id="sect0105">Introducción</span><p id="par0005" class="elsevierStylePara elsevierViewall">La enfermedad arterial periférica (EAP) afecta al 15-20% de la población mayor de 70<span class="elsevierStyleHsp" style=""></span>años<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1-5</span></a>, porcentaje que podría incrementarse si se tienen en cuenta pacientes asintomáticos<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">6</span></a>. Su presencia afecta la calidad de vida y aumenta la morbimortalidad cardiovascular<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1,6-9</span></a>, y esto hace necesario tener métodos diagnósticos que permitan implementar medidas preventivas<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">9</span></a> precozmente.</p><p id="par0010" class="elsevierStylePara elsevierViewall">El índice tobillo-brazo (ITB) es el método no invasivo con mayor rendimiento en el diagnóstico de la EAP<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a>. Se calcula dividiendo la mayor de las presiones arteriales sistólicas (PAS) a nivel del tobillo entre la mayor PAS en los brazos. Un ITB <<span class="elsevierStyleHsp" style=""></span>0,9 sugiere la presencia de EAP, mientras que cifras ≥<span class="elsevierStyleHsp" style=""></span>1,4 indican posible calcificación arterial (CA).</p><p id="par0015" class="elsevierStylePara elsevierViewall">A pesar de ser un método sencillo, sensible (90%) y específico (98%)<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">11</span></a>, el tiempo que precisa y la necesidad de experiencia han dificultado su implantación en atención primaria (AP)<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">12-15</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Para superar estas limitaciones se ha planteado que los oscilómetros automáticos podrían sustituir al método con sonda Doppler. Diversos estudios han comparado ambos métodos, aunque los resultados publicados son variables<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">16-18</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Para aportar mayor evidencia sobre el tema se diseñó este estudio, cuyo objetivo fue establecer la concordancia entre el método con sonda Doppler y dos métodos automáticos: Watch BP Office (TWIN200ABI) y MESI ABPIMDD.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Material y métodos</span><p id="par0030" class="elsevierStylePara elsevierViewall">Estudio descriptivo transversal realizado en el centro de AP de Parets del Vallès (Barcelona) en julio de 2017.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Selección de los pacientes</span><p id="par0035" class="elsevierStylePara elsevierViewall">Se incluyeron 202 pacientes de entre 50 y 77<span class="elsevierStyleHsp" style=""></span>años que espontáneamente acudieron de modo consecutivo al centro por cualquier motivo. No se invitó a participar a personas con criterios de exclusión, por mala tolerancia al procedimiento (por lesiones, flebitis, linfangitis o trombosis venosa). Los que aceptaron participar fueron citados otro día.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Variables</span><p id="par0040" class="elsevierStylePara elsevierViewall">La variable principal fue el valor continuo del ITB con los tres métodos y su categorización en sano (ITB entre 0,9-1,4), EAP (<<span class="elsevierStyleHsp" style=""></span>0,9) y CA (≥<span class="elsevierStyleHsp" style=""></span>1,4). Se recogió: sexo, edad, riesgo cardiovascular (REGICOR), riesgo REASON de padecer EAP<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a>, índice de masa corporal (IMC), tabaquismo, antecedentes patológicos y existencia de clínica de claudicación (según el test de Edimburgo<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a>).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Recogida de datos</span><p id="par0045" class="elsevierStylePara elsevierViewall">Se siguió siempre este orden (<a class="elsevierStyleCrossRef" href="#fig0010">fig. 1</a>): Doppler, Watch BP (WBP) y MESI ABPIMDD (MESI). Un observador realizó las medidas y otro rellenó los datos de la historia clínica y calculó el índice REASON.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">Solo dos enfermeras con experiencia en la técnica llevaron a cabo las mediciones.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Se registraron los casos en que no se pudo obtener el valor del ITB, las incidencias y el tiempo empleado.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Antes de la exploración se garantizó un reposo de 5<span class="elsevierStyleHsp" style=""></span>min.</p><p id="par0065" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">ITB mediante sonda Doppler</span>. Modelo Hingtleigh HealthcareDopplex<span class="elsevierStyleHsp" style=""></span>II Modelo SD2 y esfigmomanómetro manual CORYSAN tipo Manuell. Se determinó el brazo con mayor PAS (brazo control [BC]) con la sonda Doppler. A continuación se obtuvo la PAS en las piernas, primero derecha y luego izquierda, en la arteria pedia y la tibial posterior.