The issue is that if accumulated, the outcomes of fatigue may include low self-efficacy, physical pain, disturbed sleep patterns and decreasing capacity to perform certain physical activity.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In DM cases, fatigue may be triggered by episodes of hyper-glycemia, which are often observed in diabetes treatments. People with type-2 diabetes are often overweight and this alone can lead to a severe fatigue.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">4</span></a> Indeed, there is a higher risk of fatigue among DM patients, as well as Restless Legs Syndrome (RLS),<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">5</span></a> which is a sleep disorder characterised by the urge to move feet and a sensation of discomfort, even when at rest or when sleeping at night. This condition occurs in 7 to 10 per cent of the general population and the symptoms are often unrecognisable, misdiagnosed; hence are not managed properly.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">6</span></a> RLS symptoms may become severe when one is at rest, relaxation, or inactivity and this will have a major impact on daily activities and the quality of life in general.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">If not treated properly, fatigue may hinder the delivery of even the basic tasks, lower the attention span and even cause mood swings, all of which will eventually contribute to the deterioration of physical health.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">8</span></a> On a larger scale, chronic fatigue will not only impact the patients’ lives but also impact the wider community as it reduces productivity and increase health costs.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">9</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Nurses as an integral part of the healthcare system should be able to identify the overlooked and hidden risk factors for fatigue among DM patients so that healthcare planning will be more informed; thus, provide the necessary interventions.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">The research design of this study is descriptive observational by using a cross-sectional approach. The population in this study was the DM patients at the Community Health Centre in Denpasar, with the sample consisting of 50 people, selected by using a purposive sampling method. The study inclusion criteria were: age group of at least 19 years, had received a diabetes therapy for more than 6 months and were willing to participate in the study. Patients suffering from tumours, who had been hospitalised in the past three months, had hyperthyroidism or hypothyroidism, anaemia, dementia, schizophrenia, stroke were excluded from the study. The research was conducted in five Community Health Centres in Denpasar in 2019.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Measurement</span><p id="par0030" class="elsevierStylePara elsevierViewall">The severity of fatigue was assessed based on complaints from the patients in the last four weeks, assessed using the Chalder Fatigue Scale (CFS), covering two aspects of fatigue, i.e. physical fatigue (items 1–7) and psychological fatigue (items 8–11). All items were in the form of a Likert scale with a score range of 0 to 3, representing the category of mild, moderate and severe fatigue respectively. The nutritional status was assessed by using Body Mass Index (BMI) based on body weight (kilograms) against body height (metres). BMI results were categorised as being skinny, thin, normal, fat and overweight. Meanwhile, the measurement of neurological conditions that result in limb discomfort was carried out by using a restless leg syndrome (RLS) questionnaire, expressed in scores ranging from 0 to 40. The RLS was categorised into no RLS, mild, moderate, severe and very severe RLS. The measurement of blood glucose levels was conducted by using fasting blood glucose examination in the last month obtained from secondary data, i.e. patients’ records.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Data analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">Descriptive statistics summarises the characteristics of the sample and the variables being studied, which comprises fatigue, age, RLS, duration of diabetes, blood glucose levels, and BMI.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Ethical considerations</span><p id="par0040" class="elsevierStylePara elsevierViewall">This study has passed the ethical assessment from the ethics committee from Sanglah General Hospital, the Medical faculty of Udayana University. After being briefed, patients were given a written consent form to participate in the study. Patients were also informed that they had the right to participate or to withdraw from the research; and that their confidentiality and data would be safeguarded.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">The characteristics of the 50 patients are presented in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, with 74 per cent experiencing moderate levels of fatigue. The majority (52 per cent) were aged 60 years and above, 44 per cent had a severe RLS and 42 per cent had suffered DM for of 5 to10 years. In terms of fasting blood glucose, 88 per cent had ≥126<span class="elsevierStyleHsp" style=""></span>mg/dl fasting glucose level; and 58 per cent of the patients’ BMI was above normal.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Discussion</span><p id="par0050" class="elsevierStylePara elsevierViewall">The results of the current study show that the majority of patients experienced a moderate level of fatigue, in line with previous research stating that it is commonplace among DM patients, and is often caused by multidimensional factors, including physiological, psychological and lifestyle.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">10</span></a> Physiological factors include changes in blood glucose levels, either hypo-glycemia or hyper-glycemia. Research on the effect of acute hyper-glycemia on cognition and mood has been carried out among 20 type-2 DM patients, with the results indicating a strong correlation between hyper-glycemia and decreased cognitive functions, mood swings, and severe fatigue.