Also, it can increase – generally in values not higher than 100–150<span class="elsevierStyleHsp" style=""></span>U/l – in a large number of benign processes such as liver diseases (hepatitis, cirrhosis, cholangitis, cholecystitis), pancreatic (pseudocyst, pancreatitis), respiratory (pulmonary fibrosis, bronchial asthma, asbestosis, bronchiectasis, tuberculosis), renal failure, mucinous cysts, hydronephrosis or in various autoimmune diseases. In addition, it may be increased by lab test interferences.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Although there is no unanimity, the determination of CA 19-9 can be used during the study of patients with suspected neoplasms of uncertain origin, to guide a possible diagnosis.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">3</span></a> Therefore, the finding of elevated CA 19-9 is relatively common during the study of patients with nonspecific symptoms such as asthenia, abdominal pain or weight loss who, finally, show no neoplasm or for whom an established diagnosis is not obtained.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The elevation of CA 19-9 without evidence of neoplasm represents a diagnostic and follow-up problem.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Last year we treated 6 patients in our unit with elevation of CA 19-9 without demonstrated neoplasm (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Most presented with weight loss and abdominal pain and, except for cases 3 – with direct hyperbilirubinemia and striking cholestasis – and 4 – with cholestasis without hyperbilirubinemia – did not show any other relevant laboratory data. For this reason, at least one chest–abdomen–pelvis CT scan was performed in all 4 cases. Lab monitoring was scheduled for 6 months.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">In the first case, the stability of the values and the existence of pulmonary fibrosis, known cause of CA 19-9 elevation, allowed to close the study. The second patient complained of heartburn and CA 19-9 of 967<span class="elsevierStyleHsp" style=""></span>U/l. A second determination was 6.094<span class="elsevierStyleHsp" style=""></span>U/l, which coincided on the same date as the CT scan of the abdomen in which a ureteral lithiasis with pyelocaliceal ectasia and a normal pancreas was found. CA 19-9 levels went back to normal once the process was resolved.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The third and fourth cases presented with abdominal pain. After diagnosis of choledocholithiasis and ERCP, the values of CA 19-9 were normalized.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The fifth patient had a CA 19-9 of 128<span class="elsevierStyleHsp" style=""></span>U/l. After ruling out PC, follow-up was scheduled. At 9 months, the patient was asymptomatic and with a slightly elevated CA 19-9.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The last patient was diagnosed with an isolated deficiency of ACTH, and we could only see a cystic image in the ovary on the CT scan, for which follow-up was scheduled.</p><p id="par0045" class="elsevierStylePara elsevierViewall">As we see, an increased CA 19-9 has important implications in many aspects. In a Chinese cohort<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">4</span></a> of individuals with high levels of CA 19-9 (>K37<span class="elsevierStyleHsp" style=""></span>U/l), a second determination was considered 2–4 weeks later. Radiological or endoscopic tests were performed to rule out PC if the levels were still high. However, as far as we know, there are no clinical practice guidelines for the follow-up of patients with elevated CA 19-9 levels in whom the complementary tests do not show PC. In these cases, it is recommended to repeat the determination monthly, on at least 2 occasions.
