elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Rheumatoid arthritis (RA) is a chronic, autoimmune disease that is characterized by persistent synovitis, systemic inflammation and circulating autoantibodies that could lead to structural damage, irreversible disability, a higher cardiovascular risk, and several other comorbidities.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Cardiovascular disease is extremely common in the general population affecting most of adults past the age of 60 years. Age, male gender, smoking, high blood pressure, and diabetes mellitus are considered as the traditional risk factors. RA implies an increasement of a 48% of chance of suffering cardiovascular events and a 50% higher incidence of cardiovascular disease and subsequent mortality.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">3–5</span></a> Chronic inflammation seems to be the link between RA and cardiovascular disease.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">6,7</span></a> Moreover, subtle arteriosclerosis is thought to be present from early stages of the disease and all this evidence supports the statement that RA acts as an independent cardiovascular risk factors in addition to the traditional ones.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">7,8</span></a> Therefore, is of great importance an accurate assessment of cardiovascular risk.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">9,10</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">On the general population, the use of tools as Framingham Risk Score (which uses the traditional risk factors in estimating the chance of an event),<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">11</span></a> Reynolds Risk Score (which includes inflammation measured with C reactive protein),<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">12</span></a> or in Europe, the Systematic Coronary Risk Evaluation (SCORE)<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">13</span></a> has been validated. However, all those indexes might underestimate the cardiovascular risk in RA,<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">14–16</span></a> so EULAR recommended to apply a ×1.5 multiplicator to the FRS or the SCORE.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">17</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">In the last few years, carotid ultrasound has been shown as a valuable tool in estimating cardiovascular risk,<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">18</span></a> and studies focused on RA patients, demonstrated a higher intima media thickness (IMT) and a higher presence of carotid plaques,<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">19,20</span></a> findings related to a higher cardiovascular disease.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">21,22</span></a> Whether traditional risk factors only explain a part of the global burden of cardiovascular disease in RA, the objective of this study is to assess the cardiovascular risk in RA patients using carotid ultrasound additionally to the traditional risk factors.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design and population</span><p id="par0025" class="elsevierStylePara elsevierViewall">A single center cross-sectional case control study was performed. Inclusion criteria were adult patients who fulfilled the ACR/EULAR 2010 RA criteria (cases) and matched adults in terms of age, gender, and cardiovascular risk factors (controls). Population over 75 years old, patients with established CV disease and/or chronic kidney disease (from III Stage) were excluded. Controls with other inflammatory diseases, pregnant women or any malignancy were also excluded. This study was performed from July-2019 to January-2020.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Cases and controls included in the study went to a scheduled appointment at the hospital for evaluation, measurement of arterial blood pressure and the US examination. The blood test was performed at least 15 days before the appointment.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Demographic, clinical and laboratory parameters including autoantibodies (Rheumatoid Factor (RF) and anti-citrullinated protein antibodies (ACPA)) were collected. Body Mass Index (BMI), Disease activity (DAS28, SDAI), evolution, functional capacity (HAQ) and efficacy assessments were collected. Low disease activity (LDA) was considered if DAS28 value was between >2.6 and 3.2 and remission<span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>2.6.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">1</span></a> RF was determined by immunoturbidimetry (Roche Diagnostics, cut-off point 10<span class="elsevierStyleHsp" style=""></span>UI/L) and ACPA were assessed by fluoroenzyme immunoassay (Phadia 250, negative 7; dubious 7–10; positive<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>10).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Cardiovascular risk variables</span><p id="par0040" class="elsevierStylePara elsevierViewall">Patients were asked about smoking habit (former/smoker/non-smoker), physical activity, familiar history of early cardiovascular disease in first degree relatives (<55 years in men and <65 years in women) and prior history of CV risk factors. High blood pressure was considered in patients who had prior diagnosis and were under pharmacologic treatment. Blood pressure was measured according to guidelines. Diabetes mellitus was considered according to ADA guidelines. Dyslipidemia was included if total cholesterol was over 220<span class="elsevierStyleHsp" style=""></span>mg/dL in 2 or more determinations; if serum triglycerides were over 150<span class="elsevierStyleHsp" style=""></span>mg/dL or in patients who were taking lipid-lowering agents.