class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Children born with craniofacial anomalies, especially cleft lip and palate (CLP), depending on the extent and complexity of affected structures, are at risk to several alterations, which negatively influence the effective communication. These alterations include esthetic, dental, speech (especially articulation and resonance), language and auditory disorders.<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Concerning language, some authors report damage to both receptive and expressive language,<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">1,3</span></a> others have found performance within normal standards in CLP children although it suggests influence of auditory impairment in language performance of this population,<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Despite the reports in the literature about auditory alterations in childhood in this population,<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">2</span></a> this subject is difficult to understand due to the great methodological variability of studies: age range, surgical conditions, techniques and timing of surgical correction. There may be high occurrence of auditory loss and otologic diseases, mainly due to otitis media with effusion (OME) in this population, due to alteration in the velopharyngeal mechanism or in the tensor veli palatini muscle.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Another important aspect concerns the auditory sensory deprivation, caused by the fluctuating nature of OME, strongly impairing the auditory figure-ground skill,<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">6</span></a> and auditory attention.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">7</span></a> Thus, the central auditory processing disorder (CAPD) may be secondary to transitory or permanent peripheral hearing loss in this population.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">7</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Recent evidence indicates that not only the middle ear problems are related to the hearing problems of children with cleft lip and palate, considering that, especially, children with cleft palate and cleft lip and palate present high occurrence of central indicative behaviors. auditory processing disorder.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">8</span></a> The importance of including the evaluation of central auditory processing in the auditory diagnosis of children with cleft lip and palate has been highlighted in studies,<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">9,10</span></a> most of whom had a history of recurrent otitis media in childhood.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">9</span></a> A study<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">11</span></a> revealed a high percentage of children with isolated cleft palate with poor performance in central auditory processing behavioral tests, especially those that assess interaural integration ability, auditory figure ground and temporal resolution. Researchers<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">10</span></a> also found alteration in temporal resolution skills, and results that suggest impairment in bilateral hearing abilities, speech recognition in the presence of environmental noise, in children with cleft lip and/or palate. The use of auditory evoked potentials has also been considered to investigate central auditory processing in children with cleft lip and palate. A study<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">12</span></a> using auditory brainstem response (ABR), middle latency response (MLR), and mismatch negativity recordings (MMN) to explore Central auditory nervous system functioning in infants with cleft lip and palate, revealed there was no significant difference in ABR nor in MLR between the group with and without CLP.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Considering that children with CLP may have a history of hearing impairment (recurrent otitis media), the hypothesis of the present study was that these children would have worse results in terms of central auditory processing (CAP) and oral language development. So, the purpose of this study was to investigate the auditory and language skills in children with repaired cleft lip and palate.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0035" class="elsevierStylePara elsevierViewall">The prospective study was approved by the Institutional Review Board (n. 122/2007 SVAPEPE). Informed consent form was obtained from all participants. The sample was composed of 22 children registered at the hospital where the study was conducted, aged seven to nine years, being 50% females, with repaired unilateral CLP (right or left), without associated malformations. Primary palatoplasty of children in the sample was performed between 9 and 18 months of age (mean<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>12 months) at the same institution. Most of the children (36.4%) came from the State of São Paulo, Brazil, and were classified with higher socioeconomic level (54,5%). The duration of speech therapy directed to speech aspects ranged from 1 to 8 years and 2 months (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The exclusion criteria were: presence of otitis media and/or hearing loss upon evaluation; visual impairment, as observed by information obtained from the medical records; and presence of intellectual deficit, which was evaluated by a psychologist by the <span class="elsevierStyleItalic">Raven's Colored Progressive Matrices Test</span>.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">13</span></a></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">To evaluate the central auditory skills, children were submitted to four tests: <span class="elsevierStyleItalic">Random Gap Detection Test</span> (RGDT),<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">14</span></a><span class="elsevierStyleItalic">Dichotic Digit Test</span> (DDT),<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">15</span></a><span class="elsevierStyleItalic">Dichotic Non-Verbal Test</span> (DNVT),<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">15</span></a><span class="elsevierStyleItalic">Sustained Auditory Attention Ability Test</span> (SAAAT),<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">16</span></a> recorded on a CD, and applied by a Midimate 622 audiometer connected to a Toshiba <span class="elsevierStyleItalic">Compact Disc</span> (CD), in an acoustic cabin. The tests were presented at 50<span class="elsevierStyleHsp" style=""></span>dB NS from the average of tone auditory thresholds of 500, 1000 and 2000<span class="elsevierStyleHsp" style=""></span>Hz for both ears, except for RGDT,<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">14</span></a> which was presented to each ear at an intensity of 50<span class="elsevierStyleHsp" style=""></span>dB NS from the in-air auditory thresholds obtained for the frequencies analyzed. The auditory skills were considered altered when the observed performance of the child, in the respective tests, was lower than the reference criteria. The evaluation was performed in a silent room and took nearly two hours.