</p><p id="par0070" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">ITB mediante WatchBP Office (TWIN200 ABI)</span>. Se detectó el BC, medida que permite WBP, tomando automática y simultáneamente tres medidas consecutivas separadas por intervalos de un minuto, obteniendo la media de cada brazo. Luego se colocó un manguito en el BC y una tobillera en la pierna izquierda. El ITB se calculó automáticamente. Se realizó lo mismo en la derecha.</p><p id="par0075" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">ITB mediante MESI Modelo ABPIMDD</span>. Se colocó uno de los manguitos en el BC obtenido con Doppler y una tobillera en cada una de las piernas. El ITB se calculó automáticamente para cada una.</p><p id="par0080" class="elsevierStylePara elsevierViewall">En los tres métodos el registro del tiempo incluyó: reposo de 5<span class="elsevierStyleHsp" style=""></span>min, detección del BC y tiempo de ejecución de la medida.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Análisis estadístico</span><p id="par0085" class="elsevierStylePara elsevierViewall">Se usó el coeficiente de correlación intraclase (CCI) para ver el grado de acuerdo en la medida del ITB, junto a los métodos gráficos de Bland-Altman y nubes de puntos. Los cálculos se repitieron para diferentes patrones de pacientes para ver si la concordancia entre métodos era homogénea entre diferentes pacientes o dependía de sus características.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Se evaluó la concordancia usando la variable ITB categorizada en 3 grupos (EAP, sano y CA) usando kappa. La diferencia de prevalencia de EAP entre los tres métodos se evaluó mediante un test de diferencia de proporciones.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Cuando no se pudo obtener una medida se describió el motivo. Se comparó 2 a 2 la frecuencia de «no medidos» mediante test de McNemar por cada método. La comparación entre variables continuas se realizó mediante prueba t de Student apareada, y las categóricas mediante chi cuadrado.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Se calcularon: sensibilidad, especificidad, valores predictivos positivo (VPP) y negativo (VPN), y las razones de probabilidades positiva (RPP) y negativa (RPN) de los métodos automáticos versus el método Doppler agrupando los pacientes como «con EAP» y «sin EAP», así como curvas ROC y su área bajo la curva en los pacientes con ITB medido.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Se obtuvieron intervalos de confianza al 95% (IC<span class="elsevierStyleHsp" style=""></span>95%) de todos los estadísticos. Para los CCI, en el caso de que el límite inferior del intervalo fuese negativo se convirtió a cero. Las comparaciones fueron bilaterales y la significación, del 5%. Se usó el paquete estadístico Stata 15.<elsevierMultimedia ident="fig0005"></elsevierMultimedia></p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Resultados</span><p id="par0110" class="elsevierStylePara elsevierViewall">Se incluyeron 404 piernas (202 pacientes) con una edad media de 63<span class="elsevierStyleHsp" style=""></span>años con las características mostradas en la <a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">El ITB se midió con los tres métodos. Doppler no pudo obtener valor debido a la no compresión de la arteria, clasificándose como CA en un 0,7% (3 casos). WBP no dio ningún valor en 9 ocasiones (2,2%) y MESI en 32 (7,9%), siendo estas diferencias estadísticamente significativas al comparar MESI con los otros dos (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001), aunque no al comparar Doppler con WBP (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,083).</p><p id="par0120" class="elsevierStylePara elsevierViewall">El tiempo medio (DE) fue: 12,1 (1,8) min con Doppler, 14,4 (1,6) min WBP y 10,7 (1,3) min MESI (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001). El tiempo de BC fue: 2,6<span class="elsevierStyleHsp" style=""></span>min (Doppler) y 5,7<span class="elsevierStyleHsp" style=""></span>min (WBP) (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001).</p><p id="par0125" class="elsevierStylePara elsevierViewall">Hubo más incidencias con los métodos automáticos (Doppler 4%; WBP 13%; MESI 14%; p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001) (<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">La media (DE) del ITB fue: Doppler 1,05 (0,11), WBP 1,20 (0,15) y MESI 1,20 (0,11), siendo los valores con Doppler más bajos de forma estadísticamente significativa (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001). El incremento medio respecto a Doppler fue del 13,6% (WBP) y del 14,3% (MESI).</p><p id="par0135" class="elsevierStylePara elsevierViewall">La concordancia del valor ITB con Doppler respecto a WBP/MESI fue pobre: CCI<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,27 (IC<span class="elsevierStyleHsp" style=""></span>95% 0,00-0,50) para WBP y 0,20 (0,00-0,42) para MESI (<a class="elsevierStyleCrossRefs" href="#fig0020">figs. 2 y 3</a>).