Objetivos

Estudiar la efectividad y seguridad del tratamiento endoscópico de la pancreatitis y sus complicaciones en nuestro medio a corto y medio plazo.

Pacientes Y Método

Análisis retrospectivo de 43 pacientes con los diagnósticos obtenidos mediante colangiopancreatografía retrógrada endoscópica de pancreatitis crónica, pancreatitis aguda complicada con seudoquiste y fístula pancreática, que fueron susceptibles de tratamiento endoscópico.

Resultados

En 35 pacientes hubo intención de tratamiento endoscópico. La indicación de tratamiento fue dolor en 17 (48,5%), ictericia en 7 (20%), seudoquiste en 10 (28,5%) y sospecha de fístula externa en uno (3%). Se consiguió éxito técnico en 28 (80%). De los pacientes con dolor, a 13 se les insertó una prótesis pancreática y a seis se les aplicó litotricia extracorpórea. Se obtuvo mejoría en el 65% de los casos. La totalidad de los 7 pacientes con ictericia presentaba estenosis secundaria de la vía biliar. Se aplicó tratamiento en dos de ellos, con mejoría parcial. De los 15 pacientes con seudoquiste, se indicó tratamiento endoscópico en 10, obteniendo éxito técnico en ocho y resolución completa en siete (87,5%). El paciente con fístula externa fue tratado mediante una prótesis transpapilar, con resolución completa de la disrupción del Wirsung. La mejoría global de los pacientes tratados con éxito fue: completa en 19 (68%), parcial en tres (18%) y sin mejoría en cuatro (14%); se perdieron 2 pacientes para el seguimiento. Hubo 4 complicaciones a corto plazo. Se produjeron 4 muertes, una de ellas relacionada con la técnica.

Conclusiones

El tratamiento endoscópico del dolor crónico de la pancreatitis crónica, de los seudoquistes y de las fístulas ha sido efectivo en nuestro medio, con un bajo porcentaje de complicaciones.

Objectives

To study the long- and short-term safety and efficacy of endoscopic treatment of pancreatitis and its complications in our environment.

Patients and Methods

We performed a retrospective analysis of 43 patients with chronic pancreatitis, acute pancreatitis complicated with pseudocyst, and pancreatic fistula diagnosed by endoscopic retrograde cholangiopancreatography who were suitable for endoscopic treatment.

Results

Endoscopic treatment was attempted in 35 patients. The indication for treatment was pain in 17 patients (48.5%), jaundice in 7 (20%), pseudocyst in 10 (28.5%) and suspected external fistula in 1 (3%). The technique was successfully performed in 28 (80%). Of the patients with pain, pancreatic prosthesis was inserted in 13 and extracorporeal lithotripsy was applied in 6. Sixty-five percent of the patients improved. Of the 7 patients with jaundice, all had secondary stenosis of the biliary tract. Treatment was applied in 2, who showed partial improvement. Of the 15 patients with pseudocyst, endoscopic treatment was indicated in 10; the technique was successfully performed in 8 and complete resolution was achieved in 7 (87.5%). The patient with external fistula was treated with transpapillary prosthesis and complete resolution of disruption of Wirsung's duct was achieved. Overall improvement in successfully treated patients was: complete in 19 (68%), partial in 3 (18%), no improvement in 4 (14%) and 2 patients were lost to treatment. There were 4 short-term complications. There were 4 deaths and one was related to the technique.

Conclusions

Endoscopic treatment of chronic pain in chronic pancreatitis, pseudocysts and fistulas was effective in our environment with a low rate of complications.
