Especially by the concomitant use of drugs that have different mechanisms of action.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">6</span></a> The combinations of angiotensin receptor blockers (ARBs) and calcium channel blockers (CCBs) have been widely used in clinical practice.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">7</span></a> ARBs had different efficacy in lowering blood pressure.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">8</span></a> Candesartan cilexetil is the most potent long-acting ARB, although potency does not necessarily imply greater clinical efficacy; its recommended daily dose in the treatment of hypertension is 4–8 to 16<span class="elsevierStyleHsp" style=""></span>mg once daily. In comparison, the recommended dose of valsartan in the treatment of hypertension is 80–160<span class="elsevierStyleHsp" style=""></span>mg.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">9,10</span></a> Dose of 16<span class="elsevierStyleHsp" style=""></span>mg of candesartan is comparable to 160<span class="elsevierStyleHsp" style=""></span>mg of valsartan.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">11</span></a> Also, CCBs, mainly belonging to the dihydropyridine class as amlodipine, are the most effective antihypertensive agents.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">12</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Measuring PBP has limitations as the measurement of CBP represents the direct load that influences the performance and structure of target organs like; the heart, kidneys, and brain.<a class="elsevierStyleCrossRefs" href="#bib0275"><span class="elsevierStyleSup">13,14</span></a> Combining an ARB with a CCB has the potential to improve not only peripheral BP but also central hemodynamics (synergy at the vascular level).<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">12</span></a> As a result, this combination protects against hypertension-modified organ damage.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">7</span></a> The augmentation index (AIx%), as a marker of the wave reflection, is reduced differently by the antihypertensive drugs’ classes.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">15</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Many studies were used to examine the amlodipine/valsartan combination effect on peripheral and central BP and to compare it with different combinations. However, no study used to examine the candesartan/amlodipine combination in central blood pressure. Moreover, no study compared it with the amlodipine/valsartan combination. Therefore, the purpose of the current study was to compare the efficacy and safety of amlodipine/candesartan combination with the commonly used and most studied combination, amlodipine/valsartan. We hypothesized that the amlodipine/candesartan combination is non-inferior to the amlodipine/valsartan combination as regard central and peripheral BP.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Blood pressure measurements</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Peripheral blood pressure measurement</span><p id="par0025" class="elsevierStylePara elsevierViewall">An authorized automated blood pressure (Omron 10 series BP7450 Wireless Upper Arm Blood Pressure Monitor) was used to measure the first peripheral BP measurement. Then, two consecutive measurements of peripheral BP were measured 2<span class="elsevierStyleHsp" style=""></span>min apart using a standardized manual sphygmomanometer. The patient seated for 5<span class="elsevierStyleHsp" style=""></span>min before beginning in a quiet room. Appropriate-sized cuff encircling at least 80% of the arm was used. The cuff was replaced every 6 months to ensure validity of the rubber inside the cuff. We got the average of the last two readings.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Central blood pressure and its hemodynamics</span><p id="par0030" class="elsevierStylePara elsevierViewall">A Brachial Cuff-based device (Mobil-O-Graph 24<span class="elsevierStyleHsp" style=""></span>h PWA (IEM) device) was used to measure CBP and its indices non-invasively. Before the measurements, patients were requested to rest for at least 5<span class="elsevierStyleHsp" style=""></span>min in a quiet room. Measurements were taken in the sitting position. According to the size of the patients’ arm, a proper cuff was used on the patient's non-dominant left arm. The cuff is partially inflated to record the brachial waveforms. These waveforms are detected by sensing changes in the pressure inside the cuff related to arterial pulsation. The ascending aortic waveform is subsequently derived using a validated mathematical transfer function. Moreover, after the completion of the measurement, the data will transfer via Bluetooth wireless technology. CBP indices were calculated immediately during a clinic visit.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">16</span></a> It provides CBP estimations accurately as those measured invasively.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">17</span></a></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Safety and tolerability</span><p id="par0035" class="elsevierStylePara elsevierViewall">It is evaluated according to adverse events reported by the patient at each visit and also by using clinical signs and examinations.