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">11</span></a> In the current study, the majority of respondents had a fasting blood glucose value of more than 126<span class="elsevierStyleHsp" style=""></span>mg/dl. It is commonly known that postprandial blood glucose fluctuations trigger oxidative stress, which plays a role in the development of vascular complications.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">12</span></a> Oxidative stress and fatigue occur in excessive tiredness syndrome. Research conducted on 47 in patients with a risk of cardiovascular disorders due to obesity also showed an increase in isoprostane as a marker of oxidative stress, which was also a symptom of chronic fatigue.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">13</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">The majority of patients in this study were over 60 years old and mostly retired. Because ageing and diabetes—both as an independent variable or conjointly—affect fatigue, older diabetic patients are more prone to fatigue than younger or healthy diabetics.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">14</span></a> The impact of this condition might be severe among the elderly because ageing and diabetes will lower the ability to perform daily activities and the overall psychological well-being.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">15</span></a> They might experience increased life burden especially if they receive a regiment of daily treatment that leads to decreased functional ability.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">16</span></a> Moreover, fatigue among elderly diabetic patients is also affected by comorbidity and psychological factors, including depression, low sleep quality and diet-related problems.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">14</span></a> The risk factor is even higher among obese individuals with low physical activity because of the insulin resistance.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">17</span></a> Therefore, obese elderly patients who experience type-2 DM with a combination of increased insulin resistance and impaired insulin secretion,<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">15</span></a> need the most immediate attention.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Additionally, the ageing process will cause damage to pancreatic β-cells as a response to endogenous incretin, decrease insulin sensitivity and accelerate the death of β-cell by inducing mitochondrial dysfunction that impairs glucose tolerance.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">18</span></a> The majority of participants in the current research had suffered from DM for more than five years. Meanwhile, the progression of type-2 diabetes occurs over 7 to 10 years.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> The longer the patient suffers from DM, the lower the body's ability to control blood glucose levels, unless there are interventions in either pharmacological or non-pharmacological form in order to control the glycaemic levels.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Next, research has shown that high BMI may lead to fatigue.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">2</span></a> Individuals with type-2 diabetes are often overweight, and this alone can increase the risk of chronic fatigue.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">4</span></a> The current study confirms the findings in the previous research. Most participants’ BMI in this study fall into fat or overweight category. BMI has a strong relationship with diabetes and insulin resistance. Diabetic patients will have a high amount of non-esterified fatty acids, glycerol, hormones, cytokines and inflammatory markers in their system and this will cause insulin resistance,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">20</span></a> which is particularly apparent among patients with high BMI.</p><p id="par0070" class="elsevierStylePara elsevierViewall">However, it is not only patients with high BMI that experience the negative impacts. Because fatigue hinders physical activities, which subsequently lead to weight loss and lower BMI, the conditions of patients with lower BMI may be more aggravated.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">21</span></a> Episodes of hyper-glycemia, which are often observed during diabetes treatment may also trigger fatigue. The psychological condition may also worsen the situation; for example, the inability to treat oneself independently can lead to a frustration, which in turn can lower the actual physical ability to manage fatigue.</p><p id="par0075" class="elsevierStylePara elsevierViewall">RLS may also be a factor causing fatigue among DM patients. The findings in the current research show that the majority of participants (at around 52 per cent) had severe and very severe RLS, which is often characterised by a cramp that spreads to the feet and will only decrease if the feet are moved. Such a cramp is often caused by idiopathic and polyneuropathy.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">22</span></a> RLS among DM patients causes discomfort in the limb areas and may disrupt the sleep patterns, which worsen the severity of the fatigue feeling.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">23</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">However, it should be noted that this study only describes physiological factors and does not discuss psychological aspects of fatigue among the DM patients.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusions</span><p id="par0085" class="elsevierStylePara elsevierViewall">Fatigue is a common condition among diabetic patients. Age, RLS, the period of suffering from DM, blood glucose level and BMI are among the factors that contribute to fatigue. Nurses are expected to be able to identify risk factors for fatigue among DM patients so that they can provide appropriate treatment and minimise severe diabetic complications. The participants were 50 DM patients treated at Denpasar Community Health Centre, selected by using a purposive sampling technique with inclusion and exclusion criteria. The results show that 74 per cent of the patients experienced moderate, 18 per cent mild and 8 per cent severe fatigue.
