Suelen afectar principalmente al tercio medio de la diáfisis abarcando el 69 al 82% de las fracturas de clavícula<a class="elsevierStyleCrossRef" href="#bib0130">1</a> Las incidencias más altas de estas lesiones se encuentran en los varones jóvenes y predominantemente afectan el tercio medio del hueso<a class="elsevierStyleCrossRef" href="#bib0135">2</a>. En la población infantil se dice que la incidencia es de aproximadamente del 10%<a class="elsevierStyleCrossRef" href="#bib0140">3</a>.El 8,7% de los pacientes generalmente se presentan con una historia de caída sobre el hombro, 7% como un golpe directo y 6% describen una caída con la mano extendida<a class="elsevierStyleCrossRef" href="#bib0145">4</a>, esta última relacionado generalmente, con actividades como la equitación o ciclismo<a class="elsevierStyleCrossRef" href="#bib0150">5</a>. La osteoporosis se asocia a la perdida de hueso esponjoso y con el pasar del tiempo la perdida de la cortical conllevando a la vulnerabilidad de fracturas ante traumas que no se suelen considerar significativos para las mismas, dato que es relevante, especialmente en mujeres mayores de 80 años con traumas de baja energía<a class="elsevierStyleCrossRef" href="#bib0155">6</a>. En un estudio llevado a cabo post mortem se identificó la perdida de mineralización a nivel de los extremos de la clavícula por lo que esto puede evidenciar que el riesgo de fractura patológica en adulto mayor es más alto y suele afectar más estos fragmentos del hueso<a class="elsevierStyleCrossRef" href="#bib0160">7</a>.La gran mayoría de pacientes cursan con deformidad evidente y dolorosa, acompañadas de sensibilidad localizada sobre el lugar de la fractura. El diagnóstico es básicamente clínico, suele estar asociado un antecedente traumático o en su defecto a alguna alteración ósea, como la osteoporosis; sin embargo, es necesario realizar ayudas imagenológicas como la radiografía, con la cual se confirma el diagnóstico y se ubica la anatomía y severidad de la fractura.El manejo de esta entidad patológica tiene dos posibilidades; el tratamiento quirúrgico en el que existe una terapia con placa y otra con clavo endomedular<a class="elsevierStyleCrossRefs" href="#bib0165">8,9</a>, por otro lado, se encuentra el tratamiento incruento, con cabrestrillo o vendaje en ocho<a class="elsevierStyleCrossRefs" href="#bib0175">10,11</a>, buscando la resolución de la superposición de los muñones y la limitación de acortamiento clavicular<a class="elsevierStyleCrossRef" href="#bib0185">12</a>.El objetivo de nuestro estudio es caracterizar los pacientes con fractura de clavícula manejados bajo tratamiento conservador.Materiales y métodosTipo de investigación: estudio de tipo descriptivo retrospectivoPoblación: el universo de este estudio correspondía a pacientes con diagnóstico de fractura de clavícula que fueron atendidos en la clínica Medilaser de Tunja durante los años 2013 a 2019.Muestra: contando con una población aproximada de 152 000 personas para el municipio de Tunja, al realizar un estudio con un intervalo de confianza del 95% y margen de error del 5%, se debían recolectar 134 historias clínicas.Muestreo: probabilístico tipo aleatorio sistemático.Criterios de inclusión: dentro de los criterios de inclusión se tuvo en cuenta a todos los pacientes con fractura de clavícula manejada con tratamiento incruento.Criterios de exclusión: pacientes con fractura de clavícula manejados con tratamiento quirúrgico.Variables: se evaluaron los siguientes datos: edad, ocupación, sexo, mecanismo del trauma y tratamiento según historia clínica.Análisis estadístico: la base de datos fue registrada en Excel versión 2013. Para el análisis univariado se realizaron frecuencias absolutas y relativas de las variables cualitativas y, se para el caso de las variables cuantitativas se realizó medidas de tendencia central (media y mediana) con sus medidas de dispersión (desviación estándar y rango intercuartil).Sesgos: con el fin de disminuir el sesgo de selección se realizó la revisión de historias clínicas buscando los casos mediante los códigos internacionales de enfermedades (CIE-10) relacionados con fractura de clavícula (S420).Para controlar el sesgo de información, memoria y medición, se realizó una ficha de recolección de información para darle uniformidad al estudio, adicionalmente