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Patients</span><p id="par0040" class="elsevierStylePara elsevierViewall">It was an open-label, prospective, parallel, and randomized study. Between March 2019 and June 2021, 105 patients were recruited from the essential hypertension patients who attended the outpatient hypertension clinic at Beni-Suef University Hospital. They were randomized on a 1:1 basis simply into two groups. The first group, “group I” (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>52), received valsartan (160<span class="elsevierStyleHsp" style=""></span>mg) plus amlodipine (10<span class="elsevierStyleHsp" style=""></span>mg) as a fixed-dose combination. In comparison, the second group “group II” (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>53), received candesartan (16<span class="elsevierStyleHsp" style=""></span>mg) plus amlodipine (10<span class="elsevierStyleHsp" style=""></span>mg) as a free drug combination. Eighty-six patients continued the study; <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>42 “group I” and <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>44 “group II”. Nineteen patients were excluded; 15 discontinued the intervention, and four changed their medication. Each patient had his BP measurements in the hypertension clinic (office and central BP measurements) for comparison purposes. All measurements were processed before the beginning of the study, six weeks, and after 12 weeks (end of the study).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Participant inclusion criteria</span><p id="par0045" class="elsevierStylePara elsevierViewall">Adult patients over 18 years old of both genders were included in the study. Patients diagnosed with hypertension stages II or III, according to the definitions of the 2018 European Arterial Hypertension Guidelines.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">4</span></a></p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Participant exclusion criteria</span><p id="par0050" class="elsevierStylePara elsevierViewall">Fertile women, independent of contraceptive methods, and patients with known hypersensitivity to study medications were excluded. The subject has chronic kidney disease. Patients with cardiac diseases like myocardial infarction (MI), congestive heart failure (NYHA class III or IV), or any arrhythmias are also excluded.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Ethical approval</span><p id="par0055" class="elsevierStylePara elsevierViewall">The study was performed in accordance with the Declaration of Helsinki, reviewed and approved by the “Research Ethical committee” of the faculty of pharmacy, Beni-Suef University No.: REC-H-PhBSU-19006. Written informed consent was obtained from every patient in the study.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">Values were presented as means<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>SD or as numbers and proportions, as appropriate. The relations between qualitative variables were evaluated by the Chi-square test or Fisher's exact test, as indicated. Means were compared with Student's test for baseline characteristics and laboratory outcomes. Repeated measure ANOVA was done to test the effect of time and groups of treatment on blood pressure variability and pulse wave velocity. The <span class="elsevierStyleItalic">p</span>-value for multiple pairwise comparisons was adjusted using Bonferroni correction. All tests were bilateral, and a <span class="elsevierStyleItalic">p</span> value of 5% was the limit of statistical significance. Analysis was performed by statistical package software IBM-SPSS version 24 (IBM, Chicago, USA).</p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">A flow diagram of the current study is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. A total of 52 patients started with amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/valsartan 160<span class="elsevierStyleHsp" style=""></span>mg tab; 6 patients in this group discontinued the study, and 4 changed their medication. In the other group, 53 patients started with amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/candesartan 16<span class="elsevierStyleHsp" style=""></span>mg, and 9 discontinued the study. Finally, 42 patients who started with amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/valsartan 160<span class="elsevierStyleHsp" style=""></span>mg and 44 of those who started with amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/candesartan 16<span class="elsevierStyleHsp" style=""></span>mg completed the study.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Baseline patients’ characteristics</span><p id="par0070" class="elsevierStylePara elsevierViewall">Baseline demographic characteristics were comparable between both treatment groups. As shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, there was no significant difference between both groups as regard laboratory data.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">The difference within and between the two treatment groups in peripheral systolic, diastolic, and mean arterial pressure from baseline to 6 and 12 weeks</span><p id="par0075" class="elsevierStylePara elsevierViewall">There was a significant difference within group I over time in PSBP, PDBP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001). However, group II did not show any significant difference within group in PSBP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.101), PDBP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.873), and MAP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.599).