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><elsevierMultimedia ident="fig0025"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">Se repitió el cálculo del CCI según perfiles por obesidad (IMC<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>), tabaquismo, dislipemia, hipertensión y diabetes. En ningún caso el CCI superó 0,54 (datos no mostrados), indicando que no existió buena concordancia incluso en perfiles seleccionados.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Categorizando los valores de ITB en sano, EAP y CA, se asumió que la imposibilidad de compresión de la arteria con Doppler indicaba CA (3<span class="elsevierStyleHsp" style=""></span>piernas), mientras que cuando MESI indicaba error por posible EAP sin dar lectura numérica del ITB se asumía EAP (18<span class="elsevierStyleHsp" style=""></span>piernas). Se observó una proporción algo mayor de EAP detectada con Doppler, aunque no estadísticamente significativa (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,18 y p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,87), al comparar la prevalencia de EAP con Doppler vs WBP y MESI, respectivamente) (<a class="elsevierStyleCrossRef" href="#tbl0015">tabla 3</a>). Los índices kappa también fueron bajos: WBP 0,33 (IC<span class="elsevierStyleHsp" style=""></span>95% 0,26-0,40) y MESI 0,54 (IC<span class="elsevierStyleHsp" style=""></span>95% 0,47-0,62).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0150" class="elsevierStylePara elsevierViewall">Utilizando Doppler como <span class="elsevierStyleItalic">gold standard</span> para el diagnóstico de EAP, la especificidad fue elevada (WBP y MESI: 98%) pero la sensibilidad fue baja (WBP: 44%, MESI: 63%). Los VPP y VPN para WBP fueron del 65 y del 96%, y para MESI, del 68 y del 98%, mientras que las RPP y RPN fueron 27 y 0,6 (WBP) y 32 y 0,4 (MESI). El área bajo la curva ROC para la detección de EAP fue 0,80 para WBP y 0,78 para MESI. El mejor punto de corte para los métodos automáticos para la detección de EAP se estimó en 1,12 (WBP) y en 1,16 (MESI).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Discusión</span><p id="par0155" class="elsevierStylePara elsevierViewall">Diversos estudios han comparado la utilización de un oscilómetro automático y el método con sonda Doppler para la medición del ITB, obteniendo resultados variables, ya que las poblaciones y los modelos automáticos eran distintos<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">14,15</span></a> y los observadores tenían diferente grado de experiencia.</p><p id="par0160" class="elsevierStylePara elsevierViewall">El estudio de Forés et al.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a>, en población visitada en AP, compara la obtención del ITB mediante WBP y el método Doppler en 88 pacientes mayores de 55<span class="elsevierStyleHsp" style=""></span>años, hallando un CCI de 0,13 (IC<span class="elsevierStyleHsp" style=""></span>95% −0,17 a 0,41), descartando a WBP como método de cribado habitual. A pesar de que nuestro resultado ha sido ligeramente mejor, la conclusión es la misma: la concordancia entre Doppler y WBP es baja. Sinski et al.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a> también encuentran, en 80 sujetos con enfermedad coronaria, poca relación entre WBP y Doppler (coeficiente de correlación de Pearson [r] de 0,51, en nuestro estudio r<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,44). Sin embargo, Kollias et al.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> utilizan WBP en 93 sujetos con factores de riesgo cardiovascular, obteniendo una correlación moderadamente alta (r<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,80) y que aumenta discretamente al repetir la medida tres veces con WBP (segunda medida r<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,85, tercera r<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,86).</p><p id="par0165" class="elsevierStylePara elsevierViewall">Hallamos que el tiempo de ejecución de WBP fue superior al de Doppler, principalmente porque tarda más de 3<span class="elsevierStyleHsp" style=""></span>min en definir el BC.</p><p id="par0170" class="elsevierStylePara elsevierViewall">En cuanto a MESI, Span et al.