In this case, to minimize the risk and prevent injuries caused by mistakes caused by acting.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">1</span></a> One of the global and national issues for hospitals is patient safety, which is the basic principle of patient care and a critical component of quality management.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The application of patient safety management in hospitals plays a very important role in efforts to prevent or minimize the occurrence of adverse patient safety incidents. The committee that plays a role in patient safety is the Committee for Quality Improvement and Patient Safety, which reaches all work units in the hospital. The Patient Safety Committee is one of the PMKP sub-committees that plays a role in patient safety management. Among them are program preparation and reporting, monitoring and evaluation, preparation of patient safety indicators, carrying out documentation, investigations, and analysis related to patient safety.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The application of patient safety in Makassar City Hospital is ongoing since 2017 by forming the Patient Safety Committee. The existence of this application is one of the efforts to deal with incident incidents that can disrupt patient safety. Although the application has been running, there are still some Patient Safety Incidents in Makassar City General Hospital. In 2018, 13 cases were recorded where there were 6 Non-Injury events, 4 Nearly Injured Events, and 3 Unexpected Events.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">4</span></a> It means that the incidents that occurred in Makassar City Hospital exceeded the standards set by the Minister of Health Decree No. 129 of 2008 is zero events. Based on these cases and research, and evaluation of the performance of the patient safety committee is needed to determine the performance in achieving the standard number of patient safety incidents. If the goal has not been reached, the causes certainly want to be known to find ways to improve the program or find alternative programs to solve the problem.</p><p id="par0020" class="elsevierStylePara elsevierViewall">This evaluation is a form of theory-based evaluation based on a realist science philosophy that addresses the questions of what works, for whom, under what circumstances, and how it is done.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">5</span></a> Realist Evaluation can answer program evaluation more deeply. The concept of Realist Evaluation is developed through the configuration of Context, Mechanism, and Outcome (C-M-O).<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">6</span></a> For this reason, researchers are interested in knowing the performance of a patient safety committee in achieving the number of patient safety incident standards. That is by looking at the causes that have not been reached according to the standard is zero events using the Realist Evaluation approach in Makassar City Hospital. The purpose of this study was to evaluate the performance of the patient safety committee at Makassar City Hospital.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Method</span><p id="par0025" class="elsevierStylePara elsevierViewall">This research was conducted at the Makassar City Regional General Hospital from April to May 2019. The type of research is qualitative research with a realist evaluation approach. The selection of informants was carried out using purposive sampling techniques and obtained informants in this study as many as 10 people. Among them are the heads of medical services, chair of the Risk Management and Patient Safety Committee, chair of the patient safety subcommittee, deputy chair of the patient safety committee, three people a member of the patient safety committee, head of the internal care room, head of the surgical treatment room, and head of the outpatient installation. Sources of data in this study are divided into two, namely primary data and secondary data.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Primary data were obtained through a process of in-depth interviews and observation of informants using interview guidelines. Secondary data obtained from the study of related documents that are used as supporting data from primary data for research purposes. Assist instruments in this study are in-depth interview guidelines, observation guidelines, documentation tools, and notes in the field. The data analysis technique is done by data reduction, data presentation, and conclusion drawing.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">This research discusses the factors causing the achievement of the standard number of patient safety incidents seen from the performance of the patient safety committee in Makassar City General Hospital from the context to the outcome elements. The context element in this study is the availability of competent human resources. Based on the results of the triangulation of the availability of competent human resources, the available human resources are already competent. Although they are still less involved in the patient safety committee. This was revealed from the results of the following interview:<span class="elsevierStyleDisplayedQuote" id="dsq0005"><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">“<span class="elsevierStyleItalic">To be honest, if I haven’t digested the patient's safety, it's not old but it's only been in contact with us-we last year, so I just learned. Everything is still in a new stage, so it has not been well organized...</span>” (Informant 7, 37 years old)</p></span></p><p id="par0040" class="elsevierStylePara elsevierViewall">In the mechanism element in the form of providing guidance and encouragement, and supervision by the risk management and patient safety committee. Based on the triangulation carried out related to providing guidance and encouragement by the committee regarding patient safety incidents, that the risk management and patient safety committee only conveyed to the head of the room and those who continued. It means that the committee did not directly provide guidance and encouragement and some said no. Ever got that by the committee. This statement was revealed from the results of the following interview:<span class="elsevierStyleDisplayedQuote" id="dsq0010"><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">“<span class="elsevierStyleItalic">Not really, but right we usually things as soon as we convey to the heads of the rooms, then the head of the room which continues, not directly to us.”</span> (Informant 4, 40 years old)</p></span><span class="elsevierStyleDisplayedQuote" id="dsq0015"><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">“<span class="elsevierStyleItalic">That's what I said before if you want a review once a month.”</span> (Informant 8, 40 years old)</p></span></p><p id="par0045" class="elsevierStylePara elsevierViewall">Also, supervision by the risk management and patient safety committee, related to patient safety incidents, is known to be carried out by the committee through its reporting. Based on the triangulation conducted, monthly reports related to patient safety were obtained. Related to the supervision, one of the proofs from the following interview results:<span class="elsevierStyleDisplayedQuote" id="dsq0020"><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">“<span class="elsevierStyleItalic">Supervised every month, right from the report. From that report, we make the problem there; we go down to the unit.”