se definieron los criterios de inclusión y exclusión para seleccionar adecuadamente a los participantes.Consideraciones éticas: basados en la resolución 8430 de 1993, donde se establecen las normas de investigación en salud, se considera un estudio sin riesgo, puesto que se basa en la revisión de historias clínicas. Adicionalmente, se solicitó el debido permiso al comité de ética e investigaciónResultadosSe recolectaron los datos de 134 participantes, basados en el tamaño de muestra, criterios de inclusión y exclusión.Caracterización socio-demográficaSe encontró que la mayoría de personas con fractura de clavícula correspondían al sexo masculino, con edades entre los 20 a 39 años. Según la profesión u ocupación se observó que los estudiantes y los trabajadores independientes eran las personas más afectadas (<a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>).<elsevierMultimedia ident="tbl0005"></elsevierMultimedia>Características de la fracturaEn total 81 personas presentaron fractura de la clavícula derecha, 52 con fractura del lado izquierdo y una persona con afección bilateral (<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>).<elsevierMultimedia ident="tbl0010"></elsevierMultimedia>Resultados posterior a la recuperaciónEl tiempo promedio de recuperación fue de 4.2 días, con un minimo de 0.7 días y máximo de 24 días. El 55,2% de las personas retomaron sus actividades laborales con normalidad, posterior a la recuperación, sin en embargo se observa un grado alto de satisfacción en el 47.8% y el 61.2% refieren dolor residual (<a class="elsevierStyleCrossRef" href="#tbl0015">tabla 3</a>)<elsevierMultimedia ident="tbl0015"></elsevierMultimedia>DiscusiónSegún un estudio mostro que la prevalencia de fractura de clavicula varía entre el 78 al 91% de la población, en donde las personas de sexo masculino son las más afectadas<a class="elsevierStyleCrossRef" href="#bib0190">13</a>. Por otro lado el estudio de Carvajal y colaboradores reportó una incidencia en hombres adultos de 71 por 100.000, y en mujeres de 30 por 100.000 individuos<a class="elsevierStyleCrossRefs" href="#bib0195">14,15</a>. En nuestro caso se reportó un 75% en hombres y un 25% en mujeres lo que permite observar que los varones siguen siendo los más afectados.Dentro de las edades de presentación en un estudio realizado en EEU en 2019 el rango mas alto estaba entre los 10 a 19 años, luego es importante tener en cuenta el tipo de actividad a la que vaya dirigido el estudio, ya que dicha investigación estaba enfocada hacia las fracturas asociadas a actividades deportivas, afectando con mas frecuencia a la población juvenil, mientras que en el estudio que nos concierne cobijaba a todos sin importar el mecanismo de trauma en donde las edades de presentación oscilaban entre 20 y 59 años.El sistema de clasificación más ampliamente aceptado es el descrito por Allman en 1967, en el cual se dividen las fracturas de clavícula según su localización sobre el hueso, permitiendo caracterizar este tipo de fracturas y de igual forma es importante ya que se puede inferir acerca del tratamiento, luego en todas las clínicas se debiera manejar un protocolo que incluya en su proceso la aclaración de dicha clasificación.En cuanto a la profesión se encontró que los trabajadores independientes y dedicados a otros oficios no especificados fueron los mas afectados, lo cual permite inferir que el hecho de ser independientes deben realizar diferentes actividades que demanden esfuerzo físico en donde se exponen a traumas de alta y baja energía, haciéndolos más vulnerables a este tipo de alteración. Cabe resaltar que la clavícula que se vio mas comprometida es la del lado derecho, asumiendo que la gran mayoría de la población suelen ser diestros teniendo cierta asociación a ello.En un estudio publicado en 2018 el cual comparaba los dos tipos de tratamiento: quirúrgico y conservador, permitió ver que dentro de las complicaciones de los pacientes manejados con tratamiento incruento presentaron dolor residual. Para el caso de nuestro estudio en particular manifestaron dolor persistente en un 61%, cosa que coincide con el anteriormente nombrado.Históricamente los autores han abogado contra la intervención quirúrgica debido a la consolidación de la fractura y los resultados clínicos exitosos reportados con el tratamiento