The indocyanine green (ICG) mode in the thoracic approach to upper third oesophageal diverticula represents an advance in the safety of oesophageal dissection and myotomy, allowing a more adequate control of its tightness. We present the case of a 49-year-old female patient with recurrent respiratory infections secondary to a large diverticulum in the upper part of the lower third of the oesophagus. Thoracoscopy was performed to dissect the diverticulum, using the ICG mode to control the oesophageal lumen during dissection and subsequent myotomy, as well as to confirm airtightness after section of the diverticulum.
