de las parejas en todo el mundo&#46; Aproximadamente el 20-30&#37; de los casos de infertilidad est&#225;n relacionados con factores masculinos<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">1&#44;2</span></a>&#46; Una de las principales causas de infertilidad masculina se debe a defectos en la cantidad y calidad de los espermatozoides&#44; que a su vez pueden estar asociados a factores ambientales y gen&#233;ticos&#46; Sin embargo&#44; en el 30&#37; de los casos de infertilidad las causas siguen siendo desconocidas&#44; y estos se consideran como infertilidad idiop&#225;tica<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">3&#44;4</span></a>&#46; La oligozoospermia y la azoospermia idiop&#225;tica&#44; trastornos comunes de la espermatog&#233;nesis&#44; afectan a un n&#250;mero elevado de varones inf&#233;rtiles<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">5</span></a>&#46; por Single Nucleotide Polymorphism&#41; en varios genes implicados en la espermatog&#233;nesis son una causa importante de infertilidad masculina<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">8&#44;9</span></a>&#46; En los &#250;ltimos a&#241;os&#44; numerosos estudios han investigado la asociaci&#243;n entre SNP y mutaciones en genes candidatos relacionados con la infertilidad masculina y la infertilidad masculina idiop&#225;tica<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">10&#44;11</span></a>&#46; Se han identificado ciertos polimorfismos en los genes <span class="elsevierStyleItalic">LHCGR</span> y <span class="elsevierStyleItalic">NR5A1</span> que desempe&#241;an funciones cr&#237;ticas en la oligozoospermia y la azoospermia idiop&#225;tica<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">12&#44;13</span></a>&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">El gen <span class="elsevierStyleItalic">LHCGR</span> &#40;receptor de la hormona luteinizante&#47;coriogonadotropina&#41; codifica receptores acoplados a prote&#237;nas G &#40;GPCR&#41; que participan en la transducci&#243;n de se&#241;ales extracelulares mediante la activaci&#243;n de la cascada de prote&#237;nas G<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">14&#44;15</span></a>&#46; Este gen se expresa en las c&#233;lulas de la granulosa del estroma&#44; de la teca&#44; luteales y de fase tard&#237;a &#40;luteinizante&#41; del ovario y los test&#237;culos<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">16</span></a>&#46; Hasta la fecha se han identificado m&#225;s de 300 polimorfismos del gen <span class="elsevierStyleItalic">LHCGR</span>&#59; sin embargo&#44; solo en 4 de estos se trata de mutaciones no sin&#243;nimas y se producen dentro de los exones&#46; El polimorfismo no sin&#243;nimo rs2293275 &#40;c&#46;935G&#62;A&#47;p&#46;Ser312Asn&#41;&#44; localizado en el ex&#243;n 11 del gen <span class="elsevierStyleItalic">LHCGR</span>&#44; ha sido descrito como un SNP importante en la asociaci&#243;n con la infertilidad masculina<a class="elsevierStyleCrossRefs" href="#bib0280"><span class="elsevierStyleSup">17&#44;18</span></a>&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">El gen receptor nuclear del miembro 1 del grupo A de la subfamilia 5 &#40;<span class="elsevierStyleItalic">NR5A1</span>&#44; anteriormente denominado SF-1&#41; codifica un factor de transcripci&#243;n esencial que desempe&#241;a un papel importante en la regulaci&#243;n de la reproducci&#243;n y la diferenciaci&#243;n sexual<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">19</span></a>&#46; Este gen se expresa en las c&#233;lulas de Leydig y de Sertoli en los test&#237;culos&#44; en el ovario maduro y en las c&#233;lulas fetales&#46; Seg&#250;n estudios previos&#44; el gen <span class="elsevierStyleItalic">NR5A1</span> es esencial en el desarrollo gonadal y su polimorfismo se asocia a la infertilidad masculina<a class="elsevierStyleCrossRefs" href="#bib0295"><span class="elsevierStyleSup">20-22</span></a>&#46; El polimorfismo de sentido err&#243;neo &#40;o <span class="elsevierStyleItalic">missense</span>&#41; rs1057517779 &#40;c&#46;938G&#62;A&#47;p&#46;Arg313Cys&#41;&#44; localizado en el ex&#243;n 7 del gen <span class="elsevierStyleItalic">NR5A1</span>&#44; se ha descrito como un SNP altamente asociado a la infertilidad masculina<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">23</span></a>&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">Hasta