The patient is a 31-year-old woman with a history of deep vein thrombosis in the right lower limb (negative thrombophilia study). As an incidental finding in a postoperative scan after appendectomy, infrarenal inferior vena cava agenesis was observed (Fig. 1), with venous drainage through pelvic, lumbar and right abdominal wall collateral vessels (Fig. 2).
The incidence of malformations of the inferior vena cava is 4%, representing a risk factor for the development of deep vein thrombosis (up to 5% in those under 30 years of age). In the case of thrombosis, anticoagulation for 3 to 6 months would be indicated.