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Laboratory markers included in the analysis were glucose, total cholesterol, HDL cholesterol, LDL cholesterol, Triglycerides, Creatinine, Glomerular filtrate (CKD-EPI), uric acid, C reactive protein (CRP), Erythrocyte Sedimentation Rate (ESR), RF and ACPA. Cholesterol was also expressed as total cholesterol to HDL cholesterol ratio.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Carotid ultrasound</span><p id="par0050" class="elsevierStylePara elsevierViewall">The US evaluator was blinded to the case/control condition and examination included visualization of common, internal and external carotid arteries<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">18</span></a> by using B-mode imaging of the arterial wall, color doppler flow analysis, presence of plaques, plaque number and measured the IMT in both right and left carotid. According to the Mannheim Consensus, plaque was considered any focal structure encroaching into the arterial lumen of at least 0.5<span class="elsevierStyleHsp" style=""></span>mm or 50% of the surrounding IMT value or demonstrates a thickness larger than 1.5<span class="elsevierStyleHsp" style=""></span>mm as measured from the media-adventicia interface to the intima-lumen interface.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">18</span></a> The US study was performed using the Esaote MyLab™ Class C and a linear high-resolution transducer (LA523). IMT was measured with QIMT (Quality Intima Media Thickness) software by Esaote.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">Continuous variables were summarized using mean (SD, standard deviation) and median (1st, 3rd quartiles). Categorical variables were summarized using absolute and relative frequencies (%). All determinations included 95% confidence intervals for the variances. To estimate associations between different continuous variables, data were analyzed by Pearson's correlation in the case of linear relationships and by Spearman's correlation in the case of non-linear relationships. To find differences in right and left IMT between groups a MANOVA multivariate variance analysis was adjusted, including confounding factors as age and gender. A negative binomial regression, duly adjusted for confounding factors, was used to assess differences in the number of plaques between cases and controls. Previously the differences between groups with the classic CV risk factors were evaluated by means of logistic regressions corrected by gender, age, BMI, and tobacco habit.</p><p id="par0060" class="elsevierStylePara elsevierViewall">All statistical analyses were performed using software R (version 3.6.1). <span class="elsevierStyleItalic">P</span> values were set based on the number of analyses.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Ethical considerations</span><p id="par0065" class="elsevierStylePara elsevierViewall">Ethical approval was obtained from the Local Ethics Board at Hospital La Fe, Valencia, Spain. Participants provided written informed consent.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Study participants</span><p id="par0070" class="elsevierStylePara elsevierViewall">Overall, 200 patients and 111 controls were included in the study. Baseline demographic and disease characteristics were similar between the groups (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). A higher proportion of patients diagnosed with osteoporosis and gout was noted in control population.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Rheumatoid arthritis disease characteristic</span><p id="par0075" class="elsevierStylePara elsevierViewall">RA had a mean evolution of 16 years (sd: 11.38) and the 82% of the patients had an erosive disease. Seropositivity was noted in the 73% of the patient sample. Almost half of the patients were under biological or JAK inhibitors therapy. Mean disease activity measured with DAS28-ESR and SDAI was in the range of low disease activity, 73 patients (37%) were in remission and 42 (21%) in low disease activity. All this data is exposed in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Cardiovascular risk assessment</span><p id="par0080" class="elsevierStylePara elsevierViewall">There was slightly more High blood pressure and dyslipidemia prevalence in the patient group. Daily physical activity was noted in the 69% of patients and in the 58% of the controls. The rest of comorbidities and variables were balanced between groups (diabetes mellitus, BMI). Disease activity, measured by DAS28 was not related to changes in total cholesterol (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.693) nor HDL cholesterol (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.579) or the Tc/HDLc ratio (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.652). All this data is provided in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Dyslipidemia adjusted to CV risk factors was not related to RA (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.514), BMI index (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.111), female sex (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.738) nor previous smoking habit (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.159). Age was slightly related to dyslipidemia (95% [1.051; 1.111], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and former smokers evidenced lower dyslipidemia (IC 95% [0.166; 0.638], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001). Diabetes mellitus was not related to