The procedure contains instructions on how to help patients who cannot perform bathing independently. However, this bed bathing procedure is rarely implemented in the clinical practice, either by the students or nurses. A survey conducted on 69 nursing students in their clinical education phase showed that there was no respondent implementing the procedure according to the guidelines, where 47.8% of the students did not complete the bed bath according to the standard procedure that they had learnt in their education phase. The rest or 52.2% of the respondents in this study only partly complied with the given manual. This study also revealed the reasons behind these findings. Bed bathing was considered to be a time-consuming and ineffective activity. It might indicate the need for simplifying the conventional bed bathing procedure manual so that it would be more appropriate to implement in a clinical setting.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Bed bathing is one of the tasks that must be performed by nurses by first assessing the patient's level of dependency.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">1</span></a> However, in the clinical field, nurses almost never bathe patients,<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">2,3</span></a> especially in lowest-class wards,<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">4</span></a> where they usually delegate this task to younger staff or unprofessional staff,<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">5,6</span></a> or even to the patient's family.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">7,8</span></a> This finding should be taken into consideration since by its definition bathing is not simply washing a patient with water, but also an essential basic nursing procedure that requires the nurse's involvement and expertise.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">3,6,9</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Bed bathing procedure should be completed based on the hospital's guidelines to assure that the procedure is safe, patient-centered, and based on the best nursing practice.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">5,10</span></a> A bed bathing procedure performed by a nursing student may be considered a risky activity due to several reasons, as found in a preliminary study in conjunction with the present study. We surveyed 69 nursing students to investigate the occurrence of musculoskeletal complaints associated with performing a bed bathing procedure. It was found that the majority of the respondents suffered from pain on their back, arm, and shoulder, as well as their calf after completing a bed bathing procedure. These musculoskeletal complaints could have been caused by poor working postures during the procedure, such as bending down, doing some repeated movements, reaching for the bathing tools, or managing the patient's position. Unfortunately, evaluations for an effective and efficient bed bathing procedure are rarely conducted.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Bed bathing procedure was considered challenging and stressful by the nurses.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">3,11,12</span></a> This procedure has been found to precipitate musculoskeletal injuries among the nurses.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">13–15</span></a> There are three factors associated with musculoskeletal injuries in the nursing population, namely the tools, the nurse's postures, and the patient's dependency level,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">16</span></a> which are incompatible with one another. Several studies have been conducted to evaluate the most common complaint experienced by nurses and concluded that most nurses experienced back pain.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">15,17,18</span></a> Awareness of the correct working posture becomes an important part of the musculoskeletal injury prevention.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The working procedure must be designed correctly so that nurses or nursing students can work effectively and efficiently without any adverse effects on their health. An evaluation of the working procedure can be performed by a left-hand and right-hand chart (LRHC). LHRC is a tool to identify inefficient movements during a particular procedure. It can describe all movements performed by the left and right hands while a person is working and also show the proportion of the tasks done by the left and right hands.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">19</span></a> Studies about appropriate bed bathing procedures are still limited. Hence, an investigation about the most appropriate bed bathing practice to improve the patient's outcome needs to be conducted. Therefore, it is expected that through this study a better bed bathing procedure can be designed by balancing the workload proportion between the left and the right hands through an LHRC analysis by identifying any inefficient steps.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">This was an observational study involving an analysis of eight elements of motion (therbligs): Reach (RE), Use (U), Grasp (G), Release (RL), Move (M), Delay (D), Position (P), and Hold (H), which are performed on eight body parts. Before the evaluation, the respondents were trained to perform a bed bath based on the standard institutional guidelines. Once completing the training, the respondents’ ability to perform the procedure was recorded using video cameras installed in a nursing skill laboratory. The videos were then analyzed using the LHRC. Ineffective movements were eliminated based on the economic principles of movement so as to shorten work time. This study was approved by the Institutional Review Board of Udayana University and granted ethical clearance by the research and development division of Sanglah Hospital Centre, Bali, Indonesia. The participants received all the information relating to the procedure for this study. All data were stored confidentially and destroyed after the completion of the study. The participants were welcomed to ask questions about the study and were informed that they could withdraw from the study at any time.