TB management in the last few decades still has several obstacles, one of which is inadequate TB monitoring and incident reporting.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> This is in line with the results of treatment of TB patients in Indonesia (2017) showing that some patients experienced loss to follow-up (5.4%), relocated (4.0%), were not evaluated by officers (2.7%), treatment failure (0.4%) even up to died 2.5%).<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Many efforts have been made by the government as a preventive measure and to improve TB treatment, one of them is the DOTS (Directly Observed Treatment Short-Course) program, but until now DOTS is still not optimal due to insufficient resources.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a> A guideline that the WHO has recently promoted since 2015 is the treatment of TB with digital technology in supporting the implementation of the End TB strategy.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> However, there is still a lack of information about how mobile health (M-health) has been used in TB programs, thus requiring a literature review.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Thus, we conducted a literature review of the articles offering snapshots of M-health models that were evaluated and their benefits in TB patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">The method used in this paper is Systematic Review with the 2009 PRISMA checklist guidelines which are analyzed in 4 stages, namely article selection, learning description, quality assessment, and review results.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a> Then to assess eligibility, articles are filtered using the Critical Appraisal Skill Program (CASP)<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> and tools Centre for Evidence-Based Medicine (CEBM).<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Besides, the quality of articles is filtered and assessed effectively using the Cochrane Risk of Bias Tool recommended by the Cochrane Handbook for Systematic Review.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Structured literature searches were performed on the PubMed, ScienceDirect, Scopus, and ProQuest databases. By using PICO (<span class="elsevierStyleItalic">patient, intervention, comparison, and outcome</span>),<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,13</span></a> P: tuberculosis, I: <span class="elsevierStyleItalic">mobile health</span>, C: no comparison or control group was specified, dan O: the effectiveness of its use. A total of 60 articles were identified from four databases, 41 articles were excluded because they did not fit the inclusion criteria, and seven articles were double publications. Finally, there are six articles that were included with the criteria focus on interventions using M-health in TB patients, in English, and published in the last five years.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="par0020" class="elsevierStylePara elsevierViewall">The study design of the six articles included there were four designs used, namely the RCT research design, longitudinal study, cross-sectional study, and pilot study. The explanation about the participants and the purpose of selected studies are presented in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. There are several M-health models for TB treatment. Research conducted by Navin et al. (2018) uses M-health with the concept of TB track, a smartphone-based application to report drug-taking activity, can see the appropriate drug strips every day, can report symptoms experienced during the treatment period. Besides, respondents received knowledge and information about treatment, motivational messages, medication reminders, and consultation, also features telephone calls and emergency messages that are directly connected to the DOTS provider. A similar study by Nhavoto et al. (2017) used M-health with the concept of motivational messages and reminder messages taking drugs and routine checks at the clinic. Another study by Do et al. (2019) uses the video directly observed therapy (VDOT) application that functions to record and sends videos when consuming drugs and features telephone calls and SMS, take pictures, record videos, use the internet and email. Similar research was also carried out by Story et al. (2019) who compared directly observed therapy (DOT) and video observed therapy (VOT) interventions. VOT is a remote observation application for recording and sending videos of drugs taken every day. Whereas research conducted by Kumar et al. (2019) through RNTCP (Revised National Tuberculosis Control Program) is that respondents receive medication reminders via cellphone. While research by Maraba et al. (2018) aims to produce applications to facilitate reporting of TB-related data, provide quick access, and can provide laboratory examination results via mobile.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">Research conducted by Navin et al. (2018) shows smartphone-based M-health as a solution to support TB treatment. Research conducted by Nhavoto et al. (2017) shows that SMS technology for HIV and TB is effectively used to send consultation reminders, medicines, motivational texts, and health education in improving care. In addition, research by Do et al. (2019) with VDOT M-health interventions patients become more comfortable and understand more quickly. Whereas Story et al. (2019) show that VDOT is more effective for observation of TB treatment than DOT. This is also supported by research Kumar et al. (2019) shows that most respondents prefer VDOT to DOT. Subsequent research by Maraba et al. (2018) shows that mHealth interventions are acceptable and more efficient than manual visits and can improve TB care.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Discussion</span><p id="par0030" class="elsevierStylePara elsevierViewall">M-health has the potential as one of the methods in efforts to control global TB with enormous opportunities.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> This is consistent with the results of a review that supports the benefits of M-health in the form of TB case finding, rapid diagnosis, and patient compliance with treatment.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> The method often used is the design of RCT research and pilot study. An RCT was conducted to evaluate M-health compared to other interventions by randomization and to see its impact on TB care support.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> While the pilot study was conducted to assess the eligibility and response of respondents in using them-health application, assess intervention, and show that a research protocol is feasible.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a></p><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">M-health models</span><p id="par0035" class="elsevierStylePara elsevierViewall">M-health is a telephone integration as a health intervention tool in many aspects of care. Evidence of the concept of M-health has been demonstrated by several M-health models that have been applied in TB care.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Most of the M-health concepts that have been evaluated carry the concept of VDOT, which is a video observation therapy and has been conditionally recommended by WHO as an alternative to DOT.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> The VDOT application can record and send videos while taking medicine for review by the DOT team. After the data is received by the server, the smartphone application removes the video from the device to prevent the spread of patient information and saves device memory. SMS is also one of the concepts of cheap M-health, which can be run even if you only have an ordinary cellphone. Through SMS TB patients receive motivational messages and educational messages.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> The next proposed M-health solution is the TB track application, which is an application to report the activity of taking medication, see the right drug strip every day during the treatment period. Through the TB track application respondents also receive motivational messages, reminders to take medication and check their health at the hospital. The TB track application is also equipped with telephone services and emergency messages using just one tap and is directly connected to the DOTS provider.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Benefits of M-health to TB patients</span><p id="par0040" class="elsevierStylePara elsevierViewall">M-health is needed in patients with long-term care and requires routine and precise monitoring such as TB. Based on the results of the study, most patients and health workers respond to the use of M-health as a solution to TB care. The results of the review also showed that M-health could maximize TB care through mobile services. Various studies show that VOT is a concept that is flexible, cost-effective, acceptable, and patient-friendly intervention.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> A video-based approach using a mobile device provides direct observation for TB patients associated with high adherence and fewer care resources.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a></p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Conclusion</span><p id="par0045" class="elsevierStylePara elsevierViewall">The findings of this review indicate that M-health is very beneficial for the improvement of TB patients in the form of improved medication adherence, case finding, and monitoring of younger TB patients. So this research can be developed to increase understanding and add evidence related to the importance of M-health in TB patients.