</p><p id="par0080" class="elsevierStylePara elsevierViewall">Both treatment groups were highly effective in reducing peripheral blood pressure, as presented in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. There was no difference between the two treatment groups over time in reducing PSBP, PDBP, and MAP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.095, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.150 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.086, respectively). After six weeks of treatment, Group I exerted its maximum effect in reducing PSBP, PDBP, and MAP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001). Group II showed an additional reduction in PSBP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001) and MAP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.006) after the 12 weeks of follow-up, as shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">The difference within and between the two treatment groups in central blood pressure and its indices from baseline to 6 and 12 weeks</span><p id="par0085" class="elsevierStylePara elsevierViewall">In group I, there was a highly statistically significant difference within the group over time in CSBP, and CDBP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001). Both treatment groups showed a highly statistically significant effect between groups in reducing CSBP, CDBP, and Central PP but not in reducing HR and AIx. Group I reached the maximum effect in reducing CSBP and CDBP after six weeks (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001), and there was no additional reduction from 6 to 12 weeks. As CSBP decreased from 139.81<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>13.86<span class="elsevierStyleHsp" style=""></span>mmHg to 118.98<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11.16 after six weeks and decreased to 116.55<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.340 after 12 weeks. Furthermore, CDBP dropped by 13<span class="elsevierStyleHsp" style=""></span>mmHg after six weeks and one mmHg after 12 weeks. However, it continued to show an additional effect in reducing Central PP after 12 weeks (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001), as seen in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. Group II did not show any additional effect in reducing CDBP, central PP from 6 to 12 weeks except for CSBP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>, a moderately strong positive correlation was reported between the decrease in PSBP and CSBP after 6 and 12 weeks (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.578 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001) and <span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.495 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001), respectively). Also, there was a moderately strong positive correlation between the decrease in PDBP and CDBP after 6 and 12 weeks (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.554 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and <span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.535 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), respectively).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Safety</span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Regarding adverse effects after 6 and 12 weeks</span><p id="par0095" class="elsevierStylePara elsevierViewall">Both treatments were safe and tolerable. There was no statistically significant difference between groups regarding the number of patients who experienced adverse effects after 6 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.853) and 12 weeks (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.375) of treatment. After six weeks of treatment, five patients from both groups had LL edema. Although, after 12 weeks of treatment, the number of patients dropped to one in group I. Also, one patient from each group suffered from dizziness. However, after 12 weeks, two patients had dizziness in group I and not in group II. One patient had a headache in group I after six weeks of treatment and two in group II after 12 weeks. One patient suffered from both LL (lower limb) edema and dizziness in group II after six weeks. Although, one patient complained of headache and dizziness in group II after six weeks and in group I after 12 weeks.</p></span></span><span id="sec0090" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">This current study was a prospective, open-label, and randomized non-inferiority study. Compared amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/valsartan 160<span class="elsevierStyleHsp" style=""></span>mg with amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/candesartan 16<span class="elsevierStyleHsp" style=""></span>mg. The findings demonstrated that both combinations were non-inferior to each other in controlling peripheral and CBP. Both treatments were well tolerated and associated with few adverse events. This is the first study which used to examine the non-inferiority of the amlodipine/valsartan combination compared with the amlodipine/candesartan combination. Furthermore, there is no study demonstrated the effects of the amlodipine/candesartan combination on central hemodynamics.</p><span id="sec0095" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Efficacy of amlodipine/valsartan versus amlodipine/candesartan on PBP</span><p id="par0105" class="elsevierStylePara elsevierViewall">In the current study, the amlodipine/valsartan combination reached its maximum efficacy in reducing PBP after six weeks of treatment with no significant additional reduction after 12 weeks. Many previous studies and meta-analysis confirmed the efficacy of the amlodipine/valsartan combination in reducing peripheral BP.<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">18–20</span></a> A study done on 8336 patients to evaluate the efficacy of amlodipine/valsartan single-pill combination (SPC), concluded that an optimal BP reduction was achieved for all patients with different hypertension grades.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">21</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">In agreement with previous studies, our results showed the high efficacy of amlodipine/candesartan combination in reducing PBP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001). In a previous study, candesartan cilexetil in combination with amlodipine is an effective BP-lowering drug in treating moderate-to-severe essential hypertension.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">22</span></a> Also, the amlodipine and candesartan trial (ACTY) demonstrated that the amlodipine and candesartan combination significantly reduced PSBP.