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">The bed bathing procedure in this study was divided into four phases: (1) preparation,(2) bed bathing, (3) termination, and (4) evaluation. The bed bathing procedure according to the institutional guidelines used in this study consisted of the following steps: washing the patient's body with wet washcloths, wiping the body, cleaning the patient’ body with water, and drying the body with a dry towel. These steps should be completed on all the patient's body parts, except for the patient's face which should be washed without soap. All the instructions should be performed in sequence, which initially start on the patient's farthest hand, followed by the upper part of the body and the closest hand. The participants should also tilt the patient's body to clean the back part and place the patient on a recumbent position to wash the lower part of the body.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The observation was made on the movements performed on eight parts of the body: face and neck, farthest hand, closest hand, chest, abdomen, farthest thigh and foot, closest thigh and foot, and back. Ineffective movements made on each body part are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the ineffective movements during the bed bathing procedure: the waiting and the holding movements of the left hand during the washing of all the body parts, and the waiting movement of the right hand on all the body parts (except the face). The reaching movement was also performed by the left and right hands during the washing of the farthest body parts (hand or foot).</p><p id="par0050" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the analyses of the working time of the left hand and the right hand for each respondent. The longest working time was 12.13<span class="elsevierStyleHsp" style=""></span>min and the shortest was 10.40<span class="elsevierStyleHsp" style=""></span>min. <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> shows the mapping summary of the left-hand and right-hand chart on the bed bathing procedure. The analyses showed that the mean duration of the bed bathing procedure was 12.17<span class="elsevierStyleHsp" style=""></span>min. The duration did not include the time used for communication with the patient before the procedure and the time needed to change the bath water in the middle of the procedure. This study also found that there was a 2.87-min waiting period on the left hand and a 0.74-min waiting period on the right hand. Holding performed by the left hand was also found with a mean duration of 3.05<span class="elsevierStyleHsp" style=""></span>min. The left hand performed the holding of the hand and foot, which was not performed by the right hand.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Design and modification</span><p id="par0055" class="elsevierStylePara elsevierViewall">Based on the results of the observations, identifications, and measurements, the researchers propose solutions to reduce working time on the bed bathing procedure by eliminating movements that are inconsistent with the principles of motion economy. These movements include waiting, holding, reaching, and twisting. They are eliminated because they pose a risk of a musculoskeletal disorder and trigger fatigue. The proposed modification of movements is shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">There were several important findings that can be drawn from this study. Researchers found waiting periods, holding movements, and reaching movements during the bed bathing procedure. These movements are ineffective and may affect the bed bathing duration. Based on the analyses, balanced movements between the right and the left hands are possible to be performed.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">20</span></a> The unproductive movements could also be eliminated<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">19</span></a> to shorten the working duration and reduce health consequences of the procedure. Researchers also found that there was workload imbalance between the left and right hands, in that most of the tasks were performed by the right hand. To confirm the finding, the researchers assessed the musculoskeletal complaint using the Nordic Body Map questionnaire and found that the majority of the respondents complained of pain on the right shoulder, upper right arm, and right wrist.</p><p id="par0065" class="elsevierStylePara elsevierViewall">On the other hand, this study was also able to specifically identify the time consumed during the main intervention by the respondents for the bed bathing procedure, which was 11.40<span class="elsevierStyleHsp" style=""></span>min. Another study found that the mean bed bathing procedure duration was 26.06<span class="elsevierStyleHsp" style=""></span>min, but it is unclear whether this length of time was used for the main procedure only or the whole procedure from the preparation to the termination phase.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">3</span></a> A principle of bed bathing is patient-centeredness; therefore, the intervention should be conducted based on the situation and patient's condition. For example, nurses should not stop bathing patients only for the nurses’ personal interests such as resting or drinking, but should stop the activity for a while if the patient's condition seems unstable.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The bed bathing procedure is performed in standing postures. Standing work postures are unnatural and cause problems in the musculoskeletal system such as low back pain.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">21</span></a> The long-time standing position will cause blood clots because blood flows against the gravitational force.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">22</span></a> Working in this posture is also included among activities with moderate metabolic rates.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">23</span></a> Therefore, it is recommended that the duration of the bed bathing procedure should be shortened.