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The RGDT<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">14</span></a> analyzes the temporal resolution skill by binaural presentation of a sequence of pure tone pairs at frequencies of 500, 1000, 2000 and 4000<span class="elsevierStyleHsp" style=""></span>Hz, with randomly presented intervals between the two tones, which range from 0 and 40<span class="elsevierStyleHsp" style=""></span>ms, and at the expanded version of 50 to 300<span class="elsevierStyleHsp" style=""></span>ms. The expanded version was applied only when the temporal threshold for each frequency was higher than 40<span class="elsevierStyleHsp" style=""></span>ms. The child was instructed to respond orally if she heard one or two sounds, and the mean interval of response at frequencies between 500<span class="elsevierStyleHsp" style=""></span>Hz and 4000<span class="elsevierStyleHsp" style=""></span>Hz was calculated. Normal scores for children aged 7 years are 15<span class="elsevierStyleHsp" style=""></span>ms, and 10<span class="elsevierStyleHsp" style=""></span>ms for those aged 8 years and over.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The DDT,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">15</span></a> stage of binaural integration, evaluates the figure-ground skill for verbal sounds and inter-hemispheric communication at the corpus callosum. It consists of 20 pairs of digits, presenting two pairs to each ear simultaneously. Error was considered when one digit was omitted or replaced. Normal scores for children aged 9 years and over are 95% hits and above for both ears. And <span class="elsevierStyleUnderline">></span>85% for the right ear and <span class="elsevierStyleUnderline">></span>82% for the left ear for those aged 7 and 8 years.</p><p id="par0055" class="elsevierStylePara elsevierViewall">For DNVT,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">15</span></a> six non-verbal sounds were presented, combined in pairs and presenting two pairs to each ear simultaneously. They were presented in free, right-directed and left-directed attention conditions. This test evaluated the figure-ground skill in selective attention process for non-verbal sounds. It was considered as altered when one or more stages were impaired. Normal scores for children aged 7 years are 12<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2 hits in free attention, and <span class="elsevierStyleUnderline">></span>22 hits in directed mode. And 12<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2 hits in free atttention, and <span class="elsevierStyleUnderline">></span>23 hits for those aged 8 and 9 years.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The SAAAT<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">16</span></a> consists of binaural presentation, six times, without interruption, of recording of a list of 100 monosyllabic words. The child is instructed to pay attention to the words and raise the hand only for the word “no”. This test evaluates the sustained attention and auditory vigilance skill. The total error score is obtained by the sum of inattention errors (when the hand is not raised in response to the word “no”) and impulsivity (when the hand is raised for other word). The vigilance decrement, i.e. the decrease in attention in the test is obtained by subtracting the number of correct responses in the 6<span class="elsevierStyleHsp" style=""></span>th presentation from the first. Normal scores for the children aged 7 and 8 years are 8 or less to decrement vigilance, and 5 or less for 9 years. Total Error Score are less than 36, 33, and 27 for the children aged 7, 8, and 9 respectively.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Four standardized instruments evaluated language: Illinois Psycholinguistic Skills Test (IPST),<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">17</span></a> Peabody Picture Vocabulary Test (PPVT),<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">18</span></a> Token Test<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">19</span></a> and Phonological Skills Profile (PSP).<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">20</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">The <span class="elsevierStyleItalic">Illinois Psycholinguistic Skills Test</span> (IPST) analyzes the psycholinguistic skills based on the communication processes. This study used six subtests: auditory association (AA), visual association (VA), grammar closure (GC), verbal closure (<span class="elsevierStyleSmallCaps">VC</span>), auditory memory (AM) and visual memory (VM). The scoring of each sub-test followed the guidelines and as of 36 points was considered adequate.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">17</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The PPVT<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">18</span></a> aimed to evaluate the lexical level in the receptive domain, using boards organized in increasing order of difficulty, composed of four drawings. The test proposes the scoring: extremely high; moderately high; middle high; middle; middle low; moderate low; and extremely low. Children were considered as normal if they presented extremely high; moderately high; middle high; middle; and middle low scores.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The <span class="elsevierStyleItalic">Token Test</span><a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">19</span></a> was used to evaluate the receptive language in the verbal comprehension domain, by 36 orders involving 20 pieces, circles and squares, large and small, in colors black, red, yellow and white. For each correctly followed instruction, the child achieves one point. The results were scored as normal and altered, according to the age range and normality parameters.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The PSP<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">20</span></a> provided data on the child's skill to process and handle the phonological aspects of language. The PSP is composed of items that identify and compose the phonological skills for analysis, addition, segmentation, subtraction, replacement, rhymes, sequential rhyme, syllable reversion and articulatory image. In case of error the 0 value is assigned, summing the values at completion to achieve the total score, yielding the result classified as normal or under attention (altered).