The patient is an 81-year-old man with acute appendicitis. CT scan revealed an indirect right inguinal hernia containing the ipsilateral ureter, which caused mild right hydronephrosis with extrarenal pelvis, yet no clinical or analytical repercussions. Laparoscopic appendectomy was performed without incident, and a Nyhus repair technique was done in a second stage.

Ureteral inguinal hernias are rare. Some 80% are indirect hernias, which may be either preperitoneal (80%) or extraperitoneal (20%). Generally asymptomatic and diagnosed incidentally, the most common complication is inadvertent injury during hernioplasty. The radiological image of a ‘loop ureter’ is characteristic (Fig. 1). Treatment is hernioplasty with precise dissection and reduction of the ureter.