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">23</span></a> A phase II multi-center, randomized, double-blind clinical trial demonstrated the efficacy of the combination therapy amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/candesartan 16<span class="elsevierStyleHsp" style=""></span>mg compared to amlodipine monotherapy in reducing peripheral systolic and diastolic BP.<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">24</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">Furthermore, the amlodipine/candesartan combination showed additional efficacy in reducing PSBP and MAP after 12 weeks of treatment. The previous studies’ results go with our findings. A study was done on poorly controlled hypertensive patients showed that the amlodipine 5<span class="elsevierStyleHsp" style=""></span>mg/candesartan 8<span class="elsevierStyleHsp" style=""></span>mg combination was effective in reducing blood pressure after 12 weeks of treatment.<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">25</span></a> Another short-term (24 weeks) study demonstrated the efficacy of candesartan in reducing PSBP and PDBP after the end of treatment.<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">26</span></a> However, some studies suggest increasing the dose or adding another drug to the candesartan regimen if the patient cannot obtain its target after four weeks of treatment.<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">27</span></a> Our findings suggest that the effect of candesartan may be more delayed than valsartan in PSBP and MAP. However, finally, both treatments reached the same level of PBP after the 12-weeks treatment period.</p></span><span id="sec0100" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Efficacy of amlodipine/valsartan versus amlodipine/candesartan on CBP and its hemodynamics</span><p id="par0120" class="elsevierStylePara elsevierViewall">Both treatments showed greater efficacy in reducing CBP without statistically significant differences between groups. The Strong Heart Study has recommended CBP as a treatment target in future trials.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">13</span></a> A recent meta-analysis concluded that RAS blockers and CCBs were more efficacious than other agents on central hemodynamics.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">28</span></a> That's because RAS blockers produce angiotensin II, which enhances NO bioavailability and hence enhances vasodilation. In the current study, the amlodipine/valsartan combination reached its maximum efficacy in reducing CSBP and CDBP after six weeks of treatment as in PBP. The amlodipine/valsartan combination has been studied extensively in previous studies and has proven its efficacy in lowering CBP. Farag et al., in 2018, stated that the amlodipine/valsartan combination was highly effective in reducing CBP.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">29</span></a> Also, in Explore study, the amlodipine/valsartan combination effectively reduced CSBP compared with the amlodipine/atenolol combination.<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">30</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">After six weeks, the amlodipine/candesartan combination reached its maximum efficacy in reducing CDBP. Nevertheless, it showed an additional reduction in CSBP after 12 weeks of treatment. It was proven that treatment with candesartan reduced CBP compared with baseline.<a class="elsevierStyleCrossRef" href="#bib0365"><span class="elsevierStyleSup">31</span></a> The effect of candesartan cilexetil 16<span class="elsevierStyleHsp" style=""></span>mg in reducing CBP may be due to the induction of systemic and forearm vasodilatation. This results in a reduction in BP without affecting cardiac performance.<a class="elsevierStyleCrossRef" href="#bib0370"><span class="elsevierStyleSup">32</span></a> However, the new drug combination amlodipine/candesartan's effect on central blood pressure has not been studied extensively. A study was done on Japanese population to compare the peripheral BP lowering effects between the amlodipine add-on candesartan group and candesartan add-on amlodipine group, concluded that the larger decrease in BP in the CCB add-on group than in the ARB add-on group in this study. They suggested that the improvement in vascular function parameters in the CCB add-on group is derived from better BP control. However, both groups showed significant improvements in central SBP after the 12 week follow-up.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">23</span></a> More recent study showed that candesartan reduced CSBP significantly after four weeks of treatment (mean change −18.4<span class="elsevierStyleHsp" style=""></span>mmHg, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002), and this effect continued after eight weeks (mean change −22.6<span class="elsevierStyleHsp" style=""></span>mmHg, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001).<a class="elsevierStyleCrossRef" href="#bib0375"><span class="elsevierStyleSup">33</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Central PP may determine clinical outcomes over brachial PP.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">13</span></a> Central PP is associated with the macro-circulation target organ damage (heart, carotid arteries).<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">12</span></a> The two treatment groups in the current study reduced central PP significantly, but there was a significant difference between the groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.036). As group II exerted its maximum effect at six weeks of treatment (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.411), group I continued to show some efficacy (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001) after 12 weeks.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In the CAFE study, amlodipine-based therapy significantly decreases central PP compared with atenolol-based therapy.