Los objetivos de la RTUV son resecar completamente las lesiones y realizar un diagnóstico correcto para estadificar adecuadamente al paciente. Recientemente se ha publicado que, cuando hay inclusión músculo en el espécimen, la re-resección transuretral (re-RTU) no influye en la progresión ni en la supervivencia específica del cáncer de vejiga<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">1</span></a>. Sin embargo, según las directrices de la Asociación Europea de Urología (EAU), cuando se diagnostica un tumor de alto riesgo en la primera RTUV, es altamente recomendable repetir la resección con el objetivo de minimizar el riesgo de tumor residual y de infraestadificación<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">2</span></a>. Esta recomendación se basa sobre todo en las elevadas tasas de enfermedad residual de la re-RTU presentes en la literatura. El porcentaje de pacientes que presentan un tumor residual en la re-RTU alcanza el 75%, de los cuales el 28% había sido infraestadificado en la primera resección<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">3</span></a>. La re-RTU es un procedimiento invasivo con un cierto riesgo de complicaciones (hasta el 16%) que puede empeorar el estado funcional del paciente y, en consecuencia, retrasar el tratamiento posterior de la enfermedad<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">4</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Lamentablemente, los factores predictivos que pueden identificar <span class="elsevierStyleItalic">a priori</span> a aquellos pacientes con un mayor riesgo de enfermedad residual tras una primera RTUV supuestamente completa no han sido bien documentados.</p><p id="par0015" class="elsevierStylePara elsevierViewall">El objetivo del presente estudio es identificar los factores relacionados con el paciente y el tumor que pueden influir en la presencia de enfermedad residual en la re-RTU en una gran cohorte de pacientes con tumores T1G3.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material y métodos</span><p id="par0020" class="elsevierStylePara elsevierViewall">El diseño del estudio y los criterios de selección de pacientes han sido previamente comunicados por Gontero et al.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">1,5</span></a>. Brevemente, los pacientes con tumores primarios T1 G3 (OMS 1973)/de alto grado (HG) (ISUP 1998/OMS 2004) o enfermedad T1G3/HG secundaria de tumor CVNMI T1G3/HG naïve a la BCG formaron esta cohorte de estudio retrospectivo siempre que hubiesen recibido al menos un ciclo de inducción completo de BCG entre 1990 y 2011.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Se recogieron las siguientes características del paciente y del tumor: edad, sexo, historial de tabaquismo e intensidad, exposición a compuestos químicos, estado del tumor (primario o recurrente), quimioterapia intravesical previa, tamaño del tumor (< 3 cm vs. > 3 cm), multifocalidad del tumor (solitario vs. múltiple), presencia de carcinoma <span class="elsevierStyleItalic">in situ</span> (CIS) concomitante, afectación de la uretra prostática con o sin invasión del estroma, presencia de músculo en el espécimen de tejido inicial, dosis de BCG y número total de instilaciones. También se registró información sobre la re-RTU (definida como una segunda RTU realizada en las cuatro a seis semanas siguientes a la RTU inicial y antes de la administración de BCG). Los resultados de la patología en la re-RTU se clasificaron como: sin evidencia de enfermedad, enfermedad persistente en estadio bajo (Ta) o enfermedad persistente T1. Los pacientes con enfermedad músculo-invasiva en la re-RTU no cumplían los criterios de inclusión del estudio y, por tanto, fueron previamente excluidos.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Se analizaron la edad, el sexo, el estado del tumor (primario/recurrente), la terapia intravesical previa, el tamaño del tumor, la multifocalidad del tumor, la presencia de CIS concomitante y el músculo en el espécimen para evaluar los factores de riesgo de enfermedad residual en la nueva RTU, tanto en los análisis univariantes (prueba de <span class="elsevierStyleItalic">X</span><span class="elsevierStyleSup"><span class="elsevierStyleItalic">2</span></span>) como en los multivariantes mediante regresión logística lineal.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Resultados</span><p id="par0035" class="elsevierStylePara elsevierViewall">En nuestra cohorte retrospectiva de 2.451 pacientes con tumores primarios T1G3 tratados inicialmente con BCG, se dispone de los resultados patológicos de 934 pacientes (38,1%) que se sometieron a una re-RTU.