la fecha no se ha descrito una correlaci&#243;n entre los polimorfismos de los genes <span class="elsevierStyleItalic">LHCGR</span> &#40;rs2293275&#41; y <span class="elsevierStyleItalic">NR5A1</span> &#40;rs1057517779&#41; y el riesgo de infertilidad masculina en la poblaci&#243;n azer&#237; en Ir&#225;n&#46; En el presente estudio&#44; nos propusimos investigar la correlaci&#243;n entre el polimorfismo de los genes <span class="elsevierStyleItalic">LHCGR</span> &#40;rs2293275&#41; y <span class="elsevierStyleItalic">NR5A1</span> &#40;rs1057517779&#41; y la infertilidad masculina idiop&#225;tica por primera vez en la poblaci&#243;n azer&#237; de Ir&#225;n&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">M&#233;todos</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Sujetos del estudio</span><p id="par0030" class="elsevierStylePara elsevierViewall">Este estudio de casos y controles incluy&#243; a 200 varones azer&#237;es iran&#237;es de la provincia de Azerbaiy&#225;n Oriental de Ir&#225;n&#46; El grupo de casos estaba compuesto por 100 varones con confirmaci&#243;n de azoospermia y oligozoospermia idiop&#225;tica remitidos al Servicio de Infertilidad del Hospital Valiasr de Tabriz &#40;Ir&#225;n&#41; entre enero de 2018 y diciembre de 2020&#46; El grupo de control inclu&#237;a 100 varones sanos f&#233;rtiles sin anomal&#237;as esperm&#225;ticas&#44; no emparentados gen&#233;ticamente y emparejados por edad&#44; raza y etnia&#46; Los criterios de exclusi&#243;n para el grupo de casos fueron un cariotipo anormal y&#47;o microdeleciones en el cromosoma Y&#44; hipogonadismo hipogonadrot&#243;pico&#44; orquitis&#44; obstrucci&#243;n del conducto eyaculador&#44; criptorquidia e hipogonadismo&#46; Se recogieron variables demogr&#225;ficas y caracter&#237;sticas de los pacientes y los controles sanos mediante cuestionarios y entrevistas &#40;<a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>&#41;&#46; y <span class="elsevierStyleItalic">NR5A1</span> &#40;rs1057517779&#41; en pacientes inf&#233;rtiles y controles sanos&#46; El an&#225;lisis estad&#237;stico indic&#243; una correlaci&#243;n significativa entre los polimorfismos <span class="elsevierStyleItalic">LHCGR</span> &#40;rs2293275&#41; Siendo la oligozoospermia y la azoospermia idiop&#225;tica las principales causas de infertilidad masculina&#44; numerosos estudios recientes se han centrado en esta cuesti&#243;n&#46; Las principales causas de oligozoospermia y azoospermia son la infecci&#243;n&#44; las anomal&#237;as en la estructura y en la cantidad de espermatozoides&#44; alteraciones inmunol&#243;gicas&#44; cromos&#243;micas&#44; varicocele y disfunci&#243;n endocrina&#46; La oligozoospermia y la azoospermia idiop&#225;tica con recuento anormal de espermatozoides constituyen las principales causas de infertilidad masculina<a class="elsevierStyleCrossRefs" href="#bib0320"><span class="elsevierStyleSup">25&#44;26</span></a>&#46; Recientemente se han llevado a cabo amplios estudios para identificar los mecanismos moleculares subyacentes de la infertilidad masculina&#59; sin embargo&#44; muchos casos de infertilidad masculina siguen siendo idiop&#225;ticos&#46; Se sugiere que la presencia de mutaciones y SNP en genes relacionados con la espermatog&#233;nesis son la causa com&#250;n de la oligozoospermia o la azoospermia masculinas<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">7&#44;27</span></a>&#46;</p><p id="par0075" class="elsevierStylePara elsevierViewall">En este estudio se han llevado a cabo an&#225;lisis genot&#237;picos a 100 varones inf&#233;rtiles &#40;azoospermia y&#47;o oligozoospermia&#41; y 100 controles sanos de la poblaci&#243;n azer&#237; iran&#237; para investigar la asociaci&#243;n entre los polimorfismos de los genes <span class="elsevierStyleItalic">LHCGR</span> &#40;rs2293275&#41; y <span class="elsevierStyleItalic">NR5A1</span> &#40;rs1057517779&#41; y la infertilidad masculina idiop&#225;tica&#46; Los resultados indicaron una asociaci&#243;n significativa entre los polimorfismos rs2293275 y rs1057517779 y la infertilidad masculina idiop&#225;tica en esta poblaci&#243;n&#46;</p><p id="par0080" class="elsevierStylePara elsevierViewall">Seg&#250;n estudios anteriores&#44; el gen <span class="elsevierStyleItalic">LHCGR</span> normal es esencial para la