Hypertension that is not well controlled and disciplined will affect the function of all organ systems, and have an impact on disability and death. It can reduce life expectancy by approximately 10–20 years.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">According to the World Health Organization in 2010, hypertension complications caused 9.4 million deaths each year worldwide, of which 45% was caused by heart disease and 51% by stroke.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">4</span></a> About a quarter of the adult population is hypertensive, and the incidence is higher among Afro-Americans. Hypertension, known as cardiovascular disease, is estimated to have caused 30% of deaths worldwide with a prevalence of 37.4%. In 2025, it was projected that around 29% or around 1.6 billion people around the world would experience hypertension.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">5</span></a> Data from Bali Provincial Health Service show that Denpasar is the district with the second highest number of hypertensive patients in Bali. The number of visits of patients with hypertension to the community health care centers in 2016 was 13.551.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">6</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Based on the data on patient visits from the community health care centers in Denpasar, hypertension ranks second in the group of 10 diseases with the highest number of patients. The high number of patients with hypertension and the severity of potential hypertension complications require commitment in the management of hypertension. The treatment of patients with hypertension as a chronic disease should be sustainable both pharmacologically and non-pharmacologically.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">1</span></a> Therefore, appropriate health behaviors are needed, which can be referred to as self-care management.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">7</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Self-care management is an effort made by individuals to play a responsible role for their own health.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">8</span></a> A research by Peters and Templin on 306 hypertensive patients found an association between motivation and patient self-care management. Motivation can be formed by internal factors (e.g. age, level of education, gender) and external factors (e.g. family social support).<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">9</span></a> This is in accordance with another research which found that the level of education, gender, social support, and duration of illness were related to the management of patient self-care.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">10</span></a> Another study on 130 patients found that optimal self-care management could improve the quality of life of patients with/without complications. Self-care management is multidimensional.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">11</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Hypertensive patients have several self-care management needs, including the needs for physical and psychological comfort, feeling valued, privacy, involving family and relatives, and being involved in decision-making if possible. In order for the patients’ needs to be fulfilled, they need the support of others.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">12</span></a> The people that can support them to meet these needs are the family. Family is legally allowed to make decisions for the patients, has emotional closeness to them, and lives together with or close to them.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">13</span></a> Family is an important part in the management of patient self-care because they act as a protector, facilitator, information provider, spiritual support provider, and decision maker. The presence of family members in the daily lives of patients has a very high value in increasing patients’ comfort.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">14</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Based on the results of a preliminary study on 10 family members of hypertensive patients of Puskesmas III Denpasar Utara, a community health care center of North Denpasar, it was found that 5 persons accompanied patients during a check-up at the health care center, 4 persons accompanied patients during elderly program activities, and 7 persons were able to explain about hypertension, but could not explain specifically about the efforts patients needed to make at home besides taking medication regularly.</p><p id="par0035" class="elsevierStylePara elsevierViewall">A previous study about hypertension conducted by the researchers in 2017 found that family support was related to self-care behavior, and that there was lack of family support still. This could be influenced by several factors including family's knowledge, attitude, and behavior related to hypertension and treatment that can be applied in the daily lives of hypertensive patients. Based on these findings, the researchers were interested to identify family's knowledge, attitude, and behavior in supporting self-care management of hypertensive patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Design</span><p id="par0040" class="elsevierStylePara elsevierViewall">This research is a descriptive study with a cross-sectional approach.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Population and study setting</span><p id="par0045" class="elsevierStylePara elsevierViewall">The population was family members of hypertensive patients of three community health care centers in Denpasar. The total sample of 90 respondents was obtained using a consecutive sampling technique.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Instruments</span><p id="par0050" class="elsevierStylePara elsevierViewall">This research used 4 instruments, namely a respondent demographic form, a questionnaire to assess knowledge consisting of 10 statements (correct<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1 point, wrong<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0 point), a questionnaire to assess attitude consisting of 11 statements (never<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1 point, sometimes<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2 points, often<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3 points, always<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>4 points), and an observation form consisting of 6 observation moments (yes<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1 point, no<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0 point).