Transaxillary endoscopic approach, a technique well-spread in Far-East countries, could be offered in reference centers of our country. Access is through one periareolar port and two axillary port. The benchmarks are: the cricoid cartilage (upper), the sternal hairpin (lower) and the outer edge of the contralateral ECM muscle (contralateral). Lateral approach allows to preserve the recurrent laryngeal nerve without traction, during the lobectomy and istmectomy. After 48hours without incidents, the drainage is removed and the patient is discharged. This approach is a safe and feasible technique for many endocrine surgeons a technique which can be regularly offered to patients (Fig. 1).
