination was considered incorrect if:</p><p id="p0090" class="elsevierStylePara elsevierViewall">Doses were missed: Those patients who did not receive the total number of doses specified in the data sheet.</p><p id="p0095" class="elsevierStylePara elsevierViewall">- Interval errors occurred: Those who received both a primary and booster dose, without respecting the minimum or maximum time between them.</p><p id="p0100" class="elsevierStylePara elsevierViewall">Data collection: The information on the variables, as well as the vaccination record, was obtained through the registration in the primary care clinical record of Aragón, through its management tool OMI AP.</p><p id="p0105" class="elsevierStylePara elsevierViewall">Statistical analysis: Data analysis was performed with the IBM SPSS Stadistics version 24.0.0.0.0 statistical calculation programme. The main variables under study were analysed, with calculations of mean, median, percentiles, and standard deviation. The Kolmogorov–Smirnov test was performed to determine whether the quantitative variables followed a normal distribution. For hypothesis testing, Chi-square was used to compare qualitative variables, and the Kruskal–Wallis test was used to compare qualitative and quantitative variables. Those with P < .05 were considered statistically significant.</p><p id="p0110" class="elsevierStylePara elsevierViewall">Ethical considerations: This study has the support of the Research Ethics Committee of the Autonomous Community of Aragón (CEICA).</p></span><span id="s0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0080">Results</span><p id="p0115" class="elsevierStylePara elsevierViewall">Of the 11 776 patients, 51.5% (6070) were male, with a mean age of 5.04 years [4.96–5.11]. Correct vaccination was present in 83.8% (9869), missed doses in 5.5% (652), interval error in 1.4% (170), and incomplete vaccination in 9.2% (1085).</p><p id="p0120" class="elsevierStylePara elsevierViewall">The mean number of doses received was 2.16 [2.15–2.17]. In our study, 6.5% (776) received 1 dose, 2 doses 74.1% (8721), 3 doses 16% (1884), 4 doses 3.4% (402), and 5 doses 0.03% (3).</p><p id="p0125" class="elsevierStylePara elsevierViewall">Of the sample obtained, 12% (1412) were less than 3 months old, 11.5% (1349) from 4 to 11 months, 6.7% (790) from 12 to 23 months, 50.4% (5931) from 2 to 9 years, and 19.5% (2294) from 10 to 16 years.</p><p id="p0130" class="elsevierStylePara elsevierViewall">In terms of age groups in relation to the year they received the first dose, there was a progressive annual increase in the number of children under 3 months vaccinated, from 7.2% of the total number of those vaccinated in 2016 to 50.9% in 2019. On the other hand, children under 2 years of age increased until 2017 (58.6% of the total vaccinated) and then decreased until 2019 (17%).</p><p id="p0135" class="elsevierStylePara elsevierViewall">Significant differences (P < .001) were found in the vaccination schedule received according to age. Correctly vaccinated children had a mean age of 5.5 years, while those with an incomplete schedule were younger, with a mean age of 1.043 years. The same is true for interval errors, which were more common in those with a mean age of .918. The mean age was 4.4 years in those with missing doses.</p><p id="p0140" class="elsevierStylePara elsevierViewall">In relation to the number of doses and vaccination schedule related to sex, no differences were found (P > .05).</p><p id="p0145" class="elsevierStylePara elsevierViewall">The relationship between dose date and vaccination schedule could be observed (P < .001), finding that, from 2017 onwards, there is an increase in the number of incomplete schedules (<a class="elsevierStyleCrossRef" href="#t0005">Table 1</a>).</p><elsevierMultimedia ident="t0005"></elsevierMultimedia><elsevierMultimedia ident="t0010"></elsevierMultimedia><p id="p0150" class="elsevierStylePara elsevierViewall">In terms of incorrect guideline (missed dose or interval error) in each age group compared to the total number of vaccinations received by that group, it represented 5% in the under 3-month group, 10.9% in the 3–11-month group, 30.1% in the 12–23-month group, 4.2% in the 2–9 year group and 5% in the 10–16 year group.</p></span><span id="s0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0085">Discussion</span><p id="p0155" class="elsevierStylePara elsevierViewall">The current vaccination schedule has been shown to be highly effective against the MenB strain, reducing cases of IMD by around 75% in the vaccinated population. This effectiveness has been proven after receiving the complete primary vaccination.<a class="elsevierStyleCrossRef" href="#bb0045"><span class="elsevierStyleSup">9</span></a> It is important to highlight the need to comply with the minimum intervals between doses of the same vaccine product for an adequate immune response. Primary immunisation is considered the minimum number of doses needed to protect the susceptible person. The booster dose is the one that increases the immune response and makes it more durable. Therefore, the minimum interval between primary immunisation and booster must also be respected, so that the memory B lymphocytes mature.<a class="elsevierStyleCrossRef" href="#bb0050"><span class="elsevierStyleSup">10</span></a></p><p id="p0160" class="elsevierStylePara elsevierViewall">The mean age of correctly vaccinated children is 5.5 years, which may be related to the fact that the group aged 2–9 years received the most vaccinations (50.4%). In addition, they have a simpler vaccination schedule to complete (2 doses without booster). Since 2017, there has been a decrease in vaccinations in this group, in favour of an increase in the number of infants, perhaps because there is a higher prevalence of IMD in infants, and therefore more emphasis is placed on this age group in paediatric consultations. In addition, infants have regular check-ups where they are better captured by healthcare professionals.