Considering that a significant percentage of people with a delayed diagnosis have already been in contact with the healthcare system before being diagnosed with the infection, HEDs should be included in these screening programs.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In a study carried out between 2011-2015 in the regional community of Aragón, Spain, Gargallo-Bernad et al.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> found that up to 12.2% of patients with a newly diagnosed HIV infection or Acquired Immune Deficiency Syndrome (AIDS) had visited a HED, with a mean of two visits per patient within the three years preceding the diagnosis.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Routine testing for diseases with an HIV prevalence greater than 0.1% has proven to be a cost-effective method.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In general terms, a high degree of patient acceptance for routine and universal HIV serology testing has been observed in HEDs. Two studies performed in the regional community of Madrid, Spain,<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a> revealed a prevalence of HIV-positive serology tests of between 0.6% and 0.9%, which is higher than that observed in Primary Care settings (prevalence of 0.2%).</p><p id="par0035" class="elsevierStylePara elsevierViewall">Thus, we are completely in favor of the initiative of the Spanish Society of Emergency Medicine (SEMES, <span class="elsevierStyleItalic">Sociedad Española de Medicina de Urgencias y Emergencias</span>) to implement the possibility of offering HIV screening tests in HEDs to patients who consult for certain diseases with a high HIV prevalence (>4%). These indications focus on patients consulting for sexually transmitted diseases (HIV prevalence of 33%), herpes zoster (prevalence of 8%-10%), mononucleosis syndrome (prevalence of 5%), community-acquired pneumonia (prevalence of 9-16%), and those reporting engaging in chemsex practices.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The most efficient circuit to avoid a loss of patients in this situation has yet to be defined.