</p><p id="par0040" class="elsevierStylePara elsevierViewall">La distribución de las características de los pacientes y de los tumores en estos 934 pacientes se indica en la <a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>: 548 (58,7%) tenían menos de 70 años, 755 (80,8%) eran varones, 847 (90,7%) eran tumores primarios, 687 (73,6%) tenían tumores multifocales, 256/447 (57,2%) tenían tumores de menos de 3 cm de diámetro y 241 (25,8%) tenían CIS concomitante.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">No se encontró enfermedad residual en 267 pacientes (28,6%) mientras que sí hubo enfermedad residual en 667 pacientes (71,4%): Ta en 378 (40,5%) y T1 en 289 (30,9%) pacientes. Se analizaron todos los factores anteriores para evaluar los factores de riesgo de enfermedad residual en la re-RTU.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Los resultados de los análisis univariantes se recogen en la <a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>. No fueron factores de riesgo de enfermedad residual: la edad, el sexo, el estado del tumor y la quimioterapia intravesical previa. Los siguientes fueron factores de riesgo para la presencia de enfermedad residual: la ausencia de músculo en la RTU inicial, los tumores múltiples, los tumores > 3 cm y la presencia de CIS concomitante. Se obtuvieron resultados similares en los 623 pacientes con inclusión de músculo en la RTU inicial.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Debido a la correlación entre la multifocalidad y el tamaño del tumor, el modelo de regresión logística multivariante para la enfermedad residual retuvo el número de tumores o el diámetro del tumor (pero no ambos), p < 0,001. La presencia de músculo en el espécimen dejó de ser significativa, mientras que la presencia de CIS sólo siguió siendo significativa en el modelo con el tamaño del tumor, p < 0,001 (<a class="elsevierStyleCrossRef" href="#tbl0015">tabla 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discusión</span><p id="par0060" class="elsevierStylePara elsevierViewall">Igual que en estudios anteriores<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">6</span></a>, encontramos que los pacientes con tumores múltiples, y aquellos con tumores de gran tamaño y CIS concomitantes tenían una mayor probabilidad de enfermedad residual en la re-RTU. Además, en estos casos la presencia de músculo no parece reducir la tasa de tumor residual tras la primera RTU. Sin embargo, aún no se ha establecido un panel definitivo de factores pronósticos para predecir la enfermedad residual.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Históricamente, la función de la re-RTU ha sido evitar el riesgo de enfermedad residual y la infraestadificación<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">7</span></a>. Cumberbatch et al. encontraron que la mortalidad específica por cáncer era mayor en los pacientes sin RTU (17 vs. 31%). Del mismo modo, esta revisión mostró una mayor tasa de progresión en el grupo sin re-RTU<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">8</span></a>. De acuerdo con estos datos, no es sorprendente que la re-RTU confiera un beneficio en términos de supervivencia libre de recurrencia.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Sin embargo, la utilidad real de este procedimiento no está aceptada de manera homogénea y no hay claridad respecto a los beneficios a largo plazo en términos de la supervivencia libre de recurrencia (SLR) y la supervivencia libre de progresión (SLP). Algunos autores destacaron que la re-RTU representa una intervención quirúrgica adicional con costes considerables y posibles complicaciones, con un retraso global para iniciar el tratamiento adyuvante.