Los objetivos de nuestro estudio son conocer la incidencia de la DP y la capacidad de 20 características de los donantes para inducir este cuadro.</p> <span class="elsevierStyleSectionTitle">Pacientes y método</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Se han estudiado de forma prospectiva los 248 TH realizados consecutivamente durante 79 meses en el Hospital Ramón y Cajal de Madrid en 206 pacientes. Los enfermos fueron clasificados en normofunción primaria y disfunción primaria, y en esta última se reagruparon los casos de FP y función primaria inadecuada. Se definió normofunción primaria o función primaria inadecuada según si las cifras de transaminasas y la actividad de protrombina eran superiores o inferiores a 2.000 U/ml y al 50%, respectivamente, entre los días segundo y séptimo tras el trasplante hepático.</p> <span class="elsevierStyleSectionTitle">Resultados</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">La incidencia de DP fue del 9,3% (23 trasplantes hepáticos), de los cuales 12 (4,8%) casos fueron un fallo primario. El análisis univariable relacionó significativamente la edad del donante y la causa de la muerte cerebral de él con una mayor frecuencia de DP. El estudio multivariable únicamente lo relacionó con esta última variable (p = 0,04), con una odds ratio (OR) de 4,25 (intervalo de confianza [IC] del 95%, 1,13-16,0).</p> <span class="elsevierStyleSectionTitle">Conclusiones</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">La DP es una entidad clínica con una incidencia importante. El único factor que ha demostrado su influencia en el desarrollo de una DP es una causa de muerte cerebral diferente de un traumatismo craneoencefálico.</p>" ] "en" => array:1 [ "resumen" => "<span class="elsevierStyleSectionTitle">Introduction</span><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">Primary dysfunction, and its most severe form, primary failure, continue to be a serious problem in liver transplant recipients, since the etiology of this entity is unknown, its incidence is high and it has a huge impact on outcome. The aims of this study were to determine the incidence of primary dysfunction and identify the influence of 20 donor characteristic in inducing this process.</p> <span class="elsevierStyleSectionTitle">Patients and method</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">We prospectively studied 248 consecutive liver transplantations performed in 206 patients over 79 months in the Ramón y Cajal Hospital in Madrid. The patients were classified into those with normal primary function and those with primary dysfunction. The latter group was subdivided into patients with primary failure and those with inadequate primary function. Normal primary function and inadequate primary function were defined according to whether transaminase levels and prothrombin activity were above or below 2,000 U/ml and 50%, respectively, on days 2-7 after transplantation.</p> <span class="elsevierStyleSectionTitle">Results</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">The incidence of primary dysfunction was 9.3% (23 liver transplantations), of which 12 cases were of primary failure (4.8%). In the univariate analysis, a significant association was found between donor age and cause of brain death and a higher frequency of primary dysfunction. Multivariate analysis was performed with the latter variable only (p = 0.04), with an odds ratio of 4.25 (95% CI: 1.13-16.0).</p> <span class="elsevierStyleSectionTitle">Conclusions</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">The incidence of primary dysfunction is considerable.
