Regarding the methodology of the study, a descriptive analysis of epidemiological and clinical variables of the patients and the dialysis technique was carried out (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>); as well as a statistical analysis to assess the relationship between the different variables and the presence of IDH. We considered hypertension to be a difference in systolic blood pressure (BP) pre- and post-HD > 10 mmHg in 4–6 consecutive sessions.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">One of the most relevant data we obtained was that there was a prevalence of 30.2% of IDH, even higher than in other publications using the same defining criteria for IDH, which was between 5 and 25%.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> In addition, it is important to highlight that in most cases, clinical and ultrasound criteria (VEXUS/LUS score 0) ruled out volume overload as the cause of IDH, traditionally considered one of the most common causes of hypertension (HT) in haemodialysis.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">In addition, the majority of cases were found to have interdialytic HT (84.5%) and 49.5% required more than 2 antihypertensive drugs to control BP, with beta-blockers being the most commonly used drugs (58%), together with diuretics (52.3%), since 41.9% of the sample preserved residual renal function; followed by calcium antagonists (49%), ACE inhibitors/ARBs (37.6%) and alpha-blockers (32.1%). Furthermore, the statistical analysis showed statistical significance (p = 0.008) in the relationship between having hypertension and having IDH, which seems to indicate an association between both variables.</p><p id="par0025" class="elsevierStylePara elsevierViewall">It is also noteworthy that no association with the type of dialysis technique was observed, as most cases were standard HD (94.9%) with a frequency of 3 sessions per week (82.1%). In addition, sodium/bicarbonate concentrations and dialysate fluid temperature remained constant in all cases. Furthermore, no statistically significant differences were observed between dialysis quality (Kt/V) and IDH (p = 0.819).</p><p id="par0030" class="elsevierStylePara elsevierViewall">HT is a common phenomenon in haemodialysis patients. Traditionally, a dynamic BP pattern has been described consisting of a decrease during the dialysis session, with a progressive increase in BP until the next session. However, a subgroup of patients has a paradoxical rise in BP during the haemodialysis session, called intradialytic hypertension (IDH).<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> This phenomenon is poorly understood and little evidence is available in the literature.</p><p id="par0035" class="elsevierStylePara elsevierViewall">There is no consensus on the definition of IDH, as different studies use different criteria to define it, making it extremely difficult to determine its prevalence.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,3</span></a> Moreover, its pathophysiology has not been accurately determined, and this means that it is difficult to treat. Some authors suggest that it is related to the volume overload associated with interdialytic HT, others believe that it is due to activation of the renin-angiotensin-aldosterone system or the sympathetic nervous system, others associate it with the dialysis technique as a result of increased sodium gain, the use of erythropoietin analogues or the withdrawal of antihypertensive drugs during the session;