conservador, incluso en la presencia de un desplazamiento sustancial<a class="elsevierStyleCrossRef" href="#bib0205">16</a>. Es importante tener en cuenta esta alternativa no invasiva sin dejar de lado una intervención quirúrgica si se amerita, ya que toda cirugía por más pequeña e inofensiva que parezca no deja de tener complicaciones, tanto para el paciente como para la entidad de salud, ya que esta debe acarrear con más costos, así es de que si se puede evitar el manejo quirúrgico claro está, que exceptuando los casos especiales; fracturas abiertas, desplazadas, con cabalgamiento de más de 2cm<a class="elsevierStyleCrossRef" href="#bib0210">17</a>, se podría pensar en primera medida en el tratamiento incruento, si bien es cierto se ha estipulado que cualquiera de las dos opciones se puede llevar a cabo y en algunos casos incluso se da manejo con las dos alternativas, no se tiene claro que haya diferencia entre la respuesta de la una a la otra puesto que para el caso del manejo conservador según algunos estudios afirman que los tratamientos pueden variar en las complicaciones, para el caso anteriormente citado suele presentarse en ciertas ocasiones seudoartrosis o mala unión<a class="elsevierStyleCrossRefs" href="#bib0215">18,19</a>, mientras que con el manejo quirúrgico suelen presentarse complicaciones relacionadas con disección de tejidos blandos, riesgo de lesión de nervios supraclaviculares y malos resultados estéticos<a class="elsevierStyleCrossRefs" href="#bib0225">20–22</a>. Al igual que puede provocar mas solicitudes de retiro del implante por parte del paciente, debido a prominencias del mismo<a class="elsevierStyleCrossRefs" href="#bib0240">23–25</a>.Concluyendo que, si se ponen en una balanza los dos tratamientos, ambos terminarían siendo efectivos, por ende el manejo no invasivo llevaría la delantera por el hecho de no someter al paciente a un riesgo extra que presenta cualquier procedimiento quirúrgico. Respecto a lo anterior cabe mencionar que en el estudio llevado a cabo en la clínica Medilaser se identificó un gran número de casos con dolor residual, dejando ver esta respuesta como una complicación a dicho tratamiento, sería interesante hacer la compración de los pacientes que fueron manejados quirurgicamente para observar si en ellos tambien se presenta este hallazgo, lo que indicaria la posibilidad de continuar con el mismo manejo o definitivamete pensar en tratamiento quirurgico.FinanciaciónRecursos propios de los autores.Conflictos de interésLos autores no declaran algún conflicto de interés." Dentro de los objetivos del estudio se busca tener una apreciación más cercana respecto al tratamiento para dichas fracturas enfocados en el manejo no quirúrgico. Materiales & MétodosSe trata de un estudio de tipo descriptivo retrospectivo, llevado a cabo en la Clínica Medilaser de Tunja con recopilación de la información por medio de registros clínicos a pacientes que fueron ingresados entre el 2013 y 2019 con dicha afectación. ResultadosEste estudió contó con una población de 134 pacientes, entre ellos 101 varones y 33 mujeres, las edades oscilan entre los 10 y 85 años, de los cuales el 73% estaban satisfechos con su tratamiento, el otro 27% estuvieron inconformes. DiscusiónLa fractura de clavícula, es más frecuente en varones, con edades entre los 20 a 59 años y las causas más comunes fueron las caidas en general, predominando la caída en bicicleta. que representa entre el 3 y el 10&#37; de las fracturas de adultos y aproximadamente el 35&#37; de las fracturas de la cintura escapular&#46; Suelen afectar principalmente al tercio medio de la di&#225;fisis abarcando el 69 al 82&#37; de las fracturas de clav&#237;cula<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">1</span></a> Las incidencias m&#225;s altas de estas lesiones se encuentran en los varones j&#243;venes&#160;y predominantemente&#160;afectan&#160;el tercio medio del hueso<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">2</span></a>&#46; En la poblaci&#243;n infantil se dice que la incidencia es de aproximadamente del 10&#37;<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">3</span></a>&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">El 8&#44;7&#37; de los pacientes generalmente se presentan con una historia de ca&#237;da sobre el hombro&#44; 7&#37; como un golpe directo