Se comparan los resultados con un grupo control de 24 pacientes</p> <span class="elsevierStyleSectionTitle">Pacientes y métodos</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Se incluye a 14 pacientes tratados con malla de polipropileno y dermolipectomía abdominal, con aplicación de Tissucol® vaporizado. Se analizan las características clínicas, tipo de cirugía y datos técnicos de la malla colocada, volumen y coste del adhesivo, morbilidad, estancia postoperatoria y recidivas</p> <span class="elsevierStyleSectionTitle">Resultados</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">Los pacientes estudiados son de mayor edad (p < 0,006), con más cirugías previas, más obesos (índice de masa corporal [IMC] > 30 en el 50%), más patología respiratoria, hipotiroidismo y neoplasias previas</p><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">El 50% de las eventraciones eran reproducidas, 42,9% tenían malla previa; en el 78,6% el defecto fue suprainfraumbilical (p < 0,0001), y se colocó una malla de mayor tamaño (p < 0,05), situándola premuscular en el 64,3% (p < 0,0001). El Tissucol® aplicado fue de 2,2 ml/paciente</p><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">La morbilidad local en el grupo estudio fue menor (el 21,4% frente al 41,7%), con menos hematomas (el 7,1 y el 16,7%) y abscesos (el 0 y el 20,8%). La estancia media también fue menor (9,2 frente a 11,6 días). No hubo mortalidad y la recidiva ha sido nula con seguimiento entre 2-18 meses</p> <span class="elsevierStyleSectionTitle">Conclusiones</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">A pesar de ser pacientes con condiciones clínicas desfavorables, de más alto riesgo, con eventraciones más grandes, multioperadas, y cirugía más compleja, tanto la morbilidad local como la estancia media han sido menores, por lo que se puede considerar que la aplicación del adhesivo Tissucol ® disminuye la tasa de hematoma e infección locales y reduce consecuentemente la estancia media hospitalaria, sin efectos secundarios y con un mínimo gasto añadido</p>" ] "en" => array:1 [ "resumen" => "<span class="elsevierStyleSectionTitle">Introduction</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">The development of seromas or hematomas after repair of complex abdominal eventrations is common. This is especially so when associated dermolipectomy is performed as infectious complications can occur</p> <span class="elsevierStyleSectionTitle">Objectives</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">The aim of this study was to determine the efficacy of Tissucol® fibrin glue in reducing local complications (seromas, hematomas, abscesses) after prosthetic repair of eventrations and its effect on length of hospital stay. The results were compared with those obtained in a control group of 24 patients</p> <span class="elsevierStyleSectionTitle">Patients and methods</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">Fourteen patients treated with polypropylene mesh and abdominal dermolipectomy with application of vaporized Tissucol® were included. The clinical features, type of surgery and technical details of the implanted mesh, volume and cost of the glue, morbidity, postoperative stay and recurrence were analyzed</p> <span class="elsevierStyleSectionTitle">Results</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">The study patients were older (p < 0.006), had undergone a greater number of previous surgical interventions, were more obese (body mass index > 30 in 50%), and showed more respiratory disease, hyperthyroidism and previous neoplasias. Fifty percent of the eventrations were recurrences and 42.9% had previous mesh. In 78.6% the defect was suprainfaumbilical (p < 0.0001) and a larger mesh was implanted (p < 0.05) premuscularly in 64.3% (p < 0.0001). The amount of Tissuecol® applied was 2.2 cc/patient. Local morbidity was lower in the study group (21.4% vs. 41.7%) with fewer hematomas (7.1% vs. 16.7%) and abscesses (0% vs. 20.8%). The mean length of hospital stay was also lower (9.2 days vs. 11.6 days). There was no mortality and no recurrence during a followup of between 2 and 16 months</p> <span class="elsevierStyleSectionTitle">Conclusions</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">Although the patients showed unfavorable clinical status, were of high-risk, had larger eventrations, had undergone previous operations and the surgery was more complex, both local morbidity and mean hospital stay were lower.
