Cervical cancer, as one of the gynaecological cancers, is the highest prevalence of cancer in Indonesia.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">1</span></a> During 2010–2013 cervical cancer was one of the three most cancers in a cancer hospital in Indonesia, and the number of new cases and deaths from cancer continued to increase.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Chemotherapy is an option in gynaecological cancer management. Research at a district hospital in Surabaya showed that cervical cancer patients who received chemotherapy followed by radiotherapy during 2011–2013 experienced a complete therapeutic response of 88.3%.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">2</span></a> Research at a hospital in Denpasar also showed a significant decrease in tumour mass and left parametrium infiltration after 3 series of chemotherapy in cervical cancer patients.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Nurses who are in charge of administering chemotherapy for gynaecological cancer patients have a large occupational risk. Research at a central hospital in Bandung shows that nurses’ knowledge about chemotherapy and the risk of work accidents in chemotherapy services was at a moderate level and half of the number of oncology nurses had never received training in chemotherapy.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">4</span></a> Meanwhile, according to research at a district hospital in Surakarta, the level of knowledge and attitudes of nurses significantly influenced nurses’ compliance in applying universal precaution when administering chemotherapy.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The importance of chemotherapy in the management of gynaecological cancer and the magnitude of work risks for nurses who carry it out, need to be balanced with high performance. Research in a hospital in Bandung illustrates that the action of giving chemotherapy, from preparation, implementation, monitoring, and evaluation, which was carried out in accordance with the standard of operational procedures, was 72%.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">6</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Performance is strongly influenced by work motivation, one of the determining factors is remuneration. According to Herzberg's Theory, a person's motivation is influenced by motivational factors and hygiene factors, including salary or compensation.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">7</span></a> The research of Miladiyah, Mustikasari, Gayatri found that extrinsic motivation, one of which is remuneration, affected nurses’ performance twenty-six times higher after being controlled by factors of age, employment status, and work experience.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">8</span></a> Likewise, the research of Muthmainnah, Gayatri, Yetti, which proves a significant correlation between remuneration satisfaction and nurses’ work motivation.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">9</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">The high mortality rate caused by gynaecological cancer requires great attention from health workers, including nurses. The importance of chemotherapy in the management of gynaecological cancer and the magnitude of work risks for nurses who carry it out, need to be balanced with high performance. Thus, remuneration as one of the factors influencing nurses’ work motivation needs to be highly considered. On the other hand, there has been no specific research on nurses’ perceptions and expectations of remuneration that affects their work performance in gynaecological services in Indonesia. Therefore, research on the perceptions and expectations of chemotherapy nurses in gynaecology wards about their remuneration need to be explored. The purpose of this study was to explore the perceptions and expectations of chemotherapy nurses in gynaecology wards about their remuneration.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Method</span><p id="par0035" class="elsevierStylePara elsevierViewall">This study used a qualitative research method. Participants in this study were selected using a purposive sampling method. The inclusion criteria in this study were chemotherapy nurses working in a hospital in Indonesia.</p><p id="par0040" class="elsevierStylePara elsevierViewall">This research was conducted from March to October 2019, which began with the preparation of a proposal, data collection, processing of results, and compiling research reports. This study had received a Certificate of Ethical Review Passing from the Ethics Committee of the Faculty of Nursing, Universitas Indonesia number SK-253/UN2.F12.D1.2.1/ETIK.FIK.2019.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The data collection method used in this study was a focus group discussion (FGD), which was conducted two times. The first FGD was done by eight staff nurses, while the second one was performed by seven head nurses. The time spent on each FGD was around one hour. Data were analysed using thematic content analysis.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Result</span><p id="par0050" class="elsevierStylePara elsevierViewall">The participants of this study were fifteen nurses, consisting of 8 staff nurses and 7 head nurses. Staff nurses were given the code SN1 through SN8, while the head nurses were given the code HN1 through HN7. Nurses who participated in this study consisted of 13 females and 2 males, with the age range of 29–50 years old. All nurses in this study were married, with number of children between 0 and 4 children. The work experience of staff nurses was between 5 and 16 years, while head nurses’ work experience was from 16 to 26 years. The educational backgrounds of staff nurses consisted 6 nurses with diploma and 2 nurses with bachelor degree, while head nurses’ educational backgrounds were consisting of 6 nurses with bachelor degree and 1 nurse with master degree. The career ladder of staff nurses was from level 1 to 3, while head nurses’ career ladder was between level 3 and 4.