y 6&#37; describen&#160;una&#160;ca&#237;da&#160;con la&#160;mano extendida<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">4</span></a>&#44; esta &#250;ltima relacionado generalmente&#44; con actividades como la equitaci&#243;n o ciclismo<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">5</span></a>&#46; La osteoporosis se asocia a la perdida de hueso esponjoso y con el pasar del tiempo la perdida de la cortical conllevando a la vulnerabilidad de fracturas ante traumas que no se suelen considerar significativos para las mismas&#44; dato que es relevante&#44; especialmente en mujeres mayores de 80 a&#241;os con traumas de baja energ&#237;a<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">6</span></a>&#46; En un estudio llevado a cabo post mortem se identific&#243; la perdida de mineralizaci&#243;n a nivel de los extremos de la clav&#237;cula por lo que esto puede evidenciar que el riesgo de fractura patol&#243;gica en adulto mayor es m&#225;s alto y suele afectar m&#225;s estos fragmentos del hueso<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">7</span></a>&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">La gran mayor&#237;a de pacientes cursan con deformidad evidente y dolorosa&#44; acompa&#241;adas de sensibilidad localizada sobre el lugar de la fractura&#46; El diagn&#243;stico es b&#225;sicamente cl&#237;nico&#44; suele estar asociado un antecedente traum&#225;tico o en su defecto a alguna alteraci&#243;n &#243;sea&#44; como la osteoporosis&#59; sin embargo&#44; es necesario realizar ayudas imagenol&#243;gicas como la radiograf&#237;a&#44; con la cual se confirma el diagn&#243;stico y se ubica la anatom&#237;a y severidad de la fractura&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">El manejo de esta entidad patol&#243;gica tiene dos posibilidades&#59; el tratamiento quir&#250;rgico en el que existe una terapia con placa y otra con clavo endomedular<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">8&#44;9</span></a>&#44; por otro lado&#44; se encuentra el tratamiento incruento&#44; con cabrestrillo o vendaje en ocho<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">10&#44;11</span></a>&#44; buscando la resoluci&#243;n de la superposici&#243;n de los mu&#241;ones y la limitaci&#243;n de acortamiento clavicular<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">12</span></a>&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">El objetivo de nuestro estudio es caracterizar los pacientes con fractura de clav&#237;cula manejados bajo tratamiento conservador&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Materiales y m&#233;todos</span><p id="par0030" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Tipo de investigaci&#243;n&#58;</span> estudio de tipo descriptivo retrospectivo</p><p id="par0035" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Poblaci&#243;n&#58;</span> el universo de este estudio correspond&#237;a a pacientes con diagn&#243;stico de fractura de clav&#237;cula que fueron atendidos en la cl&#237;nica Medilaser de Tunja durante los a&#241;os 2013 a 2019&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Muestra&#58;</span> contando con una poblaci&#243;n aproximada de 152 000 personas para el municipio de Tunja&#44; al realizar un estudio con un intervalo de confianza del 95&#37; y margen de error del 5&#37;&#44; se deb&#237;an recolectar 134 historias cl&#237;nicas&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Muestreo&#58;</span> probabil&#237;stico tipo aleatorio sistem&#225;tico&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Criterios de inclusi&#243;n&#58;</span> dentro de los criterios de inclusi&#243;n se tuvo en cuenta a todos los pacientes con fractura de clav&#237;cula manejada con tratamiento incruento&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Criterios de exclusi&#243;n&#58;</span> pacientes con fractura de clav&#237;cula manejados con tratamiento quir&#250;rgico&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Variables&#58;</span> se evaluaron los siguientes datos&#58; edad&#44; ocupaci&#243;n&#44; sexo&#44; mecanismo del trauma y tratamiento seg&#250;n historia cl&#237;nica&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">An&#225;lisis estad&#237;stico&#58;</span> la base de datos fue registrada en Excel versi&#243;n 2013&#46; Para