She had no previous history of surgery or facial trauma. The endonasal examination was normal. A CT scan of the face/paranasal sinuses showed an extensive osteoma of the right frontal sinus.</p><p id="par0010" class="elsevierStylePara elsevierViewall">There are very few cases described in the literature of spontaneous subcutaneous orbital emphysema (SOE) due to high intranasal/paranasal pressures produced on blowing or coughing, without any history or comorbidity. The most common cause of SOE is trauma, when a bony defect occurs at the level of the lamina papyracea leading to air entering the orbit. It usually resolves spontaneously within a few weeks, but there are cases of complications with loss of vision due to compression and infection due to the entry of microorganisms (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">In the case of a patient with SOE, it is essential initially to rule out the main causes: trauma, infection, iatrogenesis or tumour. Therefore, a complete physical examination, including an in-depth ENT examination, is essential. Facial and paranasal sinus CT scans are the most useful complementary tests. In this case, it was crucial to continue the work-up to diagnose and initiate early treatment of the tumour.