fertilidad de varones y mujeres&#44; mientras que los defectos en el gen <span class="elsevierStyleItalic">LHCGR</span> pueden causar infertilidad<a class="elsevierStyleCrossRefs" href="#bib0335"><span class="elsevierStyleSup">28&#44;29</span></a>&#46; Varios estudios han informado de que los polimorfismos del gen <span class="elsevierStyleItalic">LHCGR</span> est&#225;n asociados a la fecundaci&#243;n <span class="elsevierStyleItalic">in vitro</span> &#40;FIV&#41; y a los niveles de testosterona<a class="elsevierStyleCrossRefs" href="#bib0280"><span class="elsevierStyleSup">17&#44;30</span></a>&#46; El &#250;nico estudio realizado por Simoni et al&#46; inform&#243; de que los polimorfismos del gen <span class="elsevierStyleItalic">LHCGR</span> se asocian significativamente con test&#237;culos mal descendidos e infertilidad masculina&#46; Los autores demostraron que el alelo C del polimorfismo rs2293275 en el ex&#243;n 10 del gen <span class="elsevierStyleItalic">LHCGR</span> era significativamente menos frecuente en hombres con test&#237;culos mal descendidos que en controles sanos<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">31</span></a>&#46; Nuestro estudio es el primer informe sobre la correlaci&#243;n significativa entre el polimorfismo rs2845570 de <span class="elsevierStyleItalic">LHCGR</span>&#44; la azoospermia y la oligozoospermia en todo el mundo&#46;</p><p id="par0085" class="elsevierStylePara elsevierViewall">Inicialmente se inform&#243; de funciones anormales del gen <span class="elsevierStyleItalic">NR5A1</span> en pacientes con defectos de insuficiencia suprarrenal primaria&#44; estructuras m&#252;llerianas y trastornos del desarrollo sexual<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">32</span></a>&#46; Adem&#225;s&#44; estudios anteriores sugirieron que las variantes del gen <span class="elsevierStyleItalic">NR5A1</span> est&#225;n asociadas con la infertilidad masculina<a class="elsevierStyleCrossRefs" href="#bib0360"><span class="elsevierStyleSup">33&#44;34</span></a>&#46; Hasta ahora&#44; varios estudios con resultados diferentes han informado de la correlaci&#243;n entre los polimorfismos del gen <span class="elsevierStyleItalic">NR5A1</span> y la infertilidad masculina&#46; En un estudio de R&#246;pke et al&#46; se inform&#243; de una correlaci&#243;n significativa entre las variantes del gen <span class="elsevierStyleItalic">NR5A1</span> y la infertilidad masculina en la poblaci&#243;n alemana<a class="elsevierStyleCrossRef" href="#bib0370"><span class="elsevierStyleSup">35</span></a>&#46; Sin embargo&#44; 2 estudios realizados por Sudhakar et al&#46; y Adamovic et al&#46; informaron de la ausencia de asociaci&#243;n significativa entre los polimorfismos del gen <span class="elsevierStyleItalic">NR5A1</span> y la infertilidad masculina en las poblaciones india y china<a class="elsevierStyleCrossRefs" href="#bib0310"><span class="elsevierStyleSup">23&#44;36</span></a>&#46; Nuestro estudio es el primer informe en confirmar la asociaci&#243;n significativa entre el polimorfismo del gen <span class="elsevierStyleItalic">NR5A1</span> rs1057517779 y la infertilidad masculina en la poblaci&#243;n iran&#237;&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">La contradicci&#243;n en los resultados de los estudios mencionados puede deberse a los efectos de otros genes o polimorfismos implicados en la infertilidad masculina&#44; al sesgo de selecci&#243;n de la muestra y al tama&#241;o de esta&#44; a los antecedentes gen&#233;ticos de la poblaci&#243;n&#44; a factores ambientales&#44; a diferencias en la zona geogr&#225;fica y a la heterogeneidad en el origen &#233;tnico y la raza<a class="elsevierStyleCrossRefs" href="#bib0380"><span class="elsevierStyleSup">37-39</span></a>&#46;</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Conclusiones</span><p id="par0095" class="elsevierStylePara elsevierViewall">En t&#233;rminos generales&#44; nuestro estudio sugiri&#243; que los polimorfismos de los genes <span class="elsevierStyleItalic">LHCGR</span> &#40;rs2293275&#41; y <span class="elsevierStyleItalic">NR5A1</span> &#40;rs1057517779&#41; se asocian significativamente con el riesgo de infertilidad masculina en la poblaci&#243;n azer&#237; iran&#237;&#46; Sin embargo&#44; se requieren m&#225;s estudios con un tama&#241;o de muestra mayor en otras poblaciones&#44;