</p><p id="par0075" class="elsevierStylePara elsevierViewall">There are various strategies to improve the working procedure to be more effective and safer. In this study, all strategies offered are formulated according to Barnes (1980). The refinement strategies can be developed by four methods, namely elimination, combination, re-arrangement of the procedure sequence, and simplification of the procedure.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">24</span></a> Based on these methods, the strategies for refining the bed bathing procedure can be described as follows: (1) Movement Elimination: some movements need to be eliminated because of a disproportionate workload between both hands. Repeated hand movements and twisted movements when taking the water in the washbowl is an example of a movement that needs to be eliminated. In this movement, the left hand plays the role of a “holding tool”. This movement can be considered as an unproductive movement and causes a disproportionate workload between the right and the left hands. Some reaching movements performed in a static or fixed position also need to be eliminated because they use too much muscle power; (2) Movement Combination: combinations can be made of movements on the contiguous parts of the body, for example on the upper extremity, lower extremity, and the posterior part of the body. When washing the farthest hand, reaching and holding movements are performed; at the same time, washing of the upper part of the body can be done. The same movement combination can also be applied when washing the lower extremities. These movement combinations can be applied when cleaning the contiguous parts of the body while still referring to the clean-to-dirty basic principle; (3) Simplification of the present procedure: one strategy for simplifying the present procedure is to use simpler tools for bed bathing. There were too many unused tools in the present procedure, such as towels and washbowls. Based on the guidelines, two washbowls and two dry towels are needed for the bed bathing procedure. This guideline is not suitable for the situation in the clinical field. The number of washbowls is limited, so one washbowl may be used for some patients in the same period of bed bathing. Several studies previously conducted found that the washbowl used could spread microorganisms if it was not decontaminated well.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">25–27</span></a> Therefore, it was suggested that one washbowl be used for one patient. Based on these findings, we do not recommend using a washbowl, but other types of equipment that are simpler and more suitable in the clinical field.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Other strategies that can be applied for a safer bed bathing procedure are to place all the tools for the procedure within reaching distance,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">19</span></a> to optimize the instructions which can be performed in one movement, and to increase the awareness of safe working postures through some awareness programs. Nursing students should understand the basic ergonomic concept and the health consequences of poor working postures. A working procedure plays an important role as a guideline that provides instructions during the performance of a bed bathing procedure. A working procedure is supposed to be so designed as to ensure its safe performance and to balance the workload between the right and the left hands. A simultaneous and symmetric working pattern is required to produce the most effective movements in performing a procedure. These results indicate that some insignificant movements during the bed bathing procedure need to be eliminated for the performance of a safe and balanced procedure.</p><p id="par0085" class="elsevierStylePara elsevierViewall">We concluded that the proposed procedure modification would be able to remove ineffective movements, balance left-hand and right-hand movements, and increase time efficiency by more than 50 percent. This study proposes a procedure modification based on the results of the Left-Hand and Right-Hand Chart instrument. To complement the shortcomings of a research using this method, subsequent studies can use different methods to analyze various factors that contribute to the modification of work procedures. It is expected that the results of this study can be used as a reference to improve work safety in the present time and in the future, especially in the nursing field. This signifies the need to review the bed bathing procedures used in schools, by either simplifying or conditioning them according to the practice area. This study is an analytical-descriptive research which aims to analyze ineffective movements in the conventional bed bathing procedure using the left-hand and right-hand map and further improvements of the procedure in order to improve work efficiency. The subjects of this study were six nursing students from the Nursing Study Program of Udayana University. This research was conducted by analyzing video-recorded data containing the procedures performed by the subjects using the left-hand and right-hand map. The mapping was carried out on the six students who had been trained to use the bed bathing procedure. The bed bathing procedure was divided into eight movement elements. This study found that the average time for bathing a patient was 12.17<span class="elsevierStyleHsp" style=""></span>min, excluding the time used for preparation, communication during bathing, tidying up the patient, and evaluation. The effective working time of the left hand was noted to be 6.25<span class="elsevierStyleHsp" style=""></span>min while that of the right hand was 11.40<span class="elsevierStyleHsp" style=""></span>min. The total waiting time on the left hand was 2.87<span class="elsevierStyleHsp" style=""></span>min while that of the right hand was 0.74<span class="elsevierStyleHsp" style=""></span>min. There were 3.26<span class="elsevierStyleHsp" style=""></span>min of left-hand holding of body parts such as the patient's hands and feet, which movement was not found on the right hand. Modification was conducted by eliminating ineffective movements based on the economic principles of movement and the left-hand and right-hand map. It was concluded that after modification the average time for bathing a patient was 5.73<span class="elsevierStyleHsp" style=""></span>min and work efficiency increased by 52.92%.