el an&#225;lisis univariado se realizaron frecuencias absolutas y relativas de las variables cualitativas y&#44; se para el caso de las variables cuantitativas se realiz&#243; medidas de tendencia central &#40;media y mediana&#41; con sus medidas de dispersi&#243;n &#40;desviaci&#243;n est&#225;ndar y rango intercuartil&#41;&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Sesgos&#58;</span> con el fin de disminuir el sesgo de selecci&#243;n se realiz&#243; la revisi&#243;n de historias cl&#237;nicas buscando los casos mediante los c&#243;digos internacionales de enfermedades &#40;CIE-10&#41; relacionados con fractura de clav&#237;cula &#40;S420&#41;&#46;</p><p id="par0075" class="elsevierStylePara elsevierViewall">Para controlar el sesgo de informaci&#243;n&#44; memoria y medici&#243;n&#44; se realiz&#243; una ficha de recolecci&#243;n de informaci&#243;n para darle uniformidad al estudio&#44; adicionalmente se definieron los criterios de inclusi&#243;n y exclusi&#243;n para seleccionar adecuadamente a los participantes&#46;</p><p id="par0080" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Consideraciones &#233;ticas&#58;</span> basados en la resoluci&#243;n 8430 de 1993&#44; donde se establecen las normas de investigaci&#243;n en salud&#44; se considera un estudio sin riesgo&#44; puesto que se basa en la revisi&#243;n de historias cl&#237;nicas&#46; Adicionalmente&#44; se solicit&#243; el debido permiso al comit&#233; de &#233;tica e investigaci&#243;n</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Resultados</span><p id="par0085" class="elsevierStylePara elsevierViewall">Se recolectaron los datos de 134 participantes&#44; basados en el tama&#241;o de muestra&#44; criterios de inclusi&#243;n y exclusi&#243;n&#46;</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Caracterizaci&#243;n socio-demogr&#225;fica</span><p id="par0090" class="elsevierStylePara elsevierViewall">Se encontr&#243; que la mayor&#237;a de personas con fractura de clav&#237;cula correspond&#237;an al sexo masculino&#44; con edades entre los 20 a 39 a&#241;os&#46; Seg&#250;n la profesi&#243;n u ocupaci&#243;n se observ&#243; que los estudiantes y los trabajadores independientes eran las personas m&#225;s afectadas &#40;<a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Caracter&#237;sticas de la fractura</span><p id="par0095" class="elsevierStylePara elsevierViewall">En total 81 personas presentaron fractura de la clav&#237;cula derecha&#44; 52 con fractura del lado izquierdo y una persona con afecci&#243;n bilateral &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Resultados posterior a la recuperaci&#243;n</span><p id="par0100" class="elsevierStylePara elsevierViewall">El tiempo promedio de recuperaci&#243;n fue de 4&#46;2 d&#237;as&#44; con un minimo de 0&#46;7 d&#237;as y m&#225;ximo de 24 d&#237;as&#46; El 55&#44;2&#37; de las personas retomaron sus actividades laborales con normalidad&#44; posterior a la recuperaci&#243;n&#44; sin en embargo se observa un grado alto de satisfacci&#243;n en el 47&#46;8&#37; y el 61&#46;2&#37; refieren dolor residual &#40;<a class="elsevierStyleCrossRef" href="#tbl0015">tabla 3</a>&#41;</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discusi&#243;n</span><p id="par0105" class="elsevierStylePara elsevierViewall">Seg&#250;n un estudio mostro que la prevalencia de fractura de clavicula var&#237;a entre el 78 al 91&#37; de la poblaci&#243;n&#44; en donde las personas de sexo masculino son las m&#225;s afectadas<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">13</span></a>&#46; Por otro lado el estudio de Carvajal y colaboradores report&#243; una incidencia en hombres adultos de 71 por 100&#46;000&#44; y en mujeres de 30 por 100&#46;000 individuos<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">14&#44;15</span></a>&#46; En nuestro caso se report&#243; un 75&#37; en hombres y un 25&#37; en mujeres lo que permite observar que los varones siguen siendo los m&#225;s afectados&#46;</p><p id="par0110" class="elsevierStylePara elsevierViewall">Dentro de las edades de presentaci&#243;n en un estudio realizado en EEU en 2019 el rango mas alto estaba entre los 10 a 19 a&#241;os&#44; luego es importante tener en cuenta el tipo de actividad a la que vaya dirigido el estudio&#44; ya que dicha investigaci&#243;n estaba