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Cada vez hay más pruebas que sugieren que la re-RTU podría representar un procedimiento innecesario, con costes adicionales y posible morbilidad<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">9</span></a>. Estudios recientes, que incluyeron pacientes con CV T1 con y sin tratamiento de BCG, no mostraron ninguna mejora en términos de la SLP y la supervivencia cáncer-específica (SCE) de los pacientes sometidos a una re-RTU con presencia del músculo detrusor en la RTU inicial. Gontero et al. confirmaron esta hipótesis<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">1</span></a>. En su análisis retrospectivo, la tasa de enfermedad residual en la RTU en una población de 935 pacientes fue del 71%, variando del 65 al 86% según la presencia o ausencia de músculo en la primera resección. El análisis multivariante no encontró un impacto pronóstico significativo de la re-RTU con base en la presencia o ausencia del músculo en la primera RTU<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">1</span></a>. Conclusiones similares fueron presentadas por Dutta et al. en su población de pacientes cT1 tratados mediante cistectomía. El riesgo de inexactitud en la estadificación estaba estrictamente relacionado con la presencia o ausencia de músculo en el espécimen de la primera RTU (30 y 62% de re-estadificación en la cistectomía, respectivamente)<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">10</span></a>.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Aunque nuestros análisis univariantes apoyan la importancia de la presencia de músculo en el espécimen de la RTU, en el análisis multivariante encontramos que los pacientes con tumores de gran tamaño (> 3 cm) tenían una mayor tasa de enfermedad residual, independientemente de la presencia de músculo en la primera resección.</p><p id="par0085" class="elsevierStylePara elsevierViewall">De acuerdo con esta tendencia, las directrices actuales de la EAU sugieren que la re-RTU se debe realizar en los casos de RTU inicial incompleta; cuando el músculo no está presente en la resección inicial (excepto para TaG1 y CIS primario) y en todos los tumores T1 y HG/G3 (excepto para CIS)<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">11</span></a>.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Con el fin de seleccionar mejor a los pacientes que realmente se puedan beneficiar de una segunda resección, algunos autores han tratado de identificar los factores predictivos de la enfermedad residual en la re-RTU. Recientemente, Ferro et al. presentaron los resultados de un estudio prospectivo en 1.155 pacientes con cáncer de vejiga primario T1 HG/G3 que se sometieron a una re-RTU en las seis semanas siguientes a la primera RTU<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">9</span></a>. Según estos autores, se encontró enfermedad residual de alto grado en la RTU en el 24,9% de los pacientes. Los pacientes que presentaban cáncer residual de alto grado en la re-RTU tenían más probabilidades de tener CIS, tumores múltiples y un tamaño del tumor superior a 3 cm en la primera resección<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">9</span></a>. Del mismo modo, nuestra serie confirmó que el tamaño y el CIS asociado son factores de riesgo de enfermedad residual.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Se deben reconocer algunas limitaciones de este estudio. En primer lugar, se trata de un análisis retrospectivo. Faltan datos para algunos factores pronósticos y características tumorales como el tamaño del tumor, y no hay una evaluación estandarizada. Además, no hubo un estudio patológico centralizado. En segundo lugar, nuestra serie procede de una gran población retrospectiva de T1G3<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">5</span></a> en la que la re-RTU no se determinó de forma aleatoria. En consecuencia, una serie de sesgos (multifocalidad, tumores de gran tamaño, etc.) podrían haber influido en la decisión de realizar una re-RTU, haciendo que la población de re-RTU fuese seleccionada<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">5</span></a>.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusiones</span><p id="par0100" class="elsevierStylePara elsevierViewall">En los pacientes con enfermedad de alto riesgo, aquellos con tumores múltiples y tumores mayores de 3 cm deberían someterse a una re-RTU debido al alto riesgo de tener enfermedad residual. Es bien sabido que la presencia de músculo detrusor en el espécimen es un requisito previo para minimizar el riesgo de infraestadificación.</p><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">La persistencia de enfermedad tras la resección de los tumores vesicales no es infrecuente, y es la razón por la que las guías europeas recomiendan una re-resección transuretral (re-RTU) para todos los tumores T1. Recientemente se ha publicado que, en los casos con inclusión de músculo en el espécimen, la re-RTU no afecta la progresión ni la supervivencia específica del cáncer.</p><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Presentamos aquí los factores relacionados con el paciente y el tumor que pueden influir en la presencia de enfermedad residual en la re-RTU.