MRI showed an isolated lesion over the left parietal area, raising the diagnosis of a high-grade glioma versus pseudotumoral multiple sclerosis.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Patient was referred for a [<span class="elsevierStyleSup">18</span>F]F-Choline PET/MRI, considering its availability in our setting, for cerebral tumor study. This imaging study showed heterogeneous [<span class="elsevierStyleSup">18</span>F]F-Choline uptake in the lesion of the cortical-subcortical area over the upper left parietal region, extending through centrum semiovale, with a low-signal feature, adjacent subcortical white matter edema, diffusion-weighted restriction and with focal mild gadolinium enhancement (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Imaging findings prompted the lesion being biopsied, which showed a macrophage and lymphocyte infiltration with no tumor cells seen, and immunohistochemical staining with diffuse T cells (CD3) and B cells (CD20) build-up. Therefore, these findings were considered related to a multiple sclerosis (MS) lesion, and patient neurological symptoms responded to steroids treatment. MRI lesion was reduced in the follow-up study.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Multiple sclerosis is the most common inflammatory demyelinating condition of CNS, currently with unknown etiopathogeny. MS lesions involve optic nerve, brain stem, spinal cord, cerebellum and periventricular white matter mainly. Disease course usually presents as outbreaks that include stages with inflammation, demyelination, axonal lesion, gliosis and re-myelination. MS diagnostic criteria include clinical assessment, laboratory data and MRI findings described by McDonald et al. in 2010.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Tumefactive multiple sclerosis (TMS) is a rare form of MS (representing 1.4–8% of MS cases) represented by demyelinating lesions larger than 2<span class="elsevierStyleHsp" style=""></span>cm size, causing mass effect and perilesional edema, which may be misdiagnosed by a brain tumor, since both entities share common neurological symptoms and, therefore, this is the main differential diagnosis to establish. Most of TMS cases include supratentorial involvement, especially frontal and parietal lobes. Prognosis of patients with TMS lesions seems to be better than those with MS, and favorable responses have been described after treatment with high-dose steroids.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,3</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">MRI features of TMS were described by Luccuhinetti et al., including a mass effect on 36% of patients, surrounding edema in 77%, ring-like pattern of contrast enhancement in 60–65%, and T2-weighted hypointense margin in 45%. There is scarce information in the literature with respect to PET tracers’ distribution patterns in TMS lesions. Accordingly, lack of [<span class="elsevierStyleSup">18</span>F]FDG uptake has been described for all MS lesions, including TMS.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> However, a moderate [<span class="elsevierStyleSup">11</span>C]Methionine and [<span class="elsevierStyleSup">18</span>F]F-Choline uptake have been described by some authors, probably related to their affinity for inflammatory cells, with no differences found among these both PET tracers.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Our case illustrates a false-positive case due to an isolated TMS lesion, a disease to have into account when characterizing brain findings in PET imaging.</p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "NotaPie" => array:1 [ 0 => array:2 [ "etiqueta" => "☆" "nota" => "<p class="elsevierStyleNotepara" id="npar0005">Please cite this article as: Garcia JR, Baquero M, Bassa P, Compte A, Mourelo S, Riera E. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
