Based on an international survey by WHO in 2013, 50% of adolescents have dysmenorrhea.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">1</span></a> Some research indicated that the incidence of this problem is quite high in several countries. In Ethiopia, 75.3% of female students at the University of Gondar have primary dysmenorrhea,<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">2</span></a> and 70.6% in King Khalid of Saudi Arabia.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">3</span></a> In Indonesia, there is no definite number of dysmenorrhea incidence in adolescents. Several research have also described the incidence of dysmenorrhea in universities including Jenderal Soedirman, in Purwokerto Indonesia, and the results showed that 67.1% of female students had dysmenorrhea.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">4</span></a> Furthermore, 80% of medical students had primary dysmenorrhea in Andalas University, West Sumatera.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">5</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Dysmenorrhea is a condition of physical discomfort characterized by painful cramps pain that begins before or immediately after the onset of menstrual flow and continues for 48–72<span class="elsevierStyleHsp" style=""></span>h.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">6</span></a> Another complaint is pain in the lower abdomen, which spread to the waist and thighs. In addition, other complaints besides pain are nausea, vomiting, headaches, and diarrhea. This problem is thought to be the result of excess prostaglandin formation, which causes the uterus to contract excessively and also leads to uterine arteriolar vasospasm. Furthermore, psychological factors like anxiety and tension contribute to dysmenorrhea.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">1,7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The complaints by adolescents with dysmenorrhea indicated that this problem significantly impact their daily activities such as absence from college, lack of enthusiasm about attending lectures, and organizational activities. Its impact is found in almost all countries, including the United States, Switzerland, Turkey, and Indonesia. Female workers and students have many complaints of been absent from work and study due to dysmenorrhea.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">8–10</span></a> Based on the discussions with the personnel in charge of Nursing at the Riau University Hospital, the average problem based on female students around health was dysmenorrhea. These problems disrupt student academic activities and daily life. Furthermore, the results of the initial survey of 10 randomly selected female students from representatives of 10 faculties, showed that most experienced dysmenorrhea with complaints of pain in the suprapubic area, around the waist, back, and even all over the body. Based on the data and phenomena above, it is necessary to investigate the incidence of dysmenorrhea in female students at the University of Riau.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Method</span><p id="par0020" class="elsevierStylePara elsevierViewall">This study was conducted from April to October 2019 at the University of Riau. A multistage cluster randomized sampling method was used to select a representative sample of female university students. One thousand one hundred and twenty-one female students were involved in this study, which was conducted in ten faculties, namely Mathematics and Natural Sciences, Social and Political Sciences, Fisheries and Marine Sciences, Teacher Training and Education, Economics and Business, Agriculture, Engineering, Medicine, Law, and Nursing. Eligibility criteria for these study female students who enrolled in degree program at the university and have access to the online questionnaire form. Furthermore, before participation, all participants were asked to provide informed consent, consisting of two parts. The first was sociodemographic information, and the second was the dysmenorrhea characteristic. The second part includes the presence or absence of menstrual cramps, premenstrual symptoms, effects, and treatment of dysmenorrhea severity, and duration. Its severity was determined with the visual analogue scale (VAS). The scores, which were based on previous literature, were categorized as mild, moderate, severe, and very severe pain. The questionnaire using a semistructured questionnaire. Data were analyzed descriptively (frequencies and percentage) using SPSS version 21 and the mean for numerical variables.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Ethical permission was obtained from the Faculty of Nursing 02/UN.19.5.1.8/KEPK.FKp/2019 and also the authorization of school principals. All participants were informed about the procedure and aims of the study and they joined the research group through the link <a href="https://forms.gle/7vymnWEknsFKveGR7">https://forms.gle/7vymnWEknsFKveGR7</a>. Furthermore, informed consent was provided by all individual participants included.