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material y métodos</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">De nuestra cohorte retrospectiva de 2.451 pacientes con tumores T1G3 primarios tratados inicialmente con bacilo de Calnette-Guérin (BCG), están disponibles los resultados patológicos de 934 pacientes (38,1%) que se sometieron a una re-RTU. El 74% tenía tumores multifocales, el 20% de los tumores tenía más de 3 cm de diámetro y el 26% tenía carcinoma <span class="elsevierStyleItalic">in situ</span> (CIS) concomitante. En este subgrupo de pacientes que se sometieron a una segunda RTU, no hubo enfermedad residual en 267 pacientes (29%) y se presentó enfermedad residual en 667 pacientes (71%): Ta en 378 (40%) y T1 en 289 (31%) pacientes. Se analizaron la edad, el sexo, el estado del tumor (primario/recurrente), la terapia intravesical previa, el tamaño del tumor, la multifocalidad del tumor, la presencia de CIS concomitante y la inclusión de músculo en el espécimen para evaluar los factores de riesgo de enfermedad residual en la re-RTU, tanto en los análisis univariantes, como en las regresiones logísticas multivariantes.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Resultados</span><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">La edad, el sexo, el estado del tumor y la quimioterapia intravesical previa no fueron factores de riesgo de enfermedad residual, mientras que la ausencia de músculo en la RTU, los tumores múltiples, los tumores > 3 cm y la presencia de CIS concomitante sí fueron factores de riesgo univariantes para la presencia de enfermedad residual. Debido a la correlación entre la multifocalidad y el tamaño del tumor, el modelo multivariante retuvo el número de tumores o el diámetro del tumor (pero no ambos), p < 0,001. La presencia de músculo en el espécimen dejó de ser significativa, mientras que la presencia de CIS sólo fue significativa en el modelo con el tamaño del tumor, p < 0,001.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusiones</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">Los factores más significativos para un mayor riesgo de enfermedad residual en la re-RTU en pacientes con tumores T1G3 son los tumores multifocales y los tumores de más de 3 cm. It is well known that the presence of detrusor muscle in the specimen is a prerequisite to minimize the risk of under staging.</p><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Persistent disease after resection of bladder tumors is not uncommon and is the reason why the European Guidelines recommended a re-TUR for all T1 tumors. It was recently published that when there is muscle in the specimen, re-TUR does not influence progression or cancer specific survival.</p><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">We present here the patient and tumor factors that may influence the presence of residual disease at re-TUR.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Material and methods</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">In our retrospective cohort of 2451 primary T1G3 patients initially treated with BCG, pathology results for 934 patients (38.1%) who underwent re-TUR are available. 74% had multifocal tumors, 20% of tumors were more than 3 cm in diameter and 26% had concomitant CIS.</p><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">In this subgroup of patients who underwent re-TUR, there was no residual disease in 267 patients (29%) and residual disease in 667 patients (71%): Ta in 378 (40%) and T1 in 289 (31%) patients. Age, gender, tumor status (primary/recurrent), previous intravesical therapy, tumor size, tumor multi-focality, presence of concomitant CIS, and muscle in the specimen were analyzed in order to evaluate risk factors of residual disease at re-TUR, both in univariate analyses and multivariate logistic regressions.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">The following were not risk factors for residual disease: age, gender, tumor status and previous intravesical chemotherapy. The following were univariate risk factors for presence of residual disease: no muscle in TUR, multiple tumors, tumors <span class="elsevierStyleUnderline">></span> 3 cm, and presence of concomitant CIS. Due to the correlation between tumor multi-focality and tumor size, the multivariate model retained either the number of tumors or the tumor diameter (but not both), p < 0.001.