</p><p id="par0090" class="elsevierStylePara elsevierViewall">The results of the questionnaires were tabulated and described in percentage, average and standard deviation. The sample was divided into two groups, one group with a lower socioeconomic level and the other higher group, comparing the data on frequency of otitis, frequency of language disorders and hearing. For the inferential statistical analysis, the Mann-Whitney test and the independent T test were used, varying according to the analysis of the Shapiro–Wilk test. Correlation was also performed to find out if the frequency of episodes of otitis influenced the performance of language and hearing, through Spearman's correction; and it was applied the Fisher exact test to analysis the correlation between auditory and language skills. All statistical procedures were performed on the software Jamovi version 1.2.25, at a significance level of 5% (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0095" class="elsevierStylePara elsevierViewall">According to medical records and anamnesis, 82% of the children with CLP presented otitis media with effusion between 0 to 8 years old (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0100" class="elsevierStylePara elsevierViewall">The CAP tests indicated that one child presented adequate performance in all skills, five children presented altered performance in all tests, while the others exhibited alteration in one to three tests applied, with greatest impairment of the temporal resolution skill (81%) in the present study population, followed by the figure-ground for non-verbal sounds (68%). Finally, sustained attention and figure-ground for verbal sounds were impaired in the same proportion (50%).</p><p id="par0105" class="elsevierStylePara elsevierViewall">Evaluation of language skills evidenced that three children presented adequate performance in all tests applied, and no child in the sample presented damage in all language skills.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The most impaired psycholinguistic skills (IPST) were VA and AA (40%), followed by VM (13%) and finally GC (18%). Children presented similar performance concerning the receptive aspect of language (50%) in the Token Test.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The phonological skills were altered in 22% of children (PSP). The lexical aspect in the receptive domain analyzed (PPVT) was the least affected performance (9%) among the language skills in the present sample (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">Through Fisher exact test, no correlation was observed between alterations in auditory and language skills (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">In the same way there was no correlation between otitis and CAP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.77), or otitis and language (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.51), through the Spearman correlation.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Comparing the lower socioeconomic level group and the higher group, there was no association of otitis, CAP tests and oral language alteration frequencies (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussions</span><p id="par0135" class="elsevierStylePara elsevierViewall">The investigation of communication, considering the skills of message reception order (decoding), message processing and emission (coding), contributes to understand the individual from a broader standpoint, allowing identification of the most commonly impaired skills of the child with CLP.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The significant presence of history of otitis media with effusion (OME) in the present sample, children with normal hearing and without active presence of the disorder corroborates the literature, which mentions this problem as frequent in children with CLP,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">5</span></a> with an estimated in 90% of infants under one with this type of congenital malformation.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">21</span></a> The fluctuating nature of OME may strongly compromise the normal pattern of hearing development, at least with respect to the figure-ground ability.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">6</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">Among the central auditory processing skills analyzed, alteration was observed in at least one skill in 95% of the sample, being temporal resolution (skill to detect the shorter time required to analyze acoustic events) the most impaired. Thus, articulation, reading and language errors, damage to phonemic analysis, lack of perception of normal speech and recognition of phonemes may be present, since the individual is unable to detect short silence intervals, observed in some consonants, as well as the alternance of frequencies between phonemes.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">22</span></a> The figure-ground skill for verbal and non-verbal sounds and the sustained auditory attention skill were also affected in children with CLP in this study, in agreement with a previous study that investigated a similar age range with CLP<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">23</span></a> and observed alteration in these skills in children with and without phonological disorders. In the present sample, there was phonological impairment in 22% of children, which may be influenced by the phonetic development, with consequences for the expressive language. Also, other levels of language are interconnected concerning the development, thus the lexical level may also be influenced by the limited inventory of consonants,<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">4</span></a> highlighting that, in the present study, 86% of children presented impairment in at least one level of language.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Even though no significant association was observed between the auditory and language skills, there was concomitance of auditory and language alterations in 81.82% of the children. It should be mentioned that deficits in CAP may be present in children with normal or altered language, and this is not a sovereign/sufficient causal factor for language impairment,<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">24</span></a> and several risk factors act synergistically in the different impairments of language skills. The lack of association between tests that evaluate the CAP and language skills, in this study, suggests that they measure different processes, evidencing the need that both areas should be addressed when analyzing children with CLP.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Given the alterations found in the auditory and language skills of the children of this study, although intervention is not the focus of this article, its importance is highlighted by implementing, as early as possible, appropriate auditory and language strategies to minimize the effects of altered abilities on the child's life and make use of the Central Nervous System plasticity. The use of bottom-up (sensory stimulation) and top-down (cognitive stimulation) strategies has been suggested in the scientific literature.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">25</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">Even though the study hypothesis was confirmed, further investigations should be continued on larger sample sizes, as well as including data related to OME, such as number, duration and chronicity of occurrences, history of hearing loss, and sensory deprivation time.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0165" class="elsevierStylePara elsevierViewall">Most children with CLP in this study presented impairment of the auditory skills that compose the CAP, and temporal resolution was the most impaired. Concerning the language skill, the sample presented alteration, especially concerning the receptive language, in which verbal comprehension was the most impaired.