</span> (Informant 2, 45 years old)</p></span></p><p id="par0050" class="elsevierStylePara elsevierViewall">The outcome element in this study is the performance of the patient safety committee. Based on this, what is meant by its performance is all matters in achievement related to patient safety incidents at Makassar City Regional General Hospital. Related to the background of the establishment of a patient safety committee that was formed because of accreditation. This statement was revealed from the results of the following interview:<span class="elsevierStyleDisplayedQuote" id="dsq0025"><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">“<span class="elsevierStyleItalic">His background is accreditation. Try if there is no accreditation, it will not be made.”</span> (Informant 4, 40 years old)</p></span></p><p id="par0055" class="elsevierStylePara elsevierViewall">Regarding the opinions of informants regarding the performance of the patient safety committee in achieving the number of patient safety incident standards that the committee's performance was not running as expected and the lack of coordination from the committee itself. Also, there were even informants who did not know at all the people on the patient safety committee. As for the obstacles related to patient safety incidents that there are still those who do not understand the reporting system. These barriers were revealed from the following interview results:<span class="elsevierStyleDisplayedQuote" id="dsq0030"><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">“<span class="elsevierStyleItalic">There are still those who do not understand may be the reporting system. It could be like that.”</span> (Informant 4, 40 years old)</p></span><span class="elsevierStyleDisplayedQuote" id="dsq0035"><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">“<span class="elsevierStyleItalic">Compliance from each human resource in each unit to report.”</span> (Informant 6, 37 years old)</p></span><span class="elsevierStyleDisplayedQuote" id="dsq0040"><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">“<span class="elsevierStyleItalic">There are still those who are afraid to report for fear of being returned to their mistakes.”</span> (Informant 9, 39 years old)</p></span></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">At present, judging from the competence of its own human resources, it is quite good in supporting the success of achieving patient safety incidents in hospitals. Even though those human resources are still lacking in involvement in the patient safety committee. As explained by Hasibuan et al. on the research carried out that the main resource in policy implementation is staff. Failures that often occur in the implementation of policies, one of which is caused by staff/employees who are not competent in their fields. The addition of staff and implementers alone is not enough to solve the problem of implementing the policy but requires staff with the expertise and capability needed to implement the policy.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">7</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Also, the provision of guidance and encouragement by the risk management and patient safety committee related to the achievement of patient safety incidents is not given routinely and evenly at the Makassar City Regional General Hospital. The results of this study are also in line with research conducted by Sihotang cited in the research of Bardan, et al. who examined the relationship of work motivation to the performance of nurses in providing services to patients. Based on the research data, it shows that increase motivation and achievement will improve nurse performance.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">8</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">The patient safety committee at the Makassar City General Hospital is under the auspices of the risk management and patient safety committee. The background for the establishment of a patient safety committee due to accreditation, both as a demand to become an agency, as well as a factor in improving the quality and patient safety. Because the patient safety committee was formed at the time of accreditation, this committee has not run well. This is because there are still nurses and room heads who are not aware of the patient safety committee at the Makassar City Regional General Hospital. This is in line with research from Sulahyuningsih related to the analysis of patient safety management at the Committee on Quality Improvement and Patient Safety, which considers that there are limitations in understanding and mismatched by the role of the Committee for Quality Improvement and Patient Safety, due to lack of exposure to information.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">Regarding the performance of the patient safety committee that the committee's factors have not been going well, that is related to the quality of human resources who are still lacking training in the safety incident and reporting system, and the absence of full-time human resources. This is consistent with Sulahyuningsih's research that the limitations in understanding and the inconsistency of the role of the patient safety committee, occur due to lack of exposure to information. Education and training related to the role of the committee need to be done so that the committee management understands and can carry out its role according to the responsibilities and authority.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Also, support from all parties in the hospital, especially the leadership given to the patient safety committee. This statement is following the Republic of Indonesia Ministerial Regulation No. 11 of 2017 concerning patient safety, which addresses patient safety standards. One of them is the role of leadership in improving patient safety by encouraging and ensuring the integrated implementation of patient safety.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">9</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">In this committee, they also lack in control, so they do not know the feedback from the reporting made. This is also one of the reasons for the newly formed patient safety committee. In this regard, periodic monitoring and evaluation need to be carried out at each unit which is carried out as a follow up of the programs that have been carried out. This is in line with Sulahyuningsih's research related to the analysis of patient safety management in the Quality Improvement and Patient Safety Committee, which believes that the hospital has not been able to do monitoring and evaluation regarding the role of the Quality Improvement and Patient Safety Committee in patient safety management. This is because the newly formed committee in the hospital and the implementation of the Patient Safety Targets are still in the stage of socialization.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Conclusion</span><p id="par0090" class="elsevierStylePara elsevierViewall">The patient safety committee has not been running well in the Makassar City Regional General Hospital. This is because human resources have not played an active role in it so that no one works on the committee.