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a> lo utilizaron en 136 sujetos que acudían espontáneamente a AP y que cumplían uno de estos criterios: fumador, diabético o dolor en las piernas durante el ejercicio (r<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,61; en nuestro estudio r<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,37). Aunque indican que la sensibilidad, usando Doppler como <span class="elsevierStyleItalic">gold standard</span>, fue del 85%, esto se produjo al aumentar el punto de corte a ≤<span class="elsevierStyleHsp" style=""></span>1, pero usando <<span class="elsevierStyleHsp" style=""></span>0,9 la sensibilidad fue del 57%. En nuestro estudio el punto de corte<span class="elsevierStyleHsp" style=""></span>1 para MESI aún funcionó peor, ya que se reclasificaban 4 sujetos a EAP cuando, según Doppler, no tenían EAP. En relación al tiempo, registraron 14<span class="elsevierStyleHsp" style=""></span>min con Doppler y con MESI 2<span class="elsevierStyleHsp" style=""></span>min, sin reposo, justificando que la medición se hace en las tres extremidades a la vez. Si restamos en nuestro estudio el tiempo de reposo y de BC, el tiempo de MESI sería de 3,1<span class="elsevierStyleHsp" style=""></span>min, por lo que el ahorro de tiempo al usar MESI se vería incrementado de 1,4 a 9<span class="elsevierStyleHsp" style=""></span>min.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Existe una sobreestimación del ITB por parte de los métodos automáticos que se repite en diferentes estudios, lo que hace necesario ajustar los puntos de corte para aumentar la sensibilidad. Nuestros puntos fueron ITB<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>1,12 para WBP e ITB<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>1,16 para MESI, aumentando la sensibilidad (WBP: 84%; MESI: 88%) en detrimento de la especificidad (86 y 70%).</p><p id="par0180" class="elsevierStylePara elsevierViewall">Otros dispositivos muestran resultados variables. Los estudios de Forés et al.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a> (CCI<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,27), Ena et al.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> (kappa<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,45) y Hamel et al.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> (CCI<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,35) establecieron que OMRON M6 no podía substituir al Doppler. En cambio, la concordancia entre Doppler y otros modelos fue buena: OMRON-M3 HEM-7200-E, Herráiz et al.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a> (área bajo la curva<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,94; kappa<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,68); SCVL utilizado por Rosenbaum et al.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a> (r<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,71); OMRON M4 empleado por Benchimol et al.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">29</span></a> (r<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,68 a 0,72 según extremidad) y Vega et al.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> (área bajo la curva<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,94), y OMRON BP-203 RPEIII por Ma et al.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a> (kappa entre 0,87 y 0,91 según extremidad).</p><p id="par0185" class="elsevierStylePara elsevierViewall">Algunos estudios consideran que las dificultades para detectar presiones muy bajas de los métodos automáticos podrían ser utilizadas para determinar la presencia de EAP<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a>. En 18 ocasiones MESI dio lectura de error no explicable por otro tipo de causas, lo que podría haber indicado presencia de EAP. De ellas, solamente en 11 (61%) se confirmó EAP con Doppler.</p><p id="par0190" class="elsevierStylePara elsevierViewall">La obesidad se ha relacionado con dificultades para determinar el ITB debido a problemas para comprimir la arteria. En nuestro estudio esto pasó con MESI: el 14% de los obesos no obtuvieron ITB, con respecto al 5% de los no obesos (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,002).</p><p id="par0195" class="elsevierStylePara elsevierViewall">Hubo más errores y necesidad de repetir las medidas con los métodos automáticos: 2% de repeticiones con Doppler, 4% con MESI y 10% con WBP.</p><p id="par0200" class="elsevierStylePara elsevierViewall">El único efecto adverso fue la aparición de dolor: 1% Doppler, 1% WBP y 3% MESI. Los casos en que hubo dolor con Doppler también lo hubo con los otros métodos. El hecho de que MESI fuera siempre el tercer método podría haber influido en que hubiera más dolor con él.</p><p id="par0205" class="elsevierStylePara elsevierViewall">La concordancia observada en nuestro estudio se basa en una muestra con una prevalencia de EAP similar a la de otros estudios de la zona<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">3</span></a>, pero en términos absolutos baja. Se investigó si la concordancia mejoraba al seleccionar poblaciones de mayor riesgo cardiovascular, y aunque en algunos perfiles mejoró, el CCI nunca superó 0,54 para WBP y 0,37 para MESI, y kappa no pasó de 0,59 (WBP) ni de 0,86 (MESI), obteniéndose las mejores concordancias en población diabética, aunque hay que tener en cuenta la baja muestra disponible para realizar estos cálculos por perfiles. Limitando el estudio a los sujetos con REASON positivo, las mejoras en la concordancia fueron moderadas, pasando el CCI de 0,27 a 0,35 (WBP) y de 0,20 a 0,24 (MESI), kappa de 0,33 a 0,38 (WBP) y de 0,54 a 0,64 (MESI), mientras que la sensibilidad aumentó del 44 al 55% (WBP) y del 63 al 75% (MESI), manteniéndose la especificidad superior al 96%.