<a class="elsevierStyleCrossRef" href="#bib0380"><span class="elsevierStyleSup">34</span></a> The EXPLORE study showed that the amlodipine/valsartan treatment group decreased central PP more than the amlodipine/atenolol treatment group. In a previous study, candesartan showed a significant decrease in PP results from its dose-dependent decrease in SBP with a smaller decrease in DBP.<a class="elsevierStyleCrossRef" href="#bib0385"><span class="elsevierStyleSup">35</span></a> In the CHAMP study, candesartan exerted a substantial decrease in PP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05) more than losartan.<a class="elsevierStyleCrossRef" href="#bib0390"><span class="elsevierStyleSup">36</span></a> Another study concluded that candesartan administration for a 4-weeks period significantly decreased PP.<a class="elsevierStyleCrossRef" href="#bib0395"><span class="elsevierStyleSup">37</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">CBP and PBP are highly interrelated.<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">38</span></a> Correlation analysis between CBP and PBP reductions over time may demonstrate if a drug can decrease CBP beyond decreasing PBP.A cohort study demonstrated a good correlation between central systolic pressure and peripheral SBP reductions after 12 weeks of treatment with valsartan.<a class="elsevierStyleCrossRef" href="#bib0405"><span class="elsevierStyleSup">39</span></a> These results were comparable with the current study results that demonstrated a high correlation between PSBP and CSBP and PDBP and CDBP.</p></span><span id="sec0105" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Safety and tolerability</span><p id="par0145" class="elsevierStylePara elsevierViewall">The most common side effect of the chronic treatment with CCB is LL (lower limb) edema. The use of therapeutic combinations of ARB and CCB was found to reduce the risk of that undesirable side effect because of the counterbalancing effect provoked by RAS inhibition and using lower doses of each drug.<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">40</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">In this study, the two treatment groups were well tolerated with minor side effects which were confirmed by previous studies. Many studies showed that treatment with amlodipine/valsartan combination was well tolerated with few adverse events (AEs).<a class="elsevierStyleCrossRefs" href="#bib0315"><span class="elsevierStyleSup">21,41,42</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">The CASTLE study evaluated the safety and efficacy of candesartan 8<span class="elsevierStyleHsp" style=""></span>mg/amlodipine 5<span class="elsevierStyleHsp" style=""></span>mg combination in hypertensive patients concluded that the combination was well tolerated with no side effects.<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">25</span></a> Another study showed that headache was the most frequent adverse event. Other side effects may include upper respiratory tract infections, lethargy, dizziness, and cough. Furthermore, the proportion of patients who experienced adverse effects in the candesartan arm was not statistically significant compared with placebo ((31.2%) versus (20.7%); <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.1327).<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">22</span></a> Also, it was found that the dose-dependent response effect of candesartan did not have dose-dependent adverse effects.</p></span></span><span id="sec0110" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Conclusion</span><p id="par0160" class="elsevierStylePara elsevierViewall">Amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/candesartan 16<span class="elsevierStyleHsp" style=""></span>mg combination was non inferior to amlodipine 10<span class="elsevierStyleHsp" style=""></span>mg/valsartan 160<span class="elsevierStyleHsp" style=""></span>mg combination in terms of reducing peripheral and CBP over time. Although the amlodipine/candesartan combination did not reach its maximum effect until 12 weeks, amlodipine/valsartan reached its maximum effect after six weeks. In addition, both treatments were similarly associated with fewer side effects. Peripheral BP was highly correlated to central BP.</p></span><span id="sec0115" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Funding</span><p Nevertheless, a study has yet to be done to evaluate the effect of amlodipine/candesartan on central blood pressure and compare it with amlodipine/valsartan combination. To see how “amlodipine plus candesartan combination” reduces peripheral and central blood pressure compared to the most studied combination, “amlodipine plus valsartan”.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect1015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Eighty-six patients were randomized in an open-label, prospective study by 1:1 ratio to two groups. Group I (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>42) received the amlodipine and valsartan combination, and group II (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>44) received the amlodipine and candesartan combination. Peripheral and central blood pressure (CBP) was measured at baseline, at 6 and 12 weeks of follow-up.</p></span> <span id="abst3010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect1215">Discussion</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Both treatment groups reduced peripheral systolic, diastolic, and mean blood pressure. There was no significant difference between and within both groups. The amlodipine/candesartan combination showed more reduction in peripheral systolic blood pressure (PSBP) after 12 weeks of treatment (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><0.001). Both groups decreased CBP without significant differences between groups.