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The resulting themes are as follows: (1) Nurses perceived themselves as chemotherapy nurses who must have special knowledge and skills and work in high risks that induce job stress, (2) Chemotherapy nurses perceived that the remuneration system is not transparent, unfair, and demotivate them to improve their job performance, (3) Chemotherapy nurses expected that the remuneration system could be implemented fairly and transparently, with more involvement of nurse managers and support of adequate information system.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The first theme was nurses perceived themselves as chemotherapy nurses who must have special knowledge and skills and work in high risks that induce job stress. The statements below were the examples of their expressions:<span class="elsevierStyleDisplayedQuote" id="dsq0005"><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall"><span class="elsevierStyleItalic">“We have special knowledge and skills which are different from other nurses, that is chemotherapy” (HN3)</span></p></span><span class="elsevierStyleDisplayedQuote" id="dsq0010"><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall"><span class="elsevierStyleItalic">“In the inpatient ward we administer intravenous or oral chemotherapy, so that we are exposed to carsinogenic agents.” (SN4)</span></p></span><span class="elsevierStyleDisplayedQuote" id="dsq0015"><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall"><span class="elsevierStyleItalic">“Every time we administer chemotherapy, our stress level is getting higher because we are afraid that the side effects of chemotherapy would appear.” (SN7)</span></p></span></p><p id="par0065" class="elsevierStylePara elsevierViewall">The second theme was chemotherapy nurses perceived that the remuneration system is not transparent, unfair, and demotivate them to improve their job performance. The statements below were the examples of their expressions:<span class="elsevierStyleDisplayedQuote" id="dsq0020"><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall"><span class="elsevierStyleItalic">“Sometimes the amount of remuneration received by nurses who do not administer chemotherapy are higher than those who do it. So, I think how to count the amount of remuneration is unclear and inconsistent.” (SN6)</span></p></span><span class="elsevierStyleDisplayedQuote" id="dsq0025"><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall"><span class="elsevierStyleItalic">“My work performance does not affect my income, so that I am not motivated to do my best.” (HN5)</span></p></span></p><p id="par0070" class="elsevierStylePara elsevierViewall">The third theme was chemotherapy nurses expected that the remuneration system can be implemented fairly and transparently, with more involvement of nurse managers and support of adequate information system. The statements below were the examples of their expressions:<span class="elsevierStyleDisplayedQuote" id="dsq0030"><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall"><span class="elsevierStyleItalic">“It must be very clear for us why the amount of remuneration we received is higher or lower every month.” (SN7)</span></p></span><span class="elsevierStyleDisplayedQuote" id="dsq0035"><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall"><span class="elsevierStyleItalic">“The information system needs to be improved, so that it can help the users more.” (HN7)</span></p></span></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">The first theme is nurses perceived themselves as chemotherapy nurses who must have special knowledge and skills and work in high risks that induce job stress. This perception is consistent with Wiseman et al. (2005).<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">10</span></a> Nurses have three main roles in the process of administering chemotherapy, including to educate patients, to manage the therapy, and to manage the side effects.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">10</span></a> Nurses must also provide emotional supports to patients and their families, act as learning facilitators and role models for less experienced staff, and ensure that the environment and the nurse itself are safe.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">10</span></a> Therefore, nurses must have confidence in their knowledge, abilities, and technical expertise so that they can provide effective, safe, and useful patient care.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">10</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">The perception that chemotherapy nurses must have special knowledge and skills is consistent with Roe and Lennan. Nurses must be fully involved in the assessment and management of patients undergoing chemotherapy.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">11</span></a> Consequently, a chemotherapy nurse must be a competent practitioner who has the necessary knowledge about the specific chemotherapy the patient is receiving.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">11</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">The perception that chemotherapy nurses work with high occupational risks, workload, and job stress is in accordance with Wiseman et al. (2005). Chemotherapy nurses are at risk not only when handling cytotoxic drugs, but also from handling excretions and body fluids from patients receiving chemotherapy.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">10</span></a> At the first nurses administer chemotherapy, they feel very anxious, nervous, scared, and very unsure.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">10</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">The second theme is chemotherapy nurses perceived that the remuneration system is not transparent, unfair, and demotivate them to improve their job performance.