</p><p id="par0210" class="elsevierStylePara elsevierViewall">La baja concordancia con WBP fue principalmente por diferencias en la PAS de las piernas. Al comparar las PAS de los brazos encontramos un CCI<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,79 (IC<span class="elsevierStyleHsp" style=""></span>95% 0,75-0,82) entre Doppler y WBP. Esto puede explicarse por diferencias anatómicas, que precisan adaptar los manguitos, por la mayor dificultad de obtener presiones en obesos, y por la forma en que los aparatos oscilométricos miden la presión arterial, entendiéndola como la presión en el punto de máxima oscilación de la arteria. Incrementos de la rigidez arterial causados, por ejemplo, por arteriosclerosis pueden ser motivos de falsas lecturas. No se pudieron estudiar las concordancias específicas a nivel de piernas con MESI, al no disponer de todos los datos desagregados.</p><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Limitaciones del estudio</span><p id="par0215" class="elsevierStylePara elsevierViewall">El método que confirma la presencia de EAP es la arteriografía. Debido a las connotaciones éticas de realizar una prueba invasiva no necesaria, no se hizo esta comprobación.</p><p id="par0220" class="elsevierStylePara elsevierViewall">Para disminuir la variabilidad interobservador, solo dos enfermeras realizaron la técnica.</p><p id="par0225" class="elsevierStylePara elsevierViewall">Para evitar sesgos debidos a un conocimiento previo del resultado del ITB, Doppler se aplicó siempre en primer lugar.</p><p id="par0230" class="elsevierStylePara elsevierViewall">Para evitar la posible influencia de saber que el paciente tenía un mayor riego de EAP, REASON fue siempre calculado por un observador diferente al que efectuaba la medida.</p><p id="par0235" class="elsevierStylePara elsevierViewall">MESI se utilizó después de haber aplicado compresión de forma repetida en las extremidades con los otros métodos; desconocemos si esto puede haber influido en el resultado.</p><p id="par0240" class="elsevierStylePara elsevierViewall">Los métodos automáticos presentaron mayor dificultad de colocación y de compresión en personas obesas, lo que también contribuyó a aumentar las incidencias.</p><p id="par0245" class="elsevierStylePara elsevierViewall">La muestra utilizada, si bien es representativa de las personas que pueden acudir a un centro de AP, no lo es de la población general. Aunque esto no debería afectar la concordancia.</p><p id="par0250" class="elsevierStylePara elsevierViewall">Por último, el cálculo del CCI solamente puede realizarse si se dispone de los valores numéricos de ITB de los métodos a comparar. Los bajos valores observados al comparar Doppler con MESI podrían mejorar si cuando MESI indicaba EAP pero no daba un valor concreto se hubiera asignado un ITB<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,9 a MESI.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Conclusión</span><p id="par0255" class="elsevierStylePara elsevierViewall">El método Doppler no puede sustituirse por los métodos automáticos utilizados en nuestro estudio. Convendría realizar estudios en otras poblaciones o con otros dispositivos para evaluar posibles alternativas al método tradicional.<elsevierMultimedia Se evaluó concordancia de ITB mediante coeficiente de correlación intraclase (CCI) y de ITB categorizado (enfermedad arterial periférica [EAP], sano y calcificación arterial [CA]) mediante índice kappa.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Mediciones principales</span><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">Resultado ITB mediante los tres métodos.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Resultados</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">La concordancia del ITB de los métodos automáticos respecto a Doppler fue baja: CCI (intervalo de confianza del 95% [IC<span class="elsevierStyleHsp" style=""></span>95%]) 0,27 (0,00-0,50) para WBP y 0,20 (0,00-0,42) para MESI. La concordancia de ITB categorizado respecto a Doppler también fue baja: kappa (IC<span class="elsevierStyleHsp" style=""></span>95%) 0,33 (0,26-0,40) para WBP y 0,54 (0,47-0,62) para MESI. Se observó una sobreestimación del ITB con los métodos automáticos respecto a Doppler. Usando este último como <span class="elsevierStyleItalic">gold standard</span>, los métodos automáticos fueron específicos (98%) pero poco sensibles (WBP 44% y MESI 63%). Se determinó que un ITB<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>1,12 (WBP) y <<span class="elsevierStyleHsp" style=""></span>1,16 (MESI) eran los mejores puntos de corte para comparar con ITB<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,9 medido con Doppler.