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="par0030" class="elsevierStylePara elsevierViewall">The average age of the participants was 19.41 years with a range of 16–24 years. The reported prevalence of dysmenorrhea among these female students was 82.9%. From a total of 1121 subjects, about 71.2% experienced a pain duration of 8–72<span class="elsevierStyleHsp" style=""></span>h. Its intensity was measured using a visual analog scale. Furthermore, the majority (39.7%) of dysmenorrheic females reported have moderate pain, 24.7% as severe, and 2.2% as very severe, while only 16.2% experienced mild pain. Most respondents (62.4%) perceived pain on the first day of menstruation. The result also shows the negative effect of dysmenorrhea on the students, which include the inability to attend classes, loss of appetite, and their sleep was disturbed by menstrual cramps or pain. Furthermore, 44% suffered from dysmenorrhea, which limited their daily activity. The study also assessed the use of pain relievers. Most of the students with dysmenorrhea use pain relievers and many use non-pharmacologic therapies to manage their menstrual discomfort. Their treatments for dysmenorrhea include rest (34.9%), heating pads for cramps or drinking water and herbs, and only 6.1% use medicine. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the characteristics of dysmenorrhea.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Discussion</span><p id="par0035" class="elsevierStylePara elsevierViewall">The results showed 19.41 as the mean age of the respondents, which ranges from 16 to 24 years. Generally, adolescents between the age range of 15–24 and usually begin at 2–3 years after menarche. The frequency decreases with age and usually stops after delivery.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">11,12</span></a> Research by<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">13</span></a> illustrates that the age range of respondents that experience dysmenorrhea is 18–24 years (81.8%). A total of 929 (82.9%) female students experienced dysmenorrhea, which implies that only 17.1% did not experience this problem. This data illustrates that the incidence of dysmenorrhea in female students is quite high at the University of Riau, Indonesia. Other research outside illustrates that the prevalence of dysmenorrhea is also quite high in several countries such as Tabriz University of Medical Sciences, Iran, with a record of 97.4% out of 197 respondents<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">13</span></a> and Ankara University with as much as 84%.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">14</span></a> Furthermore, 80.1% of 376 medical students experienced dysmenorrhea.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">15</span></a> Meanwhile, 63.3% of nursing students in Southern Spain experienced this problem.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">16</span></a> In Palestine,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">17</span></a> reported that as many as 85.1% of female students experience dysmenorrhea at Najah National University. However, only 47% (1921/4606) was recorded for the female university students in Hunan province, China.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">18</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">The intensity of these menstrual cramps felt by some respondents was in the moderate category, namely 158 (40.3%), severe 24.7%, and very severe 2.2%. The results of this study are the same as several others, including,<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">15</span></a> where moderate pain was 49.8% and 80.34% for moderate/severe pain .<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">15,17,19</span></a> Ref.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">18</span></a> found that the highest pain intensity felt by respondents was moderate (27.7%). Dysmenorrhea occurs due to an increase in the hormone prostaglandin, which causes uterine contractions. Most women experience menstrual pain in the moderate scale range. However, when not treated, it interferes with activities and even disrupts the learning process at school and on campus.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Dysmenorrhea also causes shock and loss of consciousness. The nature of the pain is spasms, usually limited to the lower abdomen, but spreads to the waist and thighs. In addition to the pain, nausea, vomiting, headaches, and diarrhea are other symptoms<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">20</span></a> (2). Furthermore, a total of 67.85% complained of abdominal pain during menstruation. The research by<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">18</span></a> describes that out of 2555 female students in Changsha China experienced complaints of cramps (96.9%), weakness (70%), back pain (65.1%), facial blemishes (53.3%), and irritability (55.3%).</p><p id="par0050" class="elsevierStylePara elsevierViewall">The actions taken by respondents to overcome dysmenorrhea include rest 34.9% (391/1121), compressing with and drinking warm water (16.8%), taking painkillers (6.1%), taking traditional medicines (6.1%), and others (17.1%). This action is the same as<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">21</span></a> that reported the method for relieving pain as analgesics (64.3%), rest (61.9%), medical herbs (11.7%), and warm compresses in the area of pain (22.3%). Furthermore, Refs.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">22,23</span></a> report that 90.5% of Spanish students with dysmenorrhea used pharmacological treatment, and 80% were self-medicated.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Conclusion</span><p id="par0055" class="elsevierStylePara elsevierViewall">The prevalence of primary dysmenorrhea among university of Riau students is relatively high. The findings from this study revealed that dysmenorrhea affects a large part of the university population, and is a problem that impacts students’ daily life. Moreover, 71.2% reported a dysmenorrhea duration of 8–72<span class="elsevierStyleHsp" style=""></span>h, while 39.7% had a moderate pain score, and over half (62.4%) suffered informed and extended pains during the first menstruation, while 38.85% had physical and psychological symptoms.