RA (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.652), female sex (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.457) nor smoking habit (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.645) were not related to this feature. BMI index (95% [1.039; 1.232], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.004) and age (CI 95% [1.038; 1.158], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) were related to diabetes mellitus. High Blood Pressure was related to BMI (95% [1.039; 1.232], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.004) and age (CI 95% [1.038; 1.158], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) but not to RA (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.279), female sex (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.181) or smoking habit (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.288). A higher BMI was related to sedentarism (CI 95% [0.541; 2.938], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005), gender (female sex CI 95% [−3.091; −0.43], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01), and age (CI 95% [0.039; 0.141], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) but not to RA (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.933) or smoking (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.957).</p><p id="par0090" class="elsevierStylePara elsevierViewall">Laboratory results from the blood test were also balanced between groups, although CRP and ESR were higher in patients than in controls.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Carotid ultrasonography</span><p id="par0095" class="elsevierStylePara elsevierViewall">US study revealed a greater presence of plaques in patients (51%) than in controls (29%). However, IMT ANOVA analysis demonstrated some differences between left and right arteries. In the study of the left artery, IMT was higher in cases than controls (0.022) but the statistical signification was not achieved (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.19). Right study was on average 0.039 higher in cases than controls (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.006).</p><p id="par0100" class="elsevierStylePara elsevierViewall">Moreover, multivariate ANOVA analysis also found that IMT value was related to the age of the patients (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001 in both arteries) and women had on average 0.045 less IMT than men (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.018) in the left artery and 0.018 in the right artery (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.27). Dyslipidemia also influenced the IMT value (0.039 in the left artery [<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.02] and 0.024 in the right artery [0.11]. High Blood Pressure, Diabetes nor BMI modified the IMT result in the model.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Another ANOVA model was built with only data from cases population in which DAS28 and the longer disease duration were related to higher IMT value (0.038, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.02 for DAS28 and 0.002, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005 for disease duration in years). Seropositivity, biologic failure, SDAI nor HAQ showed influence on IMT.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Plaques were found in both carotid arteries in the 32% of cases and 10% of controls. The average number of plaques detected in patients was 1.61 and 0.55 in controls. All this data is exposed in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><p id="par0115" class="elsevierStylePara elsevierViewall">In RA patients, the number of plaques was not related to disease activity measured by SDAI (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.521), DAS28 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.452) or HAQ (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.651). The longer duration of RA was related to a higher presence of carotid plaques (95% [1.015; 1.056], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). By the other hand, in the total population of the study, dyslipidemia (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.068), diabetes mellitus (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.382) or BMI (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.525) were neither related to a major presence of plaques. Furthermore, cases evidenced more presence of plaques than controls (CI 95% [1.542; 3.436], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and age was also related to plaque development (CI 95% [1.051; 1.094], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) as seen with high blood pressure (CI 95% [1.124; 2.27], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.012). Female sex protected against carotid plaques presence (CI 95% [0.367; 0.856], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.009). Uric acid levels were also statistically protectors of carotid plaques (CI 95% [0.727; 0.983], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.033).</p></span></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">Although the RA major clinical features are related to the joint involvement, RA is a systemic disease and many organs are frequently involved such as the lung, exocrine glands, eye, kidney, or gastrointestinal system. Regardless, cardiovascular disease seems to be the major cause of morbidity and mortality in RA patients.<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">23–25</span></a> In this exploratory study we have evaluated the CV risk with the Carotid Ultrasound additionally to the traditional risk factor to determinate the correlation between the classical CV evaluation and a potential new approach, excluding population with known cardiovascular disease.