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Data collection, ethical aspects, and data analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">The researchers obtained all permits for the research, conducted perceptual discussions with the enumerators about the selection of respondents and how to fill out the questionnaires and observation form, provided explanations about the research along with an informed consent form to the respondents, which ended with the respondents signing or affixing their thumbprint to the informed consent form. This research adhered to the ethical principles of research, including informed consent, autonomy, anonymity, confidentiality, beneficence, non-maleficence, to protect the rights of the respondents. The respondents completed the questionnaires for 20<span class="elsevierStyleHsp" style=""></span>min. The data were analyzed using a univariate analysis with a computer program.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows an overview of the demographic characteristics of the respondents with respect to their age, sex, and level of education. In <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, it can be seen that the most common age range of the respondents was 45–55 years (45.5%), the majority of the respondents were female (64.4%), and the most common level of education was senior high school (42.2%).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows an overview of the family's level of knowledge, attitude, and behavior in supporting self-care management of hypertensive patients. Based on <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, most of the respondents had good knowledge about self-care management (as many as 62 persons or 68.9%), and three respondents had poor knowledge (3.3%). In terms of attitude, a large number of the respondents had sufficient/moderate attitude regarding the management of hypertensive patients’ self-care (as many as 42 people or 46.7%). In terms of behavior, the majority of the respondents had sufficient/moderate behavior in supporting the management of hypertensive patient's self-care (as many as 56 people or 62.2%).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Discussion</span><p id="par0070" class="elsevierStylePara elsevierViewall">Based on the results of this study, 68.9% of the respondents had good knowledge in supporting family members with hypertension. Good knowledge can be gained in many ways, one of which is through exposure to information from health care workers. Based on the interviews, several family members stated that they often received information when accompanying the patients to the community health care center, or when attending health education programs. The topic of hypertension was one of the topics that was often discussed, both by health care workers and by students who were practicing in the community health care center at the time.</p><p id="par0075" class="elsevierStylePara elsevierViewall">In addition, family's knowledge also depends on their level of education. A large number of the respondents in this study were high school and college graduates. This allowed them to have a higher awareness to support family members who were sick. Education can affect one's knowledge and eventually one's behavior toward healthy lifestyle.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">15</span></a> A similar research also found that 66.7% of the respondents had good knowledge about hypertension. Good knowledge is useful for the development of family's attitude and behavior in the future. Family is a vital component as a facilitator in the management of hypertensive patients’ self-care. Family presence is needed not only when the patient is in an acute condition, but also throughout the treatment period.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">16</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Another research had a different result with respect to family's knowledge, where 50% of the respondents were in the poor knowledge category. This could occur because the aspects of knowledge assessed in the study were specific to the type of diet and food management for hypertensive patients.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">17</span></a> One of the efforts to increase family's knowledge is family support training. This is in accordance with Maryam's research on 60 elderly with hypertension with the result that family support training affected the quality of life of the elderly (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.012).<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">18</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">As much as 46.7% of the respondents in this study were in the moderate attitude category with respect to caring for family members who had hypertension. One aspect of support that had a low score was informational support. Some family members knew about behaviors or efforts that could worsen or ease the patient's condition, but were often reluctant to remind the patient. The family members realized that the patient also needed independence in managing their illness and to not always depend on the family. As much as 62.2% of the respondents showed a moderate behavior in supporting family members who had hypertension. The respondents often accompanied the patient during a check-up visit as it did not require a long time. However, when there were special activities, for example an elderly program, hypertension program, or health education program, the family members would usually take the patient to the location of the activity, and then return home to continue their usual activities. Then, they would pick up the patient when the program was finished.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Olowookere's research stated the importance of family support for hypertensive patients. Patients with high family support were more obedient than patients with low family support.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">19</span></a> Other studies also found that family support was strongly related to hypertension medication adherence. Chronic diseases such as hypertension require a lifetime of treatment. This is a challenge for patients and families to be able to maintain the motivation to adhere to the treatment on an ongoing basis. One way to increase motivation is through family support. Family support will increase patients’ awareness to use health services and be active in health development activities related to hypertension.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">20</span></a> Another study on 318 patients in a local community health care center found that family support increased the patients’ motivation to take medication and perform regular blood pressure checks.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">21</span></a></p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Conclusion</span><p id="par0095" class="elsevierStylePara elsevierViewall">The results of this study show that 68.9% of the respondents had good knowledge, 46.7% of them had moderate attitude, and 62.2% showed a sufficient/moderate behavior that supports patients in their self-care management. Family should always accompany the patient, not only when visiting health care facilities, but also in the daily care in relation to the motivation and self-efficacy of hypertensive patients.
