class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">In recent years, important advances have been made in the treatment of malignant diseases. However, stem cell transplantation (SCT) remains the only curative option for some patients. The development of complications during SCT may lead to admission to an intensive care unit (ICU), and has a well established deleterious effect on patient survival.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The prognosis of hematological patients that require intensive care measures has notably improved due to better supportive care, better patient selection and closer clinical collaboration between intensivists and hematologists, resulting in a higher frequency of satisfactory discharge from the ICU.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">2–5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">However, little is known about the possible consequences of a previous ICU admission on outcomes following SCT. Therefore, to study the possible adverse effects of a previous ICU admission on SCT outcome in patients with hematologic malignances, we retrospectively compared the outcomes of transplanted patients requiring a previous stay in the ICU with patients not requiring ICU admission.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">From January 2000 to April 2016, 800 SCT procedures (in 743 patients) were carried out in adult patients in our institution. Of these, 22 (2.9%) patients had been admitted to the ICU (ICU-group) due to complications related to chemotherapies administered before the SCT procedure.</p><p id="par0025" class="elsevierStylePara elsevierViewall">For this study, these patients were matched (1:2) with patients with similar baseline characteristics (age, sex, hematologic disorder, disease status at SCT, type of SCT [autologous or allogeneic], comorbidity [Sorror and EBMT scores<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6,7</span></a>], time of SCT, stem cell source, human leukocyte antigen [HLA], match and conditioning regimen [myeloablative vs. non-myeloablative]) who had not been previously admitted to the ICU (NO-ICU-group). One patient in the ICU-group needed intensive care support twice, but only the first admission was considered for this study.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The medical records of all the patients were retrospectively reviewed, and data were recorded, including demographic, hematological disease, SCT, status of disease at SCT, source of stem cells, HLA match, conditioning regimen, transplant complications, length of in-hospital stay during SCT, as well as outcome data (date of progression/relapse and death or last visit). Data regarding admission into the ICU (date and cause of admission, length of ICU stay, need for mechanical ventilation, renal replacement or vasoactive support and APACHE II [Acute Physiology And Chronic Health Evaluation II])<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">8</span></a> were also collected in the ICU-group.</p><p id="par0035" class="elsevierStylePara elsevierViewall">This study was conducted according to the Declaration of Helsinki and the rules of good clinical practice, and all patients signed written informed consent before the SCT procedure. The approval of the local ethics committee was obtained.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">A descriptive study of the main demographic and clinical variables was performed. The Mann–Whitney <span class="elsevierStyleItalic">U</span> or median tests (continuous variables) and the Pearson chi-square or Fisher exact tests (categorical variables) were used for comparison between groups. Overall survival (OS) was defined as the time from SCT to death or the last follow-up. OS curves were plotted using the Kaplan–Meier method and compared by the log-rank test. Estimates of relapse incidence (RI) and non-relapse mortality (NRM) were analyzed as competing events and were compared with Gray's test. All statistical analyses were conducted with SPSS version 23 and R version 3.2.2. The level of significance was set at <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">From January 2000 to April 2016, 320 hematological patients were transferred to the ICU and 205 were discharged alive. Twenty-two patients were finally considered for SCT based on basal disease status and absence of any major contraindication.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Sixty-six patients were included in the study (22 in the ICU-group and 44 in the NO-ICU-group). <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows their baseline characteristics. As expected, there were no significant differences in demographic, hematological and SCT procedure characteristics between the two groups.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">In patients requiring advanced medical support, the median APACHE II score at the time of ICU admission was 21 (range 11–31). The causes of admission into the ICU were: respiratory failure in 11 (50%), septic shock in 4 (18%), neurological disturbance in 4 (18%), post-surgery care in 2 (9%) and tumor lysis syndrome in 1 (5%). Among the whole group, 17 (77%) needed mechanical ventilation, and 8 out of 21 evaluable patients (38%) required vasoactive support. Six out of 20 patients (30%) showed renal function impairment, and 7 out of 21 (33%) presented liver damage and 6 (27%) neurological disturbance. Neutropenia (<0.5<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span>/L) was observed in 17 (77%) patients during ICU stay. The median time between ICU admission and the SCT procedure was 144 days (range 106–1093), and the median length of stay in the ICU was 11.5 days (range 3–57). The 5-years non-relapse mortality was 18% (2%; 46%) for patients that received a SCT earlier than 144 days after ICU admission and 50% (12%; 79%) for those that received SCT later (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.520).</p><p id="par0060" class="elsevierStylePara elsevierViewall">Comparing the SCT outcomes of both groups, there were no differences in time to neutrophil and platelet recovery (the median time to neutrophil count<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>0.5<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span>/L was 14 days [range 10–48] in the ICU group vs. 13 days [range 10–23] in the NO-ICU group, and the median time to platelet count recovery<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>20<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span>/L was 14 days [range 10–151] vs. 15 days, respectively [range 9–48]). There were also no differences in length of hospital stay during SCT.