</p><p id="p0165" class="elsevierStylePara elsevierViewall">In 2018 and 2019, we found the highest percentages of incomplete doses, especially in infants. This is due to the fact that certain age groups, such as infants, are still in time to complete primary vaccination or booster doses, and it is therefore logical that this is the group with the highest number of incomplete doses.</p><p id="p0170" class="elsevierStylePara elsevierViewall">A study conducted in the United States between 2000 and 2013 measured vaccination errors, and found that the most common error was in the vaccination schedule, due to non-compliance with intervals, minimum ages, or compatibility of vaccines in the same session (27%).<a class="elsevierStyleCrossRef" href="#bb0055"><span class="elsevierStyleSup">11</span></a></p><p id="p0175" class="elsevierStylePara elsevierViewall">However, in Europe from 2001 to 2016, the most frequent adverse events in vaccination reported in a European surveillance database were analysed, finding that, of 8167 reports, 27.1% were due to incorrect vaccination schedule.<a class="elsevierStyleCrossRef" href="#bb0060"><span class="elsevierStyleSup">12</span></a> In Brazil, adverse events in vaccination were studied, where it was found that the age group with the most errors was children under 1 year of age. In addition, the second most frequent type of error found was inadequate interval between doses (18.2%).<a class="elsevierStyleCrossRef" href="#bb0065"><span class="elsevierStyleSup">13</span></a> Among the errors analysed in our study, the most common was missing doses (5.5%), followed by interval errors (1.4%).</p><p id="p0180" class="elsevierStylePara elsevierViewall">In the United States, in 2004, the National Immunitation Survey (NIS) surveyed children aged 19–35 months. Approximately, 11% had received an invalid dose of vaccine. The hepatitis B vaccine had the most errors, as it had the most complicated vaccination schedule (minimum age of administration for the third dose and a minimum interval between first and third and between second and third doses).<a class="elsevierStyleCrossRef" href="#bb0070"><span class="elsevierStyleSup">14</span></a> Another study conducted in the United States over 13 years concluded that the greatest error found was not respecting the vaccination schedule, with a higher incidence in children under 1 year of age, especially for not respecting the minimum interval between doses or administering a dose before the recommended age. One of the reasons found for this error was the increased complexity of vaccination schedules.<a class="elsevierStyleCrossRef" href="#bb0075"><span class="elsevierStyleSup">15</span></a></p><p id="p0185" class="elsevierStylePara elsevierViewall">In our study, we found that the highest interval errors occurred in infants. Specifically, they were higher in the 12–23-month group (55.9% of the total interval errors), which may be explained by the fact that the guideline is more complex, as they require a booster dose 12–23 months after completing the first vaccination. It is worth noting that 30.1% of the 12–23-month group would be under-vaccinated either due to missed doses or interval error, compared to 4.2% of the 2–9-year group.</p></span><span id="s0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0090">Conclusions</span><p id="p0190" class="elsevierStylePara elsevierViewall">Given that vaccination errors have a direct or indirect economic impact (either by administering unnecessary doses, non-compliance with minimum intervals or incomplete guidelines), it is important to apply the vaccine guideline appropriately.<a class="elsevierStyleCrossRef" href="#bb0080"><span class="elsevierStyleSup">16</span></a></p><p id="p0195" class="elsevierStylePara elsevierViewall">Although we can conclude that 83.8% of children in Sector I of Zaragoza who received the first dose of Bexsero from 2015 to 2019 are correctly vaccinated, it is necessary for healthcare professionals to be aware of vaccination errors, as well as to design strategies to minimise and avoid them, in order to achieve safe vaccination.<a class="elsevierStyleCrossRef" href="#bb0060"><span class="elsevierStyleSup">12</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bb0080"><span class="elsevierStyleSup">16</span></a></p></span><span id="s0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0095">Limitations</span><p id="p0200" class="elsevierStylePara elsevierViewall">This study has some limitations, as our data were collected until December 2019, and we do not know whether those children who have not completed vaccination could in the future do so, and therefore receive the full number of doses. The objective of the study was to analyse the vaccination of children with Meningitis B in Zaragoza.</p></span> <span id="as0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0015">Methods</span><p id="sp0035" class="elsevierStyleSimplePara elsevierViewall">A descriptive observational study was carried out with 11 776 of 13 Health Centres of Sector I of Zaragoza, aged 0 months to 15 years, with a history of Meningitis B vaccination between January 2015 and December 2019. It was determined sex, date of birth, date and age of primary vaccination, health centre, and vaccination schedule. A descriptive analysis and hypothesis testing was carried out to analyse the results.</p></span> <span id="as0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="st0020">Results</span><p id="sp0040" class="elsevierStyleSimplePara elsevierViewall">50.4% of those vaccinated were between 2 and 9 years old.