enfocada hacia las fracturas asociadas a actividades deportivas&#44; afectando con mas frecuencia a la poblaci&#243;n juvenil&#44; mientras que en el estudio que nos concierne cobijaba a todos sin importar el mecanismo de trauma en donde las edades de presentaci&#243;n oscilaban entre 20 y 59 a&#241;os&#46;</p><p id="par0115" class="elsevierStylePara elsevierViewall">El sistema de clasificaci&#243;n m&#225;s ampliamente aceptado es el descrito por Allman en 1967&#44; en el cual se dividen las fracturas de clav&#237;cula seg&#250;n su localizaci&#243;n sobre el hueso&#44; permitiendo caracterizar este tipo de fracturas y de igual forma es importante ya que se puede inferir acerca del tratamiento&#44; luego en todas las cl&#237;nicas se debiera manejar un protocolo que incluya en su proceso la aclaraci&#243;n de dicha clasificaci&#243;n&#46;</p><p id="par0120" class="elsevierStylePara elsevierViewall">En cuanto a la profesi&#243;n se encontr&#243; que los trabajadores independientes y dedicados a otros oficios no especificados fueron los mas afectados&#44; lo cual permite inferir que el hecho de ser independientes deben realizar diferentes actividades que demanden esfuerzo f&#237;sico en donde se exponen a traumas de alta y baja energ&#237;a&#44; haci&#233;ndolos m&#225;s vulnerables a este tipo de alteraci&#243;n&#46; Cabe resaltar que la clav&#237;cula que se vio mas comprometida es la del lado derecho&#44; asumiendo que la gran mayor&#237;a de la poblaci&#243;n suelen ser diestros teniendo cierta asociaci&#243;n a ello&#46;</p><p id="par0125" class="elsevierStylePara elsevierViewall">En un estudio publicado en 2018 el cual comparaba los dos tipos de tratamiento&#58; quir&#250;rgico y conservador&#44; permiti&#243; ver que dentro de las complicaciones de los pacientes manejados con tratamiento incruento presentaron dolor residual&#46; Para el caso de nuestro estudio en particular manifestaron dolor persistente en un 61&#37;&#44; cosa que coincide con el anteriormente nombrado&#46;</p><p id="par0130" class="elsevierStylePara elsevierViewall">Hist&#243;ricamente los autores han abogado contra la intervenci&#243;n quir&#250;rgica debido a la consolidaci&#243;n de la fractura y los resultados cl&#237;nicos exitosos reportados con el tratamiento conservador&#44; incluso en la presencia de un desplazamiento sustancial<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">16</span></a>&#46; Es importante tener en cuenta esta alternativa no invasiva sin dejar de lado una intervenci&#243;n quir&#250;rgica si se amerita&#44; ya que toda cirug&#237;a por m&#225;s peque&#241;a e inofensiva que parezca no deja de tener complicaciones&#44; tanto para el paciente como para la entidad de salud&#44; ya que esta debe acarrear con m&#225;s costos&#44; as&#237; es de que si se puede evitar el manejo quir&#250;rgico claro est&#225;&#44; que exceptuando los casos especiales&#59; fracturas abiertas&#44; desplazadas&#44; con cabalgamiento de m&#225;s de 2<span class="elsevierStyleHsp" style=""></span>cm<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">17</span></a>&#44; se podr&#237;a pensar en primera medida en el tratamiento incruento&#44; si bien es cierto se ha estipulado que cualquiera de las dos opciones se puede llevar a cabo y en algunos casos incluso se da manejo con las dos alternativas&#44; no se tiene claro que haya diferencia entre la respuesta de la una a la otra puesto que para el caso del manejo conservador seg&#250;n algunos estudios afirman que los tratamientos pueden variar en las complicaciones&#44; para el caso anteriormente citado suele presentarse en ciertas ocasiones seudoartrosis o mala uni&#243;n<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">18&#44;19</span></a>&#44; mientras que con el manejo quir&#250;rgico suelen presentarse complicaciones relacionadas con disecci&#243;n de tejidos blandos&#44; riesgo de lesi&#243;n de nervios supraclaviculares y malos resultados est&#233;ticos<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">20&#8211;22</span></a>&#46; Al igual que puede provocar mas solicitudes de retiro del implante por parte del paciente&#44; debido a prominencias del mismo<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">23&#8211;25</span></a>&#46;</p><p id="par0135" class="elsevierStylePara elsevierViewall">Concluyendo que&#44;