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Even though the implementation of the remuneration system had already considered job performance, but nurses in this study perceived it was not transparent and unfair. When nurses did not reach the work target, they received less amount of remuneration as punishment. However, when nurses performed more than the target, they did not get more amount of remuneration as the reward.</p><p id="par0100" class="elsevierStylePara elsevierViewall">This study result shows that implementation of the remuneration system in this hospital does not motivate nurses to work harder and perform better. Head nurses said that it is difficult to motivate their subordinate to increase their career ladder because associate nurses commonly think the higher the career ladder, the more the responsibilities but not the higher remuneration.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Several other hospitals also have considered performance in determining the amount of remuneration for their nurses in various ways. There are several remuneration models applied in various hospitals in Indonesia. Research by Sulasmi, Herachwati, and Rachmawati revealed a job ranking system at a teaching hospital as part of developing a remuneration system.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">12</span></a> While Suprajitno explained about the development of the nursing remuneration index at a hospital in Blitar using a statistical approach.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">13</span></a> The remuneration index at this hospital is formed by three factors at work, additional work as measured by daily additional work and its length as a safety, and the type of nursing intervention measured from the number of nursing interventions in the simple, mild, moderate, severe, and special nursing interventions.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">13</span></a> In addition, Prihatin explained that the provision of compensation consisted of 2 components, namely direct financial compensation and indirect financial compensation.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">14</span></a> In contrast, another hospital applies casemix system strategy formula.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">15</span></a> A district general hospital in West Sumatra also has implemented a remuneration system and has considered the principles of remuneration, namely individual equity, internal equity, and external equity.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">16</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">The third theme is chemotherapy nurses expected that the remuneration system can be implemented fairly and transparently, with more involvement of nurse managers and support of adequate information system. Nurse's expectations are in accordance with the results of Sudibyo's research.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">17</span></a> In the study, nurses proposed that the formula for the distribution of nurse services incentives consists of two distribution groups; including the formula based on an assessment of output performance and based on the assessment of input performance.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">17</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">This nurse's expectation is also in accordance with the findings of Pembantjanawati, who evaluated the Remuneration System at a hospital in Blora Regency.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">18</span></a> The results of this study indicate that hospital employees proposed a review related to the percentage of remuneration for doctors and nurses by referring to the distribution of incentives based on the team and making new agreements on the distribution of direct and indirect incentives.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">18</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">This research has implications in the field of nursing, especially providing an overview, especially about nurses’ expectations so that a fair remuneration system can be applied to improve nurse performance. Hospital management needs to find the right formula that suits the character of each resource.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Conclusion</span><p id="par0125" class="elsevierStylePara elsevierViewall">Based on the results of the study and the discussion, it can be concluded about the perceptions and expectations of chemotherapy nurses in gynaecology wards about their remuneration. The resulting themes are as follows: (1) Nurses perceived themselves as chemotherapy nurses who must have special knowledge and skills and work in high risks that induce job stress, (2) Chemotherapy nurses perceived that the remuneration system is not transparent, unfair, and demotivate them to improve their job performance, (3) Chemotherapy nurses expected that the remuneration system could be implemented fairly and transparently, with more involvement of nurse managers and support of adequate information system.</p><p id="par0130" class="elsevierStylePara elsevierViewall">It is recommended that hospitals implement a fair and proportional remuneration system that motivates nurses to improve their performance. This study was a qualitative research with phenomenological design. Data were collected by FGD involving 15 chemotherapy nurses in a hospital in Indonesia. The themes produced were as follows: (1) Nurses perceived themselves as chemotherapy nurses with special knowledge and skills and high-risk job, (2) Chemotherapy nurses perceived that the remuneration system is not transparent, unfair, and demotivate them, (3) Chemotherapy nurses expected that the remuneration system could be implemented fairly and transparently, with support of adequate information system.