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Our sample baseline was sufficient balanced between groups; however, osteoporosis and gout were more frequently detected in controls, and this could be meaningful since gout has also been reported as an independent CV risk factor. As seen in other series published, RA cases were predominantly females, seropositive with a disease duration of more than 10 years.<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">26–29</span></a> Mean age was over 55 years old and the traditional risk factors was balanced between groups with no statistical differences.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Carotid intima-media complex was significant thicker in RA than in controls after adjustment for confounding factors. Also, a higher presence of plaques was noted in cases. These findings are consistent with data published in a metanalysis by Ambrosino P et al. in which RA cases also shown a higher IMT and plaques presence even in early RA.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">27</span></a> Similar results, reported by Wah-Suarez MI et al., revealed more plaques and greater IMT in cases than in controls.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">28</span></a> Both groups also reported relationship with DAS28, the duration of the illness, CRP and ESR value.<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">27,28</span></a> In our study, we detected association with DAS28 and disease duration but not with acute phase markers, probably explained by the lower disease activity stated in our study.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In contrast to other series,<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">27,29</span></a> we found no relationship between IMT and plaques with ACPA seropositivity. In addition, we also did not find any relation between any traditional cardiovascular risk factor and age, probably explained by a low statistical power and potentially correctable with a larger sample.</p><p id="par0140" class="elsevierStylePara elsevierViewall">On the other hand, we found no differences between taking statins and the value of the IMT, nor was it related to less presence of plaques, also drawing attention to the fact that in our sample 51.5% of patients at the time of ultrasound were taking oral glucocorticoids, without finding a relationship with an increased IMT or the presence of plaques, which would support the greater weight that inflammatory disease would have in RA patients, as described in previous studies<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">27–31</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">Because cardiovascular events are the leading cause of death in RA patients and because qualifying scores do not consider inflammatory phenomena or adjust for disease activity, it is important to establish markers of subclinical atherosclerosis that can predict cardiovascular risk and specify the development of cardiovascular events in these patients.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">20,27</span></a> Given that the IMT is a good predictor of cardiovascular risk at the systemic level, it can be used in patients with RA to estimate the CVD and study which score is closest to diagnosis.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">22</span></a></p><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Limitations</span><p id="par0150" class="elsevierStylePara elsevierViewall">A prospective study would allow to stablished causality between ultrasound findings and cardiovascular events. Would be also interesting to increase the control population. Moreover, the results are applied in a country where the cardiovascular risk is low, so the results may not be valid to extrapolate to other populations. However, the studies consulted show that the populations studied are similar.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Further evaluations of the cohort would add prognostic value of these findings and could help in assess the role of RA and CV treatment in prevention of CV events.</p></span></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Conclusion</span><p id="par0160" class="elsevierStylePara elsevierViewall">In conclusion, measuring carotid IMT and/or detecting atheroma plaques by ultrasound are useful tools in the evaluation of CRV in RA patients. A high prevalence of carotid atherosclerosis estimated by ultrasound in RA is observed in our sample compared to controls, independent to age, gender, and classic CV risk factors.</p><p id="par0165" class="elsevierStylePara elsevierViewall">A relationship of carotid atherosclerosis with the duration of RA and DAS28 is evidenced, which may indicate, as it has been stated in other studies, that inflammatory and inherent conditions to RA may affect the formation of atherosclerotic plaques, so for the management of CVD in these patients it should be paramount not only to control traditional risk factors, but also the disease activity of RA.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Funding</span><p Statistical analysis included a multivariate variance analysis (Manova) and a negative binomial regression adjusted by confounding factors.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Overall, a total of 200 cases and 111 controls were included in the study. Demographical and clinical variables were comparable between groups. A relationship between age, BMI and high blood pressure was detected in both groups.
