In the labor law, there is no clear working-hour distribution for nurses working in hospitals, and whether they have to work in two or three shifts a day is not specified.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Working in shifts not only brings about positive outcomes, but also possesses some drawbacks for nurses in the ED. One of the negative impacts identified is the increase of work-related fatigue and stress in nurses working in ED.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a> Based on a survey conducted in the United Kingdom, of a total of 700 nurses, 60% (420 nurses) suffered from work-related stress.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">4</span></a> On the other hand, another research in Brunei showed that nurses who worked in the ED were four times more likely to suffer from patient violence or patient's attendant violence compared to nurses of other departments, and 2.8 times more likely to suffer from work-related fatigue.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a> These results were underpinned by Weigl and Schneider<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> who claimed that ED nurses tended to get exhausted quickly and were 66% more likely to suffer from work-related stress compared to nurses working in wards. A problem might arise if work-related fatigue and stress accumulated continuously, which would cause chronic fatigue (burnout) in nurses.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">7</span></a> If not identified and handled immediately, this condition will affect working performance and service quality, including the implementation of patient safety in the hospital.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Based on its type, the shift work system adopted in Indonesia is classified as a rotational shift with the working hours distributed into two or three shifts in a day.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> The most common shift scheme implemented in Indonesian hospitals, both government and private hospitals, is the 6-6-12 shift scheme. Based on the findings of the preliminary study, there were other shift-work schemes implemented in those hospitals, namely the 7-7-10 and 12-12 shift schemes and never been conducted a study before. Therefore, based on these findings, it is of urgency to conduct a research to determine the difference in the work-related fatigue and stress levels among nurses working in the ED in relation to the implementation of shift schemes other than the 6-6-12 scheme in the hospitals in Badung Regency and Denpasar City.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This research was conducted by employing a quantitative research method with an analytical-observational design using a cross-sectional approach. The total number of respondents participating in this research was 45, all of which were ED nurses. This research was conducted from January 20, 2018 to February 27, 2018 in four hospitals implementing the 7-7-10 and 12-12 shift-work schemes. A univariate analysis was chosen in order to determine the characteristics of the respondents from each hospital. A bivariate analysis was conducted by using a Mann–Whitney test. The instruments used in this research were the Occupational Fatigue Exhaustion Recovery (OFER) Scale and Expanded Nursing Stress Scale (ENSS) questionnaires. Both questionnaires were declared to be valid and reliable, with <span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.396. Also, this research was approved by the Ethics Committee of the Faculty of Medicine, Udayana University, with Ethic Code No. 104/UN.14.2/KEP/2018.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Respondents’ characteristics</span><p id="par0025" class="elsevierStylePara elsevierViewall">Based on <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, most of the respondents were male (34 nurses). With respect to their age range, the respondents were mostly aged 26–35 years (33 nurses). In addition, the majority of the respondents had earned a Bachelor's degree (27 nurses), and 23 respondents were single. The length of service in the ED with the highest percentage was 25–48 months (19 nurses).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Bivariate analysis results</span><p id="par0030" class="elsevierStylePara elsevierViewall">In <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, the data were not normally distributed and it can be seen that there was a significant difference in the fatigue score between the two studied groups based on the fact that the minimum and the maximum values of the 12-12 shift group was slightly higher than those of the 7-7-10 shift group. The data displayed in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> revealed that there was no significant difference between the two shift groups, with a <span class="elsevierStyleItalic">p</span> value of 0.267.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Discussion</span><p id="par0035" class="elsevierStylePara elsevierViewall">In line with the Mann–Whitney test analysis, it is statistically proven that there is a significant difference in the work-related fatigue levels between the 7-7-10 shift group and the 12-12 shift group of the observed Emergency Departments. A similar result was also found in a previous study conducted in Surakarta, which showed that there was a significant difference in the effects of work distribution system on nurses’ work-related fatigue.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">9</span></a> A research conducted by Pryce<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">10</span></a> suggests that the longer the duration of a shift in a day, the more severe is the fatigue suffered by nurses working in the ED. In the 12-12 shift group, the level of work-related fatigue was higher than that of the 7-7-10 shift group, as shown by the different minimum and maximum values (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.012). This finding is in line with Caruso<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">11</span></a> who claimed that shift work and long working-hour durations affect the level of fatigue, which consequently affect working performance, quality of service, and risk of malpractice, and increase the possibility of conflicts within a working team.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Work-related fatigue in nursing service is an impediment which can result in serious problems if not administered.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">12</span></a> Each nurse has a different resilience level in managing work-related fatigue, which causes the unequal distribution of data for fatigue scores. Similar to work-related fatigue, nurses’ work-related stress is also a problem and a challenge for the hospital management. It can affect the working performance of nurses in providing nursing service especially in the ED. Based on the statistical analysis there is no significant difference in the effect of work distribution system on work-related stress between the two observed groups. This is probably caused by the difference in the workload, motivation, and working environment. This finding is in line with the finding of Murhayati and Kismanto,<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">13</span></a> which suggests that workload, work motivation, and working environment will affect nurses’ work-related stress differently.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In addition, consistency of working hours is also pivotal. Although schedules are generally created for a fair working-hour distribution among all nurses working in the ED, in reality there are still working hours that are not properly scheduled due to personal issues or local cultural factors. For instance, nurses who are supposed to work morning shifts have to reschedule or take annual leaves because of religious activities, cultural activities, family matters, or sickness. This will obviously affect the working hours and the recovery duration in the following days which is crucial factors that lead to chronic work-related fatigue and stress in nurses.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">14</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Other factors that trigger the increase of work-related stress in nurses working in the ED are being targeted for threats or violence by patients or the patients’ relatives, workplace discrimination, and the characteristics of the respondents. In accordance with D’Arc et al.,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">15</span></a> the presence of threats and discrimination in the workplace will cause ED nurses to suffer from a higher level of stress and fear making mistakes in performing their duties. The other factor is the characteristics of the respondents, such as age, gender, and working experience in each hospital. According to a research conducted by Marinaccio et al.,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">16</span></a> age and gender affect the level of nurses’ stress, which is caused by the presence of roles and control functions which are predominantly held by men. Meanwhile, age-wise, nurses who are 30 years old and older, regardless of their gender, and with more working experiences, are more dominant in holding the control of the workplace.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">17</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">Nurses who have worked within shifts must be given ample time for recovery. This is relevant with a research that claims that work-related stress is common to happen and is heightened when it is not balanced with an adequate recovery phase.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">18</span></a> A research conducted in China suggests that 30–60<span class="elsevierStyleHsp" style=""></span>minutes’ rest in each shift will alleviate the level of fatigue and increase the concentration necessary to continue performing duties.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">19</span></a> This research concludes that work distribution system (shift work) contributes significantly to the level of work-related fatigue and stress in nurses working in the ED, and that the duration of recovery after every shift will help reduce the level of work-related fatigue suffered by nurses.</p><p id="par0060" class="elsevierStylePara elsevierViewall">One of the reasons for why there was no difference in the work-related stress score between the two groups was the nurses’ internal factors, which include self-motivation. The nurses who were working in the ED of each studied hospital showed high self-motivation, which caused their work-related stress score to show no difference even though they were working under different shift schemes.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Conclusion and recommendation</span><p id="par0065" class="elsevierStylePara elsevierViewall">There was a significant difference in the influence of work distribution system on work-related fatigue among the nurses of the EDs in the hospitals of Badung Regency and Denpasar City. However, there was no significant difference in the level of work-related stress between the two studied groups.