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Conversely, microbiologically documented infections were more common in the ICU group (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). In the ICU group a total of 23 infections were documented; 19 (82%) caused by bacteria. Of these, 11 (57%) were caused by Gram-negative bacteria and 5 (26%) by multidrug-resistant pathogens. Six out of 23 patients presented the same recurrent infection during SCT process, three of them with a multi-drug resistant pathogen. In the NO-ICU group, 22 infections were reported; 20 (90%) by bacteria, 12 (60%) of which were Gram-negative, and only one (5%) was multidrug-resistant (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">With a median follow-up after SCT of 5.47 years (range 0.50–16.22) in the ICU group and 4.52 years (range 0.73–15.85) in the NO-ICU group, the 5-year OS probability (95% CI) was 49% (28–70%) vs. 45% (29–61%), respectively (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.353). The 5-yr RI was 34% (14–56%) vs. 42% (25–58%) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.755) and the 5-yr incidence of NRM was 32% (14–52%) vs. 24% (12–38%) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.333), respectively (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1–3</a>). No differences were observed in the allogeneic and autologous SCT subgroups considered separately (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Six patients (27%) in the ICU group and 8 (18%) in the NO-ICU group required admission to the ICU during or after the SCT procedure (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.293). At the time of the analysis, 12 (54%) patients in the ICU and 22 (50%) in the NO-ICU group had died. The causes of death were similar in both groups. In the ICU group 5 (42%) patients died of relapse, 4 (33%) of infection and 3 (25%) of GVHD. In the No-ICU group, the main cause of death was relapse in 8 (37%) patients, infection in 4 (18%), GVHD in 8 (37%), hepatic sinusoidal obstruction syndrome in 1 (4%) and secondary malignancy in 1 patient (4%).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">The aim of this study was to determine whether admission into the ICU during induction and consolidation chemotherapies prior to SCT had an impact on the outcome of patients after SCT. The OS, RI and NRM after SCT was similar in both study groups. According to the design of the study, both groups of patients were comparable for the main demographic, disease- and SCT-related characteristics. It is of note that the time to hematological recovery and length of hospital stay for SCT were similar in both groups. Conversely, patients with a previous ICU stay presented more microbiologically documented infections and by more drug-resistant pathogens, although this had no adverse effect on patient survival.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Previous studies have shown that the survival of patients with hematological disorders admitted to the ICU, either before or at the time of SCT, is still poor, although the prognosis has improved over the last few years.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">1,2,5,9–11</span></a> However, there are few data on the impact of a previous ICU admission on the survival of patients who ultimately undergo SCT.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The results of our study are similar to those described in patients with newly diagnosed AML who received intensive chemotherapy, where patients who survived an ICU stay had a similar long term survival to those patients who never required intensive care support.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">2</span></a> In this latter study, the survival to hospital discharge was decreased in patients who were admitted into the ICU, as compared to those that were not, because the decrease in survival was caused by the complication that prompted the admission into the ICU. Once the patients overcame ICU admission and were discharged, their probability of survival was similar to that of AML patients who did not require intensive care measures.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">2–5</span></a> Similarly, diffuse large B-cell lymphoma patients that required ICU admission also had a worse OS but a similar disease-free survival<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">12</span></a> than patients who did not.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Another relevant finding of our study was that very few transplanted patients (2.9%) had previously been admitted to the ICU, which is likely explained by the diversity of hematological diseases requiring SCT. However, it has been reported that 12–28% of patients with AML<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">2,5,11,13</span></a> need intensive care at some point. This seems to show that a large proportion of patients with hematological disorders admitted to the ICU do not eventually undergo SCT, probably due to the high mortality rate related to ICU stay, or perhaps, because patients with a previous admission into the ICU are deemed ineligible for SCT due to an assumed high risk of future complications. Limitations aside, the results of our study seem to indicate that a previous ICU admission should not warrant denial for SCT, when otherwise indicated.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The number of ICU survivors that may need a SCT is likely to rise in the future, as the prognosis of patients with hematological disorders admitted to the ICU is improving.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">1,2,9,14</span></a> New techniques of non-invasive ventilation (leading to less frequent use of mechanical ventilation), increase knowledge of oxygen-induced lung damage, the use of better antimicrobial drugs for treatment and prophylaxis of infections, as well as better patient selection<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">8</span></a> have led to an increase in the number of ICU survivors with hematological disorders who can be considered as candidates for SCT consolidation.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The leading causes of admission into the ICU in our study were respiratory failure and septic shock, with many patients requiring mechanical ventilation, consistent with previous series<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">1,2,5,9–11</span></a> and indicating that despite the limited sample size, this is a representative sample of daily care. Similarly, the causes of death (mainly relapse, infection and graft-versus-host-disease) were similar in the two groups and to those described in larger series,<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">9</span></a> also underscoring the previous point. Surprisingly, there was no difference between the two groups in terms of the number of patients requiring admission to the ICU during the transplant period. This may be explained by strict selection criteria for SCT with respect to a previous ICU stay.</p><p id="par0110" class="elsevierStylePara elsevierViewall">This study has several limitations. It was a small, retrospective, single center study including patients with a broad range of disorders. However, taking into account that very few patients with a previous admission into the ICU undergo SCT, large series are difficult to achieve. Conversely, one of the strengths of this study was that all the patients were treated by the same medical team, implying homogenous medical criteria and indication for ICU admission as well as ICU care.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusion</span><p id="par0115" class="elsevierStylePara elsevierViewall">The results of this study indicate that patients previously admitted to the ICU and finally undergoing SCT have a similar outcome to those who have never been admitted into the ICU.
